Objetivos: El objetivo de este trabajo es comparar las características articulares de los pies de pacientes con ciática, con las de los pies de personas sin esta patología. Pacientes y métodos: Se trata de un estudio descriptivo transversal. Se incluyeron 20 pacientes con ciática y 20 pacientes sin esta patología emparejados por edad y sexo. Se cuantificó el dolor del pie mediante la Numeric Pain Rating Scale 11 y se midió la movilidad de las articulaciones del tobillo, subastragalina, antepié, primer radio y primer dedo. Se clasificó el tipo de pie mediante el Foot Posture Index, y se cuantificó la discapacidad relacionada con el dolor del pie mediante el cuestionario Manchester Foot Pain and Disability Index. Se compararon estas variables entre los dos grupos de participantes (con ciática y sin ciática). Resultados: Se observó un menor rango de movimiento de pronación subastragalina, así como mayor dolor en el pie y mayor discapacidad relacionada con el dolor en el pie, en los participantes con ciática en comparación con los del grupo control. La extensión del hallux también fue significativamente menor en los sujetos con ciática, aunque solo en el pie izquierdo. Además, la fuerza muscular en estos pacientes fue menor que en los que no tenían ciática. Los participantes de ambos grupos presentaron valores de flexión dorsal del tobillo por debajo de la normalidad. Conclusiones: Los participantes con ciática presentaron ciertas diferencias articulares y musculares con respecto al grupo control, aunque no se puede establecer una relación causa-efecto debido al diseño del estudio (AU)


Objectives: The objective of this work is to compare the joint characteristics of the feet of patients with sciatica, with those of people without this pathology. Patients and methods: This is a cross-sectional descriptive study. Twenty patients with sciatica and 20 patients without this pathology were included. Foot pain was quantified using the Numeric Pain Rating Scale 11, and mobility of the ankle, subtalar, forefoot, first ray, and hallux joints were measured. Foot type was classified using the Foot Posture Index, and disability related to foot pain was quantified using the Manchester Foot Pain and Disability Index questionnaire. These variables were compared between the two groups (participants with and without sciatica). Results: Less subtalar pronation range of motion, as well as greater foot pain and greater foot pain-related disability, were observed in participants with sciatica compared with those in the control group. Hallux dorsalflexion was also significantly less in subjects with sciatica, although only in the left foot. Muscle strength in these patients was lower than in those without sciatica. Participants in both groups presented values of ankle dorsiflexion below normal.Conclusions: The participants with sciatica presented certain joint and muscle differences with respect to the control group, although a cause-effect relationship cannot be established due to the study design (AU)

El dolor es uno de los trastornos que más afecta y preocupa a las personas y es el síntoma acompañante que con mayor frecuencia motiva una consulta médica. La frecuencia de dolor crónico en la población adulta española es muy elevada y constituye un problema generalizado de salud pública. La Escuela de Pacientes ha demostrado su eficacia en el abordaje terapéutico de determinados procesos (hipertensión arterial, diabetes mellitus y otros). Presentamos la primera Escuela de Paciente con Dolor Crónico No Oncológico y Tratamiento Opioide implementada en el Sistema Sanitario Público de Andalucía, que ha permitido formar a pacientes en el tratamiento opioide para su proceso de dolor crónico no oncológico con muy elevado grado de satisfacción


Pain is one of the disorders that most affects and worries people and is the accompanying symptom that most often motivates medical consultation. The frequency of chronic pain in the spanish adult population is very high and constitutes a general publich health problem. The school of patients has demonstrated its effectiveness in the therapeutic approach of certain processes (arterial hypertension, mellitus diabetes and others). We present the first school of patients with non-onologic chronic pain and opioid treatment implemented in the Public Health System of Andalusia, which has allowed training patients with opioids treatment for their pain processes with a very high satisfaction degree

RESUMO

Primary colorectal lymphoma is a rare form of presentation of gastrointestinal tract lymphomas. Inflammatory bowel disease and its treatment are risk factors for its development. We report a 47-year-old male patient with Ulcerative Colitis of two years of evolution, treated initially with azathioprine and later on with infliximab. Due to a relapse in symptoms after the second dose of infliximab, a new coloncoscopy was performed showing a rectal ulcerative lesion, corresponding to a large cell Non-Hodgkin's Lymphoma. The patient was successfully treated with RCHOP chemotherapy (Rituximab cyclophosphamide doxorubicin vincristine prednisone). He is currently in disease remission.

