AIMS: The rapid endothelialization of bare metal stents (BMS) is counterbalanced by inflammation-induced neointimal growth. Drug-eluting stents (DES) prevent leukocyte activation but impair endothelialization, delaying effective device integration into arterial walls. Previously, we have shown that engaging the vascular CD31 co-receptor is crucial for endothelial and leukocyte homeostasis and arterial healing. Furthermore, we have shown that a soluble synthetic peptide (known as P8RI) acts like a CD31 agonist. The aim of this study was to evaluate the effect of CD31-mimetic metal stent coating on the in vitro adherence of endothelial cells (ECs) and blood elements and the in vivo strut coverage and neointimal growth. METHODS AND RESULTS: We produced Cobalt Chromium discs and stents coated with a CD31-mimetic peptide through two procedures, plasma amination or dip-coating, both yielding comparable results. We found that CD31-mimetic discs significantly reduced the extent of primary human coronary artery EC and blood platelet/leukocyte activation in vitro. In vivo, CD31-mimetic stent properties were compared with those of DES and BMS by coronarography and microscopy at 7 and 28 days post-implantation in pig coronary arteries (n = 9 stents/group/timepoint). Seven days post-implantation, only CD31-mimetic struts were fully endothelialized with no activated platelets/leukocytes. At day 28, neointima development over CD31-mimetic stents was significantly reduced compared to BMS, appearing as a normal arterial media with the absence of thrombosis contrary to DES. CONCLUSION: CD31-mimetic coating favours vascular homeostasis and arterial wall healing, preventing in-stent stenosis and thrombosis. Hence, such coatings seem to improve the metal stent biocompatibility.

Stents are cardiovascular implants deployed on atherosclerotic arteries that aid in reopening, sustaining, and avoiding their collapse. Nevertheless, postimplantation complications exist, and the risk of the renewal of the plaque subsists. Therefore, enhanced properties are mandatory requirements for clinics. For that purpose, a novel approach allowing the direct-grafting of bioactive molecules on cobalt-chromium devices (L605) has been developed. This original strategy involves the direct plasma functionalization of metallic surfaces with primary amines (-NH2). These groups act as anchor points to covalently graft biomolecules of interest, herein a peptide derived from CD31 (P23) with proendothelialization and antithrombotic properties. However, the biological activity of the grafted peptide could be impacted by its conformation. For this study, glutaric anhydride (GA), a short chain spacer, and polyethylene glycol (PEG) with antifouling properties were used as linking arms (LAs). The covalent grafting of the CD31 agonist on L605 by different LAs (GA-P23 and PEG-P23) was confirmed by XPS and ToF-SIMS analyses. The biological performance of these functionalized surfaces showed that, compared to the electropolished (EP) alloy, grafting the P23 with both LA increases adhesion and proliferation of endothelial cells (ECs) since day 1: EP = 68 ± 10%, GA-P23 = 101 ± 7%, and PEG-P23 = 106 ± 5% of cell viability. Moreover, ECs formed a complete monolayer at the surface, preventing clot formation (hemoglobin-free >80%). The potential of this plasma-based strategy for cardiovascular applications was confirmed by promoting a fast re-endothelialization, by improving the hemocompatibility of the alloy when coupled with the CD31 agonist and by its transfer onto commercial L605 stents, as confirmed by ToF-SIMS.

Stents are cardiovascular devices used to treat atherosclerosis, and are deployed into narrowed arteries and implanted by expansion to reopen the biological lumen. Nevertheless, complications after implantation are still observed in 10-14% of the implantations. Therefore, functionalizing these devices with active molecules to improve the interfacial effects with the surrounding tissue strongly impacts their success. A plasma-based procedure to directly graft biomolecules to the surface of cobalt chromium alloys, without any polymeric coating, has been recently reported. Assuring the stability of the coating during plastic deformation generated during the implantation whilst avoiding the corrosion of the surface is crucial. This study explores different surface treatments to be used as a pre-treatment for this novel procedure. The effects of (i) electropolishing, (ii) thermal treatments, and (iii) the plasma immersion ion implantation of oxygen on the chemical composition, roughness, wettability and efficiency during the plasma-amination procedure whilst avoiding cracks after deformation, thus maintaining corrosion resistant behaviour, were investigated by XPS, AFM, ToF-SIMS imaging and depth profile, and WCA. Furthermore, the hemocompatibility of the surface and cell viability assays were also performed. Results showed that all of the treatments created a different surface chemical composition: EP mainly of chromium oxide, PIII with a layer of cobalt oxide and TT with a mixture of oxides, as observed by XPS and ToF-SIMS. Moreover, EP was the process that generated a surface with the highest efficiency to amination and the most corrosion resistance among the treatments, and it appeared as the most suitable pre-treatment for stent functionalization.

