A 67-year-old female presented to the emergency department with acute-onset severe left flank pain as well as nausea and vomiting. Physical examination was notable for left-sided abdominal, flank tenderness, and costovertebral angle tenderness. Laboratory workup revealed an elevated lactate of 9.2 mmol/L and elevated serum creatinine of 1.14 mg/dL, with an estimated glomerular filtration rate of 53 mL/minute/1.73m2. Urinalysis showed moderate leukocyte esterase with microscopy showing 12 white blood cells and three red blood cells per high-power field. CT of the abdomen and pelvis with intravenous contrast was notable for moderate amounts of left-sided perinephric and periureteric fluid without the presence of an obstructing calculus. Due to the amount of perinephric and periureteric fluid without associated nephrolithiasis, the differential diagnosis was broadened to include spontaneous ureter rupture as well as concern for malignancy. A delayed post-contrast CT scan of the abdomen and pelvis was obtained, which confirmed a spontaneous proximal and mid-ureter rupture. Spontaneous ureter rupture is a rare disease process with significant morbidity and mortality. It often poses a diagnostic dilemma due to a lack of clinical awareness and varied presentation. Diagnosis rests upon obtaining delayed post-contrast CT of the abdomen and pelvis. Currently, there are no standardized treatment guidelines, although most experts utilize minimally invasive endourological approaches in their treatment plans.

A 22-year-old female patient was seen in the emergency department with a two-week onset of progressively worsening pain and swelling to the medial aspect of her distal left femur. The patient was involved in an automobile versus pedestrian accident two months prior sustaining superficial swelling, tenderness, and bruising to the affected area. Radiographs revealed soft tissue swelling without osseous abnormalities. Examination of the distal femur region revealed a large, tender, ovoid area of fluctuance with a dark crusted lesion and surrounding erythema. Bedside ultrasonography revealed a large anechoic fluid collection in the deep subcutaneous plane with mobile internal echogenic debris which was suspicious for a Morel-Lavallee lesion. The patient underwent contrast enhanced CT of the affected lower extremity demonstrating a 8.7 cm x 4.1 cm x 11.1 cm fluid collection superficial to the deep fascia of the distal posteromedial left femur, confirming the diagnosis of a Morel-Lavallee lesion. A Morel-Lavallee lesion is a rare, post-traumatic degloving injury that results in the separation of the skin and subcutaneous tissues from the underlying fascial plane. The resultant disruption of the lymphatic vessels and underlying vasculature leads to progressively worsening hemolymph accumulation. If not recognized and treated in the acute or subacute period, complications can ensue. Complications of Morel-Lavallee include recurrence, infection, skin necrosis, neurovascular injury, as well as chronic pain. Treatment is based on the size of the lesion and ranges from conservative management and surveillance for small lesions to percutaneous drainage as well as debridement, sclerosing agents, and surgical fascial fenestration approaches for larger lesions. Additionally, the utilization of point-of-care ultrasonography can help in the early identification of this disease process. This is important as a delay in diagnosis and subsequent treatment of this disease state is associated with long-term complications.

A 59-year-old female presented to the emergency department with an acute onset of unilateral facial droop and aphasia. Collateral information obtained from her family revealed a diagnosis of prolonged subacute bacterial sinusitis with initiation of multiple antimicrobial regimens. The patient underwent a non-contrast computed tomography of the brain, which demonstrated a 1.2-cm lobulated extra-axial collection overlying the left frontotemporal region with an associated midline shift consistent with subdural empyema. The patient was initiated on intravenous antibiotics and underwent emergent burr hole placement with successful evacuation of the subdural empyema. Additionally, the patient's hospital course consisted of a bilateral maxillary antrostomy, bilateral total sphenoethmoidectomy, and bilateral frontal sinusotomy, septoplasty, as and excision of her right concha bullosa. Subdural empyema is a pyogenic infection of the subdural space located between the dura mater and the arachnoid matter, with high rates of morbidity and mortality. Treatment is based on rapid identification and treatment of the underlying causative factor as well as surgical evacuation to obtain source control.

