RESUMO

A mucoid degeneration of the anterior cruciate ligament (ACL) is regarded as a degenerative change in the ligament, which is clinically presented with pain on full extension or flexion. Regarding morphological factors, it has been reported that an increased posterior tibial slope can be a cause of ACL degeneration secondary to the repetitive overload. The increase in the tibial slope is among the potential problems after medial opening wedge high tibial osteotomy (OWHTO). Especially, a large wedge opening in the correction of severe varus deformity may lead to non-physiologic bony geometry including an increased posterior tibial slope and medial tibial coronal inclination. We present a 69-year-old man had undergone OWHTO with a wedge correction angle of 12.4° for Kellegren-Lawrence grade 2, medial uni-compartmental osteoarthritis of the left knee. Evaluations of the postoperative radiographs revealed postoperative changes in radiological parameters with mechanical medial proximal tibial axis (mMPTA) from 81.3° to 94.3°, and posterior tibial slope (PTS) from 12.2° to 15.8°. Physical examination at 3 years after surgery revealed a knee extension of 0° and a limitation to knee flexion with maximum flexion of 110° and, and severe knee pain was elicited when the knee approached deep flexion. MRI revealed an increased signal intensity along the substance of the ACL and multiple cystic lesions indicative of a ganglion formation around the proximal ACL attachment site extending into the adjacent lateral femoral condyle. Microscopic examination of the resected tissues showed mucoid degeneration and mucous cysts indicative of ganglions formation within the ligament substance and the bone at the attachment site. The reported case illustrates the importance of being aware of this potential complication following OWHTO.

Cesium lead halide perovskite nanocrystals are widely studied as among the most attractive emissive nanomaterials because of their high photoluminescence quantum yield and tunable emission wavelengths over the whole visible-light region by the halide ion-exchange reaction. However, the reactions were often observed in solution and generally very fast, which interferes with the fine-tuning capability of the emission properties. Here, we report a novel nanocrystal-organogel hybrid soft material in which the perovskite nanocrystals in a supramolecular gel exhibit extremely slow and inhomogeneous anion-exchange reactions that are different from those in solution. Furthermore, the inhomogeneous emission in the gel became homogeneous over several days due to a slow diffusion.

Plant defense against herbivores is modulated by herbivore-associated molecular patterns (HAMPs) from oral secretions (OS) and/or saliva of insects. Furthermore, feeding wounds initiate plant self-damage responses modulated by danger-associated molecular patterns (DAMPs) such as immune defense-promoting plant elicitor peptides (Peps). While temporal and spatial co-existence of both patterns during herbivory implies a possibility of their close interaction, the molecular mechanisms remain undetermined. Here we report that exogenous application of rice (Oryza sativa) peptides (OsPeps) can elicit multiple defense responses in rice cell cultures. Specific activation of OsPROPEP3 gene transcripts in rice leaves by wounding and OS treatments further suggests a possible involvement of the OsPep3 peptide in rice-herbivore interactions. Correspondingly, we found that simultaneous application of OsPep3 and Mythimna loreyi OS significantly amplifies an array of defense responses in rice cells, including mitogen-activated protein kinase activation, and generation of defense-related hormones and metabolites. The induction of OsPROPEP3/4 by OsPep3 points to a positive auto-feedback loop in OsPep signaling which may contribute to additional enhancement of defense signal(s). Finally, the overexpression of the OsPep receptor OsPEPR1 increases the sensitivity of rice plants not only to the cognate OsPeps but also to OS signals. Our findings collectively suggest that HAMP-DAMP signal integration provides a critical step in the amplification of defense signaling in plants.

The effects of dietary soybean ß-conglycinin on lipid metabolism and energy consumption were studied in Wistar adult rats. Rats were fed, a diet containing casein (control group) or ß-conglycinin (ß-conglycinin group), for 4 weeks. Carbohydrate consumption was higher and fat consumption was lower in the ß-conglycinin group than in the control group, whereas the total energy consumption was the same between the two groups. Serum adiponectin was higher in the ß-conglycinin group than in the control group. Serum triacylglycerol levels in the ß-conglycinin group were significantly lower than those in the control group. The secretion rate of triacylglycerols from the liver after the administration of tyloxapol, an inhibitor of lipolysis, was significantly lower in the ß-conglycinin group than in the control group. These results suggest the possibility that ß-conglycinin exerts hypolipidemic effects through an acceleration in carbohydrate consumption associated with an increase in adiponectin in rats.

