PURPOSE: To evaluate if topical povidone iodine (alone (PI) or combined with dexamethasone (PI-DXM)) is superior to placebo for treating adenoviral conjunctivitis (AC). METHODS: A systematic review was performed according to Preferred Reporting Items for the Systematic Review and Meta-Analyses (PRISMA) Statement. An electronic search was made on PubMed, Embase and Cochrane Library. Randomized control studies that compared PI or PI-DXM with placebo were included. At least three researchers were involved in all phases. Primary outcomes were AC duration and the number of clinical resolutions during the first week. Secondary outcomes were conjunctival redness and serous discharge one week after starting treatment and the rate of AC complications. RESULTS: Only five studies met the inclusion criteria. PI-DXM reduced the duration of the disease by 2.4 days (IC95% 4.09-0.71), however this result was based only in one study. PI and PI-DXM did not modify the probability of clinical resolution during the first week; relative risk (RR) = 1.77 (IC95% 0.63-4.96) and 1.70 (IC95% 0.67-4.36). The impact of PI on the probability of pseudomembranes could not be estimated. PI-DXM did not influence the risk of developing subepithelial infiltrates RR = 0.73 (IC95% 0.02-33.38). CONCLUSIONS: At this time there is great uncertainty about the usefulness of PI on the course of adenoviral conjunctivitis. PI-DXM may have a small effect on AC duration. To make future reviews possible, it is important to standardize the way in which these results are reported. Futures studies should include etiological confirmation, unit of study (eyes vs patients) and report on those aspects that are more relevant for patient quality of life (duration of the disease, development of complications: pseudomembranes and subepithelial infiltrates).

The emergence of collective intelligence has been studied in much greater detail in small groups than in larger ones. Nevertheless, in groups of several hundreds or thousands of members, it is well-known that the social environment exerts a considerable influence on individual behavior. A few recent papers have dealt with some aspects of large group situations, but have not provided an in-depth analysis of the role of interactions among the members of a group in the creation of ideas, as well as the group's overall performance. In this study, we report an experiment where a large set of individuals, i.e., 789 high-school students, cooperated online in real time to solve two different examinations on a specifically designed platform (Thinkhub). Our goal of this paper 6 to describe the specific mechanisms of idea creation we were able to observe and to measure the group's performance as a whole. When we deal with communication networks featuring a large number of interacting entities, it seems natural to model the set as a complex system by resorting to the tools of statistical mechanics. Our experiment shows how an interaction in small groups that increase in size over several phases, leading to a final phase where the students are confronted with the most popular answers of the previous phases, is capable of producing high-quality answers to all examination questions, whereby the last phase plays a crucial role. Our experiment likewise shows that a group's performance in such a task progresses in a linear manner in parallel with the size of the group. Finally, we show that the controlled interaction and dynamics foreseen in the system can reduce the spread of "fake news" within the group.

The characteristics of food allergen labelling are relevant for avoiding accidental exposure to the allergens of interest but no Latin American country has evaluated these characteristics. Our aim was to evaluate the characteristics of food allergen labelling and precautionary allergen labelling (PAL) in six Latin American countries. All data were collected directly from the supermarkets surveyed. A total of 10,254 packaged food products were analyzed, of which 63.3% (n = 6494) and 33.2% (n = 3405) featured allergen labelling and/or PAL, respectively. Most products complied with local regulations (≥87.4% for both locally produced and imported). Thirty-three types of PAL statements were detected; the most frequent was "may contain traces of…" (35.1%). Countries without regulations on the characteristics of allergen labelling had two-fold more products that contained allergens in their ingredients lists but no food allergen labelling. The use of PAL in countries that regulate it (38.2%) was as high as that in countries without PAL regulations (19.2%-44.7%). The findings suggest that the lack of regulations for the characteristics of allergen labeling increases the risk of accidental exposure to allergens of interest. Our findings also suggest that beyond regulations, a scientific approach is required for minimizing and standardizing the use of PAL.