A 60-year-old male patient presented to the emergency department of our hospital with right-sided chest wall pain and a palpable subcutaneous cord-like structure along the right anterior chest wall. Examination revealed tenderness over the cord-like structure, and the skin overlying the structure was freely mobile and did not have any sign of infection or inflammation. Bedside ultrasonography revealed an uncompressible tubular structure with the absence of a color Doppler flow signal. The patient's presentation was suggestive of Mondor's disease. The patient was discharged with instructions to utilize anti-inflammatory drugs, perform warm compresses, and seek primary care follow-up to ensure resolution. Mondor's disease is a rare disorder characterized by a superficial thrombophlebitis of the subcutaneous veins of the chest wall. For its diagnosis, a thorough examination of the patient's medical history and physical condition is suggested; further, the performance of point-of-care ultrasonography has also been suggested. Once recognized, further emergency department workup is typically unnecessary in cases of primary Mondor's disease. Despite being a mostly self-limited condition, greater awareness of this rare disease entity is required to ensure and coordinate close outpatient follow-up as well as monitor resolution due to its association with secondary causes such as vascular and breast carcinoma, vasculitis, and hypercoagulable disorders.

A 12-year-old female with primary amenorrhea presented to the emergency department with episodic low back pain, suprapubic discomfort, and acute urinary retention. A protruding purple mass was noted at the vaginal introitus consistent with an imperforate hymen. Point-of-care bedside transabdominal ultrasonography revealed a distended uterus containing hypoechoic material. The patient underwent formal pelvic ultrasonography, which revealed a markedly enlarged uterus containing a large number of blood products, thinned myometrium, and a distended vaginal canal consistent with hematometrocolpos secondary to imperforate hymen. Imperforate hymen is a rare congenital anomaly of the female urogenital tract, in which the hymen obstructs the vaginal opening causing a vaginal outlet obstruction. Vaginal outlet obstruction secondary to imperforate hymen may lead to retrograde menstruation with a collection of blood within the uterus and vagina, which is termed hematometrocolpos. Treatment is based on identifying and treating the underlying imperforate hymen with surgical approaches. The growing use of bedside ultrasonography allows the clinician to rapidly and accurately diagnose hematometrocolpos. The use of point-of-care bedside ultrasonography can serve as an essential tool as delayed diagnosis and treatment of this rare condition are associated with significant morbidity and lifelong infertility.

In this case report, we present a case of a woman in her late 30s who presented with bilateral motor weakness, headache, hyperglycaemia and hypertension. Investigations revealed very high 24-hour urine cortisol of 90 524, ectopic adrenocorticotropic hormone secreting pheochromocytoma, normal metanephrine levels and empty sella, which has never been reported. CT chest and abdomen revealed enlarged left adrenal gland with large necrotic mass 4.7×4.0 cm most likely neoplastic lesion. The patient was cured after surgical removal of the left adrenal mass. Histological examination revealed 3.5×2.5×2 cm mass consistent with pheochromocytoma. After surgery and a short stay in the intensive care unit, the patient started improving and both antihypertensive treatment and insulin injections were withdrawn.

Vehicle make and model recognition plays an important role in monitoring traffic in a vehicle surveillance system. Identifying vehicle make and model is a challenging task due to intraclass variation, view-point variation, and different illumination conditions (Hassan et al., 2021). In this domain, many datasets regarding car make and model e.g. Stanford Car (Krause et al., 2013), VMMRdB (Tafazzoli et al., 2017, Yang et al., 2015), have already been experimented with by different researchers. However, most of the images in these datasets are high-quality images with no illumination conditions. Further, these images are collected through web crawling or image scraping. This enabled the researchers to achieve good results using deep learning models (Luo et al., 2015). In this article, we have presented an image dataset of 3847 images, designed from high-resolution (1920 1080) videos collected from camera units installed on a highway at different viewpoints with variable frame rates. This helped in collecting images demonstrating a real-world scenario and made this dataset more challenging. Due to consideration of different viewpoints and illumination effects, the dataset will help researchers to evaluate their machine learning models on realworld data (Manzoor et al., 2019).

