RESUMO

BACKGROUND: Individual- and population-level socioeconomic disadvantages contribute to unequal outcomes among childhood cancer survivors. Reducing health disparities requires understanding experiences of survivors from historically marginalized communities, including those with non-English language preference. PROCEDURE: We partnered with a community-based organization (CBO) serving families of children with cancer in a rural region in California with low socioeconomic status and majority Hispanic/Latino (H/L) residents. We interviewed English- and Spanish-speaking adolescent/young adult (AYA) childhood cancer survivors (≥15 years old, ≥5 years from diagnosis), parents, and CBO staff to evaluate post-treatment needs and impact of CBO support. Data were analyzed qualitatively using applied thematic analysis. Themes were refined through team discussions with our community partners. RESULTS: Twelve AYAs (11 H/L, 11 bilingual), 11 parents (eight H/L, seven non-English preferred), and seven CBO staff (five H/L, five bilingual) participated. AYAs (five female, seven male) were of median (min-max) age 20 (16-32) and 9 (5-19) years post diagnosis; parents (nine female, two male) were age 48 (40-60) and 14 (6-23) years post child's diagnosis. Themes included challenges navigating healthcare, communication barriers among the parent-AYA-clinician triad, and lasting effects of childhood cancer on family dynamics and mental health. Subthemes illustrated that language and rurality may contribute to health disparities. CBO support impacted families by serving as a safety-net, fostering community, and facilitating H/L families' communication. CONCLUSIONS: Childhood cancer has long-lasting effects on families, and those with non-English language preference face additional burdens. Community-based support buffers some of the negative effects of childhood cancer and may reduce disparities.

ABSTRACT This study aimed to characterize clinical-neurological factors and the functional swallowing capacity of patients with malignant infarction submitted to decompressive craniectomy during the hospital stay. This retrospective, descriptive, observational study was conducted between January 2020 and December 2021. The following data regarding up to eight stages were extracted for descriptive analysis: age, location of the lesion, level of awareness according to the Glasgow Coma Scale, neurological impairment according to the National Institutes of Health Stroke Scale, and the functional swallowing capacity according to the Functional Oral Intake Scale. Data on five patients were collected, with a mean of 0.2 days for the first neurological assessment. Decompressive craniectomy was performed in a mean of 2 days after admission. The speech-language-hearing assessment occurred in a mean of 8.2 days, and the speech-language-hearing discharge took a mean of 35.4 days. The neurological impairment score remained the same as in the first neurological assessment after decompressive craniectomy, with a mean score of 16.6. The functional swallowing capacity was the same in the first assessment after decompressive craniectomy, on FOIS level 1, improving considerably by the discharge, with a mean level of 4.8. It is concluded that clinical-neurological factors can interfere with the functional swallowing capacity, although they did not hinder either speech-language-hearing treatment or their evolution to a full oral diet during the hospital stay.


RESUMO O objetivo do presente trabalho é caracterizar fatores clínicos-neurológicos e capacidade funcional de deglutição de pacientes com Acidente Vascular Cerebral Isquêmico Maligno submetidos a craniectomia descompressiva durante internação hospitalar. Trata-se de um estudo observacional, descritivo e retrospectivo, realizado de janeiro de 2020 a dezembro de 2021. Os dados foram extraídos em até oito momentos, incluindo idade, local da lesão, nível de consciência de acordo com Glasgow, comprometimento neurológico pelo National Institutes of Health Stroke Scale e capacidade funcional de deglutição pela Escala Funcional de Ingestão por Via Oral. Realizou-se análise descritiva. Foram coletados dados de 5 pacientes sendo a média de tempo para primeira avaliação neurológica de 0,2 dias. A realização da craniectomia descompressiva levou em média 2 dias após admissão. Avaliação fonoaudiológica ocorreu, em média, 8,2 dias e alta fonoaudiológica levou, em média, 35,4 dias. O comprometimento neurológico manteve-se na pontuação da primeira avaliação neurológica pós-craniectomia descompressiva, média de 16,6. A capacidade funcional de deglutição foi a mesma na primeira avaliação pós-craniectomia descompressiva, com nível 1 na FOIS, melhorando consideravelmente, com média de nível 4,8, na alta. Concluiu-se que fatores clínicos-neurológicos podem interferir na capacidade funcional de deglutição, contudo não limitaram a atuação fonoaudiológica e evolução com dieta oral exclusiva na internação hospitalar.