The management of cancer patients' symptoms in doctor consultations is a cornerstone in clinical care, this process being fundamental for the follow-up of the evolution of these. This article presents an application that allows collecting periodically and systematically the data of cancer patients and their visualization by the medical team. In this article, we made the analysis, design, implementation, and final evaluation by analyzing the correlation of this data collection with interaction patterns to determine how the user information can be enriched with information from the interaction patterns. We have followed an agile methodology based on the iterative and incremental development of successive prototypes with increased fidelity, where the requirements and solutions have evolved over time according to the need and assessments made. The comprehensive analysis of the patient's condition allowed us to perform a first analysis of the correlation of the states of patients concerning mood, sleeping quality, and pain with the interaction patterns. A future goal of this project is to optimize the process of data collection and the analysis of information. Another future goal is to reduce the time dedicated to reporting the evolution of symptoms in face-to-face consultations and to help professionals in analyzing the patient's evolution even in the period that has not been attended in person.

RESUMO

An increasing number of cancer patients is treated and recover each year, and consequently there are survivors that require specialized and coordinated follow-up. The physical, social, working, psychological and emotional aspects of these survivors have to be characterized, investigated and treated by multidisciplinary and interdisciplinary teams. Nowadays, oncology community is focused on tracking records of interest in Patient-Reported Outcome (PROs) for patients of different cancer types. In the last years, several articles have proven that PROs are an effective method to improve the management of patient symptoms and, subsequently, clinical care. In this scenario, patient engagement is one of the most relevant aspects for PROs success. In this sense, one of the most promising strategies for increasing engagement is gamification, that is, the introduction of game elements in systems that are not games. Therefore, in this work we introduce a methodology for developing gamification apps for cancer survivors that aims at increasing engagement when collecting PROs data.

The prevalence of food allergy (FA) has not been estimated at a population level in Central American countries and, consequently, the magnitude and relevance of the problem in the Central American region remains unknown. Thus, our aim was to evaluate the parent-reported prevalence of FA in a population of schoolchildren from the Central American country El Salvador. A Spanish version of a structured questionnaire was utilized. Five hundred and eight (508) parents returned the questionnaire with valid responses (response rate, 32%). The estimated prevalence rates (95% CI) were: adverse food reactions 15.9 (13.0⁻19.3), "perceived FA, ever" 11.6 (9.1⁻14.6), "physician-diagnosed FA, ever" 5.7% (4.0⁻8.0), "immediate-type FA, ever" 8.8% (6.6⁻11.6), "immediate-type FA, current" 5.3% (3.6⁻7.6), and anaphylaxis 2.5% (1.5⁻4.3). The most common food allergens were milk (1.7%), shrimp (1.3), chili (0.7%), chocolate (0.7%), and nuts (0.3%). Most of the "food-dependent anaphylaxis" cases (60.5%) sought medical attention, but only one case reported the prescription of an epinephrine autoinjector. Mild and severe FA cases are not uncommon among Salvadoran schoolchildren and both the prescription of epinephrine autoinjectors by healthcare personnel and the use of the autoinjectors by anaphylactic individuals should be encouraged.

Cyclodialysis is the result of the separation of the longitudinal ciliary muscle fibers from the scleral spur, which creates an abnormal pathway for aqueous humor drainage that may lead to ocular hypotony. For many years cyclodialysis was considered a treatment option for glaucoma. However, today it usually occurs as a complication of blunt trauma or more rarely as a complication of anterior segment ocular surgery. Ocular hypotony can lead to cataract development, optic disk swelling, refractive changes, and several retinal complications, making accurate identification and timely intervention of the cleft mandatory. Traditionally gonioscopy was the only available technique to diagnose and localize the cleft. However, other tests such as optical coherence tomography, magnetic resonance imaging, transillumination, and specially ultrasound biomicroscopy are now available for the diagnosis of cyclodialysis. Multiple treatment options are also available for this condition. Although medical treatment can be effective to close small clefts, surgery is needed in most patients to restore ocular pressure.

