Introducción La traqueobroncopatía osteocondroplástica es una rara enfermedad crónica benigna de etiología desconocida. La broncoscopía sigue siendo el estándar de oro para el reconocimiento de traqueopatía osteocondroplástica. Sus hallazgos típicos se describen como un empedrado, un jardín de rocas, una apariencia de paisaje montañoso o de una cueva con estalactitas. El objetivo del presente trabajo es mostrar las principales características clínicas de una patología poco conocida. Casos clínicos Se analizaron los datos clínicos de cuatro pacientes de mediana edad, tres fueron hombres y una mujer. Los principales síntomas clínicos fueron tos crónica, disnea, disfonía. Los pacientes tuvieron un diagnóstico preliminar mediante tomografía axial computarizada de tórax, confirmado por examen video broncoscópico e histopatológico. El tratamiento incluyó medicamentos para los síntomas y en un solo caso criocirugía y coagulación con argón plasma. Discusión El diagnóstico de traqueobroncopatía osteocondroplástica no fue sencillo por ser una entidad rara, cuyos síntomas son inespecíficos y muy frecuentes en otras patologías. En Perú no se han publicado artículos de serie de casos sobre esta patología. Por lo tanto, tomamos como referencia artículos originales publicados en otros países para compararlos con nuestros hallazgos. Conclusión La traqueopatía osteocondroplástica es una enfermedad benigna que predispone a los adultos, los hombres tienen más probabilidades de verse afectados. Sus manifestaciones clínicas son inespecíficas; frecuentemente de origen faríngeo y la causa no está aún definida. La tomografía axial computarizada de tórax combinada con video broncoscopía son los principales procedimientos para el diagnóstico. No existe un estándar de tratamiento con efectos terapéuticos consistentes.

Objetivos: Describir la prevalencia de la enfermedad del hígado graso no alcohólico (EHGNA), la asociación entre el índice de fibrosis hepática 4 (FIB4) y los hallazgos en la ecografía y las características clínicas de los pacientes con artritis psoriásica. Material y métodos: Estudio transversal observacional de todos los pacientes con artritis psoriásica vistos de forma consecutiva en consulta desde el 01/01/2020 hasta el 30/11/2020. Resultados: De los 90 pacientes estudiados, la prevalencia de EHGNA fue del 56,67%. EL FIB4 presenta asociación con la ecografía (p=0,030), la ausencia de entesitis (p=0,036) y la mayor duración de la enfermedad (Rho 0,213, p=0,042). También con la presencia de hipertensión (p=0,027) y el consumo de alcohol (p=0,021). Sin embargo, el tratamiento biológico puede considerarse como un factor protector (p=0,005). El FIB4 actúa como predictor de EHGNA con una sensibilidad del 69,2% y una especificidad del 70,4%. Conclusiones: La prevalencia de EHGNA fue superior a la población general. El índice FIB4 puede ser una herramienta válida en el cribado de EHGNA en nuestra práctica clínica diaria.(AU)


Objectives: To describe the prevalence of non-alcoholic fatty liver disease (NAFLD), the association between Liver fibrosis 4 score (FIB4) and ultrasound findings, and the clinical characteristics of psoriatic arthritis patients. Material and methods: We carried out an observational cross-sectional study of patients seen in the outpatient clinic from January 1st, 2020, to November 30th, 2020, with psoriatic arthritis. Results: Of the 90 patients studied, the prevalence of NAFLD was 56.67%. FIB4 presents an association with ultrasound findings (p=.030), the absence of enthesitis (p=.036), and longer duration of disease (Rho .213 p=.042). It also presents an association with hypertension (p=.027) and alcohol consumption (p=.021). However, biological treatment can be considered as a protective factor (p=.005). FIB4 acts as a NAFLD predictor with 69.2% sensitivity and 70.4% specificity.Conclusion: The prevalence of NAFLD was higher in our sample than in the standard population. FIB4 index may be useful in screening for silent liver damage in psoriatic arthritis in clinical practice.(AU)

