RESUMO

The purpose of the present study was to develop a surgical treatment for congenital microphthalmos, by using the biomaterial Alloplant that causes an increase in eyeball sizes. There were 48 cases of unilateral congenital microphthalmos prior to surgery. The patients' age ranged from 2 to 14 years; the duration of follow-ups was 1 to 6 years. Data processing by parametric variance analysis indicated that there was an increase in the mean sizes of the anteroposterior and transverse axes (APA and TA) of the eye 2-3 years following surgery. With the use of the authors' method, the conjunctival cavity is not deformed and, on the contrary, after surgery it acquires a regular slit-like shape with vaults of sufficient depth, which upgrades the quality of prosthetic repair in future. The increases in the sizes (APA and TA) and volume of the eyeball not only just after surgery, but also in the late periods (up to 2-3 years) suggest that the developed operation using the biomaterial Alloplant triggers the mechanism of coordinated eyeball growth.

The effects of operations using alloplant biomaterial on the time course of visual functions were evaluated in patients with pigmented degeneration of the retina in various terms after surgery. 198 patients with terminal and subterminal stages of pigmented degeneration of the retina were operated on by inserting alloplant into the suprachoroidal space, after which they were observed for 1, 2, and 3-5 years. Visual acuity, kinetic and static visual fields were measured and computer perimetry was carried out. Analysis of the results indicates that the operation making use of alloplant leads to statistically significant improvement of visual fields and visual acuity and to stabilization of these functions during the periods of observations in patients with subterminal and terminal stages of pigmented degeneration of the retina. These data allow us to regard the operation with alloplant as one of the methods inhibiting the development of pigmented degeneration of the retina and in some cases improving the visual function.

PURPOSE: To reduce antigenicity of allografts and stimulate their replacement by natural recipient tissues. METHODS: Experimental allotransplantation of different tissues (fascias, tendons, derma, fat, etc.) with histological, histochemical, electron microscopical, electron histochemical examination 3, 5, 7, 14, 21, 30, 60, 120, 180 and 360 days postoperatively. RESULTS: Allografts of different tissues with glucosaminoglycans extracted from collagen fibers have low antigenicity and can be replaced by natural tissues. Allografts with these properties were named 'Alloplant'. CONCLUSIONS: Alloplant biomaterials can selectively stimulate natural tissue regeneration. Hence, Alloplant biomaterials can be utilized in surgery for the restoration of different tissues.
