Comprender desde la determinación social de la salud el papel de los procesos generales, particulares y singulares sobre la realización del derecho a salud bucal de mujeres en periodo de post-parto en Santiago de Chile. Diseño cualitativo exploratorio con mujeres primíparas o multíparas en primer trimestre postparto, beneficiarias FONASA usuarias de Chile Crece Contigo. Se aplicó encuesta de clasificación social, exámenes clínicos, entrevistas semiestructuradas o grupos de discusión. Para el nivel general se analizó profusa información referencial. Existen procesos generales que favorecen la realización del derecho a salud bucal como las políticas de protección integral a la infancia, la priorización de la atención en salud en gestantes; la existencia de derechos adquiridos para la embarazada: permiso pre y post natal y fuero maternal. Desfavorece el modelo de desarrollo neoliberal y consecuentes condiciones de pobreza, la inequidad de género y el énfasis cultural en la responsabilidad femenina sobre el cuidado. Los procesos críticos actúan en los tres niveles del modelo teórico. Si bien en el nivel particular se identifica un avance en las políticas públicas, este no redunda en una efectiva realización del derecho a nivel singular pues enfrenta elementos estructurales del nivel general, sobre todo en el ámbito laboral y cultural.


Objective: To understand from the framework of social determination of health the role of general, particular and singular processes in the realization of the right to oral health for women in the postpartum period and their newborn children during the first year of life, in Santiago de Chile. Materials and Methods: Exploratory qualitative design with FONASA beneficiary women and newborn children. A social classification survey, semi-structured interviews and focus groups were applied to primiparous or multiparous women of childbearing age in the first postpartum trimester, beneficiaries of the Intersectoral Program ÒChile Crece ContigoÓ. Discussion: For the general level, extensive reference information was analyzed. Results: Some general processes favor the realization of the right to oral health, such as comprehensive protection policies for children, the dynamics of the health care system, and the existence of acquired rights for pregnant women, such as pre and post-natal, and maternity leave. The processes that disfavor its realization are the neoliberal development model, the conditions of poverty, gender inequity (income, employment/unemployment) and cultural factors such as the emphasis on female responsibility for care. Conclusions: The critical processes for the realization of the right to oral health act at the three levels of the theoretical model. There is an advance in public policies at the individual level. However, this does not result in an effective realization of the right at a singular level since it confronts structural elements of the general level, especially in the work and cultural field.

The jellyfish genera Stomolophus spp. is one of the most abundant in the Pacific Ocean, yet it has not been thoroughly studied. Until recently, research has been developed and directed to its knowledge because of the economic interest in its exploitation. The genus Stomolophus in the Pacific Ocean is composed of five species (S. agaricus, S. chunii, S. collaris, S. fritillaria, and S. meleagris), and Stomolophus sp. 2 has been recently reported in the central part of the Gulf of California. Therefore, this study aimed to describe in vivo the different developmental stages of Stomolophus sp. 2 life cycle. As a result, multiple polyp reproduction forms were described, such as polyp-stolon formation, polydisc strobilation with more than 20 ephyrae formed by each strobila, and polyp formation directly from juvenile ephyra. In the degenerating phase, the polyps turned into cysts induced by stress conditions, such as changes in temperature, oxygen, and food availability. The life cycle of Stomolophus sp. 2 can be distinguished from that of S. meleagris by showing various asexual reproduction mechanisms and polydisc-like strobilation. The formation of polyps directly from the ectoderm of degenerating juvenile medusae suggests the possibility of a reversion cycle. Because of the different life cycles between S. meleagris and S. sp. 2, in addition to their morphological and genetic differences, this study proposes that Stomolophus sp. 2 should be considered a new species and suggests the name Stomolophus yaquilli, in reference to the indigenous community that lives in the species distribution area.

BACKGROUND: Monitoring tobacco control policy implementation is one of the keys for tobacco consumption reduction in Latin America (LA). This study reports on the adaptation of the Tobacco Control Scale (TCS) for use in LA countries and the level of tobacco control policy implementation in this region according to the scale. METHODS: Ecological cross-sectional survey. The questionnaire to measure tobacco control policies was a translated (into Spanish and Portuguese) and adapted version of the last TCS as used in Europe. The resulting TCS-LA maintains the same structure as the original TCS, with eight policy domains and 100 points (pts) as maximum score; however, four policy domains were adapted because the exact same rationale could not be applied. At least two non-governmental tobacco control experts were contacted per country to answer the TCS-LA. RESULTS: Informants from 17 out of 18 countries completed the questionnaire. Using the TCS-LA, Panama (70 pts), Uruguay (68 pts) and Ecuador (61 pts) exhibited the strongest tobacco control policies, while Guatemala (32 pts), Bolivia (30 pts) and Dominican Republic (29 pts) have implemented a lower number of tobacco control policies. Eight countries reached 50% of the TCS-LA total possible score, indicating a relatively good implementation level of tobacco control policies. CONCLUSIONS: Panama, Uruguay and Ecuador are the tobacco control policy leaders in LA; however, tobacco control in the region has room for improvement since nine countries have a total score under 50 pts. The TCS is a feasible and adaptable tool to monitor tobacco control in other WHO regions beyond Europe.

