Las eosinofilias pulmonares constituyen un grupo variado de entidades cuyo nexo común es la inflamación eosinofílica que puede o no asociarse a eosinofilia periférica. En ese reporte describimos una forma atípica y no descripta de presentación como "pseudotumor pulmonar" que remitió con tratamiento corticoesteroideo.


Pulmonary eosinophilia are a varied group of entities sharing the eosinophilic inflammation that may or may not be associated with peripheral eosinophilia. This report describes an atypical, undescribed presentation, the "pulmonary pseudotumor" which showed regression with corticosteroid treatment.

RESUMO

El día 30 de enero de 2020 la Organización Mundial de la Salud (OMS) reconoce la condición de pandemia por el virus SARS-CoV2 responsable de la enfermedad por coronavirus de 2019 (COVID-CoV2 por su denominación en inglés). Se ha demostrado que la vía respiratoria de pacientes infectados por COVID-19 tiene una alta carga viral, especialmente en nariz, garganta y tráquea. Los procedimientos que involucran el tracto aerodigestivo superior, como la broncoscopía, se consideran de alto riesgo de infección. Recomendaciones de diferentes sociedades establecen lineamientos para la realización de estos procedimientos, recomendando medidas de seguridad específicas evitando en lo posible la realización de broncoscopía rígida. Nuestra unidad corresponde a un servicio de neumonología intervencionista del sistema de salud público. Para hacer frente a la demanda de atención, necesidad de realizar procedimientos impostergables y una limitación en infraestructura y recursos, se inició un plan de contingencia y prueba de elementos alternativos orientados a potenciar la seguridad del personal y sus pacientes. Se presentan en este trabajo las medidas generales y nuevos dispositivos incluidos en nuestra secuencia de trabajo con el fin de mejorar la bioseguridad para el personal de salud y el paciente ante la necesidad de continuar realizando procedimientos.


On January 30, 2020, the World Health Organization (WHO) declares the pandemic of SARS-CoV2, responsible for the coronavirus disease of 2019 (COVID-CoV2). It has been shown that the airways of patients infected with COVID-19 have a high viral load, especially in the nose, throat and trachea. Procedures involving the upper aerodigestive tract, such as the bronchoscopy, are considered to carry a high risk of infection. Recommendations from different societies establish certain guidelines for these procedures, suggesting specific safety measures and, where possible, avoiding the performance of rigid bronchospies. Our unit belongs to an interventional pulmonology service of the Public Health System. In order to be able to meet the demand for medical care, the need to perform urgent procedures and the limitation to infrastructure and resources, we began a contingency plan and testing of alternative elements intended to increase the safety of the staff and the patients. This study introduces general measures and new devices included in our work sequence with the purpose of improving biosafety for the health staff and the patient when there is still a need to follow the procedures.

Background: Laryngeal paralysis, failure of arytenoid cartilage, and vocal fold abduction are commonly seen in older medium to large breed dogs. Observation of laryngeal function in dogs and cats is performed by transoral visualization. There are a variety of surgical techniques; aspiration pneumonia is the most common complication associated with surgical correction of laryngeal paralysis. The aim of this case series is to report on the placement of a laryngeal silicone stent in seven dogs with laryngeal paralysis and its use as an alternative treatment of respiratory distress caused by laryngeal paralysis and/or its use for laryngeal stenosis as complication of laryngeal paralysis surgery. Case description: Seven dogs presented with either episode of gagging, mild-to-severe inspiratory distress, or cyanosis because of a laryngeal paralysis or laryngeal stenosis. In each case, the laryngeal paralysis was diagnosed by direct laryngoscopy. They were treated with a silicone laryngeal stent (Stening®) that substantially improved the clinical signs. Each dog had a different outcome because of other pathologies; however, the laryngeal pathology was successfully treated with the stent. Conclusion: The placement of the laryngeal stent is an easy technique to learn and practice, it could avoid the life-threatening complications of the laryngeal paralysis at the acute phase, and it could be a noninvasive and long-term alternative therapy for laryngeal paralysis in dogs. The results in these clinical cases are encouraging for considering the laryngeal stent as a therapeutic alternative.

Pulmonary alveolar proteinosis (PAP) is a rare disease characterized by the intra-alveolar accumulation of a proteinaceous phospholipid-laden material called surfactant. Clinically, this disease should be suspected with respiratory failure in association with a crazy paving pattern on high-resolution chest computed tomography. We report a 24-year-old gentleman who was referred to us for a history of respiratory failure, treatment with invasive ventilation and tracheostomy. His blood exams and biochemistry were normal. His infectious and rheumatological panel was negative for a secondary disease. A flexible bronchoscopy with a transbronchial biopsy through a CryoProbe was performed. An anatomopathological analysis was periodic acid-Schiff positive for PAP. A CryoProbe is a recently developed diagnostic tool that improves the diagnostic yield in diffuse lung diseases compared to bronchoscopy with transbronchial biopsy. This method should be considered for patients with diffuse lung disease and PAP.

