RESUMO

This paper describes the design steps carried out to prove the concept of a wideband monopole antenna system to be used in a wearable device conceived for the evaluation of electromagnetic field radiation. Such a device is envisaged to be integrated into protective vests worn by professional users in their working space environment as part of intelligent multi-risk protection. Initially, the main characteristics of a simple straight monopole are reviewed to serve as a reference. A modified octagonal monopole antenna element is introduced, and a two dual-linearly polarized configuration of such monopoles is designed, fabricated, and tested to be used in the frequency range of 0.7-3.5 GHz. The expected radiation characteristics (input reflection coefficient and isolation between vertically and horizontally polarized ports) are confirmed experimentally. The effects of a thick lossy foam substrate layer used to mitigate the presence of the metal shield, employed in the vest lining as a Faraday cage protection, are analyzed both by simulation and experimentally. Finally, electromagnetic simulations are carried out to confirm that a system of five dual-linearly polarized monopole elements located in the chest, shoulders, back, and helmet of the user can provide an adequate estimation of the incident electromagnetic field radiation.

The extravasation of doxorubicin, a cytostatic from the anthracycline group, is a rare and feared complication of intravenous chemotherapy due to tissue toxicity of these drugs. We describe a case of a 64-year-old woman with breast cancer undergoing adjuvant chemotherapy with doxorubicin and cyclophosphamide using a tunneled central catheter. After a chemotherapy cycle, the patient developed cough, dyspnoea and chest pain, due to a pleural effusion secondary to cytostatic leakage. A pleural drainage was placed and dexrazoxone was administred with resolution of the condition. The authors wish to highlight that the use of dexrazoxone should be considered after intra-pleural extravasation of anthracyclines given its effectiveness in preventing tissue damage and long-term sequelae in peripheral extravasation.

PURPOSE: To reduce false-negative rates (FNR) in sentinel lymph node biopsy (SLNB) of clinically positive (cN+) axilla in patients undergoing neoadjuvant chemotherapy (NAC). The removal of three or more lymph nodes with dual-tracer mapping including a radioisotope was used. However, in the Brazilian Unified Health System, the radioisotope tracer is not feasible in some hospitals. We conducted a cross-sectional study to evaluate the detection rate of sentinel lymph node (SLN) in patients who converted from cN+ to ycN0 after NAC using blue dye as a single-agent mapping tracer. METHODS: During the period of March 2018 to September 2019, 34 patients who underwent NAC with cN+ who converted to ycN0 were enrolled in the study. The SLNB was performed using blue dye as a single-agent mapping followed by axillary lymph node dissection (ALND). RESULTS: The detection rate of sentinel lymph node was of 85.3%, being SLNB not possible for five patients (14.7%), due to fibrosis. The mean number of removed SLN was 2.5. CONCLUSIONS: The use of blue dye as a single-agent mapping tracer demonstrated an acceptable detection rate of 85.3%. Although the FNR was possible to be determined, the small sample size might overestimate this rate. The removal of three or more lymph nodes with single-agent mapping tracer might be indicated for breast cancer patients who converted to ycN0 after NAC in the Brazilian health public services, in which radioisotope tracer is not suitable.

Objective The objective of the present study was to evaluate the clinical and radiographic results of our series regarding ulnar shortening osteotomy, as well as to briefly review the pathology, indications and surgical options of ulnocarpal conflict. Methods We performed a retrospective analysis of consecutive patients who were treated with ulnar shortening osteotomy between January 2012 and June 2017 at our hospital. We clinically evaluated pain, articular range of motion, grip strength and functional outcomes using the quick-DASH questionnaire. We radiographically measured the pre- and postoperative ulnar variance and the shortening performed. Results We identified eight operated patients, and it was possible to evaluate seven of them. Pain decreased in this population (visual analogue scale [VAS] score changed from 7 to 2.6, p < 0.05), there was a decrease in quick-DASH (64 to 28, p < 0.05) and we found a decrease in the articular amplitude ∼ 7° for flexion ( p = 0.2), and of 5.5° for supination ( p = 0.3), as well as decreasing grip strength to about 86% on the contralateral side ( p = 0.07). The ulnar variance changed from a mean of + 5.5 mm to - 1.1 mm ( p < 0.05). Two out of 8 patients (25%) presented plaque-related symptoms and one of them underwent a new intervention to extract the material. Conclusions Ulnar shortening osteotomy is an effective surgical procedure both in the treatment of ulnocarpal conflict and in the discharge of the ulna. The results presented agreement with other results published in the literature, with good clinical and radiographic results.