La anemia es una condición altamente prevalente a nivel mundial y, el déficit de hierro, la causa más frecuente, sin excepción; la mujer embarazada está particularmente en riesgo dada la mayor demanda de hierro que la gestación significa. La anemia se asocia a mayor morbilidad y mortalidad materno-perinatal. En mujeres embarazadas sin anemia, la prevención, mediante el uso de multivitamínicos que contienen hierro en dosis de 30-60 mg de hierro elemental, ha demostrado ser efectiva y se recomienda durante todo el embarazo. En casos de anemia, el diagnóstico de déficit de hierro se establece cuando la ferritina es menor a 30 mg/L y/o la saturación de transferrina es menor a 20 por ciento. La severidad de la anemia y la situación temporal en el embarazo, son factores modificantes del tratamiento. En mujeres embarazadas, con hemoglobina ≥9.0 g/dL y que tengan <34 semanas de embarazo, la indicación es tratamiento con hierro oral, en dosis de 100 mg al día, en días alternos, hasta normalizar los parámetros antes mencionados. Si la hemoglobina es <9.0 g/dL, o el embarazo es ≥34 semanas, el uso de hierro intravenoso ha demostrado ser más efectivo en corregir la anemia y el déficit de hierro y en disminuir la morbilidad materno-perinatal. En estos casos, sugerimos el uso del hierro carboximaltosa, dado su perfil de seguridad y efectividad favorable. Recomendamos conocer y practicar estas recomendaciones para el diagnóstico y manejo de la anemia por déficit de hierro durante el embarazo.(AU)


Anemia is a highly prevalent condition worldwide, and iron deficiency the most frequent cause, without exception; pregnant women are particularly at risk given the increased iron demand of gestation. Additionally, anemia directly correlates with increased maternal-perinatal morbidity and mortality. For non-anemic pregnant women, prevention using 30-60mg of elemental iron-containing multivitamins, has proven effective, and is recommended throughout all pregnancy. In the case of anemia, the diagnosis of iron deficiency is established when ferritin is under 30 mg/L or when transferrin saturation is under 20 percent. The severity of the anemia and the time course of pregnancy, are treatment determining factors. Pregnant women, with hemoglobin ≥9.0 g/dL, and <34 weeks-pregnant, are best treated with oral iron, at dose of 100 mg of elemental iron, on alternate days, until all altered parameters are corrected. Otherwise, when hemoglobin is <9.0 g/dL, or pregnancy is ≥34 weeks, intravenous iron has demonstrated to be more effective to normalize anemia, iron deficiency and diminish maternal-perinatal morbidity. In those cases, we suggest the use of carboximaltose iron, due to its favorable safety and efficiency profile. We recommend knowing and practicing these recommendations for the diagnosis and management of iron deficiency anemia during pregnancy.(AU)

Thrombotic thrombocytopenic purpura, an immune/non-immune thrombotic microangiopathy (TTP/TMA) is associated with high morbidity and mortality, even with appropriate treatment. In patients refractory to standard treatment with plasmapheresis there is no certainty about the best therapeutic strategy. This report shows our experience in eight refractory patients who survived after treatment with rituximab.

Este estudio valida, a nivel confirmatorio, la escala de comportamiento ecológico que mide la realización de acciones que procuran proteger y/o conservar el medio ambiente y las escalas de las variables utilizadas en la literatura para explicar dicho comportamiento: el conocimiento medioambiental, las creencias asociadas a la preocupación ambiental, los valores y las intenciones de comportamiento ecológico. Sobre una muestra de 497 individuos en España, seleccionada por muestreo no probabilístico por cuotas, se analizan las propiedades psicométricas de las escalas mediante análisis factorial exploratorio y confirmatorio. Los resultados muestran el carácter multidimensional de las escalas de conocimiento (conocimiento básico [CB] y conocimiento experto [CEX]) y comportamiento ecológico (gestión ecológica de basuras [GEB] e implicación medioambiental [IM]) y unidimensional del resto de medidas. La validación a nivel confirmatorio de las escalas para las variables seleccionadas proporciona una importante contribución para futuros trabajos de investigación.


This study validates using confirmatory factor analysis the scale of ecological behavior, which measures the implementation of actions that intend to protect and/or preserve the environment, and the scales of variables used in literature in order to explain that behavior: environmental knowledge, beliefs associated with environmental awareness, values and ecological behavior intention. Using a sample of 497 individuals in Spain selected by non-probability quota sampling, the psychometric properties of the scales were analyzed by exploratory and confirmatory factor analysis. The results show the multidimensional nature of knowledge (basic knowledge -CB and expert knowledge -CEX) and environmental behavior (waste management -GEB and environmental implication -IM) scales and the one-dimensional nature of the rest of measures. The validation, using confirmatory factor analysis of the selected variables, provides an important contribution to future research.