Paciente de sexo femenino de 55 años de edad, con diagnóstico de melanoma cutáneo en miembro inferior izquierdo hace 3 años, que evolucionó con remisión completa posterior al tratamiento quirúrgico. Un mes antes de su ingreso, presentó disnea y expectoración mucosa de coloración oscura. Al ingreso no presentaba estridor, ortopnea ni tos. En el examen físico se auscultaron sibilancias inspiratorias en hemitórax derecho, murmullo vesicular conservado e hipoventilación generalizada. La tomografía de tórax mostró la presencia de una masa endoluminal en el tercio inferior de la tráquea con compromiso hasta el bronquio fuente derecho, generando una obstrucción de casi el 90% de la vía aérea

Introducción: La necesidad de la fibrobroncoscopía (FBC) en pacientes con hemoptisis y radiografía de tórax (RxTx) no localizadora (NL) es todavía controversial, aunque en general continúa siendo el procedimiento recomendado como rutina de evaluación. Objetivo: Determinar la rentabilidad diagnóstica de la FBC en pacientes con hemoptisis y RxTxNL. Material y métodos: Se analizaron en forma retrospectiva los registros estandarizados de 340 FBC realizadas en forma programada entre enero de 2008 y diciembre 2012 en un único centro. La hemoptisis fue clasificada como trivial (esputo hemoptoico), leve (menos de 100 ml en 24 horas) y moderada (100-500 mL en 24 horas). El procedimiento se realizo en los primeros 7 días de hemoptisis, y se consideraba positiva si se obtenía el diagnóstico final. Resultados: De 396 pacientes, 102 (25,7%) tenían indicación de fibrobroncoscopia por hemoptisis, de los cuales, 52 presentaba Rx de Tórax no localizadora. En el 42.3% de los casos, la tomografía proveyó información diagnóstica adicional. 30 pacientes (29,4% de todos los casos de hemoptisis) presentaron radiografía y tomografía no localizadora. En un paciente el hallazgo broncoscópico fue un carcinoma bronquial no sospechado. 7 pacientes tenían hallazgos endobronquiales benignos. Todos los pacientes que presentaron hallazgos positivos en la fibrobroncoscopía eran tabaquistas, mayores de 40 años, tenían alguna sospecha clínica previa o presentaban hemoptisis por más de 1 semana. Todos los pacientes asintomáticos, menores de 40 años y con hemoptisis menor a los 7 días, la fibrobroncoscopía fue normal. Conclusión: En los pacientes con hemoptisis, mayores de 40 años, con antecedentes de tabaquismo y síntomas constitucionales se obtiene una alta rentabilidad diagnóstica cuando se asocian la Tomografía computada de tórax y la FBC, pero pueden evitarse en ausencia de estas condiciones o si existe la sospecha clínica de otras causas identificables.

Objective: The aim of this study was to examine the yield of FOB in patients with haemoptysis and normal findings on chest roentgenogram and CT scan. Methods: We reviewed the medical records of all consecutive patients undergoing elective flexible bronchoscopy between January 2008 and December 2012. All patients underwent FOB within 7 days of active bleeding. Amount of bleeding was classified as trivial (drops of blood, bloody sputum), mild < 100 mL in 24 h and moderate 100-500 mL in 24 h. Bronchoscopy was considered positive if it led to the final diagnosis. We also assessed whether the bronchoscopic evaluation located the bleeding site. Results: Amongst 396 patients examined with FOB, in 102 patients (25.7%) FOB was prescribed for evaluation of haemoptysis; in 52 patients with haemoptysis, the chest radiography was normal. In 42.3% of these patients, CT scan provided additional diagnostic information. In 30 patients (29.4% of all the haemoptysis cases), CXR and CT did not show images which could explain the haemoptysis . Bronchoscopy findings showed an unsuspected bronchial carcinoma in one patient. In seven patients FOB disclosed some benign abnormality. All patients with a positive finding in the FOB were smokers, older than 40 years, had some clinical suspicion and had suffered haemoptysis for more than one week. FOB was normal in asymptomatic patients, who were younger than 40 years and with hemoptysis lasting less than one week. Conclusion: In patients older than 40 years, smokers, with constitutional symptoms or with hemoptysis lasting for more than one week the combined use of bronchoscopy and chest CT gives the best diagnostic yield. In younger, asymptomatic, non-smoking patients with haemoptysis and normal CT scan, further investigations may not be justified, but these results need to be confirmed in a prospective multicenter study.