Resumo Objetivo O objetivo do presente estudo foi avaliar os resultados clínicos e radiográficos da nossa casuística relativamente a osteotomias de encurtamento da ulna, bem como rever sumariamente a patologia, as indicações e as opções cirúrgicas do conflito ulnocárpico. Métodos Realizamos uma análise retrospectiva de pacientes consecutivos que foram tratados com osteotomia de encurtamento da ulna entre janeiro de 2012 e junho de 2017 no nosso hospital. Avaliamos clinicamente a dor, amplitude articular, força da garra e resultados funcionais recorrendo ao questionário quick-DASH. Medimos radiograficamente a variância ulnar pré- e pós-operatória e o encurtamento realizado. Resultados Identificamos oito pacientes operados, tendo sido possível avaliar sete destes. Nesta população, obtivemos uma diminuição da dor (escala analógica visual [VAS, na sigla em inglês] de 7 para 2.6, p< 0,05), uma diminuição do quick-DASH (64 para 28, p< 0,05) e constatamos uma diminuição da amplitude articular ∼ 7° para a flexão (p= 0.2), de 5.5° para a supinação (p = 0,3), bem como diminuição da força da garra para cerca de 86% do lado contralateral (p = 0,07). A variância ulnar foi alterada de uma média de + 5.5 mm para −1.1 mm (p < 0,05). Dois em 8 pacientes (25%) apresentaram sintomatologia relacionada com a placa, sendo que um deles foi submetido a nova intervenção para extração do material. Conclusões A osteotomia de encurtamento da ulna é um procedimento cirúrgico eficaz, tanto no tratamento do conflito ulnocárpico, como na descarga da ulna. Os resultados apresentados vão ao encontro aos demais publicados na literatura, tendo-se obtido bons resultados clínicos e radiográficos.

BACKGROUND: Norway has one of the best health systems in the world. However, it has a low birth rate, which decreased by 21.2% between 2009 and 2018, and one of the highest rates of infertility prevalence. The aim of this study is to understand how Norwegian doctors perceive female infertility diseases, namely those that are more difficult to diagnose and to treat, and that are more common in their practice. METHOD: Descriptive qualitative study was conducted with gynecologists and general practitioners. The sample resulted from the establishment of five criteria and on the doctors' acceptance to participate in this study. Our sample comprised thirteen highly qualified and experienced doctors. Qualitative content analysis was the method chosen to analyze the collected data. RESULTS: Clinical diseases (polycystic ovary syndrome, endometriosis and vulvodynia) and consequences of these diseases were the pinpointed themes. These led to a set of sub-themes: the main symptoms and the treatment of the diseases, from the perspective of both women and doctors (stigmatization, disturbances in women's daily life, diagnostic delay, and governmental support). CONCLUSIONS: The three most relevant disorders mentioned were polycystic ovary syndrome, endometriosis and vulvodynia. These diseases cause several impacts on the lives of women, because they feel stigmatized and limited in their daily life and sexuality, and the diagnosis of these diseases takes too much time. Governments should better redistribute the financing of women's health and allocate resources to specialized centers.

Arthroscopic subtalar arthrodesis has recently gained popularity in the treatment of primary subtalar or post-traumatic arthritis, coalition, or inflammatory diseases with subtalar arthritis. The present study reports the clinical and radiologic results of 19 patients (19 feet) who underwent posterior arthroscopic subtalar arthrodesis using 2 posterior portals. A total of 19 posterior arthroscopic subtalar arthrodeses (minimum follow-up of 24 months) performed without a bone graft and with 2 parallel screws were prospectively evaluated. The fusion rate was 94% (mean time to fusion 9.8 weeks). Modified American Orthopaedic Foot and Ankle Society ankle-hindfoot scale score (maximum 94 points) improved significantly from 43 to 80 points and the visual analog scale for pain score improved from 7.6 to 1.2. The 12-item short-form physical and mental scores at the last follow-up point were 52.5 and 56.4, respectively. One (5.3%) patient underwent open repeat fusion for nonunion, 2 (10.5%) patients required a second procedure for implant removal, and 1 (5.3%) experienced reversible neuropraxia. In conclusion, posterior arthroscopic subtalar arthrodesis is a safe technique with a good union rate and a small number of complications in patients with no or very little hindfoot deformity.

