The experiment was based on observations of 120 juveniles of tilapias of Nile, Oreochromis niloticus, sexually reverted, with an initial medium weight of 116.65 0.85 g, distributed in a completely randomized design for 168 days. The objective of this work was to evaluate the effect of introducing tank-nets on the function Taguchi's loss fed with 0% (control), 20%, 40% and 60% (tests) of yeast distillery protein in substitution of traditional source of protein. The test of Tukey was accomplished showing that there were not statistical differences (p > 0.05) among the effi ciency, the length and the weight of the fi sh. With relationship to the survival, it was observed that the treatments (T2, T3, T4) were statistically superior (p < 0.01) to the treatment T1. Taguchi's loss for the cost of production and for the dead fi shes in tank-nets in the treatment T2 = 20%, T3 = 40% and T4 = 60% (tests) were lower the group control (T1 = 0%). This result confi rm the direction of yeast distillery protein in substitution of traditional source of protein.


Foram utilizados 120 juvenis de tilápia do Nilo, Oreochromis niloticus, sexualmente revertidos, com peso médio inicial de 116,65 ± 0,85 g, distribuídos num delineamento inteiramente casualizado, durante 168 dias. O objetivo foi avaliar o efeito da introdução de tanques-rede com cultivo de peixes alimentados com 0% (grupo padrão), 20%, 40% e 60% (grupos testes) de proteína oriunda de levedura de destilaria em substituição à proteína de fontes tradicionais sobre a função perda de Taguchi. O teste de Tukey foi realizado mostrando que não houve diferenças estatísticas (p > 0,05) entre a efi ciência, o comprimento e o peso dos peixes. Com relação à sobrevivência, foi observado que os tratamentos (T2 , T3 , T4 ) foram estatisticamente superiores (p < 0,01) ao tratamento T1 . As perdas de Taguchi para o custo de produção e para os peixes mortos nos tanques para os tratamentos T2 = 20%, T3 = 40% e T4 = 60% , foram inferiores ao grupo controle (T1 = 0%). Este resultado confi rma o direcionamento da proteína oriunda de levedura de destilaria em substituição à proteína de fontes tradicionais.


Fueron utilizados 120 joveniles de tilapia del Nilo (Oreochromis niloticus), sexualmente revertidas con peso mediano inicial de 116,65 mas o menos 0,85g distribuido en un diseno totalmente seleccionado al azar por 168 dias. El objetivo de este trabajo fue evaluar el efecto de la sustitución de 0% (control), 20%, 40% y 60% (grupo teste) de proteína oriunda de levadura de destilería en sustitución a la proteína de fuentes tradicionales sobre la función pérdida de Taguchi. La prueba de Tukey era demonstración lograda que no había diferencias estadísticas (p>0,05) entre la eficacia, la longitud y el peso de los pescados. Con relación a la supervivencia, fue observado que los tratamientos (T2, T3, T4) eran estatisticamente superior (p<0,01) ao T1 del tratamiento. Las pérdidas de Taguchi para el coste de producción y para los peces muertos en los estanques-red para los tratamientos T2 = 20%, T3 = 40% y T4=60%, fueron inferiores al grupo control (T1 = 0%). Este resultado confirma el direccionameiento de la proteína oriunda de la levadura de destilería en sustitución a la proteína de fuentes tradicionales.

OBJECTIVES: Ageing of the surgical population and the evolution in anaesthetic techniques have led the Club d'anesthésie-réanimation et techniques en chirurgie cardiaque (ARTECC) to conduct a survey among French cardiac surgery centers. The aim was to profile patient population undergoing cardiac surgery and perioperative techniques employed. STUDY: National prospective study including all adult patients undergoing cardiac surgery on January 23rd, 24th and 25th, 2001. Data were collected during the first 48 postoperative hours. MATERIAL AND METHODS: Seven referent centers drafted a record form. Sixty-one centers sent back 425 patient forms, 399 were analyzed. The following were statistically studied: type of surgery, patient characteristics, preoperative treatment, monitoring, anaesthesia, cardio-pulmonary bypass (CPB) characteristics, duration of mechanical ventilation, length of stay in intensive care unit, postoperative complications. RESULTS: Patient mean age was 64.3 +/- 13.3 years. Patients over 80-year-old represented 2.5% of the population. Beating heart coronary aortic bypass grafts (13% surgery) and preoperative transoesophagal echography were not frequent. Propofol and sufentanil were the main anaesthetic agents used. There was a marked trend for fast-track procedures. CONCLUSIONS: The ARTECC study pointed out some reserve in practices and that the impact of new techniques seems limited. Regular use of studies of that kind will provide an effective tool to compare national practices.

We report a single-case study of peripherally acquired dyslexia that meets the clinical criteria of "alexia without agraphia." The patient, AA, has a large infarct involving the left posterior cerebral artery. The most striking feature is a severe impairment in recognizing single visually presented letters that precludes explicit or implicit access to reading, even in a letter-by-letter fashion. AA can, however, differentiate letters from similar nonsense characters and digits, and he is also able to identify alphanumeric signs when the visual channel is bypassed (through somesthesic or kinesthesic presentation). Spelling tasks are also well performed. Since there is a breakdown in mapping a visually presented letter to its abstract graphemic representation, we propose the term "visuographemic alexia" for this kind of reading disorder. The pattern of deficits is interpreted following theoretical models previously developed in cognitive neuropsychology. An alexia for arabic numerals with preserved comprehension lends additional support for the crucial processing of different notational systems (e.g., phonographic vs logographic). More general perceptive disorders do not seem to account for these patterns; they are material-specific. Finally, we attempt to specify functional correlations with the implied neural networks.

