RESUMO

We examined the spectral reflectance of fundus structures in the visible and near-infrared (400-1300 nm) range for contributing to the medical diagnosis of fundus diseases. Spectral images of healthy eye fundus and other ocular diseases were acquired using a novel multispectral fundus camera. Reflectance metrics were computed based on contrast to analyze the spectral features. Significant differences were observed among the structures in healthy and diseased eye fundus. Specifically, near-infrared analysis allows imaging of deeper layers, such as the choroid, which, to date, has not been retrieved using traditional color fundus cameras. Pathological structures, which were hardly observable in color fundus images owing to metamerism, were also revealed by the developed multispectral fundus camera.

The near point of convergence test is widely used to evaluate binocular vision. It assesses the ability of the eyes to converge at short distances. Although the test consists of a pure symmetrical vergence task, small involuntary saccades occur concurrently. The main goal of this study was to analyze saccadic characteristics as a function of vergence demand when testing the near point of convergence. To this purpose, the eye movements of 11 participants were registered with an eye-tracker while they performed the near point of convergence test by following a target that traveled forward and backward on a motorized bench. Saccade amplitude increased and, on average, saccade rate decreased with vergence demand. In general, the direction of the concurrent vergence movement had no significant effect on saccade characteristics. However, each individual subject showed idiosyncratic behavior. Most saccades tended to be corrective in terms of both binocular disparity and individual fixation position errors. In particular, most participants tended to correct the fixation position error of the dominant eye.

RESUMO

Whole eye visualization and morphometry are of high relevance in clinical practice. However, most standard ophthalmic OCT instruments are dedicated either to retinal or to anterior segment imaging. We demonstrate a swept source optical coherence tomography system (SS-OCT) that images both the whole anterior segment and the retina alternately using a single source and detector. A pilot population was imaged with the proof of concept prototype. We demonstrate the clinical potential of whole eye OCT screening for the description and early detection of relevant clinical features in the anterior segment and retina of several patients.

We present a multispectral fundus camera that performs fast imaging of the ocular posterior pole in the visible and near-infrared (400 to 1300 nm) wavelengths through 15 spectral bands, using a flashlight source made of light-emitting diodes, and CMOS and InGaAs cameras. We investigate the potential of this system for visualizing occult and overlapping structures of the retina in the unexplored wavelength range beyond 900 nm, in which radiation can penetrate deeper into the tissue. Reflectance values at each pixel are also retrieved from the acquired images in the analyzed spectral range. The available spectroscopic information and the visualization of retinal structures, specifically the choroidal vasculature and drusen-induced retinal pigment epithelium degeneration, which are hardly visible in conventional color fundus images, underline the clinical potential of this system as a new tool for ophthalmic diagnosis.

SIGNIFICANCE: The effect of predictability in changes of time, magnitude, and direction of the accommodation demand on the accommodation response latency and its magnitude are insignificant, which suggests that repetitive accommodative tasks such as the clinical accommodative facility test may not be influenced by potential anticipation effects. PURPOSE: The purpose of this study was to investigate the effect of stimulus' time, magnitude, and direction predictability, as well as their interactions, on accommodation latency and response magnitude. METHODS: Monocular accommodative response and latency were measured in 12 young subjects for nine different conditions where the stimulus accommodative demand changed several times in a steplike fashion for a period of 120 seconds. Each change in accommodative demand could have different time duration (i.e., 1, 2, or 3 seconds), magnitude (1, 2, or 3 diopters), and/or direction (i.e., accommodation or disaccommodation). All conditions were created permuting the factors of time, magnitude, and direction with two levels each: random and not random. The baseline condition was a step signal from 0 to 2 diopters persisting for 2 seconds in both accommodative demands. After each condition, subjects were asked to provide a score from 1 to 5 in their perceived predictability. RESULTS: Friedman test conducted on the perceived predictability of each condition resulted in statistically significant differences between the nine conditions (χ = 56.57, P < .01). However, repeated-measures analysis of variance applied to latency and accommodative response magnitude did not show significant differences (P > .05). In addition, no correlation was found between the perceived predictability scores and both latency and accommodative response magnitudes between the most predictable and the most unpredictable conditions. CONCLUSIONS: Subjects were able to perceptually notice whether the stimulus was predictable or not, although our results indicate no significant effect of stimuli predictability on either the accommodation latency or its magnitude.