La coccidiodomicosis es una micosis sistémica endémica de América, producida por los hongos del género Coccidioides, C. immitis y C. posadasii. La mayor parte de las infecciones son benignas y autolimitadas, muy pocas desarrollan una enfermedad pulmonar grave y, un porcentaje mínimo, una enfermedad diseminada. Las infecciones complicadas del espacio pleural se han comunicado en muy pocos casos en la literatura. Se presenta el caso de un paciente inmunocompetente con una forma cavitada extensa asociada a empiema.

Most of the infections are mild and self-limited; only a few of them develop a serious lung disease and a much smaller proportion cause a systemic disease. A few cases of infections complicating the pleural space infections have been reported in the literature. We present a case of an immunocompetent patient, with a pulmonary Coccidiodiomycosis with extensive cavitary dissease associated to empyema.

Introducción: La estenosis traqueal (ET) es una complicación de la intubación traqueal o de la traqueostomía. La dilatación con endoscopía rígida (DER) es el tratamiento inicial en presencia de ET sintomática. Habitualmente estos pacientes presentan hallazgosespirométricos sugestivos de ET: aplanamiento en las ramas inspiratoria, espiratoria o ambas de la curva flujo-volumen (CFV). Asimismo, índices numéricos correlacionan valores espirométricos y sustentan el diagnóstico presuntivo de ET. Objetivos: Evaluar las características espirométricas de los pacientes con ET post intubación, previo y posterior a la DER. Materiales y métodos: Se analizaron todas las DER realizadasen pacientes con ET entre Noviembre de 2006 y Marzo de 2009. Todos los pacientes incluidos contaban con espirometría previa y posterior a la DER. Se analizó el volumen espiratorio forzado en 1° segundo (VEF1), pico flujo espiratorio (PFE), índice VEF1/PFEy la morfología de la CFV. Resultados: Veinticinco dilataciones traqueales fueron realizadas en 15 pacientes con ET. Posterior a la DER se observó un incremento en el VEF1 y PEF (total y %), así como una disminución en el índice VEF1/PEF. La CFV mostró un mayor área bajo la curva como consecuencia de menor aplanamiento de una o ambas ramas. Conclusiones: La espirometria pre-dilatación confirmó el diagnóstico de ET utilizando el VEF1, PEF y su índice (análisis numérico). La morfología de la CFV sustentó dicho diagnostico (análisis visual). La espirometría post-dilatación permitióobjetivar la mejoría de la ET a través de ambos tipos de parámetros, coincidiendo con el aumento del diámetro traqueal observado endoscópicamente.


Introduction: Tracheal stenosis (TS) is a complication of endotracheal intubation or tracheostomy. Rigid endoscopic dilatation (RED) is the initial treatment in symptomatic TS. Usually these patients have spirometric results that suggest such alteration: flatteningof the flow-volume curve (FVC), either in the espiratory, the inspiratory or bothcurves. Also, quantitative indexes among different spirometric values are correlated suggesting the presence of TS. Objectives: To assess spirometric characteristics in patients who suffer from symptomatic TS due to airway intubation, before and after therapeutic RED. Materials and methods: All RED procedures in patients with symptomatic TS performed between November 2006 and March 2009 were included in the analysis. All included patients should have a spirometry performed before andafter tracheal dilatation. Forced expiratory volume in 1st second (FEV1), peak expiratory flow rate (PEFR), FEV1/PEFR index and FVC morphology were taken into consideration. Results: Twenty five ED were performed in 15 patients with TS. An increase in FEV1(total and %) and PEFR (total and %) and a decrease in FEV1/PEFR index were observed in the post-dilatation values. The FVC showed an improvement in its morphology through a larger area under the curve because of less flattening in one or both curves.Conclusions: Pre-dilatation spirometry confirmed TS diagnosis using FEV1, PEFR and its index (quantitative analysis) and by the morphology of the FVC (visual analysis). Post-dilatation spirometry allowed the documentation of TS improvement through both kinds of parameters, in agreement with the improvement of the luminal diameter seen endoscopically.