BACKGROUND: Guillain-Barre Syndrome (GBS) is considered a complex disorder with significant environmental effect and genetic susceptibility. Genetic polymorphisms in CD1E, CD1A, IL-17, and/or ICAM1 had been proposed as susceptibility genetic variants for GBS mainly in Caucasian population. This study explores the association between selected polymorphisms in these genes and GBS susceptibility in confirmed GBS cases reported in mestizo population from northern Peru during the most recent GBS outbreak of May 2018. METHODS: A total of nine nonrelated cases and 11 controls were sequenced for the polymorphic regions of CD1A, CD1E, IL-17, and ICAM1. RESULTS: We found a significant protective association between heterozygous GA genotype in ICAM1 (241Gly/Arg) and GBS (p < .047). IL-17 was monomorphic in both controls and patients. No significant differences were found in the frequency of SNPs in CD1A and CD1E between the group with GBS patients and healthy controls. CONCLUSION: ICAM1 polymorphisms might be considered as potential genetic markers of GBS susceptibility. Further studies with larger sample size will be required to validate these findings.

The white spot syndrome virus (WSSV), the most lethal pathogen of shrimp, is a dsDNA virus with approximately a 300,000 base pairs and contains approximately 180-500 predicted open reading frames (ORFs), of which only 6% show homology to any known protein from other viruses or organisms. Although most of its ORFs encode enzymes for nucleotide metabolism, DNA replication, and protein modification, the WSSV uses some of its encoded proteins successfully to take control of the metabolism of the host and avoid immune responses. The contribution of the shrimp innate immune response to prevent viral invasions is recognized but yet not fully understood. Thus, the role of several components of Toll pathway of the shrimp Penaeus vannamei against WSSV has been previously described, and the consequential effects occurring through the cascade remain unknown. In the current study the effects of WSSV over various components of the shrimp Toll pathway were studied. The gene expression of Spätzle, Toll, Tube, Cactus and Dorsal was altered after 6-12 h post inoculation. The expression of LvToll3, LvCactus, LvDorsal, decreased ~4.4-, ~3.7- and ~7.3-fold at 48, 24 and 48 hpi, respectively. Furthermore, a remarkable reduction (~18-fold) in the expression of the gene encoding LvCactus in WSSV infected specimens was observed at 6 hpi. This may be a sophisticated strategy exploited by WSSV to evade the Toll-mediated immune action, and to promote its replication, thereby contributing to viral fitness.

RESUMO

This study describes the first record of the whale shark Rhincodon typus association with the cannonball jellyfish Stomolophus meleagris in the Gulf of California, Mexico. Whale sharks were observed swimming and feeding among swarms of jellyfish, suggesting competition and predatory behaviour given the overlap in food preferences between both species. This finding is relevant because of the species-wide distribution and the importance of these interactions, which should be considered in conservation strategies of R. typus and management of cannonball jellyfish fisheries.

OBJECTIVE: Characterize the implementation process, barriers, and facilitators of evidence-based recommendations in the context of developing clinical practice guidelines (CPGs) generated by the Ministry of Health of Chile, in order to make proposals to optimize the process. METHODS: Qualitative "action-oriented research" study. Nineteen semi-structured interviews were conducted and nine discussion groups were organized at various levels of the Chilean public health system. The analysis was conducted using Atlas.ti® software and manually, in a content analysis framework, by categorizing and coding information according to pre-specified dimensions and with the inclusion of emerging categories where relevant. RESULTS: The main challenge mentioned with regard to implementing recommendations is the lack of an explicit and structured process. Actors in the health system recognize difficulties specific to the context in which the recommendations are followed. In this unprecedented institutional review, participants suggested a series of strategies that could be implemented to overcome these challenges, presented in a management flow chart optimized for the development and implementation of CPGs. This process has raised awareness of the importance of implementing CPGs in Chile. CONCLUSION: After characterizing the implementation process, barriers, and facilitators, a plan to implement recommendations was developed in order to guide and monitor the process. It would facilitate the implementation of strategies and the introduction of improvements to the CPG development process if key informants inside and outside of the Ministry of Health were included in the review process. Studies of this kind should be conducted with physicians and patients in order to complement the collected information.

