RESUMO

Clear cell tumour or "sugar tumour" of the lung is a rare primary neoplasm with unique histologic and electron microscopic features that may resemble those of metastatic renal cell carcinoma. An immunohistochemical studies are useful in a differential diagnosis these tumours: HMB45 in combination with a panel of various antibodies. The authors present a benign clear cell tumour of the lung, diagnosed on the base of its morphological, immunohistochemical and ultrastructural features.

RESUMO

BOOP is a disease with characteristic histology features which can occur as a secondary lung reaction to the various toxic agents or as a primary idiopathic disease. Idiopathic form of BOOP is a rare disease and may be found in 6-7 patients of 100,000 hospital admissions. We described 3 patients with idiopathic BOOP confirmed by the histologic lung examination. The time from the beginning of symptoms till the microscopical diagnosis ranged from 6 to 12 months. At the beginning of the disease the patients had symptoms compatible with the respiratory infection. In one of the patients the clinical course of the disease had a progressive character. In two patients spontaneous regression of radiological lung lesions was observed. This regression was however only temporary in one of them. In two cases treated by steroids regression of lung lesions was noticed.

The authors present the results of surgical treatment of chronic empyemas of the pleural cavity in 294 patients treated at the Surgical Department in the years 1980-1992. The methods of treatment are discussed taking in consideration the etiology of the empyemas, clinical presentation, coexistent diseases. In 86 patients (29.3%) decortication with partial resection of the lung was carried out (most often a lobe), in 123 patients (41.8%) a decortication was performed, in 8 (2.7%) thoracoplasty, in the remaining 41 (13.9%) drainage of the pleural cavity was carried out. Out of the 245 subjects treated surgically 3 (1.2%) died; in the remaining patients a full recovery was noted. A full recovery was noted in 22 (53.7%) out of the 41 treated with pleural cavity drainage. Eight patients with esophageal-pleural fistulas in the course of a advanced esophageal cancer died due to advanced malignancy. 11 were discharged with chronic drainage. The latter could not be treated surgically due to a existence of high risk factors.