OBJECTIVE: To determine the experience at the Aga Khan University Hospital in diagnosing and treating adult patients with primary lymphoma of bone. METHODOLOGY: All patients with Primary lymphoma of bone (PLB) that were diagnosed and/or treated at Aga Khan University Hospital, Karachi from 2005 to 2019 were included as part of this study. RESULTS: There were 17 patients with PLB including 13 (76.5%) males and 4 (23.5%) females with a mean age of 44 ± 16.5 years. Nine patients were between 30-59 years of age at diagnosis. The mean follow-up time of patients was 80±46.7 months. Six patients had tumours of pelvic bone followed by tibia (5) and femur (4). Four patients had a pathological fracture at the time of presentation whereas 2 (11.8%) required surgical fixation of the pathological fracture. The stage of the tumour was based on Ann Arbor classification. Nine (52.9%) cases had Stage 1 disease, 7 (41.2%) had stage IV disease with metastasis in extra nodal tissues. As for treatment, every patient received chemotherapy whereas 5 (29.4%) received adjuvant radiotherapy. Complete remission in the size of the tumour was seen in 11 (64.7%) patients while 6 (35.3%) had partial remission. Post-treatment, 4 (23.5%) patients expired. The mean Overall Survival (OS) time was 80.18 ± 46.71months with a survival rate of 76.5. CONCLUSIONS: Primary lymphoma of the bone can be treated with medical regime and good prophylactic surgeries to avoid pathological fracture such as intramedullary nailing.

OBJECTIVE: To determine the functional outcomes in total hip arthroplasty with a dual mobility cup, performed in our hospital. METHODS: After receiving an exemption from the Ethics review committee of the hospital, data collection for audit was started in January 2019. Records from July 2016 to June 2018 were included. All patients who underwent total hip arthroplasty with dual mobility prosthesis without any age limit were included. A proforma was prepared to collect the required information. Data was entered and analyzed on SPSS v. 21. RESULTS: Two hundred and ten patients were included, 114 females and 96 males. Of the total, 188 patients underwent unilateral surgery while 22 had bilateral hip arthroplasty. The mean postoperative hospital stay was 5.91±3.9 days. . Mean pre-op Harris score was 33.7±7.6 and the post-op mean score was 75.9± 5.34. Eighty-three (39.5 %) patients had the neck of femur fracture, 31(14.8%) had osteoarthritis while 28(13.3%) had avascular necrosis. Post-surgery complications included, wound infection, surgical site haematoma, NSTEMI, and only one patient reported dislocation after use of dual mobility cup. CONCLUSIONS: The dislocation rate which was the prime concern, has been reduced with the use of dual mobility implant in total hip arthroplasty patients.

OBJECTIVE: Verbal autopsy is a technique used to collect information about a decedent from his/her family members using questionnaires, conducting interviews, making observations, and sampling. In substantial parts of the world, particularly in Africa and Asia, many deaths are unrecorded. In 2017, globally pregnant women were dying daily around 810 and 295,000 in a year because of pregnancy-related problems, pointed out by World Health Organization. Identifying the cause of a death is a complex process which requires in-depth medical knowledge and practical experience. Generally, medical practitioners possess different knowledge levels, set of abilities, and problem-solving skills. Additionally, the medical negligence plays a significant part in further worsening the situation. Accurate identification of the cause of death can help a government to take strategic measures to focus on, particularly increasing the death rate in a specific region. METHODS: This research provides a solution by introducing a semantic-based verbal autopsy framework for maternal death (SVAF-MD) to identify the cause of death. The proposed framework consists of four main components as follows: (1) clinical practice guidelines, (2) knowledge collection, (3) knowledge modeling, and (4) knowledge codification. Maternal ontology for the framework is developed using Protégé knowledge editor. Resource description framework application programming interface (API) for PHP (RAP) is used as a Semantic Web toolkit along with Simple Protocol and RDF Query Language (SPARQL) is used for querying with ontology to retrieve data. RESULTS: The results show that 92% of maternal causes of deaths assigned using SVAF-MD correctly matched manual reports already prepared by gynecologists. CONCLUSION: SVAF-MD, a semantic-based framework for the verbal autopsy of maternal deaths, assigns the cause of death with minimum involvement of medical practitioners. This research helps the government to ease down the verbal autopsy process, overcome the delays in reporting, and facilitate in terms of accurate results to devise the policies to reduce the maternal mortality.

The glenohumeral joint is the most mobile in humans. Of all the joints 50% of dislocations involve the shoulder, mostly young males. When the first dislocation occurs in a patient under 20 years age the risk for recurrent instability increases to 90%. Many techniques are available to reduce and stabilise the glenohumeral joint; in cases of anterior dislocations one of which is the famous Bristow's procedure, originally described in 1954 by Laterjet. The purpose of this study is was to determine the Functional Outcomes of the modified Bristow procedure. This retrospective review was conducted at Aga Khan University Hospital, Karachi from January 2000- December 2015, comprising patients who underwent the modified Bristow procedure. All patients recruited in the study underwent modified Bristow procedure. A total of 70 patients were included, which comprised of 61(87.1%) males and 9(12.9%) females with a mean age of 31.6±11.0 years The maximum number of shoulder dislocations occurred primarily due to road traffic accidents in 48 (68.57%) patients while the second highest cause in 13 (18.57) patients was due to playing sports. The mean number of dislocations before surgery were 3.50±0.5 whereas no patient had an episode of dislocation in the post-operative period. Two patients presented with subluxations but none required further surgical intervention. The Modified Bristow-Latarjet procedure is considered an effective surgical treatment for the recurrent glenohumeral instability of the joint.

