RESUMO

AIM: To evaluate the efficacy and safety of renal artery embolisation (RAE) for renal arteriovenous malformation (AVM) as well as its outcomes. MATERIALS AND METHODS: The technical and clinical success rates, radiological and laboratory findings, and complications of RAE for 31 renal AVMs in 24 patients (M:F=9:15, mean age 46 years) at two separate medical institutions were retrospectively evaluated. Technical success was defined as complete occlusion of feeding arteries with no residual nidus seen on post-treatment angiography. Clinical failure was defined as recurrence of haematuria, presence of AVM on follow-up ultrasound or computed tomography, repeated RAE or surgery for the control of haematuria. Overall clinical success was defined as resolution haematuria or disappearance of AVM on follow-up imaging after single or multiple sessions of RAE. RESULTS: Types of renal AVM were AVM, arterio-venous fistula (AVF) with intranidal aneurysm, and acquired AVF in 19, 1, and 4 patients, respectively. 18 patients (75%) underwent a single session of RAE, while 6 patients (25%) had two or more sessions of RAE. The level of embolisation was feeder, segmental artery, and main renal artery in 28 (90%), 2 (6%), and 1 (4%) procedures, respectively. Coil, n-butyl 2-cyanoacrylate, and polyvinyl alcohol were the most frequently chosen embolic materials and were used in 19, 14, and 8 procedures, respectively. The clinical success rate after initial RAE was 67% (16/24). Overall clinical success rate, including multisession RAE, was 88% (21/24). The technical success rate of 31 procedures was 65% (20/31). Among 11 technical failures in 10 patients, 4 achieved clinical success without additional RAE, 3 underwent second session RAE to achieve clinical success, and 3 patients underwent nephrectomy due to recurrence. CONCLUSION: RAE is a safe and effective treatment for renal AVM. Technical failure of RAE does not always lead to clinical failure and multiple embolisation sessions may be effective for recurrent renal AVM.

BACKGROUND AND PURPOSE: Diagnostic test accuracy studies for ultrasonography-guided fine-needle aspiration and ultrasonography-guided core needle biopsy have shown inconclusive results due to their heterogenous study designs. Our aim was to compare the diagnostic accuracy of ultrasonography-guided fine-needle aspiration versus ultrasonography-guided core needle biopsy for detecting malignant tumors of the salivary gland and for the tissue-specific diagnosis of salivary gland tumors in a single tertiary hospital. MATERIALS AND METHODS: This retrospective study was approved by our institutional review board and informed consent was waived. Four hundred twelve patients who underwent ultrasonography-guided fine-needle aspiration (n = 155) or ultrasonography-guided core needle biopsy (n = 257) with subsequent surgical confirmation or clinical follow-up were enrolled. We compared the diagnostic accuracy of ultrasonography-guided fine-needle aspiration and ultrasonography-guided core needle biopsy regarding malignant salivary gland tumors and the correct tissue-specific diagnosis of benign and malignant tumors. We also tested the difference between these procedures according to the operator's experience and lesion characteristics. RESULTS: The inconclusive rates of ultrasonography-guided fine-needle aspiration and ultrasonography-guided core needle biopsy were 19% and 4%, respectively (P < .001). The overall accuracy of ultrasonography-guided core needle biopsy for diagnosing malignant tumors was significantly higher than that of ultrasonography-guided fine-needle aspiration (P = .024). The correct tissue-specific diagnosis rates of ultrasonography-guided fine-needle aspiration and ultrasonography-guided core needle biopsy were 95% versus 97% for benign tumors (P = .648) and 67% versus 80% for malignant tumors (P = .310). Trainees showed significantly lower accuracy with ultrasonography-guided fine-needle aspiration than with ultrasonography-guided core needle biopsy for diagnosing malignant tumors (P = .021). There was no difference between the diagnostic accuracy of ultrasonography-guided fine-needle aspiration and ultrasonography-guided core needle biopsy according to the internal composition of the lesions. There were no complications requiring intervention or hospitalization in our patients. CONCLUSIONS: Ultrasonography-guided core needle biopsy is superior to ultrasonography-guided fine-needle aspiration in detecting and characterizing malignant tumors of the salivary gland and could emerge as the diagnostic method of choice for patients presenting with a salivary gland mass.

