Fistulous communications between the abdomen and pleural space are rare. The Authors report a case of hydropneumothorax following transdiaphragmatic perforation of an anterior wall gastric ulcer.

OBJECTIVE: Describe a rare localization of spontaneous cerebrospinal fistula. CASE REPORT: A 75-year-old women was admitted for bacterial meningitis arising from a sinus infection. She had not experienced facial or head trauma. The CT-scan revealed pneumocephaly and a defect in the posterior part of the sphenoid sinus. DISCUSSION: Spontaneous cerebrospinal fistula is uncommon. The exact etiology remains unknown.

We report two cases of a choledochal cyst associated with dilatation of the cystic duct. This unusual variant of choledochal cyst was explored by ultrasonography and MR cholangiopancreatography.

Manubrio-sternal joint involvement in arthritis is described and may be evidenced by erosion, reactive sclerosis and ankylosis. Some reports of such involvement appear in the radiological literature in association with psoriatic arthritis and spondylarthropathy. Involvement of the manubrio-sternal joint in Behçet's disease is very uncommon. To our knowledge, only one case has been described. We report a case of manubrio-sternal arthritis in a women with Behçet's disease. Two others cases with suspicion of Behçet's disease are described.

Lipoblastoma is an uncommon benign soft tissue tumor arising from embryonal mesenchymal cells. It occurs mostly during early childhood with a predilection for the extremities, trunk, head and neck. This tumor tends to spread locally and no metastatic potential has been reported. Complete surgical resection is mandatory to prevent local recurrence. MRI provides excellent presurgical delineation of the tumor and confirms its fatty nature. Histology demonstrates the presence of lipoblasts in different stages of maturation; cytogenetic evaluation often discloses chromosomal anomalies of tumoral cells. A case of lipoblastoma of the buttock in a 10 month infant associated with anomalies of chromosome 8 is reported.

We report the case of a 27 years-old man with a left colo-colic intussuception due to a lipoma. CT scan allowed diagnosis of the obstruction, of the intussuception and showed his cause as an intraluminal fatty mass in the left colic angle, typical of lipoma. Resection of the mass was performed and mature fat cells were identified microscopically.

Ultrasound and CT in a young man, admitted for violent left flank pain, revelead a large heterogeneous left renal mass, with hypodense collection along the psoas. The diagnosis was spontaneous renal hematoma. A check CT 3 weeks later found a large tumor of the inferior pole. After radical nephrectomy, histopathology diagnosed a primitive neuroectodermal tumor. The patient died one year later despite neoadjuvant chemotherapy. This article reports the clinical, radiological and histopathological aspect of a primitive neuroectodermal tumor of the kidney, compared with the other cases in literature.

Osteosarcoma is a malignant bone tumour of mesenchymal osteoid tissue origin. Osteosarcoma can be primitive, but it may also complicated a Paget's disease of bone. We described a 75yearold man with a right thigh mass discovered after a motoring accident. Histopathologic examination revealed the diagnosis of osteosarcoma. Plain film Xrays showed Paget's lesion of the right pelvis bone. CT and MRI allowed complete evaluation of this lesion. Final diagnosis was those of malignant transformation of Paget disease. The patient was refused for surgery in view of the size of the tumour and chimiotherapy followed.

We report the changes in imaging findings over a 5 year period of a patient with unilateral malakoplakia involving renal parenchyma and pelvis. The imaging findings of malakoplakia will be reviewed.

Paraganglioma is a rare tumor arising from indifferentiated cells of the primitive neural crest. These tumors are most commonly found in the adrenal gland but other localisations are possible. We described a 60-year-old men with a right retro-peritoneal mass discovered on ultrasound examination. This tumor had an heterogeneous appearance on i.v. contrast injected CT (computed tomography) and MR (magnetic resonance) images. Plasma and urinary catecholamines were normal. This tumor was surgically removed. Immunohistochemical analyses revealed that the tumor cells were strongly positive for neurone specific enolase and chromogranin A. Histopathologic examination diagnosed a non secreting paraganglioma in the right retroperitoneum. After surgery, the patient remained asymptomatic without treatment.

OBJECTIVE: To evaluate the indications and practical modalities of prostatic ultrasonography in routine radiological practice. METHODS: An anonymous questionnaire composed of 18 multiple choice questions was sent to 122 radiologists. RESULTS: Among the 76 responses, 90% were men, 90% were in private practice and 94% regularly performed prostatic ultrasonography without clinical information (81%). The prostatic examination was performed by transrectal ultrasonography (60%) combined with vesical and renal ultrasonography. The indications were: urinary functional disorders (56%) and abnormal PSA (40%). Prostatic ultrasonography was considered to be useful to calculate prostatic volumes by 59% of radiologists and in the staging of prostatic cancer (81%). 80% of radiologists did not start by performing digital rectal examination. 59% of patients were informed. CONCLUSION: The observed indications did not correspond to urological consensus recommendations, but ultrasonography was performed according to classical recommendations. This study could justify a more precise definition of the role of ultrasonography in prostatic disease.
