INTRODUCTION: A multitude of topical wound treatments are used today. Although it is well established that the micro-environment of healing wounds can be altered to improve healing, it is difficult to measure the subtle differences in outcome where therapies are compared. METHOD: We compared wound healing properties between four different topical agents in surgically incised wounds in a pig model. The four topical agents, 5% Povidone-Iodine cream, 1% Silver-Sulphadiazine, 2% Mupirocin, and 1% Silver-Sulphadiazine plus 1mg/100g recombinant-human epithelial growth factor (EGF) were randomly assigned to four test animals each. Test agents were compared to each other and to untreated controls. We investigated existing and new methodologies of measurement of wound healing: clinical and histological visual scoring systems, immuno-histochemistry, and computerized image analysis of the wounds on days 3, 7, and 28. RESULTS: All agents were found to have improved healing rates with better cellular architecture. Healing was faster, histological appearance resembled normal architecture sooner, clinical appearance improved, mitotic activity was stimulated and more collagen was deposited in comparison to the wounds with no agents. EGF-treated wounds showed an increased rate of epithelisation, but the rate of healing did not correlate well with evaluation of cosmetic outcome. CONCLUSION: Topical agents improve all aspects of wound healing. The addition of a human recombinant EGF to Silver-Sulphadiazine increases epithelial growth and amounts of collagen in the regenerating wounds at day 7.

Acral lentiginous melanoma is a particularly aggressive tumour with a worse prognosis than other varieties of primary cutaneous melanoma. In order to study the biology of this disease, the activity of the c-myc oncogene was studied in tumours from 45 patients using flow cytometry. High levels of oncoprotein were found in all tumours and exceeded that documented in other varieties of cutaneous melanoma. Survival analysis with stratification of patients according to oncogene activity provided a useful prognostic marker with shorter disease free interval (Log-Rank test, chi 2 = 16.7, P < 0.0001) and overall survival (Log-Rank test, chi 2 = 8.9, P = 0.002) in tumours with high oncoprotein levels. Multivariate analysis revealed c-myc oncogene expression to be more accurate in predicting clinical outcome than all existing clinicopathological parameters including the Breslow depth (Cox's proportional hazards model, P = 0.0011). This study provides biological evidence to explain the aggressive behaviour of acral melanoma and supports the application of oncogene measurement as an accurate prognostic marker.

Subungual melanoma is a particularly aggressive tumour. However, biological investigations of its behaviour are presently lacking due to its comparative rarity. In order to study the biology of this disease, the activity of the c-myc oncogene was studied in tumours from 24 patients with subungual melanoma using the technique of flow cytometry. High levels of oncoprotein were found in all tumours and exceeded that documented in other varieties of cutaneous melanoma. Survival analysis revealed that stratification of patients according to oncogene activity provided a useful prognostic marker with shorter disease free interval (log rank test, chi 2 = 6.6, P = 0.01) and overall survival (log rank test, chi 2 = 3.6, P = 0.07) in tumours with high oncoprotein levels. This is the first study to investigate oncogene expression in subungual disease and supports its potential application as a prognostic marker.

RESUMO

A primary mucosa-associated lymphoid tissue lymphoma of the lung in a 27-year-old man was found. The tumor was composed mainly of centrocyte-like cells and plasma cells. These two components were demarcated sharply from each other, resulting in a zoning or biphasic pattern. This characteristic pattern also was present in the involved regional lymph nodes. Monoclonality of tumor cells was shown immunohistochemically and by in situ hybridization techniques. This article also highlights the use of in situ hybridization in detecting light chain mRNA in paraffin sections.

RESUMO

Fifty sections showing cervical intra-epithelial neoplasia grade III were stained with mucicarmine and periodic acid-Schiff reagents to demonstrate mucin production. Seven of these showed mucin within the neoplastic cells (14%); these cases were considered to be a form of adenosquamous carcinoma in situ and lend support to the hypothesis that invasive adenosquamous carcinomas arise from an indifferent reserve cell.

A 57-year-old man had renal stones, and biochemical investigation led to a diagnosis of primary hyperparathyroidism. Surgical exploration revealed bilateral inferior parathyroid enlargement. Both glands were removed; macroscopically, small cysts were seen on cut sections. Histologic examination showed broad bands of fibrosis, lymphoid follicles, and plasma cells that diffusely effaced the parathyroid architecture. Such features--if seen in the thyroid gland--would be reminiscent of an autoimmune process. The cysts were lined by respiratory and squamous epithelia and contained lymphoid follicles in their walls. Less affected areas of the parathyroid tissue were hyperplastic. It is believed that the inflammatory response in the parathyroid glands that is described in this article may be characteristic. It may result from the cysts or their contents or from an autoimmune reaction.

RESUMO

Muco-epidermoid and adenoid cystic carcinomas of the respiratory tree are uncommon and warrant attention because of controversy and uncertainty surrounding their behaviour and management. Ten cases of these tumours are detailed, with emphasis on approach to management. Careful attention should be paid to histological grading of muco-epidermoid carcinomas. Low grade tumours, completely excised, behave the best. Adenoid cystic carcinomas are slow-growing, relentlessly progressive tumours and difficult to eradicate locally. Surgery remains the best therapeutic tool available.
