Statement of the Problem: Dentin margins have a higher microleakage than enamel in composite restorations due to weaker bond durability. Clinically, most margins are located apical to cementoenamel junction on the dentin or cementum. Different surface preparation methods may have dissimilar effects on the shear bond strength of resin-modified glass ionomer (RMGI) to composite resin, regarding the layering technique employed in restoration of these cavities. Purpose: This study aimed to investigate the shear bond strength of RMGI to composite resin using different mechanical surface preparation methods. Materials and Method: In this in vitro study, 72 RMGI samples were prepared in six groups (n=12): control, acid etching, air abrasion with aluminum oxide (Al2O3), bur, Er: YAG laser, and Er, Cr: YSGG laser groups. Two samples from each group were randomly examined under a scanning electron microscope (SEM). A bonding layer and composite resin were applied and cured. Then the samples were subjected to 5000-cycles thermocycling procedure. The shear bond strength was then evaluated using a universal testing machine. Finally, the failed surface of the samples was evaluated under a light microscope to examine the failure mode. For the statistical analysis, one-way ANOVA and Games-Howell post hoc test was used to compare quantitative variables and chi-squared test to investigate the relationship between the failure mode and the groups. The significant level was set at 0.05. Results: The results showed significant differences between the mean of the strength variable in the groups (p< .001). The acid-etch and air abrasion groups exhibited significantly the lowest and highest values, respectively. The chi-squared test results also showed a significant relationship between the failure mode in the study groups (p= .008). Conclusion: Acid etching before completing RMGI polymerization could adversely affect shear bond strength. The air abrasion and bur methods are among the best methods to increase the shear bond strength between composite resin and RMGI. Using Er, Cr: YSGG and Er: YAG lasers could increase the bond strength results.

Objectives: The present study aimed to investigate the effect of preheating on the color stability and surface roughness of a silorane-based composite resin. Materials and Methods: A total of 44 Filtek P90 composite resin disks, (10mm×1mm), were fabricated using plastic molds and were divided into two groups. In group 2, the composite resin syringes were placed in a thermostatically controlled water bath at 55‒60°C before preparing the disks. After polishing the samples with silicon carbide papers, they were stored in distilled water for 48 hours. Roughness and color parameters were then measured in two stages: immediately after retrieval from distilled water and after 40 days of storage in tea solution. Finally, the differences in roughness and color parameters were recorded. Independent sample t-test and regression analysis were used at a significance level of P<0.05. Results: Based on the findings of the present study, there was no significant difference among the mean ∆E values (P=0.4); however, a significant difference in mean surface roughness (P=0.01) was found between the two groups. Regression analysis showed a significant relationship between the study groups in terms of surface roughness and ΔE values​ (preheated: r2 = 0.73; non-preheated: r2= 0.76). Conclusion: Filtek P90 silorane-base composite showed ΔE>3.3 and surface roughness above 0.2µ under preheated and non-preheated conditions and discoloration increased following preheating.

In this work, through a combination of photoluminescence spectroscopy, X-ray powder diffraction and magnetic measurements, it is determined that ZnO rods, made hydrothermally using a combination of magnetic field with respect to the force of gravity, exhibit superparamagnetic properties which emerge from Zn defects. These Zn defects result in a size-dependent superparamagnetic property of the rods. Red emissions, characteristic of Zn vacancies, and magnetic susceptibility both increased with decreasing rod size. The ZnO rods have significantly larger superparamagnetic cluster sizes (one order of magnitude) and lower fluctuation rates when compared to other superparamagnetic particles.