RESUMO

Aneurysms of the infra-renal abdominal aorta or iliac arteries result in ilio-caval compression in about 10% of cases which may cause venous thrombosis by stasis and pulmonary embolism. Fistulisation of these aneurysms into the inferior vena cava or an iliac vein is rare and paradoxical pulmonary embolism from arterial thrombus of the aneurysmal pouch is exceptionally rare. The authors report a new case in which the ilio-iliac arteriovenous fistula caused high output cardiac failure, ischaemia of the homolateral leg and pulmonary embolism. Doppler ultrasonography diagnosed the fistula and excluded a deep vein thrombosis. This case illustrated the essential value of clinical examination and of Doppler ultrasonography, especially of the abdomen, in the investigation of the causes of pulmonary embolism.

BACKGROUND AND AIMS OF THE STUDY: Air embolism during open heart surgery seems to be a common occurrence and may be responsible for neuropsychological deficit or myocardial damage. MATERIAL AND METHODS: Forty-two consecutive patients undergoing valvular surgery were studied using the long axis view of the heart by two dimensional transesophageal echocardiography (TEE). The patients were randomized into two groups of 21 each. In group 1, the routine air evacuation method was used. In group 2, the same air evacuation method was used and controlled with a Doppler ultrasonic probe adjusted around the root of the aorta. At the end of air evacuation, intracardiac microbubbles and retained air were analyzed with TEE and when air was founded, its location was communicated to the surgeons who tried to remove it by shaking the heart and tilting the operating table for 15 minutes. The patients were assessed for detection of cardiac or neurological postoperative complications. RESULTS: The incidences of microbubbles and retained air were 57% and 43% in group 1, and 62% and 38% in group 2 respectively (ns). The mean grade of microbubbles was lower in group 2: 1.4 +/- 0.8 vs. 2.2 +/- 0.9, p < 0.05. TEE allowed to significantly decrease (p < 0.05) retained air and mean grade of microbubbles to 14% and 1.3 +/- 0.8 in group 1, and to 10% and 0.8 +/- 0.8 in group 2, without statistical difference between the two groups. Despite the help of TEE, manual attempts to eradicate retained air were unsuccessful in five patients (three in group 1, two in group 2). CONCLUSIONS: The use of aortic ultrasonic probe allowed to reduce the amount of microbubbles. TEE was a useful tool not only for the detection of retained air but also for locating it, and guiding the procedure to eliminate it.

RESUMO

Urapidil is a vasodilator acting on peripheral vessels by alpha-1 adrenoceptor blockade, and on central nervous system by alphaadrenoceptor blockade on the tractus solitarius nuclei and stimulation of serotoninergic receptors. Its hepatic metabolism and central action explain the absence of rebound effect when treatment is stopped. Urapidil is an arterial and venous vasodilator in systemic and pulmonary circulation. Therefore urapidil decreases systemic and pulmonary arterial pressures and left and right ventricular preload. These effects are accompanied by moderate effects on heart rate, and increased cardiac output in patients with heart failure. Some adverse effects observed with other vasodilators are absent (increased intracranial pressure and intrapulmonary shunt, negative inotropic effect, tachycardia). IV urapidil is used to treat systemic arterial hypertension perioperatively and hypertensive emergencies. Moreover urapidil has been administered to treat pulmonary artery hypertension in chronic pulmonary disease. Its haemodynamic effects seem proportional to the basal activity of alpha-adrenoceptors. Therefore, wide individual variations in active doses are observed from 12.5 to 50 mg as i.v. bolus in adults. Thereafter i.v. continuous administration or oral treatment may be necessary, depending on the cause of hypertension.

RESUMO

A case of left pulmonary artery sling and tracheobronchial tree malformation is reported in a 3 month-old infant. Acute respiratory presenting symptoms required mechanical ventilatory support. The malformation was suspected because of ventilatory support difficulties. Barium oesophagogram showed an anterior indentation, fiberoptic bronchoscopy and tracheobronchography showed a severe tracheal stenosis from a right bronchus lobaris superior to the carina. This was worsened by the right side compression of trachea end part, due to the abnormal left pulmonary artery as demonstrated by pulmonary angiography. A surgical left artery transposition relieved lateral compression and allowed weaning of ventilatory support. After a 27 month-follow-up, the girl's respiratory status is satisfactory.

The authors report the case of a 42 years-old electrician man, with an 8 months progressive manual difficulties in his specific job, with a complete disability for the last 2 months. These symptoms were associated with anemia, deterioration of the general condition, and slight fever. Three days after the first neuropsychological examination he, suddenly, developed a left hemiparesis. Laboratory investigations confirmed the occurrence of a systemic lupus erythematosus. The neuropsychological examination showed: normal intellectual level with slight memory deficit; severe constructional and ideomotor apraxia; gaze ataxia; optical ataxia; hemisomatoagnosia, anosognosia, and autotopagnosia; agraphia. The authors discuss this particular syndromatic association, remarking the alterations of oculomotor control, the optical ataxia and the role which both may have in the disorganization of gesture activity, concerning also the gnosics disturbances. Furthermore, agraphia is stressed as well as its relation with apraxic disturbances. On the basis of the semiological analysis, the authors come to the conclusion of a bilateral temporo-parieto-occipital involvement.

Two cases of diffuse squamous cell carcinoma of the oesophagus simulating the roentgenographic appearance of oesophageal varices are presented. Careful fluoroscopic examination may permit accurate diagnosis; however if the lesions are equivocally demonstrated by X-ray examination, oesophagoscopy confirms the diagnosis of carcinoma. R94.
