RESUMO

The management of tumour lysis syndrome remains problematic despite the rigorous use of preventative measures. Continuous arteriovenous haemofiltration (CAVH) is well suited to its use in both the prevention and treatment of metabolic abnormalities and renal insufficiency associated with tumour lysis. We report the successful use of CAVH in the treatment of a patient with tumour lysis syndrome.

RESUMO

Forty seven totally implantable venous access systems have been inserted in 45 patients for the management of malignant disease (n = 29), haematological disorders (n = 5), and cystic fibrosis (n = 11). We report our experience with them and discuss the problems we have encountered during the past three years.

RESUMO

Two children previously treated for Hodgkin's disease had relapse associated with anaemia and weight loss but no other abnormal clinical findings. It was not possible to confirm the diagnosis by bone marrow aspirate or trephine and in each case laparotomy was required in order to establish a histological diagnosis.

A double blind, placebo controlled trial was performed in nine sexually transmitted diseases (STD) clinics in the United Kingdom and the Netherlands to investigate the efficacy of trisodium phosphonoformate (foscarnet) cream in treating recurrent genital herpes simplex virus (HSV) infection. The study group comprised 145 male and 85 female patients. Men received 0.3% foscarnet cream and women 1% foscarnet cream for five days. The difference in time to healing between patients receiving foscarnet or placebo was not significant. Fewer patients treated with foscarnet had positive viral cultures after treatment, but the difference was not significant. The development of new lesions, however, was significantly less common in patients given foscarnet. Though topical foscarnet is a safe drug, no appreciable efficacy in treating recurrent genital HSV infection could be shown.

RESUMO

The results of treatment are reviewed in 18 cases of childhood nasopharyngeal carcinoma. Since 1976 adjuvant chemotherapy with cyclophosphamide, methotrexate, and adriamycin has been used in Manchester and Leeds. The outcome is compared in 9 patients treated with radiotherapy alone and 9 patients treated later with both radiotherapy and adjuvant chemotherapy. Relapse-free survival rates were 0% for those treated with radiotherapy alone and 78% at 13-80 months after diagnosis for those treated with both radiotherapy and chemotherapy. It is concluded that adjuvant chemotherapy improves relapse-free survival in childhood nasopharyngeal carcinoma.

RESUMO

Surveys were conducted on three traffic-less islands: Tory and Aran, off the coast of Ireland, and Sark, one of the Channel Islands off the coast of France. Identical methods were used in surveys in three other areas, all of which have heavy gasoline driven traffic. These were Jersey, another of the Channel Islands, Ebbw Vale--a mixed industrial area, and Cardiff--the capital city of Wales. Environmental lead levels were very low in two of the traffic-less islands, but on the third, house dust lead levels were comparable with levels found throughout Wales. Blood lead levels on one of the islands were similar to those which have been reported for unaccultured remote tribes, but on the other two traffic-less islands blood lead levels were comparable with those of areas on the mainland of Wales.

RESUMO

A baby with clinical and serological evidence of congenital varicella syndrome died 36 hours after birth. Her mother had developed clinical varicella at 12 weeks gestation. Ultrasound at 37 weeks gestation had shown poor foetal growth and a lacuna within the foetal skull. Varicella-zoster-specific IgM was demonstrated in the baby's serum.
