RESUMO

We present the recommendations of the Preventive Activities and Health Promotion Programme (PAPPS) of the semFYC (Spanish Society of Family and Community Medicine) to promote healthy lifestyles using intervention methodology, and preventive actions against tobacco and alcohol use, healthy eating, physical activity in leisure time, prevention of traffic accidents, and child restraint systems. The recommendations have been updated, and new aspects highlighted, such as the definition of low-risk alcohol consumption, and the references have been updated. For the main recommendations, we include specific tables showing the quality of the evidence and the strength of the recommendation.

INTRODUCTION: Breast milk is the optimal food during the first months of life. The main objective was to analyse the factors associated with not initiating breastfeeding in terms of maternal lifestyles, epidemiological characteristics and information on breastfeeding. MATERIAL AND METHOD: Observational analytical study of cases and controls in 6 primary care centres. The sample included women with a child under 5 years, with an estimated necessary sample size of 166 cases (women who did not initiate breastfeeding) and 166 controls (women who began breastfeeding). Sociodemographic and maternal lifestyle variables were measured before and during pregnancy, such as physical activity using the BPAAT questionnaire, diet with the MEDAS-14 questionnaire, and tobacco and alcohol consumption. We assessed the information and opinions about breastfeeding as well as previous experience with it. RESULTS: The study included 348 women (174 cases and 174 controls) with a mean age of 33.4 years (SD, 5.4). The variables independently associated with not initiating breastfeeding were: absence of previous experience with breastfeeding (odds ratio [OR], 12.75), making the decision during pregnancy or delivery (OR, 10.55), not being married or in a partnership (OR, 3.42) and being sedentary for periods of 2 hours or greater/day during pregnancy (OR, 1.77). CONCLUSIONS: In our study, the determining factors associated with not initiating breastfeeding were the lack of previous experience with breastfeeding, the timing of the decision about breastfeeding, and marital status. When it came to lifestyle, only a sedentary lifestyle was associated with not initiating breastfeeding, while dietary or other habits had no influence.

RESUMO

Primary and secondary health determinants explain a large part of the morbidity and mortality observed in primary care. The recommendations of the Program of Preventive Activities and Health Promotion (PAPPS) of the semFyC are presented, for the promotion of a healthy lifestyle through intervention methodology and preventive actions in tobacco consumption, alcohol consumption, healthy eating, physical activity in free time and prevention of traffic accidents and child restraint systems. The most common clinical prevention guidelines are outlined. The recommendations are updated, new aspects are pointed out, such as the definition of low-risk alcohol consumption, and the bibliography is updated. For the main recommendations, specific tables are included that show the quality of the evidence and the strength of the recommendation.

OBJECTIVES: To know the pneumococcal vaccination coverage in patients≥65 years old, as well as the risk of pneumococcal disease according to whether or not they received such vaccination. DESIGN: Cross-sectional study, followed by historical cohort. LOCATION: Urban area. PARTICIPANTS: By systematic sampling, 2,805 people≥65 years from the city of Albacete were selected. MAIN MEASUREMENTS: Dependent variable: diagnosis of pneumococcal disease and date. INDEPENDENT VARIABLES: age, sex, chronic pathologies, medication, pneumococcal vaccination and date. The computerized medical records were reviewed, from 1-1-2009 to October-December 2015. A descriptive analysis was carried out, the relative risk of the onset of pneumococcal disease according to vaccination has been calculated, and survival analysis has been carried out, with the statistical program SPSS 17.0. RESULTS: Median age 71 years; 57.2% were women; 46% received polysaccharide vaccine (95% CI 44.1-47.8). Only 10 people received conjugated vaccine. Twenty-two people were diagnosed with invasive pneumococcal disease and 153 non-invasive. The relative risk of pneumococcal disease in vaccinated versus unvaccinated, respectively for invasive and non-invasive, was 1.59 (95% CI 0.69-3.68) and 1.84 (95% CI 1.33-2.54). Cox regression showed a higher risk of non-invasive disease for COPD (1.95; 95% CI 1.32-2.89), smoking (1.87; 95% CI 1.28-2.73), corticoid-therapy (1.73; 95% CI 1.08-2.79), polysaccharide vaccination (141.41; 95% CI 5.92-3,378.49) and age (1.11; 95% CI 1.08-1.14), with interaction between these 2 (0.94, 95% CI 0.91-0.98). CONCLUSIONS: There is an increased risk of pneumococcal disease in patients≥65 years vaccinated with polysaccharide, although with a protective effect in vaccinated older.

Objetivo: Determinar conocimientos y actitudes hacia la violencia de género de la población consultante en Atención Primaria y su relación con factores sociodemográficos y experiencias personales.DiseñoEstudio descriptivo transversal.EmplazamientoCentros de Salud urbanos.ParticipantesPacientes ≥ 18 años que acuden a consulta de Atención Primaria.Mediciones principalesSe utilizó un cuestionario que incluía preguntas relativas a conocimientos, actitudes y experiencias vividas en relación con la violencia de género y en el ámbito doméstico, y que también recogía edad, sexo, nivel de estudios, estado civil y detección de experiencias personales de violencia de género mediante la versión corta del Woman Abuse Screening Tool (WAST).ResultadosRespondieron 673 personas, de 18-86 años, con un 68% de mujeres. Solo un 18,2% tenía un adecuado conocimiento sobre quién se considera que ejerce la violencia de género. La mitad de los participantes consideraban que la violencia de género incluía lesiones físicas, psicológicas, cohibición de libertad y violación. A través de un análisis de regresión logística se encontró una asociación independiente con el conocimiento de la respuesta correcta sobre qué es la violencia de género del estado civil, siendo menos probable en los casados con respecto a los viudos (OR: 0,28; IC 95%: 0,11-0,72) considerar que la violencia de género implica lesión física (OR: 2,55; IC 95%: 1,28-5,08), pero no lesiones psicológicas (OR: 0,52; IC 95%: 0,28-0,96), y no dar la respuesta correcta sobre qué es la violencia doméstica (OR: 0,06; IC 95%: 0,03-0,12).ConclusionesExiste una gran dispersión en las respuestas con relación a qué consideran los pacientes que es la violencia de género y qué aspectos abarca(AU)