OBJECTIVE: To evaluate the quality of children's menus in restaurants located in shopping malls. METHODS: To select the sample, restaurants from 30% of shopping malls in each region of the city of São Paulo were included and, after considering only one restaurant per chain, the total was limited to 151 restaurants, 30.2% of which (n=35) presented a children's menu. Data were collected through a form on Google Forms. RESULTS: Of the restaurants with children's menu, 60% (n=21) were conventional restaurants and 40% (n=14) takeaway/fast-food. The large number of chains present in most visited malls showed a democratization of the way of eating, with popular and accessible menus, regardless of social status. Most of the analyzed foods were cooked (41.5%). Most preparations did not use grease in their preparation and there was a notable lack of fruit and vegetables (FV) (4%). Sweet desserts were offered in 11.4% of the places and 20% included gifts with meals. CONCLUSIONS: The scarce offer of children's menus, few options and low FV offer indicate the need for a new look at the development of children's menus and a greater integration between the possibilities of restaurants and the expectations of parents and children, in the challenge of integrating the relationship between the supply of new foods that promote healthier habits and their consumption.

OBJECTIVE: To evaluate the characteristics of aggression, care, and follow-up of sexual violence in adolescent women at a university service in Campinas, São Paulo, Brazil. METHODS: In a retrospective cohort study from 2011 to 2018, we assessed sociodemographic characteristics, type and form of sexual violence perpetration, background and perception of the victim about violence, and characteristics of care and social/legal support during follow-up. RESULTS: Of 521 adolescents 242 (46.5%) were aged 10-14 years, 354 (68%) had not started sexual life, 465 (89%) perceived the violence and they had low prevalence of alcohol (71, 16%) and other psychoactive substance consumption (25, 6%), and 24 (4.6%) had an intellectual disability. Intimidation through physical force, acute abuse, vaginal penetration, acquaintance aggressor, being approached in a public place, and family/acquaintance residence were the most prevalent characteristics of aggression. In the 2017-2018 biennium, we observed a decrease in the prevalence of adolescents who were students (P < 0.001), an increase in the number of acquaintance aggressors (P = 0.008), and medical care after 72 hours (P < 0.033). CONCLUSION: Adolescents were victims of severe sexual violence. There was a decrease in prophylactic treatments and half of the adolescents did not complete outpatient follow-up. The economic worsening over the last decade may have contributed to these results.

The south of Brazil has a high production of peaches which generates the stone as residue. The recycling of biomass and plastic waste is a challenge that must be overcome. Therefore, co-pyrolysis emerges as a possibility for joint conversion of peach stone and plastic bags into value-added products. Thus, the objective is to obtain new products, emphasizing the organic fraction of the bio-oil obtained under the best experimental conditions, and to characterize it by GC-MS. The experiments provided a three-phase bio-oil: two organic phases with different densities and an aqueous phase. It was observed that the central point experiment (temperature at 500 °C and blend composition, % peach stone/polyethylene, 80/20) generated the highest yield of bio-oil. However, the experiment at the lowest temperature and with greater addition of plastic waste produced a higher organic fraction. Using gas chromatography, 161 chemical compounds were identified in the denser organic phase, emphasizing the phenols. In the lighter organic fraction, hydrocarbons were the majority among the 70 compounds identified. The results suggest that the interaction between biomass and polyethylene influences the characteristics of the products obtained, since polyethylene acts as dispersant and as hydrogen donor. Co-pyrolysis, in this research, proves to be efficient and viable, allowing the joint destination of those environmental liabilities.