The prevalence of symptomatic adverse reactions to gluten and adherence to gluten-free diet in Latin American countries is unknown. These measurements are strongly linked to gluten-related disorders. This work aimed to estimate the prevalence of adverse reactions to oral gluten and the adherence to gluten-free diet in the adult Mexican population. To reach this aim, a self-administered questionnaire was designed and tested for clarity/comprehension and reproducibility. Then, a self-administered questionnaire-based cross-sectional study was conducted in the Mexican population. The estimated prevalence rates were (95% CI): 11.9% (9.9-13.5) and 7.8 (6.4-9.4) for adverse and recurrent adverse reactions to gluten respectively; adherence to gluten-free diet 3.7% (2.7-4.8), wheat allergy 0.72% (0.38-1.37); celiac disease 0.08% (0.01-0.45), and NCGS 0.97% (0.55-1.68). Estimated pooled prevalence of self-reported physician-diagnosis of gluten-related disorders was 0.88% (0.49-1.5), and 93.3% respondents reported adherence to gluten-free diet without a physician-diagnosis of gluten-related disorders. Symptom comparisons between those who reported recurrent adverse reactions to gluten and other foods showed statistically significant differences for bloating, constipation, and tiredness (p < 0.05). Gluten-related disorders may be underdiagnosed in the Mexican population and most people adhering to a gluten-free diet are doing it without proper diagnostic work-up of these disorders, and probably without medical/dietician advice.

The purpose of this study is to measure the impact of tamsulosin intake on five postoperative cataract surgery complications (toxic anterior segment syndrome, rebound uveitis, retinal detachment, macular edema, and postoperative endophthalmitis). This retrospective cohort study was conducted at University Hospital of Henares. The study included 660-eyes of 660 patients submitted to cataract surgery at the ophthalmology unit of Hospital del Henares (Madrid) between 2 March 2009 and 28 February 2010. Extracapsular cataract extraction, combined glaucoma and cataract surgery phacovitrectomy, posterior capsule rupture and zonular damage were considered exclusion criteria. Clinical charts were reviewed during July 2012. Patients were divided in two groups (exposed and non-exposed to tamsulosin). Cumulative incidence of toxic anterior segment syndrome, rebound uveitis, retinal detachment, macular edema and postoperative endophthalmitis were compared in both groups. Rebound uveitis (relative risk [RR] 3.39; confidence interval [CI] 1.63-7.08) and macular edema (RR 4.15; CI 1.06-16.22) were more common in the tamsulosin-exposed group. Retinal detachment had a similar incidence in both groups. We observed no cases of endophthalmitis or toxic anterior segment syndrome in either of the two groups. Tamsulosin exposure in this cohort was associated with a higher risk of rebound uveitis and macular edema but the other three studied postoperative complications had a similar incidence in both groups.

The Mexican fruit fly Anastrepha ludens Loew (Diptera: Tephritidae) is one of the most important insects infesting mangoes, citrus, and other fruits in Mexico and other Latin-American countries. Quarantine methods approved to destroy this insect decrease the shelf life of commodities. The objective of this study was to determine the effect of high-pressure processing using an initial temperature of 50 degrees C on the survivorship of eggs and larvae of the Mexican fruit fly. Eggs and larvae were pressurized at 25, 50, 75, 100, or 150 MPa for 0, 5, 10, or 20 min. The hatching ability of pressurized eggs of 1, 2, 3, and 4 days old and survivorship of the first, second, and third instars were registered. Further, the ability to pupate was studied in surviving third instars. The results showed that eggs were more resistant than larvae to the high-pressure processing. Treatments at 150 MPa at initial 50 degrees C for 10 min destroyed all eggs and larvae of A. ludens, indicating that this process might be useful as a quarantine method for infested mangoes or other fruits.