ABSTRACT BACKGROUND AND OBJECTIVES: Since the dawn of civilization, pain was and still is a major concern for humankind. It is estimated that 80% of the world population look for the health system due to pain complaints. American epidemiological studies show that 20% of adults assisted by primary health attention services suffer from chronic pain. This study aimed at identifying the incidence of pain in adults in the Primary Health Attention. METHODS: Documental research carried out in a family health unit of the city of Tupanatinga/PE. Sample was made up of medical charts of users (adults) registered in the Unit. A total of 1071 charts they analyzed. A verbal scale was used to evaluate the incidence of pain. RESULTS: According to the verbal scale, the prevalence of pain among users of the family health unit was 42%. Associating pain to gender, it was observed that there has been statistically significant association (p<0.001), where the presence of pain in women (69.1%) was more frequent as compared to males (30.9%). CONCLUSION: The results of this study show us the “scenario” of pain at the preferential “Gateway” of the Single Health System. The high incidence of pain in Primary Attention warns for the magnitude of the problem, making imperative to treat pain as a severe public health problem with social relevance.


RESUMO JUSTIFICATIVA E OBJETIVOS: Desde os primórdios da civilização, a dor foi e continua sendo uma das maiores preocupações da humanidade. Estima-se que 80% da população mundial procure o sistema de saúde devido a queixa de dor. Estudos epidemiológicos americanos demonstram que 20% dos adultos atendidos nos serviços de atenção primária à saúde, sofrem de dor crônica. O objetivo deste estudo foi identificar a incidência de dor em Adultos na Atenção Primária à Saúde. MÉTODOS: Pesquisa Documental realizada em uma unidade de saúde da família do município de Tupanatinga/PE. A amostra foi constituída por prontuários de usuários (adultos) cadastrados na Unidade. Um quantitativo de 1.071 prontuários foi analisado. Para a avaliação da incidência da dor foi utilizada a escala verbal. RESULTADOS: Segundo a escala verbal, a prevalência de dor entre os usuários da unidade de saúde da família foi de 42%. Associando a percepção de presença de dor em relação ao gênero, observou-se que houve uma associação estatisticamente significativa (p<0,001), onde a presença de dor em mulheres (69,1%) foi mais frequente quando comparadas aos homens (30,9%). CONCLUSÃO: Os resultados deste estudo nos colocam frente ao “cenário” da dor na “Porta de Entrada” preferencial do Sistema Único de Saúde. A alta incidência de dor na Atenção Primária alerta para a magnitude do problema, tornando-se imprescindível tratar a dor como um problema sério de saúde pública e de significado social.

Biodiesel has become attractive due to its environmental benefits compared with conventional diesel. Although the enzymatic synthesis of biodiesel requires low thermal energy, low conversions of enzymatic transesterification with ethanol (ethanolysis) of oils to produce biodiesel are reported as a result of deactivation of the enzyme depending on the reaction conditions. The synthesis of biodiesel via enzymatic ethanolysis of sunflower and soybean oils was investigated. Kinetic parameters for the overall reactions were fitted to experimental data available in the literature with the Ping Pong Bi-Bi mechanism including the inhibition effect of the ethanol on the activity of lipase Novozyme 435. The model was applied to a batch reactor and the experimental conversions were successfully reproduced. The modeling of a semibatch reactor with continuous addition of ethanol was also performed and the results showed a reduction of roughly 3 h in the reaction time in comparison with the batch-wise operation.

OBJETIVO: Avaliar a influência do diâmetro e do tempo na obstrução da janela pleuro-pericárdica em cães com pericárdio normal. MÉTODO: Trinta e seis cães mestiços foram divididos em seis grupos: 1 a, 1 b,1 c, 2 a, 2 b, 2 c; n=6 por grupo. Nos grupos 1 a , 1 b, 1 c, à janela foi de 2cm de diâmetro e nos grupos 2 a, 2 b, 2 c, foi de 4cm. Os animais foram reoperados respectivamente após 2, 8 e 12 semanas. Na re-operação, avaliou-se o grau de obstrução através de uma escala de aderência pericárdio-epicárdica e a histopatologia das bordas do pericárdio. RESULTADOS: Numa análise global, observou-se 89 por cento de janelas abertas e 11 por cento de janelas totalmente obstruídas. No grupo com 2cm de diâmetro original, na reoperação, encontrou-se um diâmetro maior em 89 por cento dos cães, enquanto que nos cães com janela original de 4cm, isto ocorreu em 61 por cento. Quando comparou-se os resultados nos cães com janelas de diâmetro igual, mas re-operados em diferentes tempos de pós-operatório, não se observou diferença estatisticamente significante. O mesmo ocorreu quando comparou-se os cães com janelas de diâmetro diferente e re-operados em tempos iguais de pós-operatório. O grau de aderência pericárdio-epicárdica, de acordo com a escala de gradação, não foi diferente entre os vários grupos em função do tempo e diâmetro com exceção do grupo com janela de 4cm e re-operado com 8 semanas (Grupo 2b). As alterações histopatológicas não foram estatisticamente significantes entre os grupos. Em nenhum cão observou-se obstrução da janela pleuropericárdica pelo pulmão. CONCLUSÃO: o tempo e o diâmetro da janela pleuropericardica não influenciaram na obstrução da mesma.