AIMS: To establish an efficient genetic transformation protocol for Leuconostoc species, methods for competent-cell preparation and electroporation conditions were optimized. METHODS AND RESULTS: Leuconostoc mesenteroides subsp. mesenteroides ATCC8293 cells were sequentially treated with penicillin G and lysozyme, and the plasmid pLeuCM was subsequently transformed into the cells. Our results demonstrated that transformation efficiencies were significantly increased (100-fold), and increased electric field strength also contributed to enhance transformation efficiency. Maximum transformation efficiency (1×10(4) or more transformants per µg DNA) was achieved when cells were grown in De Man, Rogosa, Sharpe (MRS) media containing 0·25 mol l(-1) sucrose and 0·8 µg ml(-1) penicillin G, followed by treatment with 600 U ml(-1) lysozyme and electroporation at a field strength of 10 kV cm(-1). When this protocol was used to transform pLeuCM into Leuc. mesenteroides, Leuconostoc gelidum, Leuconostoc fallax and Leuconostoc argentinun, successful transformations were obtained in all cases. Furthermore, this procedure was applicable to species belonging to other genera, including Lactobacillus plantarum, Pediococcus pentosaceus and Weissella confusa. CONCLUSIONS: The results demonstrate that the transformation efficiency for Leuconostoc spp. could be increased via optimization of the entire electroporation procedures. SIGNIFICANCE AND IMPACT OF THE STUDY: These optimized conditions can be used for the extensive genetic study and the metabolic engineering of not only Leuconostoc spp. but also different species of lactic acid bacteria.

RESUMO

Scattering of an obliquely incident plane wave by a general-shaped groove engraved on a perfectly conducting plane is rigorously solved. The scattered field is represented by a Fourier-integral representation. To analytically represent the fields in a general-shaped groove, the groove is divided into L number of layers. Fields are then expressed in each layer as summations of 2D spatial harmonic fields with unknown coefficients. Matching the boundary conditions between layers provides a linear set of equations connecting all the unknown harmonic coefficients. Judicious use of Fourier transform on the equations resulting from matching boundary conditions at the groove aperture provides a series representation of the scattered field in the spectral domain with unknown harmonic coefficients of the first layer in the groove. A stable solution is obtained by solving the complete system of equations with an adaptive choice for the number of modes in each layer.

The purpose of this study was to test the hypothesized factor structure of the Group Environment Questionnaire (GEQ) with confirmatory factor analysis and to ascertain the degree of factorial invariance across gender and across type of sport (team, individual). Additionally, the underlying theoretical structure on which the GEQ was developed was examined by fitting the data to alternative hierarchical factor structures. These models were tested with GEQ data collected on 740 high school varsity athletes (426 males, 314 females). It was shown that the males and females had different factor structures and that neither group exhibited the hypothesized structure. Attempts to fit the data to a two-factor and a second-order hierarchical factor model were unsuccessful. Finally, exploratory factor analysis did not produce a satisfactory four-factor solution. It was concluded that the data from this sample do not support the hypothesized four-factor structure of the GEQ and that further examination of the factor structure of this instrument is required.

The purpose of this study was to develop and test a method to analyze and evaluate sequential skill performances in a team sport. An on-line computerized system was developed to record and summarize the sequential skill performances in volleyball. Seventy-two sample games from the third Federation of International Volleyball Cup men's competition were videotaped and grouped into two categories according to the final team standing and game outcome. Log-linear procedures were used to investigate the nature and degree of the relationship in the first-order (pass-to-set, set-to-spike) and second-order (pass-to-spike) transition plays. Results showed that there was a significant dependency in both the first-order and second-order transition plays, indicating that the outcome of a skill performance is highly influenced by the quality of a preceding skill performance. In addition, the pattern of the transition plays was stable and consistent, regardless of the classification status: Game Outcome, Team Standing, or Transition Process. The methodology and subsequent results provide valuable aids for a thorough understanding of the characteristics of transition plays in volleyball. In addition, the concept of sequential performance analysis may serve as an example for sport scientists in investigating probabilistic patterns of motor performance.

The purpose of this study was to investigate the playing characteristics of team performance in international men's volleyball. The specific purposes were (a) to examine differences in playing characteristics (in particular, the set and spike) between the Attack Process and the Counterattack Process; (b) to examine changes in playing characteristics as a function of team success (as indicated by single-game outcomes and by final tournament standings); and (c) to determine the best predictor, or a set of predictors, of team success among the selected skill components. Seventy-two sample games from the Third Federation of International Volleyball Cup men's competition were recorded using a computerized recording system. Results showed that the significant differences between Team Standing and Game Outcome were due to better performances on those skills used in the Counterattack Process. Among the eight selected skills, the block and spike were the most important in determining team success. The methodology used in this study and the subsequent results provide valuable aids for the coach in the evaluation of team performance and ultimately in the preparation of training sessions in volleyball.