Métodos. Estudio cualitativo del tipo “investigación-acción”. Se realizaron 19 entrevistas semiestructuradas y se armaron nueve grupos de discusión a distintos niveles del sistema público de salud chileno. El análisis se realizó mediante el software Atlas ti® y en forma manual, desde un marco de análisis de contenido, mediante la categorización y codificación de la información según dimensiones preespecificadas y con la inclusión de categorías emergentes cuando fue pertinente. Resultados. El principal desafío de implementación de recomendaciones mencionado es la falta de un proceso explícito y estructurado. Los actores del sistema de salud reconocen dificultades dependientes del contexto al momento de usar las recomendaciones. En esta experiencia inédita de revisión institucional, los participantes sugirieron una serie de estrategias a poner en práctica para superar dichos desafíos, representadas en un flujograma de gestión optimizada para el desarrollo e implementación de GPC. El mismo proceso ha permitido tomar conciencia de la importancia de la implementación de GPC en Chile. Conclusión. Tras caracterizar el proceso de implementación, barreras y facilitadores se articuló un plan de implementación de recomendaciones que permitiría orientar y monitorizar dicho proceso. Hacer partícipes del proceso de revisión a informantes claves dentro y fuera del Ministerio de Salud facilitaría la implementación de estrategias y la introducción de mejoras al proceso de desarrollo de GPC. Estudios de este tipo deberían ser realizados en médicos y pacientes para complementar la información recogida.

Se presenta el caso de un paciente de sexo masculino, 29 años de edad, insuficiente renal crónico, con hiperparatiroidismo secundario y múltiples lesiones osteolíticas en las estructuras craneomaxilofaciales, diagnosticadas histológicamente como lesiones de células gigantes. La lesión de mayor tamaño afecta al maxilar superior, estructura raramente comprometida por este tipo de lesiones. Este tumor pardo produce gran deformidad facial y compromiso del piso orbitario, seno maxilar y fosa nasal correspondiente. No se evidenció regresión tumoral tras la paratiroidectomía y la normalización de los niveles de calcio séricos. Finalmente se realiza el remodelamiento quirúrgico del tumor pardo maxilar, con ausencia de recurrencia durante 7 años (AU)


A case is presented of a 29 year-old male patient suffering from chronic renal failure, secondary hyperparathyroidism and multiple osteolytic lesions in cranio-maxillo-facial structures, and with a histological diagnosis of giant cell lesions. The larger lesion affected the superior maxillary, a structure rarely compromised by this type of injury. This brown tumour caused a large facial deformity and compromised the orbital floor, maxillary sinus and the corresponding nostril. There was no evidence of tumour regression after parathyroidectomy and the serum calcium levels returned to normal. Finally, surgical remodeling of the maxillary brown tumour was performed, and no recurrence has been observed for 7 years (AU)

Objective: The objective of this study is to describe the oral health status and the factors associated with oral health-related quality of life (OHRQoL) in people aged 65 and older institutionalized in Barcelona in 2009. Study Desing: Cross sectional study in 194 elderly. The dependent variable was poor OHRQoL, according to the Geriatric Oral Health Assessment Index (GOHAI). The independent variables were socio-demographic data, last dental visit, subjective and objective oral health status. Robust Poisson regression analysis was used to determine the factors associated with OHRQoL as well as the strengths of association (Prevalence Ratios with respective confidence intervals at 95%). Results: According to GOHAI, 94 women (68.1%) and 36 men (64.3%) had poor OHRQoL. The average DMFT index (number of decayed, missing and filled teeth) was 22.8, with mean 10.2 remaining teeth. According to the Community Periodontal Index only 1.9% were healthy. 33.8% of the sample (35.5% of women and 30.4% of men) presented edentulism, 54.2% needed upper dental prostheses (51.1% of women and 60.7% of men) and 64.7% needed lower ones (61.6% of women and 71.4% of men). Only 7.2% had visited a dentist in the past year (8.8% of women and 3.6% of men). After fitting several multivariate adjusted robust Poisson regression models, poor OHRQoL was found to be associated to self-reporting problems with teeth or gums, self-reporting poor opinion about teeth/gums/denture and also associated to functional edentulism, needing upper denture, but not to sociodemographic factors or time since last dental visit (AU)

RESULTS: According to GOHAI, 94 women (68.1%) and 36 men (64.3%) had poor OHRQoL. The average DMFT index (number of decayed, missing and filled teeth) was 22.8, with mean 10.2 remaining teeth. According to the Community Periodontal Index only 1.9% were healthy. 33.8% of the sample (35.5% of women and 30.4% of men) presented edentulism, 54.2% needed upper dental prostheses (51.1% of women and 60.7% of men) and 64.7% needed lower ones (61.6% of women and 71.4% of men). Only 7.2% had visited a dentist in the past year (8.8% of women and 3.6% of men). After fitting several multivariate adjusted robust Poisson regression models, poor OHRQoL was found to be associated to self-reporting problems with teeth or gums, self-reporting poor opinion about teeth/gums/denture and also associated to functional edentulism, needing upper denture, but not to socio-demographic factors or time since last dental visit. CONCLUSIONS: The study population has poor objective oral health. A high percentage has poor OHRQoL associated to subjective and objective oral health conditions. Dental care is required and these services should be included in the Spanish National Health System.