Application of high hydrostatic pressure has been proposed as an alternative quarantine process for the Mexican fruit fly, Anastrepha ludens Loew (Diptera: Tephritidae), one of the most important insects infesting mangoes, citrus fruits, and other fruits in Mexico and other Latin American countries. The present study was performed to determine the effect of high pressure treatments at 0 degrees C on the survivorship of eggs and larvae of this pest. The effect of time and pressure level at near-freezing temperatures on the egg hatch and survival of larvae is discussed. Eggs and larvae were pressurized at 25, 50, 75, 100, or 150 MPa for 0, 5, 10, or 20 min at 0 degrees C. Hatch was recorded for 1-, 2-, 3-, and 4-day-old pressurized eggs. For pressurized larvae (first, second, and third instars), percentage of survival was registered. Further, third instars were studied for their ability to pupate and develop to adulthood. The results showed that eggs had higher resistance than larvae to pressure. Larvae were not able to resist pressure treatments at 75 MPa for 20 min at 0 degrees C. Treatments at 150 MPa for 20 min at 0 degrees C were needed to destroy all eggs and larvae of A. ludens, indicating that this process might be useful as a quarantine method for infested fruits. However, more studies involving combination of high hydrostatic pressure at low temperature with precooling treatments are needed in order to decrease the pressure level to avoid fruit damage.

A proposta desenvolvida a partir dos profissionais técnicos do Centro de Saúde El Progresso, consiste em um início de um processo de intervenção e participação da comunidade para enfrentar os problemas e contribuir com a melhoria da qualidade de vida da população da área de abrangência, contando com serviços, recursos existentes e o apoio das instituições. Com estas reflexões se promoveu o Processo Comunitário da Margem Direita do Rio Guadiana descreveremos ao longo deste relato. A administração sanitária assume a formação da assistente social da equipe em torno das experiências comunitárias assessoradas pelo consultor externo contratado pelo governo, Dr Marco Marchioni, que contribuiu com a formação necessária para o desenvolvimento do processo comunitário. Para que esta coordenação evolua para uma intervenção conjunta, é necessária a realização de um Diagnóstico Comunitário que possibilite uma visão compartida, global e integral da comunidade. Esta primeira etapa do Processo Comunitário desenvolveu o Diagnóstico com todas as pessoas envolvidas e divulgou os resultados a toda a comunidade. Seguindo o esquema da análise DAFO, se pode dizer que a Margem Direita do Guadiana tem umas debilidades e umas fortalezas (análise interna) que se aproximam uma série de ameaças e de oportunidades (análise externa). Para conseguir participação comunitária e gestão participativa efetiva há que ter persistência e aproveitar as oportunidades. O tecido associativo da Margem direita do Rio Guadiana e o Diagnóstico da área servem para unir as pessoas em um objetivo comum e diminuir a dicotomização existente, fazendo com que as mudanças sejam positivas e que as ameaças se transformem em oportunidades.


The proposal developed from the professional technicians from the Center for Health Progress, consists of a beginning of a process of intervention and participation of the community to address problems and contribute to improving the quality of life of people in the area of coverage, with services, resources and support of existing institutions. With these thoughts are promoted Procedure Committee of the right margin of the Guadiana river describe throughout this article. The administration takes the training of health social worker team's experiences around the community advised by March Marchioni, who helped with the training necessary for the development of the Community procedure. For this coordination to evolve a joint Intervention, it is necessary to carry out a Community Diagnosis enabling a shared vision, comprehensive and integrated community. This first stage of the process developed the Community Diagnosis with everyone involved and reported the results to the entire community. Following the pattern of analysis Dafoe, we can say that the right margin of the Guadiana river has some strengths and some weaknesses (internal analysis) approaching a series of threats and opportunities (external review). To achieve community participation and effective participatory management should be taken persistence and seize the opportunities. The fabric of voluntary right margin of Guadiana River Diagnóstico of area and serve to unite people in a common goal and to reduce existing dichotomization, so that the changes are positive and that threats become opportunities.