Resumo Objetivo: Avaliar a qualidade dos menus infantis de restaurantes localizados em shopping centers. Métodos: Para a seleção da amostra foram incluídos os restaurantes de 30% dos shopping centers de cada região da cidade de São Paulo e, após considerar-se somente um restaurante por rede, o total ficou limitado a 151 restaurantes, dos quais 30,2% (n=35) apresentaram cardápio infantil. Os dados foram coletados por meio de um formulário no Google Forms. Resultados: Dos restaurantes com menu infantil, 60% (n=21) eram convencionais e 40% (n=14) eram de takeaway/fast-food. O grande número de redes presente na maioria dos shopping centers visitados mostrou a democratização da forma de comer, com cardápios populares e acessíveis, independentemente de classe social. A maior parte dos alimentos analisados era cozida (41,5%). A maioria das preparações não utilizava gordura em seu preparo e foi notável a falta da oferta de frutas, legumes e verduras (FLV) (4%). Sobremesas doces eram oferecidas em 11,4% dos locais e 20% incluíam brindes com as refeições. Conclusões: A escassa oferta de menus infantis, as poucas opções e a baixa oferta de FLV indicam a necessidade de um novo olhar no desenvolvimento de cardápios infantis e de um maior entrosamento entre as possibilidades dos restaurantes e as expectativas de pais e crianças, no desafio de integrar a relação entre a oferta de novos alimentos, que promovam hábitos mais saudáveis, e o seu consumo.

RESUMO O processamento de pêssego em conserva gera o caroço como resíduo agroindustrial de difícil destinação. Assim, a técnica de pirólise do endocarpo lenhoso é um eficiente tratamento para conversão da biomassa em produtos com valor agregado. Entre os produtos, o bio-óleo é composto de duas fases imiscíveis (orgânica e aquosa), com potencial para suprir a produção de produtos químicos em diferentes ramos industriais. O objetivo deste estudo foi obter e avaliar a constituição da fase orgânica dos bio-óleos com base nas pirólises rápida (reator de quartzo) e lenta (reator de aço inox) do endocarpo lenhoso, ambos de leito fixo a 700°C evidenciando assim a influência dos processos na constituição dos bio-óleos obtidos. As frações líquidas foram derivatizadas com N-metil-N-trimetilsililtrifluoracetamida para posterior caracterização em cromatografia gasosa acoplada à espectrometria de massas. Pela metodologia empregada, detectaram-se nas fases orgânicas dos bio-óleos da pirólise rápida e lenta um total de 59 e 41 compostos, respectivamente; destes, os fenóis são majoritários com 59,9% e 67,2%, em massa, em que os núcleos metoxifenol (rápida) e os alquilfenóis (lenta) são os compostos predominantes. Conclui-se que a etapa de derivatização e a técnica de cromatografia gasosa acoplada à espectrometria de massas foram satisfatórias para a caracterização qualitativa e semiquantitativa dos bio-óleos, os quais mostraram potencial de utilização como fonte de produtos químicos e beneficiamento no reaproveitamento de resíduos.


ABSTRACT The processing of pickled peaches generates the peach stone as an agro-industrial residue that is difficult to dispose of. Thus, the woody endocarp pyrolysis technique is an efficient treatment for converting biomass into value-added products. Among the products, bio-oil is composed of two immiscible phases (organic and aqueous), with potential to supply the production of chemical products in different industrial sectors. The objective of this study was to obtain and evaluate the constitution of the organic phase of the bio-oils based on the fast (quartz reactor) and slow (stainless steel reactor) pyrolysis of the woody endocarp, both with fixed bed at 700°C, thus evidencing the influence of processes in the constitution of the obtained bio-oils. As liquid fractions, they were derived with N-methyl-N-trimethylsilyltrifluoracetamide for further characterization in gas chromatography coupled with mass spectrometry. Through the methodology employed, a total of 59 and 41 compounds were detected in the organic phases of the fast and slow pyrolysis bio-oils, respectively; Phenols are the majority, with 59.9 and 67.2% by weight, where the methoxyphenol (fast) and cresols (slow) are the predominant compounds. It is concluded that the derivatization stage and the gas chromatography/mass spectrometry technique were satisfactory for the qualitative and semi-quantitative characterization of bio-oils, which they can potentially use as a source of chemical products and to benefit from reuse of residue.