CASE PRESENTATION: A 48-year-old-female presented to the emergency department with dislodgement of her percutaneous endoscopic gastrostomy (PEG) tube, necessitating bedside replacement. Replacement was done without difficulty and gastrografin radiography was obtained to confirm positioning. Radiography revealed contrast filling the colon at the splenic flexure and proximal descending colon suggestive of colocutaneous fistula formation. DISCUSSION: The patient required hospitalization with surgical consultation, initiation of parenteral nutrition, and conservative management of the fistula with surgical replacement of the PEG tube. Although rare, it is paramount for the emergency physician to be aware of this complication when undertaking bedside replacement of PEG tubes.

The spread of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has infected more than 6 million people globally; and changed the dynamics of the entire world. In March 2020, novel coronavirus disease 2019 (COVID-19) was declared as a pandemic by the World Health Organization (WHO). In Pakistan, the first case of COVID-19 was reported on 26th February, 2020. The virus is highly contagious and spreads mainly by aerosols. Its major symptoms include sore throat, cough, high-grade fever, anosmia, etc. Like other countries, it has also affected all fields of life in Pakistan, but its impact on dental education and dentistry has been particularly devastating. Dental institutes are at particularly high risk for infection transmission through their aerosol-generating dental procedures. In the scenario of a lockdown, dental students are suffering a lot and the dental colleges and faculties must try their best to overcome this situation. This article aims to review the challenges faced by dental community to carryout teaching and training in dental education during the COVID-19 pandemic. INTRODUCTION: Respiratory viral illnesses are associated with diverse neurological complications, including acute transverse myelitis (ATM). Among the respiratory viral pathogens, the Coronaviridae family and its genera coronaviruses have been implicated as having neurotropic and neuroinvasive capabilities in human hosts. Despite previous strains of coronaviruses exhibiting neurotropic and neuroinvasive capabilities, little is known about the novel severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) and its involvement with the central nervous system (CNS). The current pandemic has highlighted the diverse clinical presentation of SARS-CoV-2 including a possible link to CNS manifestation with disease processes such as Guillain-Barré syndrome and cerebrovascular disease. It is critical to shed light on the varied neurological manifestation of SARS-CoV-2 to ensure clinicians do not overlook at-risk patient populations and are able to provide targeted therapies appropriately. CASE REPORT: While there are currently no published reports on post-infectious ATM secondary to SARS-CoV-2, there is one report of parainfectious ATM attributed to SARS-CoV-2 in pre-print. Here, we present a case of infectious ATM attributed to SARS-CoV-2 in a 24-year-old male who presented with bilateral lower-extremity weakness and overflow urinary incontinence after confirmed SARS-CoV-2 infection. Magnetic resonance imaging revealed non-enhancing T2-weighted hyperintense signal abnormalities spanning from the seventh through the twelfth thoracic level consistent with acute myelitis. CONCLUSION: The patient underwent further workup and treatment with intravenous corticosteroids with improvement of symptoms and a discharge diagnosis of ATM secondary to SARS-CoV-2.

