OBJECTIVES: To verify whether there are differences in the judgments of listeners' attitudes as a function of the presence, severity, and type of vocal deviation, and to identify the predictive factors of listeners' attitudes toward dysphonic voices in Brazilian Portuguese speakers. METHODS: A sample of CAPE-V sentences was used from 44 subjects of both genders. The samples presented different severities of vocal deviation (general grade - GG) (healthy, mild, moderate, and severe) and different degrees of roughness (GR), breathiness (GB), and strain (GS), characterized by a perceptual-auditory judgment performed by speech therapists. The samples were presented to 152 listeners of both genders who performed the judgment of 12 attitudes inserted in a semantic differential scale previously validated for this study. Here, we used a Logistic Regression Model. RESULTS: There was no association between listener's gender and the judgment of attitudes. Subjects with vocal quality deviation were judged more negatively in relation to vocally healthy individuals. We observed that higher the GG, higher the negative judgment of listeners in relation to the speakers. Listeners judged breathy and strained voices as more negative. In the general model for female and male voices, the increase in GB and GG reduced the chance of a voice being evaluated positively by 16 and 20%, respectively. For female voices, GB and GS reduced the chance of a female voice being evaluated positively by 36 and 19%, respectively. GG reduces the chance of a male voice being evaluated positively by 46%, while GR increases the chance of a male voice being judged positively by 62 %. CONCLUSIONS: There is an association between the presence, severity, and type of vocal deviation and listeners' attitudes. We identified a predictive model of listeners' attitudes related to dysphonic voices among Brazilian Portuguese speakers. In general, GG and GB were predictors of negative attitudes toward listeners. GB and GS were predictors of negative attitudes toward female voices. The GG is a predictor of negative attitudes toward male voices, while the GR increases the chance of positive judgments.

AIM: To analyze the accuracy of traditional acoustic measurements (F0, perturbation, and noise) and formant measurements in discriminating between women with and without voice disorders, and with different laryngeal disorders. STUDY DESIGN: A descriptive, cross-sectional, and retrospective. METHOD: Two hundred and sixty women participated. All participants recorded the spoken vowel /Ɛ/ and underwent laryngeal visual examination. Acoustic measures of the mean and standard deviation of the fundamental frequency (F0), jitter, shimmer, glottal-to-noise excitation ratio, and the values of the first three formants (F1, F2, and F3) were obtained. RESULTS: Individual acoustic measurements did not demonstrate adequate (<70%) performance when discriminating between women with and without voice disorders. The combination of the standard deviation of the F0, shimmer, glottal-to-noise excitation ratio, F1, F2, and F3 showed acceptable (>70%) performance in classifying women with and without voice disorders. Individual measures of jitter as well as F1 and F3 demonstrated acceptable (>70%) performance when distinguishing women with different laryngeal diagnoses, including without voice disorders (healthy larynges), Reinke's edema, unilateral vocal fold paralysis, and sulcus vocalis. The combination of acoustic measurements showed excellent (>80%) performance when discriminating women without voice disorder from those with Reinke's edema (mean of F0, F1, and F3) and with sulcus vocalis (mean of F0, F1, and F2). CONCLUSIONS: Individual formant and traditional acoustic measurements do not demonstrate adequate performance when discriminating between women with and without voice disorders. However, the combination of traditional and formant measurements improves the discrimination between the presence and absence of voice disorders and differentiates several laryngeal diagnoses.

PURPOSE: Investigate the accuracy of isolated and combined acoustic measurements in the discrimination of voice deviation intensity (GD) and predominant voice quality (PVQ) in patients with dysphonia. METHODS: A total of 302 female patients with voice complaints participated in the study. The sustained /ɛ/ vowel was used to extract the following acoustic measures: mean and standard deviation (SD) of fundamental frequency (F0), jitter, shimmer, glottal to noise excitation (GNE) ratio and the mean of the first three formants (F1, F2, and F3). Auditory-perceptual evaluation of GD and PVQ was conducted by three speech-language pathologists who were voice specialists. RESULTS: In isolation, only GNE provided satisfactory performance when discriminating between GD and PVQ. Improvement in the classification of GD and PVQ was observed when the acoustic measures were combined. Mean F0, F2, and GNE (healthy × mild-to-moderate deviation), the SDs of F0, F1, and F3 (mild-to-moderate × moderate deviation), and mean jitter and GNE (moderate × intense deviation) were the best combinations for discriminating GD. The best combinations for discriminating PVQ were mean F0, shimmer, and GNE (healthy × rough), F3 and GNE (healthy × breathy), mean F 0, F3, and GNE (rough × tense), and mean F0 , F1, and GNE (breathy × tense). CONCLUSION: In isolation, GNE proved to be the only acoustic parameter capable of discriminating between GG and PVQ. There was a gain in classification performance for discrimination of both GD and PVQ when traditional and formant acoustic measurements were combined.