INTRODUCTION: Salmonellosis, a zoonotic and foodborne disease, is a public health problem in developing countries. With the aim of identifying human carriers of Salmonella, a survey was performed in five regions of Colombia with reported salmonellosis outbreaks. METHODOLOGY: The general population and cholecystectomy surgical patients were included in this study. Stool samples from 667 volunteers and gallbladder bile samples from 199 surgical patients were examined. Detection of Salmonella from cultured stool and bile samples was determined by polymerase chain reaction (PCR). Multiplex PCR and biochemical and serological tests were performed to identify the serovars of the isolates. RESULTS: Nine (1.35%) stool samples were positive for Salmonella: two S. Newport, two S. Anatum, one S. Sinstorf, and four Salmonella spp. A total of 11 gallbladder bile samples were positive: S. Enteritidis was isolated from 3 bile cultures (1.5%), and 8 samples (4%) were positive for Salmonella spp. CONCLUSIONS: Our results show the presence of Salmonella carriers in the inhabitants of regions with reported outbreaks and suggest that these carriers are potential sources of infection in endemic and epidemic cases. Carriers also suggest Salmonella zoonotic transmission, since broiler and beef cattle are hosts to the Salmonella serotypes isolated. It is important to establish the source of infection in regions where salmonellosis is endemic in order to control transmission.

INTRODUCTION: Gallbladder stones are a very frequently occurring condition. Despite bile bactericidal activity, many bacteria have been detected inside the gallbladder, and gallstones facilitate their presence. Between 3% and 5% of the patients with Salmonella spp. infection develop the carrier stage, with the bacteria persisting inside the gallbladder, shedding bacteria in their feces without signs of infection. The aim of this study was to isolate bacteria from Colombian patients with gallstones, using standard culturing methods, and to identify Salmonella spp. carriers by molecular techniques. METHODOLOGY: A total of 149 patients (120 female and 29 male) diagnosed with gallstones who underwent cholecystectomy and who did not have symptoms of acute inflammation were included. Gallbladder tissue and bile were cultured and used for DNA extraction and Salmonella spp. hilA gene detection. RESULTS: Of the 149 patients 28 (19%) had positive cultures. Twenty-one (75%) patients with positive cultures were from Medellin's metropolitan area. In this geographical location, the most frequent isolations were Pseudomonas spp. (38%), Klebsiella spp. (23%), and Proteus spp. (9%) in addition to unique cases of other bacteria. In Apartado, the isolates found were Enterobacter cloacae (50%), Raoultella terrigena (32%), and both Enterobacter cloacae and Raoultella terrigena were isolated in one (18%) male patient. Five (3.3%) of the 149 patients had positive polymerase chain reaction (PCR) results for the hilA gene of Salmonella spp., all of whom were female and residents of the Medellín metropolitan area. CONCLUSIONS: The gallbladder microbiota variability found could be related to geographical, ethnic, and environmental conditions.

Coatings for medical devices are expected to improve their surface biocompatibility mainly by being bioactive, i.e. stimulating healing-oriented interactions with living cells, tissues and organs. In particular, for stent applications, coatings are often designed to enhance the endothelialization process. The coating strategy will be primarily responsible for the interfacial properties between the substrate and the coating, which must show high stability. Therefore, the present work aims at comparing the stability of adsorbed and grafted fibronectin, a protein well-known to promote endothelialization. Fibronectin coatings were deposited on fluorocarbon films generated by a plasma-based process on stainless steel substrates. Then, deformation tests were performed in order to simulate the stenting procedure and stability tests were completed under static and under-flow conditions. Coatings were characterized by XPS, AFM, water contact angle, immunostaining and ToF-SIMS analyses. The results show higher stability for the grafted coatings; indeed, the integrity of the protein simply adsorbed was strongly compromised especially after under-flow tests. Both coatings exhibited similar behavior after deformation and static tests. These results clearly show the impact of the coating strategy on the overall stability of the coatings as well as the importance of under-flow investigations.