Nuestro objetivo fue evaluar si el ritmo circadiano del cortisol y los niveles de IgA secretoria (IgAs) se alteraban por el ejercicio intenso en actitud moderada. Material y Métodos: Se evaluaron 4 escaladores masculinos. La travesía duró 5 días, alcanzando una altura máxima de 4270 msnm. Se tomaron muestras de sangre antes y después del viaje. Durante el ascenso, se recolectaron muestars de saliva cada mañana y cada noche en tubos estériles manteniéndolas a temperatura inferior a 10ºc. El mal agudo de montaña (MAM) se evaluó con el test de Lake Louise. Los resultados se presentaron como pormedio y DS. Resultados: los participantes no mostraron déficit previo de cortisol: 14,7ug/dl (3,45) y todos mantuvieron el ritmo circadiano: mañana: 0,87ug/dl (0,37) vs noche: 0,32ug/dl (0,22) (p<0,01); dos de ellos presentaron MAM moderado que normalizaron con la aclimatación. El cortisol salival matutino mostró valores superiores a mayor altitud (r: 0,27). Ninguno de los participantes tenía déficit previo de IgA o IgAs: 162mg% (52,3) y 17,9mg% (8,64) respectivamente. Los valores de IgAs matinal fueron superiores a los nocturnos: 21,03mg% (18,9) vs 9,88mg% (7,50) p: 0,03. El coeficiente de correlación entre IgA y cortisol salival fue positivo (r: 0,35). Conclusión: La potencia estadística es baja debido al número de escaladores. Este hecho no nos permiten establecer conclusiones firmes, pero el ritmo cardíaco del cortisol no fue alterado por ejercicio intenso en altitud moderada ni por el MAM moderado. Los nieveles elevados de cortisol salival no afectaron a la producción de IgA.

INTRODUCTION: Flexible bronchoscopy (FB) is a procedure usually carried out in the hospital setting. Occasionally, home hospitalized (HH) patients or those at long-term acute care facilities (LTACF) are not likely to be transferred to hospitals, either because they refuse to be transferred or because such transfer may be risky or complex. Under such circumstances, FB could be performed in both sites, especially in technically simple procedures or with a low complication rate. OBJECTIVES: The purpose of this study was to assess whether FB carried out at LTACF or in the home is safe, useful, well tolerated, and free of complications. PATIENTS AND METHODS: This study analyzed FB carried out at LTACF or in HH patients from October 2007 to November 2009. The procedures chosen were structural and functional assessment of the airway (SFAA) owing to potential tracheal stenosis, tracheomalacia, tracheo-esophageal fistula, phonation or swallowing disorders, and as earlier control to decannulation. Other techniques used were: aspiration of tracheobronchial mucus (ATBM), complex tracheostomy tube changes (TTC), tracheal prosthesis control (TPC), evaluation of mild hemoptysis (MH), and bronchoalveolar lavage (BAL). FB was carried out under local anesthesia. The endoscope entered through the nose, mouth, tracheostomy tube, and/or T-tracheal prosthesis. RESULTS: Thirty-three FB were carried out in 25 LTACF patients: 33 SFAA, 3 MH, 3 TTC, 2 ATBM, 1 BAL, and 2 TPC. Ten FB were carried out in 8 HH patients: 10 SFAA, 4 TTC, and 7 ATBM. Procedures were well-tolerated. There was no evidence of complications. CONCLUSIONS: FB carried out outside the hospital setting can be done safely and effectively, at least in a selected group of patients and carrying out selected procedures.

Smooth muscle neoplasms are more frequent in human immunodeficiency infected children than in HIV seropositive adults. Endobronchial leiomyoma is a rare benign tumor in HIV infected adult patients. Epstein-Barr virus (EBV) has been implicated in the pathogenesis of these tumors. Here we describe an adult patient with HIV infection with atelectasis of the left upper pulmonary lobe as the first clinical expression of an intrabronchial leiomyoma. In this case, we can not show the association with EBV. Our report suggests that smooth muscle tumors as leiomyoma should be included in the differential diagnosis of endobronchial masses in AIDS patients.

Neoplasmas da musculatura lisa são mais freqüentes em crianças infectadas pelo vírus da imunodeficiência humana do que em adultos HIV-soropositivos. Leiomioma endobronquial é um tumor benigno em pacientes adultos infectados por HIV. Vírus Epstein-Barr (EBV) tem sido implicado na patogenia destes tumores. Descrevemos paciente adulto infectado pelo HIV com atelectasia do lobo pulmonar superior esquerdo como primeira manifestação clínica de leiomioma intrabronquial. Neste caso não pudemos demonstrar a associação com EBV. Nosso relato sugere que tumores de musculatura lisa como leiomioma deveriam ser incluídos no diagnóstico diferencial de massas endobronquiais em pacientes com AIDS.