PURPOSE: To assess forward intraocular scattering by means of a new parameter (Frequency Scatter Index, FSI3 ) based on the analysing double-pass retinal images in the frequency domain, which minimizes the impact of aberrations on the results. METHODS: A prospective observational study was carried out in the Department of Ophthalmology, Hospital Universitari Mútua de Terrassa (Spain) on a group of 19 patients diagnosed with nuclear cataracts of various LOCSIII grades and a control group (CG) with nine healthy eyes. We recorded double-pass retinal images with a custom set-up based on a high-sensitivity digital camera. The FSI3 was then computed using spatial frequencies below three cycles per degree. A preliminary validation of the FSI3 was performed on an artificial eye and two eyes of volunteers with and without commercial diffusers, and under defocus. RESULTS: The FSI3 was hardly affected by defocus values up to 2.50 D. The mean (and standard deviation) FSI3 values were as follows: for the CG, 1.19 (0.21); and for LOCSIII grades nuclear opacity 1, 2 and 3, 1.30 (0.12), 1.62 (0.21) and 1.85 (0.21), respectively. Eyes with cataracts showed FSI3 values significantly different than healthy eyes (p = 0.001). A good correlation (ρ = 0.861, p < 0.001) was found between the FSI3 and objective scatter index provided by a commercial instrument. CONCLUSION: Since aberrations have little impact on the FSI3 , the light scatter assessment becomes less dependent on the patient's refractive error compensation and the presence of higher-order aberrations. The FSI3 can further the clinical and scientific understanding of forward intraocular scattering.

Double-pass systems rely on backscattering of light by the human ocular fundus to assess the optical quality of the eye. In this work, we present a method to reduce double-pass image degradation caused by undesired multiple scattering effects in the eye fundus. The reduction is based on combined data processing of simultaneous measurements using two different configurations: one symmetric with equal entrance and exit pupils and another asymmetric with unequal entrance and exit pupils. Under certain conditions, such scattering effects may be effectively suppressed. Measurements of human eyes show that, although multiple fundus scattering imposes a shift on the estimations, double-pass systems can be used to predict the optical quality of the eye within a population.

Heterophoria is the relative deviation of the eyes in absence of fusional vergence. Fusional vergence can be deprived by, for example, occluding one eye while the other fixates a visual target. Then, the occluded eye will presumably deviate from its initial position by an amount that corresponds to the heterophoria. Its assessment in clinical practice is crucial for the diagnosis of non-strabismic binocular dysfunctions such as convergence insufficiency. Traditional clinical methods, like the cover test or the modified Thorington test, suffer from practitioner's subjectivity, impossibility to observe the occluding eye or unusual viewing conditions. These limitations could be overcome by using eye tracking systems to measure objectively the heterophoria. The main purpose of this study was to compare the performance of an automated and objective method to measure near heterophoria using an eye-tracker with two conventional methods: the cover-uncover test and the modified Thorington test. The eye tracking method gave us the possibility to measure the heterophoria as the deviation of the occluded eye (mimicking the cover test) or as the deviations of the occluded and fixating eyes (adhering to the theoretical definition of heterophoria). The latter method provided smaller results than the former, although on average the differences might not be clinically relevant. The proposed objective method exhibited considerably better repeatability than the two conventional clinical methods. It showed better agreement with the modified Thorington test than with the cover-uncover test, and a similar level of agreement was obtained between the two clinical methods. To conclude, the use of eye-trackers to measure heterophoria provides objective and more repeatable measures. As eye-trackers become common tools in clinical settings, their use to measure heterophoria should be the new gold standard.

Current methods to measure intraocular scattering provide information on the total scattered light, which consists of the combined contributions originating from different ocular structures. In this work, we propose a technique for the objective and independent assessment of scattering caused by the cornea and the lens based on the analysis of the contrast of the third and fourth Purkinje images. The technique is preliminarily validated first by using artificial eyes with different levels of corneal and lens scattering; second, it is validated in eyes wearing customized contact lenses to simulate corneal scattering and eyes with nuclear cataracts. Finally, it is tested on a larger population of eyes with cataracts and corneal disorders to prove its clinical usefulness.