La sepsis neonatal es causa importante de morbilidad y mortalidad. La OMS reporta que anualmente mueren 2,4 millones de bebés durante el primer mes de vida, siendo la sepsis neonatal una de las principales causas. El objetivo es actualizar en el diagnóstico y tratamiento de la sepsis neonatal. En esta investigación se realizó una revisión de la literatura para sintetizar hallazgos relevantes sobre sepsis neonatal, que se define como afección sistémica, caracterizada por cambios hemodinámicos y otras manifestaciones clínicas, como respuesta desregulada a la infección. La sepsis neonatal precoz se relaciona con factores de riesgo maternos y los microorganismos causantes son del tracto genitourinario materno, mientras que la tardía es causada por gérmenes intrahospitalarios, en neonatos con estancia hospitalaria prolongada y sometidos a procedimientos invasivos. Los signos y síntomas de sepsis neonatal son inespecíficos, lo que hace difícil su diagnóstico, se recomienda identificar factores de riesgo y evaluación clínica minuciosa. Los establecimientos de salud deben realizar vigilancia epidemiológica de gérmenes más frecuentes y sensibilidad antibiótica, para garantizar tratamiento antibiótico adecuado.


Neonatal sepsis is a major cause of morbidity and mortality. The WHO reports that 2.4 million babies die annually during the first month of life, with neonatal sepsis being one of the main causes. The objective is to update the diagnosis and treatment of neonatal sepsis. In this research, a review of the literature was carried out to synthesize relevant findings on neonatal sepsis, which is defined as a systemic condition, characterized by hemodynamic changes and other clinical manifestations, as a deregulated response to infection. Early-onset neonatal sepsis is related to maternal risk factors and the causative microorganisms are from the maternal genitourinary tract, while late-onset sepsis is caused by nosocomial germs, in neonates with prolonged hospital stay and subjected to invasive procedures. The signs and symptoms of neonatal sepsis are nonspecific, which makes its diagnosis difficult, it is recommended to identify risk factors and a thorough clinical evaluation. Health establishments must carry out epidemiological surveillance of the most frequent germs and antibiotic sensitivity, to guarantee adequate antibiotic treatment.

Objetivo: Determinar las características maternas de mujeres infectadas con COVID-19 y de sus neonatos nacidos en el Hospital de atención referencia Covid-19 (HRDT) entre abril y setiembre del 2020. Material y Métodos: Estudio observacional y transversal realizado en el HRDT que incluyó 703 pacientes obstétricas con COVID-19. Resultados: La edad promedio fue de 27 años. El 35.7 % fueron nulíparas y el 95% fueron asintomáticas para COVID-19. Tuvieron COVID-19 leve, moderado y severo el 3%,1% y 1%; respectivamente. Los síntomas más frecuentes fueron tos (84,85%) y anosmia (39,39%). Los signos más comunes fueron Taquipnea (60.61%) y dificultad respiratoria (51,52 %). La gasometría, dímero-D y ferritina sérica estuvieron alterados en el 71%, 54% y 30%; respectivamente. Las complicaciones médicas más frecuentes fueron: sepsis (2,28%), neumonía (2,13%), insuficiencia respiratoria (2,13%) y shock séptico (1,14%). Las complicaciones obstétricas más frecuentes fueron Rotura prematura de membranas (6,69%), preeclampsia severa (5,41%), síndrome abortivo (3.27%) y trabajo de parto pretérmino (2,56%). El 73,9% de partos fue por Cesárea. El 0,99 % de pacientes requirió unidad de cuidados intensivos y la letalidad materna fue del 0,2%. El 85,7% de neonatos fueron a término y el 97,2 % tuvo APGAR a los 5 minutos ≥7. Hubo 4 muertes neonatales (0,64%) y 16 óbitos (2,58%). Hubo 8 RT-PCR positivas en neonatos (1,3%). Conclusión: La mayoría de pacientes fueron nulíparas, menores de 35 años y asintomáticas para COVID-19. La mayoría de neonatos nacieron por cesárea y evolucionaron favorablemente. Hubo dos muertes maternas, 16 óbitos fetales y 4 muertes neonatales.

There were 4 neonatal deaths (0.64%) and 16 deaths (2,58%). There were 8 positive RT-PCR in neonates (1.3%). Conclusion: The majority of patients were nulliparous, under 35 years of age and asymptomatic for COVID-19. Most of the newborns were born by cesarean section and evolved favorably. There were two maternal deaths, 16 stillbirths, and 4 neonatal deaths.