Diphtheric aspergillosis tracheitis is an uncommon syndrome described in human pathology, usually associated with immunosuppression in the affected individuals. Interestingly, no comparative/equivalent cases were found in domestic animals. This report describes the pathological and mycological findings associated with diphtheric aspergillosis tracheitis in an immunocompromised calf. The main pathological findings were diphtheric tracheitis and rhinitis, and necrotizing ruminitis associated with intralesional septate, acute branching fungal hyphae consistent with Aspergillus spp. Mycological culture and isolation confirmed the fungal hyphae as A. fumigatus due to characteristic features. Immunohistochemistry (IHC) assays identified intralesional antigens of bovine viral diarrhea virus (BVDV) and malignant catarrhal fever virus (MCFV) at the trachea and small intestine; IHC detected intralesional antigens of bovine alphaherpesvirus 1 (BoHV-1) only at the trachea. These findings confirmed the simultaneous occurrence of A. fumigatus with concomitant infections due to BVDV, MCFV, and BoHV-1 in this calf. Since ovine gammaherpesvirus-2 (OvHV-2) is the cause of MCF in Brail, it is likely that the intralesional MCFV antigens identified were those of OvHV-2. In this case, disseminated aspergillosis was probably associated with the undeveloped immunological status of the calf that was further impaired due to the combined immunodepressive effects of BVDV and BoHV-1 infections. Although BVDV and BoHV-1 are infectious disease pathogens frequently associated with the development of bovine respiratory disease (BRD) in feedlot and dairy cattle, the identification of intralesional OvHV-2-like antigens in several parts of the lungs suggest that this MCFV also played a role in the BRD-associated lesions identified in this calf.

The effectiveness of 7% hypertonic saline solution (HSS) and 6% hypertonic bicarbonate solution (HBS) followed by intraruminal water administration for the treatment of acute rumen lactic acidosis (ARLA) has not been evaluated yet. The hypothesis of the study is that treatment with HBS causes a faster correction of imbalances and clinical recovery than treatment with HSS. ARLA was induced in six healthy mature female sheep with sucrose twice. The sheep received both treatment regimens in a cross over design. After 18 h of induction, rumen lavage was performed and IV infusion of 7% HSS (4 mL/kg BW) or 6% HBS (6.7 mL/kg BW) was done over 5 min, followed by intraruminal administration of water (8% BW). The solutions provided 4.8 mmol/kg BW of sodium. Physical and laboratory tests were performed for 168 h. Both treatments increased plasma volume. After HSS treatment, pH and HCO3- returned to baseline values at 144 h and BE at 168 h. With HBS treatment, pH returned to baseline at 24 h, and HCO3- and BE at 48 h. In both treatments, the ruminal motility and the maximum hay intake were restored at 96 and 120 h, respectively. In conclusion, administration of HSS or HBS followed by intraruminal water corrects dehydration and mild-to-moderate metabolic acidosis in sheep with ARLA. Treatment with HBS promoted a faster correction of metabolic acidosis and could be indicated for the most severe cases.

Desejando ampliar a escuta aos usuários de uma Unidade Básica de Saúde, criou-se o “Acolhimento em Grupo”, um espaço semelhante a um grupo de sala de espera, com foco em uma concepção ampliada de saúde. Tem como objetivo a construção de conhecimentos na coletividade e o fortalecimento do usuário como protagonista, fundamentando-se na concepção de educação popular, processos grupais e análise institucional. Os temas abordados foram funcionamento e qualidade dos serviços de saúde, aspectos de trabalho, reforma psiquiátrica, controle social e mobilização popular. Observamos como resultados a possibilidade do serviço refletir sobre sua organização e conceito de saúde, utilidade desta prática no processo de territorialização e maior aproximação entre profissionais e usuários. Apontamos como desafios a inclusão de mais profissionais na atividade, o desgaste emocional provocado naqueles que a realizam e a intersetorialidade(AU)


Desiring to extend the listening to the users in the actions from a Primary Care Unit, it was created the “Welcome in Group”, a space alike to a “waiting room”, which focuses on a health extended conception. It has as a main goal the construction of knowledge based on collectivism, in search of strengthening the user as a main character in the Welfare State System’s (SUS/BR) actions, grounded on popular educational concepts, group processes and institutional analysis. The themes that were dealt with come as it follows health operation and quality, work aspects, psychiatric reform, social control and popular mobilization. As results the possibility of work reflecting on its organization and concept of health, the useful point of this practice on territorial process, better and bigger approach among professionals and users. Challenges to be thought about inclusion of more professionals in the activity, emotional stress caused on those who work in this area and intersectorial practice(AU)
