RESUMO

Complete superior vena cava (SVC) and inferior vena cava (IVC) obstruction is not uncommon and most commonly associated with malignancy. The risk increases in patients with central lines and hypercoagulable states such as with malignancy, thrombophilia, or use of oral contraceptive pills. According to our knowledge, complete SVC and IVC obstruction associated with systemic-to-pulmonary venous shunts in patients with prothrombin G20210A gene mutation has not been reported in the literature. Here we report the case of a 34-year-old female with complete SVC and IVC obstruction presenting with oxygen desaturation and shortness of breath due to systemic-to-pulmonary venous shunts. The unusual collateral pathway was secondary to SVC obstruction. The patient was managed conservatively, and she remained stable.

A tilapia farm experiencing endemic streptococcosis was selected to study the effect of vaccination with a feed-based vaccine on naturally ocurring streptococcosis. A total of 9000 red tilapia, Oreochromis niloticus × Oreochromis mossambicus of 100 ± 20 g were divided into 9 cages. Fish of Group 1 in cages 1, 2 and 3 were not vaccinated. Group 2 in cages 4, 5 and 6 were vaccinated on days 0 and 14 (single booster) while Group 3 in cages 7, 8 and 9 were vaccinated on days 0, 14 and 42 (double booster). Vaccination was done by oral administration of the feed-based bacterin vaccine at 4% bodyweight. Samples of serum for antibody study and the brain, eyes and kidney for bacterial isolation were collected at 14-day intervals. The study was carried out during the critical months between April and June. Following vaccination and booster, there was significant (p < 0.05) increase in the antibody levels in all vaccinated groups from week 1 that reached the peak at week 3 before declining gradually until week 6. However, second booster on week 6 significantly (p < 0.05) increased the antibody level that remained high until the end of the 16-week study period (double booster). Streptococcus agalactiae was isolated at the start of the experiment (day 0) at an average of 10 ± 5.0% of the sampled fish. In week 4, the isolation rate was 13 ± 5.7% but increased to 18 ± 7.6% in week 8, to 25 ± 10.0% in week 10, to 28 ± 5.8% in week 12 and 25 ± 7.3% in week 14. The average isolation rate was 28 ± 7.2%, 18 ± 7.1% and 13 ± 8.2% of the fish sampled from unvaccinated, single booster and double booster groups, respectively. At the end of the study period, the survival rate was 45.2 ± 2.45% for unvaccinated, 65.3 ± 4.8% for single booster and 75.1 ± 2.1% for double booster groups. Vaccinating fish in endemic farm might not eliminate the disease but was able to significantly improve the survival rate.

On fractionation the ethanolic extract of H. rosa sinensis leaves, 5 fractions were obtained. Of these, fraction-3 (F3) and fraction-5 (F5) were chosen for detailed investigation on non obese diabetic (NOD) mouse to study anti-diabetic properties because they were more active than others. Serum glucose, glycosylated hemoglobin, triglyceride, cholesterol, blood urea, insulin, LDL, VLDL, and HDL were estimated. Both fractions F3 and F5 on oral feeding (100 and 200 mg/kg body weight) demonstrated insulinotropic nature and protective effect in NOD mice. These fractions may contain potential oral hypoglycemic agent.

UNLABELLED: Most scholars, old and modern, agree that the vowel system of the Arabic language is composed of 3 vowels only, namely /i/, /ε/ and /u/. The spoken Cairo dialect suggests that there are 6 identifiable vowels, with a short and long variant for each. OBJECTIVE: The aim of this study is to test the validity of the notion that there are 6 × 2 distinct vowels, with a more central one. SUBJECTS AND METHODS: Spectral analysis was used to measure F(1) and F(2) for the vowels of 14 real words. Data was collected from 60 healthy adult informants, 30 males and 30 females. They were native Egyptians speaking the colloquial Cairene dialect. RESULTS: The values of the 6 long and short vowels plus the central one are presented. A significant difference was found between each of them. The long and short vowels differed only in the duration but did not differ in their formant values. CONCLUSION: The study illustrates the distinctive features of the vowels of the Arabic language. Each of the 7 vowels represents a distinct entity. This will have important implications in assessment and management of language, speech and voice disorders in children and adults.

Adipokines are important for regulation body metabolism and immune response. Many studies have shown that variants in adipokines genes play a role in age-associated diseases. In this study, we investigated the contribution of rs266729 (-11377G/C), rs2241766 (+45T/G), and rs1501299 (+276 G/T) SNPs of adiponectin gene (ADIPQO) and rs7799039 (-2548C/A) SNP of leptin (LEP) gene to human longevity phenotype in Jordanian population. Polymorphisms were genotyped in 110 randomly selected elderly subjects (>85 years old) with mean age of 90.2 years, and 120 young control subjects (range from 20 to 50 years) with mean age of 32.0 years. No significant differences were detected in the genotype and allele frequencies of examined gene variants between the two groups (p > 0.05). However, when gender was considered, genotypes and alleles frequencies of rs1501299 SNP in ADIPOQ gene and rs7799039 in LEP gene were significantly associated with longevity in men (p < 0.02) but not in women (p > 0.05). Thus, ADIPOQ and LEP genes polymorphisms might play a gender-specific role in the pathway to men's longevity.

SETTING: Of all the provinces in Indonesia, the highest tuberculosis (TB) case notification rates are reported from North Sulawesi Province. OBJECTIVE: To estimate the annual risk of tuberculous infection (ARTI) among schoolchildren in the 6-9 year age group. METHODS: A cross-sectional survey was carried out in 99 schools selected by a two-stage sampling process. Children attending grades 1-4 in the selected schools were administered intradermally with 2 tuberculin units (TUs) of purified protein derivative RT23 with Tween 80, and the maximum transverse diameter of induration was measured about 72 h later. A total of 6557 children in the 6-9 year age group were satisfactorily test-read, irrespective of their bacille Calmette-Guérin (BCG) vaccination status. RESULTS: Based on the frequency distribution of reaction sizes obtained among satisfactorily test-read children (without and with BCG scar), the estimated ARTI rates when estimated by different methods (anti-mode, mirror-image and mixture model) varied between 1.9% and 2.5%. BCG-induced tuberculin sensitivity was not found to influence the ARTI estimates, as the differences in estimates between children without and with BCG scar were not statistically significant. CONCLUSION: TB control efforts should be further intensified to reduce the risk of tuberculous infection.

Patients complaining of chest disorders were clinically and serologically examined for evidence of echinococcal infection. Serologically, the patients were screened by the indirect haemagglutination test and reactors were confirmed by the bentonite flocculation and/or latex-agglutination tests. Of 755 patients 6.2 percent had echinococcal antibodies in their sera. Of 393 patients given a chest radiography one patient (0.25%) had distinct pulmonary hydatid cysts. The bentonite flocculation technique picked more of the positives (91.5%) than did the latex-agglutination (14.9%) technique. Females were more exposed to echinococcal infection (8.4%) than males (2.3%). Among positives, 71-75 per cent were below thirty years old. Prevalence of infection was higher among individuals having direct contact with dogs (4.6%) than among individuals with no direct contact with dogs (3.2%). Evidently cross-reactions between the echinococcal antigen and antibodies of some bacterial and parasitic infections including schistosomiasis interfere with the haemagglutination test results.