INTRODUCTION: COVID-19 disease can affect women at any stage of pregnancy, and newborns could become infected with SARS-CoV-2 through vertical or horizontal transmission. OBJECTIVE: To determine clinical and epidemiological characteristics of mothers with COVID-19, associated neonatal outcomes, and to evaluate SARS-CoV-2 vertical transmission. METHODS: We conducted an observational, descriptive, cross-sectional study. We included all mothers with positive serology for SARS-CoV-2 and their newborns at the Hospital Regional Docente de Trujillo from April 18 to September 30, 2020. Variables were collected from the medical records, and descriptive statistics were used for the analysis. RESULTS: A total of 647 mothers and 656 neonates were enrolled. Of all live births, 85.3% and 14.7% were term and preterm neonates, respectively. We found 1.7% (11/656) of newborns with positive RT-PCR for SARS-CoV-2; and that 27.3% (3/11) of these neonates required hospitalization. Neonatal mortality was 4/656 (0.6%), and no case was attributed to COVID-19. Of all mothers affected with COVID-19, 95.7% were asymptomatic, and 4.3% presented clinical symptoms attributed to COVID-19, most of which were mild. The most frequent obstetric complications were preeclampsia-eclampsia, prelabour rupture of membranes, and acute fetal distress. All the mothers were discharged. CONCLUSION: We found 1.7% of newborns with positive RT-PCR test for SARS-CoV-2; and that 20.1% of these neonates were hospitalized. The most frequent morbidity was neonatal sepsis and prematurity. The infection was mild among newborns, showing a 0.6% overall mortality, with no cases attributed to COVID-19. We found that only 5% of mothers presented symptoms, most of which were mild to moderate symptoms. There was no record of maternal mortality in this study group. It is not possible to conclude whether vertical transmission or intrapartum-acquired infection is responsible for neonatal COVID-19 infections.


INTRODUCCIÓN: La enfermedad de COVID-19 puede afectar a gestantes en cualquier trimestre del embarazo. Por su parte, los neonatos podrían infectarse con SARS-CoV-2 por transmisión vertical u horizontal. OBJETIVO: Determinar las características clínicas y epidemiológicas de madres con COVID-19, de sus neonatos y la transmisión vertical del SARS-CoV-2. MÉTODOS: Estudio observacional, descriptivo, transversal. Se incluyeron todas las madres con serología positiva para SARS-CoV-2 y sus neonatos nacidos en el Hospital Regional Docente de Trujillo desde el 18 de abril hasta el 30 de septiembre de 2020. La información para las variables se recogió de las historias clínicas. Para el análisis se usó estadística descriptiva. RESULTADOS: Participaron 647 madres y 656 neonatos. El 85,3% de los neonatos nació de término y el 14,7% fue prematuro. El 1,7% (11/656) tuvieron PCR-RT positivos para SARS-CoV-2, y de ellos el 27,3% (3/11) requirió hospitalizados. La mortalidad fue de 4/656 (0,6%), no atribuida a COVID-19. De las madres afectadas con COVID-19, 95,7% fue asintomática, el 4,3% presentó sintomatología clínica atribuida a COVID-19, siendo en su mayoría casos leves. Las complicaciones obstétricas más frecuentes fueron preeclampsia, eclampsia, rotura prematura de las membranas y sufrimiento fetal agudo. Todas las madres fueron dadas de alta. CONCLUSIÓN: De los neonatos estudiados, el 1,7% presentó prueba PCR-RT para SARS-CoV-2 positiva. El 20,1% fue hospitalizado. La morbilidad más frecuente fue sepsis neonatal y prematuridad. La mortalidad fue de 0,6%, ningún caso atribuido a COVID-19. El cuadro clínico de esta patología fue leve en los neonatos. El 95% de las madres con COVID-19 fueron asintomáticas. De las gestantes que presentaron cuadro clínico, tuvieron sintomatología leve a moderada. No se tuvo registro de mortalidad materna en el grupo de estudio. No se puede concluir si se trata de casos de transmisión vertical del SARS-CoV-2 o estamos frente a casos de posible infección neonatal adquirida intraparto.

