RESUMO

This study evaluated a new tibial torsiometer that is universally applicable to all limb sizes and requires no special training, jigs, or radiographic equipment. To compare the reliability of measurements obtained with the torsiometer, 160 tibias were evaluated with both the torsiometer and computed tomography. Results for both methods were identical in 51 tibias, within 1 degrees in 66 tibias, and within 2 degrees in 43 tibias. The difference between the two methods was not significantly different. No significant difference was found among testers. This device should prove to be useful for the quantitation of tibial torsion.

RESUMO

Primary leiomyosarcoma of the bone is a rare malignancy. Clinical follow-up suggests that primary osseous leiomyosarcoma has an aggressive biologic behaviour with poor survival time. We report a case of primary leiomyosarcoma arising from the proximal metaphysis of the right tibia of a 55-year old woman with a long follow-up period, without recidivism. Primary leiomyosarcoma has to be considered as a differential diagnostic possibility in the case of bone tumours seen on routine initial plain radiographs as lytic lesions. If the tumour has been adequately excised at the time of diagnosis, as in the present case, with adjuvant therapy protocol, the long-term prognosis of such an aggressive tumour can be exceptionally good.


El leiomiosarcoma primario del hueso es un tumor maligno raro. El seguimiento clínico sugiera que el leiomiosarcoma óseo primario tiene un comportamiento biológico agresivo con escaso tiempo desobrevivencia. Reportamos un caso de leiomiosarcoma primario proveniente de la metáfisis proximal de la tibia derecha de una mujer de 55 años de edad con un largo período de seguimiento, sin recidivas. El leiomiosarcoma tiene que ser considerado una posibilidad de diagnóstico diferencial en el caso detumores óseos observados en radiografías simples iniciales de rutina como lesiones líticas. Si el tumorha sido debidamente extirpado en el momento del diagnóstico – como en el caso presente – con un protocolo de terapia adyuvante, la prognosis a largo plazo de un tumor tan agresivo puede ser excepcionalmente buena.

We studied 18 adult male New Zealand rabbits with a critical right-sided ulnar defect of 15 mm. In six animals the defect was grafted with homologous compressed cancellous bone, in six animals with homologous compressed cancellous bone including 300 micro g bone morphogenetic protein (BMP)-7 and in six animals with homologous compressed cancellous bone including 0.5 ml autologous bone marrow. The defect was studied using radiographs every second week for 10 weeks. At the conclusion of the experiment the animals were killed and the defect studied by histology and histomorphometry. In all animals treated with the addition of autologous bone marrow and in five of six animals treated with the addition of BMP-7, the defect healed. There was no union in animals treated with homologous compressed cancellous bone without additive. The histological picture of the regenerated area was similar in the two experimental groups. Woven bone contained small marrow spaces with fibrous tissue and capillaries. The osteoid seams were on average greater in animals that received autologous bone marrow as compared to animals that received BMP-7.

AIM: To evaluate the effect of cement use in porous-coated anatomic (PCA) total knee prosthesis on its survival. METHODS: The study was a retrospective analysis of 142 PCA total condylar arthroplasties performed in 124 patients from 1985 to 1991. Uncemented prostheses were used in 87 knees, the prostheses were cemented in 44 knees, and hybrid prosthesis components were used in 11 knees. The average follow-up time was 88 months (range 66-140). The survival of prosthesis was assessed using the Kaplan-Meier's method. The Baltimore score was evaluated as a measure of clinical performance in 115 replacements. RESULTS: The overall cumulative survival rate of a PCA total knee prosthesis was 77% over the average follow-up period of 88 months. No significant differences in survival rates among cementless, cemented, or hybrid fixations could be demonstrated. The survival rate of the prostheses in patients with rheumatoid arthritis (82.5%) was significantly higher than in patients with osteoarthritis (73.8%). Revision was necessary in 29 (20.4%) replacements. CONCLUSION: The survival of PCA endoprosthesis, regardless of the components used for implantation, is not satisfactory.
