RESUMO

Patients with chronic kidney disease who are candidates for transplant may experience changes in capillary permeability, coagulation, and the endocrine system; alterations in the pulmonary vasculature; and cardiac structural and functional changes. Patients with renal replacement by hemodialysis have a mortality rate 30 times higher than those who do not have uremia. According to the onset, duration, and severity of chronic kidney disease, cardiovascular disease will be reflected in baseline function and response to stress. For this reason, it is important to establish the functional condition so that preoperative management can be started and factors inherent to the patient and the surgical procedure can be modified. Here, we report a 29-year-old male patient with chronic kidney disease secondary to renal hypoplasia, with no family history. The patient had renal replacement therapy with peritoneal dialysis and had progressive deterioration of general status, decreased functional capacity, decreased tolerance to physical activity, presence of fatigue and pulmonary congestion, and retention of liquids, with a diagnosis of moderate to severe mitral regurgitation.

Kidney transplantation continues to be the treatment of choice in patients with end-stage chronic kidney disease. Unfortunately, most patients in the adult population have coexisting diseases. Anemia, platelet dysfunction, and changes to the cardiovascular system are not the exception in the evolution of the disease. This group of patients has an increased risk of intraoperative and postoperative complications associated with the general conditions of their body and other complications related to the surgical procedure. For this reason, adequate control of fluids and electrolytes under adequate monitoring and hemodynamic support are essential; these can optimize the patient's vital signs and improve the conditions of the implant to guarantee proper functioning. In this series, 18 kidney transplant recipients with peripheral insertion central venous catheter were reviewed. There was an acceptable rate of success with proper placement. No complications inherent to the installation technique were found. In all patients, the installation was successful.

Chronic kidney disease is defined as irreversible and progressive damage of the kidney. Chronicity is defined by the presence of renal dysfunction for at least 3 months, and renal dysfunction is defined through combinations of investigation (abnormal radiologic findings, abnormal urine or abnormal biochemistry reflecting renal dysfunction) and/or documentation of glomerular filtration rate below 60 mL/min/1.73 m². The case patient was a girl of 11 years of age, with diagnosis of chronic renal disease, of unknown cause, under renal replacement therapy with peritoneal dialysis, with progressive deterioration of general status and decrease of functional capacity and tolerance to physical activity, presence of fatigue, pulmonary congestion, retention of liquids, and edema in lower extremities, even with adjustment of medical treatment. Transplant was performed from a related living donor, without incidents or complications. The results of pediatric kidney trans plant are excellent, offering a high quality of life for recipients; many patients return to school. It is consi dered the criterion standard for the treatment of pediatric endstage renal disease with excellent allograft function and subsequent resolution of systolic dysfunction.
