RESUMO

Objective:To study the role of myocardial work parameters in early identification of myocardial injury in neonatal asphyxia.Methods:From July 2020 to December 2021, neonates diagnosed with mild neonatal asphyxia admitted to the Department of Neonatology of our hospital within 24 h after birth were prospectively enrolled into the asphyxia group. Neonates without asphyxia during the same period were selected as the control group and matched with the asphyxia group for gender, gestational age and birth weight at a ratio of 1:1~1:2. The asphyxia group was subgrouped into preterm asphyxia group and term asphyxia group. All neonates received echocardiography within 24 h after birth. Multiple parameters were measured including M-mode, two-dimensional image, Doppler image, global longitudinal strain (GLS) and myocardial work parameters [global work index (GWI), global constructive work (GCW), global wasted work (GWW), global work efficiency (GWE)]. The level of serum N-terminal pro brain natriuretic peptide (NT-proBNP) was recorded in the asphyxia group. The data were compared between the asphyxia group and the control group. Correlations between myocardial work parameters and other parameters were analyzed.Results:A total of 33 cases were in the asphyxia group and 43 cases were in the control group. The preterm asphyxia group (18 cases) showed significantly lower GWI and GCW than the preterm control group (18 cases) [GWI: (702±153) mmHg vs. (879±205) mmHg, GCW: (1 016±221) mmHg vs. (1 200±271) mmHg] ( P<0.05). No differences existed in GLS, GWW and GWE. The term asphyxia group (15 cases) showed significantly lower GWW than the term control group (25 cases) [45.0 (30.0, 65.0) mmHg vs. 71.0 (35.5,85.5) mmHg] ( P<0.05). No differences existed in GLS, GWI, GCW and GWE. GWI was negatively correlated with serum NT-proBNP level ( r=-0.327, P<0.05). Conclusions:GWI and GCW may indicate myocardial injury in preterm neonates with mild asphyxia.

OBJECTIVE: To observe the therapeutic effect of acupuncture at thirteen evil acupoints in patients with hepatic encephalopathy, and to explore its possible mechanism. METHODS: Patients with hepatic encephalopathy were randomly divided into acupuncture group (n=38) and western medicine group (n=36). Patients in the western medicine group were treated by intravenous injection of aspartate ornithine and branched chain amino acids, and those in the acupuncture group were treated with acupuncture at thirteen evil acupoints on the basis of the western medicine. All the patients were treated for 1 week. The liver function and blood ammonia were measured by automatic biochemical analyzer. The changes of plasma ß-endorphin were measured by enzyme-linked immunosorbent assay (ELISA). RESULTS: Following the treatment, of the 36 and 38 hepatic encep-halopathy patients in the western medicine and acupuncture groups, 12 and 18 experienced a marked improvement in their symptoms, 13 and 16 were effective, and 11 and 4 invalid, with the effective rates being 69.4% and 89.5%, respectively. Compared with pre-treatment, the levels of serum aspartate aminotransferase, alanine aminotransferase, total bilirubin, and plasma ß-endo-rphin and blood ammonia were significantly lower in both western medicine and acupuncture groups(P<0.01), and the therapeutic effects of the acupuncture group were obviously superior to those of the western medicine group in the above mentioned indexes (P<0.05, P<0.01). CONCLUSIONS: Treatment of acupuncture at thirteen evil acupoints combined with western medicine can enhance the curative effect of hepatic encephalopathy, improve patients' liver function and decrease the levels of plasma ammonia and ß-endorphin.

OBJECTIVE: To assess the clinical therapeutic effects of elastic intramedullary nail on extremity fractures in children. METHODS: From June 2005 to March 2008, 40 children with extremity fractures were treated by elastic intramedullary nail, in whom femoral shaft fractures occurred in 26 cases, tibiofibular fractures in 8 cases, radial capitular fractures in 4 cases, ulnoradial fractures in 2 cases. All patients were treated by closed reduction and elastic intramedullary nail fixation. RESULTS: All the fractures gained satisfactory reduction and healing. The average duration needed for fracture healing was 1-2 months. Postoperative follow-up confirmed a sound functional recovery. CONCLUSIONS: The elastic intramedullary nail is a minimally invasive and effective surgical approach for treatment of extremity fractures in children. It allows early functional exercises after operation and secures a satisfactory bone union and functional recovery.

During the noninvasive measurements of body blood glucose, the result will be effected by many factors, such as the measuring conditions including temperature, contact pressure and so on, and in addition the change of body's state also will induce some error. However, among so many factors the temperature is very important and should be discussed. To find the quantitative value of the result bias caused by temperature in the wavelength range from 1 100 to 1 700 nm, the aqueous glucose with the concentration ranging from 10 mg x dL(-1) to 200 mg x dL(-1) and 10 mg x dL(-1) interval was detected at temperature of 15, 20, 25, 30, 35 and 40 degrees C. Then six different models at different temperature were founded and predicted one another. The maximum RMSEP result of models is 11. 227 9 mg x dL(-1) and the minimum is 3. 298 8 mg x dL(-1). The correlation is about 0.98. The authors have also found that 1 degrees C change of temperature will induce deltac = 2.662 (mg x dL(-1) x degrees C(-1)) change of the prediction result, so these show that the detect error will be minimal when the temperature of measuring is the same as that of modeling. Moreover, the authors put forward two approaches to decreasing or compensating the error induced by the temperature.

RESUMO

Near-infrared spectroscopy is a fast and efficient analytical technique based on multivariate calibration model, which correlates near-infrared spectra with the property of samples (such as concentration). The reliability of analytical results depends mostly on the accuracy of measured spectra. But outliers do not make for reliable data. The authors combined RHM (Resampling by Half-Means) with SHV (Smallest Half-Volume) method to detect the outliers of the near-infrared spectra of milk samples, and the results were satisfactory. The performance of the new method is superior to the traditional outliers detecting algorithms such as Mahalanobis distances and hat matrix leverage. And this combined method is simple and fast to use, conceptually clear, and numerically stable, so it is recommended to be used for the detection of multiple outliers in multivariate data, especially the online measurement and discriminant analysis.