Loyalty is important in the tourism sector since tourists are the key to returning to a destination or recommending it, which is a determining factor in the management of tourist sites. The tourism of Mosques, is a contextualized tourism within religious and cultural tourism. This research aims to analyze the loyalty of tourists of Islamic origin in the Cathedral Mosque of Cordoba. Unlike previous studies, this research adopts a comprehensive approach by considering cultural factors in the analysis of loyalty of Islamic tourists in mosque tourism. The methodology used in this study was a structural equation model with a partial least squares (PLS) analysis. The sample is made up of 262 tourists of Islamic origin at Cordoba Cathedral Mosque. This model does not correspond to factors identified by the previous literature, which adopts an religious perspective of Islamic tourists in mosque tourism. Internal marketing involves the development of organizational strategies that promote the welfare of the employees who, in turn, play a transcendental role in achieving institutional goals. Ecuadoran co-operativism lacks relevant studies of this construct and, because of this, this research intends to analyze the dimensions of internal marketing, through the validation of a measuring instrument that has been adapted to this sector of the social economy, in a developing country. The methodology is based on the completion of field work, where a structured questionnaire for a representative sample of 2,499 employees and officers of the operatives of Ecuador was applied. The suitability of the scale was determined through the means of a model of Ecuadoran structural equations. The results show that internal marketing is a multi-dimensional construct and it can be measured in six dimensions: Identify Value Exchange (IVE), Internal Market Segmentation (IMS), Internal Communication (IC), Management Concern (MC), Training (TR), and Work/Family Balance (WFB).

Tourism in synagogues is an activity which is increasing in popularity due to the current interest in cultural and patrimonial heritage. The synagogue in Córdoba has become an irreplaceable Jewish tourist destination in Spain due to its origins, construction, conservation, and recognition by international organizations. This article analyzes the loyalty of tourism in synagogues using variables such as satisfaction, loyalty and the expected and perceived value of the destination and the historical monument using a structural research equations methodology. No previous studies on tourism at synagogues using structural equations have been made. The data was obtained by means of a survey given to 350 tourists visiting this cultural heritage site. The results showed that there is strong loyalty to the heritage site as well as the destination.

Several previous studies have proposed the segmentation of tourists based on their motivations, their sociodemographic profile and the characteristics of their trip. This study has focused on proposing a segmentation of tourists that uses a peer-to-peer accommodation (p2p) based on their motivations, socio-demographic profile and the characteristics of their trip. This investigation is based on the application of a questionnaire to a representative sample of visitors in the city of Córdoba and who choose a peer-to-peer accommodation. Factorial analysis has been used as a data reduction technique in order to find homogeneous groups from the point of view of the reasons for choosing "peer-to-peer" tourism. The results confirm the existence of four types of p2p tourists clearly differentiated according to the motivations that have led them to choose this type of accommodation. The study shows the profile and preferences relative to the characteristics of the trip for each of the segments detected. Likewise, a high degree of fidelity towards the p2p accommodation modality was detected in the respondents-seven out of ten choose collaborative accommodation.

Objetivo: Comparar la supervivencia de los anti-TNF subcutáneos utilizados durante el periodo 2008-2012 según práctica clínica. Material y métodos: Estudio observacional retrospectivo de todos los pacientes diagnosticados de AR que habían iniciado tratamiento con un anti-TNF subcutáneo y mantenido durante al menos 6 meses. Los datos fueron analizados mediante SPSS V17,0. Resultados: Cuarenta y nueve pacientes con AR iniciaron tratamiento con anti-TNF subcutáneo (32 con etanercept y 17 con adalimumab). La media de edad fue de 45,94 años (75,5% mujeres). La media de duración de la enfermedad previa al inicio del anti-TNF fue de 2,67 años. La media de edad al inicio del tratamiento fue de 51,84 años, índice de actividad de la enfermedad en 28 articulaciones medio de 4,93. La supervivencia media del tratamiento anti-TNF fue de 8,40 años, mostrando una mayor supervivencia etanercept. La principal razón de discontinuación fue por fallo secundario (90,9%). Conclusión: En la práctica clínica habitual, la supervivencia a largo plazo de los tratamientos anti-TNF subcutáneos fue elevada e independiente de que tuvieran o no tratamiento inmunosupresor concomitante