OBJECTIVE: The study aimed to determine whether there is a relationship between acoustic measures and self-evaluation in patients with voice disorders. STUDY DESIGN: This is a descriptive, transversal, and observational study. METHODS: Patients (257) who answered the Voice Handicap Index protocols (VHI) and the Voice Symptoms Scale (VoiSS) and recorded the vowel /ε/ were included. Standard deviation (SD) measures of the fundamental frequency (F0), jitter, shimmer, and the glottal to noise excitation ratio (GNE) vowel /ε/ were taken. RESULTS: There was a weak positive correlation between all scores of VoiSS and the SD of the F0 and jitter. The overall scores, physical limitation, and VoiSS showed weak positive correlations with shimmer. The overall scores, limitation, and emotional VoiSS showed weak negative correlations with the GNE. The VHI did not correlate with any of the acoustic measurements. There was no difference in the mean of the acoustic measures of the SD of F0, jitter, and GNE because of a voice problem detected from the cutoff points of VoiSS. There was no difference in any of the acoustic measurements when patients with and without voice problems were compared from VHI cutoffs. CONCLUSIONS: There is a correlation between the scores of VoiSS and acoustic measurements. Patients with self-reported voice problems in VoiSS present greater deviations in acoustic measures, mainly in jitter. There is no correlation between the VHI scores and the acoustic measures and no difference in the averages of these measures between patients with and without voice problems detected from the VHI cutoffs.

RESUMO Objetivo investigar se existe associação entre a frequência de ocorrência dos sintomas vocais, a intensidade do desvio vocal e o diagnóstico laríngeo em pacientes com diferentes distúrbios da voz. Métodos Participaram do estudo 330 pacientes com queixa vocal, distribuídos em cinco grupos de acordo com o diagnóstico laringológico: G1-sem lesão laríngea, G2-lesões benignas das pregas vocais, G3-distúrbio neurológico da voz, G4-fenda glótica sem causa orgânica ou neurológica e G5-distúrbio de voz secundário a refluxo gastroesofágico. Foram alocados em cinco grupos, de acordo com o diagnóstico laríngeo. Todos os participantes responderam à Escala de Sintomas Vocais (ESV) e gravaram a vogal /ɛ/ sustentada, que foi analisada quanto à intensidade do desvio vocal por meio de uma escala analógico-visual. Resultados Houve diferença nos escores total, de limitação e físico da ESV entre diferentes grupos diagnósticos. Pacientes com lesão na porção membranosa das pregas vocais apresentaram maior número de sintomas vocais em relação aos outros grupos diagnósticos. Observou-se correlação positiva entre a intensidade do desvio vocal e os escores total, de limitação e emocional da ESV. Pacientes com desvio vocal moderado e intenso apresentaram maior número de sintomas vocais que pacientes com vozes saudáveis ou com desvio leve. Conclusão Existe associação entre o diagnóstico laríngeo, a intensidade do desvio vocal e a frequência de ocorrência dos sintomas vocais. Pacientes com lesão benigna nas pregas vocais e com desvio vocal mais intenso apresentam maior frequência de sintomas vocais.


ABSTRACT Purpose To investigate whether there is an association between the frequency of occurrence of vocal symptoms, the intensity of vocal deviation and laryngeal diagnosis in patients with different voice disorders. Methods The study included 330 patients with vocal complaints. The patients were divided into five groups according to the laryngeal diagnosis: G1- without laryngeal lesions; G2 -benign vocal fold lesions; G3 - voice disorder of neurological origin; G4 -incomplete glottal closure without organic or neurologic cause; and G5 - voice disorder secondary to gastroesophageal reflux. All participants responded to the Voice Symptom Scale (VoiSS) and recorded the sustained /ɛ/ vowel, which was analyzed for the intensity of vocal deviation using a visual analog scale. Results There were differences in total VoiSS scores and in scores for the impairment and physical domains among the different diagnostic groups. Patients with lesions in the membranous portion of the vocal folds presented more vocal symptoms than those in the other diagnostic groups. There was a positive correlation between the intensity of vocal deviation and the total, limitation and emotional VoiSS scores. Patients with moderate and intense vocal deviation showed a greater number of vocal symptoms than patients with healthy voices or with mild deviations. Conclusion There is an association between laryngeal diagnosis, the intensity of vocal deviation and vocal symptoms. Patients with benign vocal fold lesions and more intense vocal deviations have a higher frequency of vocal symptoms.
