RESUMO

PURPOSE: Matrix based autologous chondrocyte implantation is an established method for treatment of full cartilage defects in the knee joint, but little is known about the influence of the implanted autologous chondrocyte quality and its clinical value. The aim of this study is to evaluate the influence of the gene expression of the implanted autologous chondrocytes on the clinical outcomes in the follow-up period of three years. METHODS: Biological parameters of the implanted chondrocytes were analysed histologically and immunohistologically. An analysis of the gene expression of the relevant chondrogenic dedifferentiation markers was performed as well. In order to detect the rate of apoptosis, we analysed specific apoptosis markers. To evaluate the clinical outcome, the International Knee Documentation Committee (IKDC) subjective score and the visual analogue scale (VAS) were used pre-operatively and after 0.5, one, two and three years. RESULTS: Significantly improved clinical scores were observed. The subjective IKDC score increased from 50.6 ± 20.7 points to 69.3 ± 26.1 points at three years (p = 0.001). The implanted chondrocytes showed a typical expression of the chondrogenic markers with a high rate of collagen type II expression without dedifferentiation. The analysed differentiation, dedifferentiation and apoptosis markers showed no influence on the clinical outcome. CONCLUSIONS: The implanted autologous chondrocytes have a high chondrogenic quality with a high rate of collagen type II expression without dedifferentiation. An influence of differentiation, de-differentiation and apoptosis marker gene expression on the clinical outcomes could not be found in a follow-up period of three years. BACKGROUND: Autologous chondrocyte implantation (ACI) using tissue-engineered cartilage is a successful therapy for full-thickness cartilage lesions in the knee joint. However, in vivo graft maturation is still unclear. PURPOSE: The aim of this prospective study was to analyze graft maturation after ACI in the knee using objective T2 mapping in correlation with the clinical outcomes within a 3-year postoperative course. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: A total of 13 patients with isolated cartilage defects of the knee were treated with Novocart 3D, a matrix-based ACI procedure in the knee joint. The patients had complete data from International Knee Documentation Committee (IKDC) scores and MRI examinations for 6 to 36 months postoperatively. All cartilage defects were arthroscopically classified as Outerbridge grades III and IV. The mean area of the cartilage defect was 5.6 cm(2). Postoperative clinical and MRI examinations were conducted at 6, 12, 24, and 36 months after surgery. The modified magnetic resonance observation of cartilage repair tissue (MOCART) score was used to evaluate the quality and integration of the Novocart 3D implants on MRI. The T2 relaxation time values of the ACI graft and healthy native cartilage areas were determined to assess graft maturation using T2 mapping. RESULTS: The T2 relaxation times of the ACI graft showed significant improvement, with decreasing values from 41.6 milliseconds at 6-month follow-up to 32.4 and 30.9 milliseconds after 24 and 36 months, respectively. These values were similar to the T2 relaxation times of the native surrounding cartilage. There was no correlation between the clinical outcomes (IKDC score) and T2 relaxation time values. CONCLUSION: The T2 relaxation time in the repaired tissue showed similar values compared with normal hyaline cartilage. Graft maturation after ACI in the knee joint needs at least 1 year, with ongoing adjustment of the T2 relaxation time values compared with native surrounding cartilage. A correlation between increasing ACI graft maturation and clinical outcomes (IKDC score) could not be found with the data available.

PURPOSE: Low-grade joint infections are characterized by infiltration of granulocytes, which mediate aspects of inflammatory changes. We evaluated retrospectively the contribution of SPECT/CT as an addition to planar scintigraphy with (99m)Tc-labelled antigranulocyte antibodies for diagnosing and localizing low-grade joint infections. METHODS: Planar scintigraphy using (99m)Tc-labelled antigranulocyte BW 250/183 antibodies was performed in 31 patients with suspected joint infections at 5 min, 5 h and 24 h after injection, with additional SPECT/CT performed 6 h after injection. With reference to gold standard clinical data, we assessed the diagnostic sensitivity of scintigraphy alone and in conjunction with SPECT/CT. RESULTS: Joint infections were diagnosed clinically in 9 of the 31 patients (1 hip and 8 knee prostheses). Planar scintigraphy revealed 6 true-positives, 13 true-negatives, 9 false-positives and 3 false-negative results, indicating sensitivity, specificity, positive and negative predictive values of, respectively, 0.66, 0.60, 0.4 and 0.81. With the addition of SPECT images, corresponding sensitivity, specificity, positive and negative predictive values increased to 0.89, 0.45, 0.40 and 0.91. Implementation of fused SPECT/CT led to a further increase to 0.89, 0.73, 0.57 and 0.94. CONCLUSION: Relative to planar scintigraphy, SPECT with and without CT substantially improved the utility of imaging with (99m)Tc-labelled antigranulocyte antibodies for diagnosis and localization of suspected joint infections. Optimal accuracy was obtained through image fusion, which permitted anatomical allocation of foci of pathological tracer accumulation as well as providing information on the extent of the infection. This imaging method seems suited for selection of patients requiring surgical therapy.

In this article we study the settling process of a colloidal particle under the influence of a gravitational or centrifugal field in an unbounded electrolyte solution. Since particles in aqueous solutions normally carry a non-zero surface charge, a microscopic electric field develops which alters the sedimentation process compared to an uncharged particle. This process can be mathematically modelled via the Stokes-Poisson-Nernst-Planck system, a system of coupled partial differential equations that have to be solved in an exterior domain. After a dimensional analysis we investigate the influence of the various characteristic dimensionless numbers on the sedimentation velocity. Thereby the linear-response (weak-field) approximation that underpins almost all existing theoretical work on classical electrokinetic phenomena is relaxed, such that no additional assumption on the thickness of the double layer as well as on its displacement is needed. We show that there exists a strong influence of the fluid Reynolds number and the ionic strength on the sedimentation velocity. Further we have developed an asymptotic expansion to describe the limit of small values of the surface potential of a single particle. This expansion incorporates all nonlinear effects and extends the well-known results of Booth (1954) [1] and Ohshima et al. (1984) [2] to higher fluid Reynolds numbers.