OBJECTIVE: To assess effectiveness and safety of certolizumab PEGol (CZP) in rheumatoid arthritis (RA) patients after 12 months of treatment and to detect predictors of response. METHODS: Observational longitudinal prospective study of RA patients from 35 sites in Spain. Variables (baseline, 3- and 12-month assessment): sociodemographics, previous Disease Modifying Anti-Rheumatic Drug (DMARD) and previous Biological Therapies (BT) use; TJC, SJC, ESR, CRP, DAS28, SDAI. Response variables: TJC, SJC, CRP, ESR, and steroids dose reductions, EULAR Moderate/Good Response, SDAI response and remission, DAS28 remission. Safety variables: discontinuation due to side-effects. Descriptive, comparative and Logistic regression analyses were performed. RESULTS: We included 168 patients: 79.2% women, mean age 54.5 years (±13.2 SD), mean disease duration 7.5 years (±7.3 SD). Mean number of prior DMARD: 1.4 (±1.2 SD), mean number of prior BT was 0.8 (±1.1). Mean time on CZP was 9.8 months (±3.4 SD). A total of 71.4% were receiving CZP at 12-month assessment. Baseline predictors of response: lower prior number DMARD; low number prior BT; higher CRP, ESR, TJC, SJC, DAS28 and SDAI (p < 0.05) scores. A 25/46.4% Moderate/Good Response, a 20% SDAI remission, and a 44% DAS28 remission were observed. We observed 48 discontinuations (28.6%), 31 due to partial or complete ineffectiveness, and 17 due to side-effects. CONCLUSIONS: CZP showed benefit in severe RA patients, with significant reduction of all effectiveness parameters, despite the high prevalence of previous BT exposure in our series. We found CRP, ESR, prior DMARD/BT number, TJC, SJC, DAS28, and SDAI as baseline predictors of response. CZP was mostly well tolerated.

This study aimed to identify differentially expressed genes in Procambarus clarkii crayfish collected from locations of different environmental qualities in the Doñana National Park surrounding areas. The pollution sustained by the crayfish was confirmed by their hepatopancreatic metal concentration. We generated forward and reverse libraries by suppression subtractive hybridization (SSH) to analyze the transcriptional profiles of crayfish from moderately and highly polluted zones in comparison with the control site within the Doñana Biological Reserve. Forty-three differentially expressed genes were detected, and most of them were identified as genes involved in a variety of biological functions, particularly in the innate immune response. To verify the SSH results and assess interindividual variability nine transcripts (ALP, AST, BTF3, CHIT, CTS, ferritin, HC, HC2, and SPINK4) were selected for absolute quantification by real-time qRT-PCR. The qRT-PCR data revealed substantial differences in the absolute amounts of the nine transcripts and confirmed their up- or down-regulation in the polluted sites. Additionally, a positive and significant linear correlation was found between the hepatopancreatic copper concentration and the levels of the transcripts encoding hemocyanins. Finally, the transcriptomic study was complemented with a detailed analysis of SNP profiles of the selected transcripts that revealed point mutations that might underlie adaptive response to environmental stress in P. clarkii. Overall, this work provides novel insights into the molecular pathways that could mediate the response to environmental pollutants in P. clarkii emphasizing the central role of the immune function and thus, should clearly benefit further immunotoxicological research in this organism.

Biomonitoring requires the application of batteries of different biomarkers, as environmental contaminants induce multiple responses in organisms that are not necessarily correlated. Omic technologies were proposed as an alternative to conventional biomarkers since these techniques quantitatively monitor many biological molecules in a high-throughput manner and thus provide a general appraisal of biological responses altered by exposure to contaminants. As the studies using omic technologies increase, it is becoming clear that any single omic approach may not be sufficient to characterize the complexity of ecosystems. This work aims to provide a preliminary working scheme for the use of combined transcriptomic and proteomic methodologies in environmental biomonitoring. There are difficulties in working with nonmodel organisms as bioindicators when combining several omic approaches. As a whole, our results with heterologous microarrays in M. spretus and suppressive subtractive hybridization (SSH) in P. clarkii indicated that animals sustaining a heavy pollution burden exhibited an enhanced immune response and/or cell apoptosis. The proteomic studies, although preliminary, provide a holistic insight regarding the manner by which pollution shifts protein intensity in two-dimensional gel electrophoresis (2-DE), completing the transcriptomic approach. In our study, the sediment element concentration was in agreement with the intensity of protein expression changes in C. maenas crabs. In conclusion, omics are useful technologies in addressing environmental issues and the determination of contamination threats.

This case report describes a 20-year-old immunocompetent man with an episode of chest pain radiating into both arms, an increase in the level of myocardial enzymes, electrocardiogram abnormalities (widespread ST-segment elevation and q waves in leads V(4)-V(6)) and serological evidence for acute Epstein-Barr Virus infection preceding typical signs and symptoms of infectious mononucleosis.