BACKGROUND: to evaluate the influence of diameter and time on the patency of transthoracic pericardial window in normal pericardium. METHODS: Thirty-six mongrel dogs were divided into 6 groups: 1a, 1b and 1c; 2a, 2b and 2c; n=6 per group. In groups 1a, 1b and 1c the pericardial window diameter was 2cm and in groups 2a, 2b and 2c it was 4cm. Animals were sacrificed 2, 8 and 12 weeks , respectively and we evaluated the patency and histology of the pericardial margins. RESULTS: Eighty-nine per cent of the windows were open and 11 percent were completely obstructed by pericardial-epicardic adhesions. In the groups with a 2cm-pericardial window, we observed during sacrifice an increase of the original diameter in 89 percent of the animals. In dogs with a 4cm-pericardial window a greater diameter was observed in 61 percent of the animals. We did not observe statistical differences comparing animals with the same window diameter but sacrificed at different time points. Likewise, we did not observe differences among dogs with different window diameters but sacrificed at the same time points. The degree of pericardial-epicardic adhesions, according to a graduation score, was not different among the groups when analyzing the influence of time and diameter, except for the group 2b ( 4cm-window diameter and animals sacrificed at 8 weeks). Histopatology was not different among the different groups. We did not observe any lung blockade of the transthoracic pericardial windows. CONCLUSIONS: time and diameter did not influence the patency of the transthoracic pericardial window.

Relato do caso de uma mulher com 83 anos apresentando nódulo e retração de pele na mama direita com oito meses de evolução. Ao exame físico verificou-se nódulo sólido de 5 cm, localizado no quadrante súpero-lateral de mama direita, associado a presença de retração de pele correspondente e linfonodos axilares não coalescentes ipsilaterais. O resultado da mamografia evidenciou nódulo de 4 cm de diâmetro irregular no quadrante súpero-lateral da mama direita (bi-rads V). Estádio clínico: T2N1M0 (IIB). O tratamento cirúrgico incluiu mastectomia radical modificada (à Maden) com dissecção axilar níveis I, II e III. Avaliação histopatológica demonstrou a presença de carcinoma lobular infiltrativo que mediu 2,5 cm (T2), presença de linfadenite granulomatosa causada por tuberculose em linfonodos dos níveis I, II e III, associados a metástase de carcinoma lobular em um único nível linfático, nível I. Estádio patológico: pT2pN1aM0. A paciente recebeu tratamento para tuberculose ganglionar com rifampicina, isoniazida e pirazinamida por um ano. Foram solicitados receptores hormonais, os quais mostraram-se positivos, sendo feito terapia adjuvante com tamoxifeno. Durante o primeiro ano de seguimento a paciente evoluiu bem, sem sinais de recidiva local ou metástases a distância

Thymolipoma is a rare benign tumor of the thymus. Because of its large size and pliability, the mass usually drapes itself around the heart, conforming to its borders and producing a large radiographic shadow easily mistaken as cardiomegaly. This report describes a 48-year-old man with a chest radiograph strongly suggestive of cardiomegaly. Careful radiological study showed the existence of a large mass in the anterior mediastinum, simulating cardiomegaly. A thymolipoma weighing 2500g was resected through an esternotomy

The effects of radiation on the reproductive capacity and the longevity of Panstrongylus magistus were studied. An indirect correlation between longevity and radiation doses was observed. Males were more affected than females. Longevity of females submitted to 10 Gy was not different when compared to controls. Some of the irradiated males copulated and in these cases semen was transferred to females, but only few eggs were laid. Fertility was seriously affected in all irradiated groups, decreasing with increasing intensity of dose. The dose of 80 Gy induced sterility in males and females. Gelatinous spermatophores that were expelled by females irradiated with 20 and 40 Gy, may be a consequence of irradiation that induced modification in the bursa copulatrix pH

Os autores apresentam casuística de 20 pacientes submetidos à punçäo-biópsia por agulha de Cope, demonstrando a predominância de tuberculose, neoplasias e pleuris crônico nos exsudatos pleurais, descrevem a técnica de exame, correlacionando os achados patológicos com o sexo, idade, cor e teor protéico do líquido pleural e mostram a baixa incidência de complicaçöes do exame