Shrimp postlarvae were reared into different microcosm systems without water exchange; a traditional system based on simple fertilization to improve microalgae concentration (control), an autotrophic system (AS) based on the promotion of biofloc and biofilm by the addition of fertilizer and artificial substrates and a heterotrophic system (HS) based on the promotion of heterotrophic bacteria by the addition of nitrogenous and carbonaceous sources and artificial substrates. Better growth performance and survival were registered in shrimp from the AS and HS compared to the control. Feed conversion ratios were below 0.7 for all treatments, but AS and HS were significantly lower than the control. Regarding digestive performance, no significant differences were observed for trypsin, amylase and lipase activities among AS and control shrimp; however, shrimp from HS showed a higher trypsin and amylase activities, suggesting a higher digestive activity caused by the presence of microbial bioflocs. The presence of biofilm and bioflocs composed by either autotrophic or heterotrophic organisms in combination with formulated feed improved the growth performance and survival of shrimp. Apparently, such combination fits the nutritional requirements of shrimp.

El tumor odontogénico calcificante quístico (TOCQ) es una lesión poco frecuente. Ésta deriva de restos epiteliales odontogénicos presentes en los maxilares, mandíbula o encía.El TOCQ fue descrito por primera vez como una entidad patológica propia por Gorlin y col. en 1962. Clínicamente, el TOCQ corresponde al 1% de las lesiones odontogénicas. Se puede manifestar desde el primer año hasta la octava década de la vida. Afecta por igual los maxilares y la mandíbula, siendo más común en las zonas dentadas, con incidencia algo mayor en las zonas anteriores al primer molar.Se presentan 2 casos clínicos de TOCQ en dos etapas de la vida, ambos en el sexo femenino, uno a los 5 y el otro a los 35 años de edad. El tratamiento fue la enucleación del tumor.El objetivo de este artículo es presentar una revisión de la literatura a propósito de dos casos de TOCQ y su tratamiento,atendiendo a la bajísima incidencia de esta lesión, poniendo énfasis en su etiología, comportamiento biológico y tratamiento

RESUMO

Osteochondroma is the most common benign neoplasia of the skeleton. In the head it was been described in cranial base, posterior maxillary surface, maxillary sinus, and different mandibular areas like condyle, ramus, body and symphysis. Osteochondroma of the coronoid process are rare. We present a review of the literature and the report of the cas of a 44 years old female patient presenting limited mouth opening and swelling of the left cheek, with diffuse limits, bony consistency, painless, and covered of normal skin. No temporomandibular joint disease was present. In panoramic radiographs was evident a coronoid tumor localized in the union of zigomatic arch and bone. Under general anaesthesia coronoidectomy was made, recovering mouth opening until 43 mm. The post-operative period was performed without complications. Histopatological examinations revealed normal trabecular bone covered with hyaline cartilage. The histopatological diagnosis was osteochondroma. Clinical and pathological aspects, treatment and differential diagnosis with other lesions are discussed.

El osteocondroma es la neoplasia benigna más común del esqueleto. En la cabeza se ha descrito su localización en base de cráneo, cara posterior del maxilar, senos maxilares, y en diferentes áreas de la mandíbula, como cóndilo, rama, cuerpo y región sinfisiaria, siendo los osteocondromas coronoídeos de baja frecuencia. Presentamos una revisión de la literatura y el informe de un nuevo caso. Una mujer de 44 años que consulta por limitación de la apertura bucal y deformidad en la mejilla izquierda, de límites difusos, consistencia ósea, indolora y cubierta por piel de aspecto normal. No presentaba patología en la articulación témporomandibular. En la radiografía panorámica se evidencia un tumor coronoídeo localizado en la zona de unión del arco cigomático y el hueso malar. Se realizó la coronoidectomía bajo anestesia general, recuperándose inmediatamente la apertura bucal hasta 43 mm. El postoperatorio se desarrolló sin complicaciones. El estudio histopatológico reveló hueso esponjoso normal sobre el cual se encontró cartílago hialino. El diagnóstico fue osteocondroma. Los aspectos clínicos y patológicos, su tratamiento y diagnóstico diferencial son discutidos