OBJECTIVES: Studies of palliative care are often performed using single-arm pre-post study designs that lack causal inference. Thus, in this study, we propose a novel data analysis approach that incorporates risk factors from single-arm studies instead of using paired t-tests to assess intervention effects. METHODS: Physical, psychological and social evaluations of eligible cancer inpatients were conducted by a hospital-based palliative care team. Quality of life was assessed at baseline and after 7 days of symptomatic treatment using the European Organization for Research and Treatment of Cancer QLQ-C15-PAL. Among 35 patients, 9 were discharged within 1 week and 26 were included in analyses. Structural equation models with observed measurements were applied to estimate direct and indirect intervention effects and simultaneously consider risk factors. RESULTS: Parameters were estimated using full models that included associations among covariates and reduced models that excluded covariates with small effects. The total effect was calculated as the sum of intervention and covariate effects and was equal to the mean of the difference (0.513) between pre- and post-intervention quality of life (reduced model intervention effect, 14.749; 95% confidence intervals, -4.407 and 33.905; p = 0.131; covariate effect, -14.236; 95% confidence interval, -33.708 and 5.236; p = 0.152). CONCLUSION: Using the present analytical method for single-arm pre-post study designs, factors that modulate effects of interventions were modelled, and intervention and covariate effects were distinguished based on structural equation model.

BACKGROUND: In May 2012, two schoolchildren developed allergic symptoms after eating a school meal of fried bread in Osaka, Japan. One specific raw material, milk, should not be present in fried bread. However, we suspected the unintentional mixing of milk in the bread manufacturing process. Our aim was to verify the reason if this was so. METHODS: We first manufactured bread that contained milk as one of its components. We then thoroughly cleaned the manufacturing line except for the dough divider and, as per the revised instruction manual, continuously manufactured bread that did not contain milk. In this manner, we tried to simulate the conditions at the time of the incident. Casein is a major milk protein. We, therefore, determined casein concentration in the milk-free bread, using casein enzyme-linked immunosorbent assay. RESULTS: We observed casein levels higher than 1000 ppm in the initial lots of breads, but lower levels in the later lots. Casein levels also decreased upon frying of bread. CONCLUSION: High casein levels, which were observed in the earlier lots of breads, were lower in the later lots. In the manufacturing line, the remnants of the bread dough that contained milk presumably got mixed with the dough used in the subsequent manufacture of milk-free bread.

BACKGROUND: The prevention of contrast-induced acute kidney injury (AKI) after coronary computed tomography angiography (CCTA) is important because patients referred for CCTA often need further contrast exposure such as an invasive coronary angiography. We aimed to examine the effects of oral volume intake on renal function in patients with preserved renal function referred for CCTA. METHODS: We enrolled 180 patients who were referred for CCTA. The serum creatinine (SCr) and estimated glomerular filtration rate (eGFR) levels were measured before, 24h, and a mean of 4.8 days after CCTA. The amount of unrestricted oral fluid intake for 24h was checked. The patients were divided into two groups: 106 subjects with a rise in SCr after CCTA (group A); and 74 without (group B). RESULTS: Significant correlations were observed between the amount of oral fluid intake and the percentage changes in SCr (%SCr) (r=-0.66, p<0.0001) as well as the absolute changes in eGFR (ΔeGFR) (r=0.65, p<0.0001). The percentage of patients showing hemoglobin-A1c (HbA1c)≥6.5% was greater in group A than in group B (29% vs. 18%, p<0.001). Patients with HbA1c≥6.5% showed higher %SCr and lower ΔeGFR compared to those without it. Multiple regression analysis revealed that the amount of oral fluid intake was the only independent predictor for a rise in SCr (ß=-0.731, p<0.0001). CONCLUSION: Oral volume intake after CCTA is a very simple but important prophylactic procedure for contrast-induced AKI especially in diabetic patients.