PURPOSE: To study the accommodative dynamics for predictable and unpredictable stimuli using manual and automated accommodative facility tests Materials and Methods: Seventeen young healthy subjects were tested monocularly in two consecutive sessions, using five different conditions. Two conditions replicated the conventional monocular accommodative facility tests for far and near distances, performed with manually held flippers. The other three conditions were automated and conducted using an electro-optical system and open-field autorefractor. Two of the three automated conditions replicated the predictable manual accommodative facility tests. The last automated condition was a hybrid approach using a novel method whereby far and near-accommodative-facility tests were randomly integrated into a single test of four unpredictable accommodative demands. RESULTS: The within-subject standard deviations for far- and near-distance-accommodative reversals were (±1,±1) cycles per minute (cpm) for the manual flipper accommodative facility conditions and (±3, ±4) cpm for the automated conditions. The 95% limits of agreement between the manual and the automated conditions for far and near distances were poor: (-18, 12) and (-15, 3). During the hybrid unpredictable condition, the response time and accommodative response parameters were significantly (p < 0.05) larger for accommodation than disaccommodation responses for high accommodative demands only. The response times during the transitions 0.17/2.17 D and 0.50/4.50 D appeared to be indistinguishable between the hybrid unpredictable and the conventional predictable automated tests. CONCLUSIONS: The automated accommodative facility test does not agree with the manual flipper test results. Operator delays in flipping the lens may account for these differences. This novel test, using unpredictable stimuli, provides a more comprehensive examination of accommodative dynamics than conventional manual accommodative facility tests. Unexpectedly, the unpredictability of the stimulus did not to affect accommodation dynamics. Further studies are needed to evaluate the sensitivity of this novel hybrid technique on individuals with accommodative anomalies.

Double-pass (DP) and Hartmann-Shack (HS) are complementary techniques based on reflections of light in the ocular fundus that may be used to estimate the optical properties of the human eye. Under conventional data processing, both of these assessment modes provide information on aberrations. In addition, DP data contain the effects of scattering. In the ocular fundus, this phenomenon may arise from the interaction of light with not only the retina, but also deeper layers up to which certain wavelengths may penetrate. In this work, we estimate the response of the ocular fundus to incident light by fitting the deviations between DP and HS estimations using an exponential model. In measurements with negligible intraocular scattering, such differences may be related to the lateral spreading of light that occurs in the ocular fundus due to the diffusive properties of the media at the working wavelength. The proposed model was applied in young healthy eyes to evaluate the performance of scattering in such a population. Besides giving a parameter with information on the ocular fundus, the model contributes to the understanding of the differences between DP and HS estimations.

RESUMO

We describe a new wavefront sensor for ocular aberration determination, based on the curvature sensing principle, which adapts the classical system used in astronomy for the living eye's measurements. The actual experimental setup is presented and designed following a process guided by computer simulations to adjust the design parameters for optimal performance. We present results for artificial and real young eyes, compared with the Hartmann-Shack estimations. Both methods show a similar performance for these cases. This system will allow for the measurement of higher order aberrations than the currently used wavefront sensors in situations in which they are supposed to be significant, such as postsurgery eyes.

PURPOSE: Wavefront sensors provide quite useful information on the optical quality of the eye. However, in eyes where very high-order aberrations and scattered light are prominent, wavefront sensors may overestimate retinal image quality. This study showed that, in those cases, the double-pass technique is a complementary tool for better estimation of ocular optical quality. METHODS: A double-pass (DP) instrument was used, based on recording images of a point source in near-infrared light after reflection in the retina and double-pass through the ocular media. The aberrations were also measured with a prototype of near-infrared Hartmann-Shack (HS) wavefront sensor adapted to the clinical environment. From the wave aberrations, the modulation transfer function (MTF) was calculated (MTF_HS). The MTF was also obtained from the double-pass images (MTF_DP). Both techniques were applied in normal young subjects as the control and in three other groups of eyes: older subjects, after LASIK refractive surgery, and after IOL implantation. RESULTS: The MTFs obtained from DP and HS techniques were compared. In the group of normal eyes with low levels of intraocular scattering, these estimates were quite similar, indicating that both techniques captured well most of the optical degradation. However, in eyes where scatter was more predominant (e.g., early cataract, posterior capsular opacification after IOL implantation) the MTF provided by the HS sensor was always higher than the MTF obtained from DP. A single parameter was used to indicate the differences. CONCLUSIONS: In eyes with low scattering, DP and HS techniques provided similar estimates of the retinal image quality. However, in a patient's eye with mild to severe amount of scatter, wavefront sensors might overestimate image quality, whereas the DP technique produces a more accurate description of the optical quality, better correlated with the quality of vision.