BACKGROUND: Pakistan is fifth among high burden countries for tuberculosis. A steady increase is seen in extrapulmonary tuberculosis (EPTB), which now accounts for 20% of all notified TB cases. There is very limited information on the epidemiology of EPTB. This study was performed with the aim to describe the demographic characteristics, clinical manifestations and treatment outcomes of EPTB patients in Pakistan. METHOD: We performed descriptive analysis on routinely collected data for cohorts of TB patients registered nationwide in 2016 at health facilities selected using stratified convenient sampling. FINDINGS: Altogether 54092 TB including 15790 (29.2%) EPTB cases were registered in 2016 at 50 study sites. The median age was 24 years for EPTB and 30 years for PTB patients. The crude prevalence of EPTB in females was 30.5% (95%CI; 29.9-31.0) compared to 27.9% (95%CI; 27.3-28.4) in males. The likelihood of having EPTB (OR), was 1.1 times greater for females, 2.0 times for children, and 3.3 times for residents of provinces in the North-West. The most common forms of EPTB were pleural (29.6%), lymphatic (22.7%) and abdominal TB (21.0%). Pleural TB was the most common clinical manifestation in adults (34.2%) and abdominal TB in children (38.4%). An increase in the prevalence of pleural and osteoarticular and decline in lymphatic and abdominal TB was observed with advancing age. Diversity in demography and clinical manifestations were noted between provinces. The treatment success rate for all type EPTB was significantly high compared to bacteriology confirmed PTB with the exception of EPTB affecting CNS with a high mortality rate. CONCLUSIONS: The study provides an insight into demography, clinical manifestations and treatment outcomes of EPTB. Further studies are needed to explain significant diversities observed between provinces, specific risk factors and challenges concerning EPTB management.

A 21-year old female presented to the emergency department with an abnormal appearance of her left eye after trivial trauma the previous day. A protruding, opaque appearing area was noted on the left cornea. The patient underwent visual acuity testing, fluorescein staining with slit lamp examination revealing a conical corneal protrusion with corneal stromal edema suggestive of corneal hydrops. Corneal hydrops is a rare complication of keratoconus and various corneal ectasias where aqueous humor can leak into the corneal stroma causing edema and protruding pockets of fluid. Treatment is based on treating the underlying keratoconus or corneal ectasia in addition to cycloplegics, topical corticosteroids, topical antibiotics, and topical hypertonic saline.

A 47-year-old male presented to the emergency department with 12 hours of nausea, vomiting, abdominal pain, and a widespread skin eruption with mottled, irregular, purpuric lesions with subsequent rapid decompensation. Laboratory analysis revealed thrombocytopenia, bandemia, elevated metamyelocytes, abnormal coagulation panel, decreased fibrinogen, elevated fibrin split products, renal dysfunction, bacterial rods, dohle bodies, and toxic granulation. Acute promyelocytic leukemia (APML) was confirmed via bone marrow biopsy, flow cytometry, and fluorescence in situ hybridization analysis. Disseminated intravascular coagulation (DIC) may be the initial presentation of APML. When treated promptly, APML can achieve high remission rates; however, conditions such as DIC continue to increase mortality requiring early recognition to improve survival rates.

A 29-year-old male presented to our emergency department with complaint of abdominal pain after allegedly ingesting a 4-gram packet of heroin in an attempt to evade detection. Initial evaluation including computed tomography (CT) of the abdomen/pelvis with intravenous and oral contrast, as well as laboratory workup was negative and the patient was discharged. The patient returned 3 days later with complaint of "I feel high" and severe constipation, and demonstrated an opiate toxidrome requiring naloxone with improvement of symptoms. A repeat CT of the abdomen/pelvis, this time without contrast revealed a 2.1 × 1.8 cm foreign body in the gastric antrum. The patient was promptly taken to endoscopy with surgical backup. Foreign body removal included multiple plastic bags encasing heroin, which had sustained a small leak causing a gastric outlet obstruction as well as a slow opiate toxidrome. The foreign body was removed and the patient was observed and discharged with a favorable outcome.

BACKGROUND: The reduction and control over neonatal, infant, and maternal mortality is a collective mission of the World Health Organization under United Nations. METHODS: This article summarizes the automation of verbal autopsy reporting for neonatal, infant, and maternal mortality with primary focus on user-centered design for technologically illiterate workforce with minimum available resources. The diminution in neonatal, infant, and maternal deaths is not possible until grassroot level quality data are available for mortality. The estimated data are less effective for developing countries like Pakistan because it has heterogeneous demographic pockets with respect to mortality causes. The Neonatal, Infant, and Maternal Death E-surveillance System is a project in which a real-time reporting system is innovated that is useful in detecting the causes of mortality and effective in adopting appropriate countermeasure policies. In a pilot study, the system was implemented initially in nine districts of Punjab, Pakistan. The initial system was refined after getting detailed feedback from district management staff including Lady Health Workers and Lady Health Supervisors. The refined surveillance system was finally implemented in all 36 districts of Punjab, Pakistan. RESULTS: The results exhibited 31% improvement in infant data collection and 6% improvement in maternal data collection regarding mortality. CONCLUSION: This research will be helpful in achieving the milestone of gathering real-time mortality data from grassroot level using user-centered design methodology.