OBJECTIVES: The aim of this study was to compare the microleakage in normal and caries-affected dentin (CAD) and to investigate the effect of three hemostatic agents on the microleakage of Class V composite resin restorations in CAD. MATERIALS AND METHODS: Ninety-six Class V non-beveled cavities were prepared in 48 third molars at 1 mm below the cementoenamel junction (CEJ) in the cervical margin with the occlusogingival size of 2 mm, mesiodistal dimension of 3 mm, and a depth of 1.5 mm. Next, the teeth were divided into 8 groups (n=12): G1-4 included normal dentin (N) substrate, while G5-8 were exposed to mineralization/demineralization cycles to produce CAD substrate. Groups 1 and 5 were the controls. ViscoStat was used in groups 2 and 6, ViscoStat Clear was used in groups 3 and 7, while trichloroacetic acid (TCA) was used in groups 4 and 8. The cavities were restored with composite resin. The samples were sectioned after thermocycling and immersion in 2% fuchsin for 24 hours. The degree of dye penetration was evaluated under a stereomicroscope at 40× magnification. Data were evaluated using Kruskal-Wallis and Mann-U-Whitney tests in SPSS 15 software (α=0.05). RESULTS: Significant differences were recorded on the mean microleakage of different groups (P=0.047). There was a significant difference in the mean dentinal microleakage between N and CAD groups (P=0.014). The dentinal microleakage in group 3 was significantly higher than that in groups 4 to 8. CONCLUSIONS: According to the results, CAD showed less microleakage in comparison with intact dentin. ViscoStat Clear caused a greater microleakage than did ViscoStat or TCA.

Air pollution contains a complex mixture of poisonous compounds including particulate matter (PM) which has wide spectrum of adverse health effects. The main purpose of this study was to estimate the potential health impacts or benefits due to any changes in annual PM10 level in four major megacities of Iran. The required data of PM10 for AirQ software was collected from air quality monitoring stations in four megacities of Iran. The preprocessing was carried out using macro coding in excel environment. The relationship between different presumptive scenarios and health impacts was determined. We also assessed the health benefits of reducing PM10 to WHO Air Quality Guidelines (WHO-AQGs) and National Ambient Air Quality Standards (NAAQSs) levels with regard to the rate of mortality and morbidity in studied cities. We found that the 10 µg/m3 increase in annual PM10 concentration is responsible for seven (95% CI 6-8) cases increase in total number of deaths per 2 × 105 person. We also found that 10.7, 7.2, 5.7, and 5.3% of total death is attributable to short-term exposure to air pollution for Ahvaz, Isfahan, Shiraz, and Tehran, respectively. We found that by attaining the WHO's proposed value for PM10, the potential health benefits of 89, 84, 79, and 78% were obtained in Ahvaz, Isfahan, Shiraz, and Tehran, respectively. The results also indicated that 27, 10, 3, and 1% of health impacts were attributed to dust storm days for Ahvaz, Isfahan, Shiraz, and Tehran, respectively.

OBJECTIVE: Resistance to anti-tuberculosis (anti-TB) drugs is becoming a major and alarming threat in most regions worldwide. METHODS: This was a descriptive cross-sectional study at a tertiary hospital in Iran, using patient medical records for 2000-2003. The findings were analyzed following the same framework as that used for previous reports from this center. RESULTS: Among 1556 TB patients, drug susceptibility testing (DST) was performed for 548 culture-positive cases. Anti-TB drug resistance to both isoniazid and rifampin was identified in 10 (2.8%) of the new TB cases (multidrug-resistant TB; MDR-TB). Any resistance was detected in 228 (41.6%), showing an increasing trend in both new and retreatment cases. The data analysis revealed that drug-resistant TB had a statistically significant association with Afghan ethnicity, age>65 years, and the type of disease (retreatment vs. new TB case) (p<0.05). Also, assessment of the drug resistance trends showed a significant increase in resistance to any anti-TB agent, to isoniazid, and to streptomycin in new cases, and to all of the first-line anti-TB drugs in retreatment patients. CONCLUSIONS: There has been an increasing trend in drug resistance in recent years, particularly in retreatment cases. Hence, revision of the national TB control program, reevaluation of the role of the World Health Organization category II (CAT II) regimen, as well as the conducting of a nationwide drug resistance survey, are recommended.
