RESUMO

BACKGROUND: Patients undergoing rotator cuff repairs commonly experience postoperative pain and functional limitations. Various socioeconomic and pain-related factors have been recognized as influential in the prognosis of such patients. This study aims to investigate the associations between postoperative pain and functionality and preoperative pain-related outcomes and socioeconomic status in patients undergoing rotator cuff repairs. METHODS: This cross-sectional study examines the relationship between the outcomes of rotator cuff repairs and participants' socioeconomic status and pain-related measures. Socioeconomic status was assessed through indicators such as educational level, monthly household income, and occupation. Pain-related outcomes included measures of kinesiophobia and pain self-efficacy. RESULTS: A total of 105 patients (68 male, 37 female) were included in the analysis. The findings revealed no significant association between postoperative pain or functionality and the patients' socioeconomic status (p > 0.05). However, postoperative pain levels demonstrated a significant association with preoperative kinesiophobia (p < 0.05) and pain self-efficacy (p < 0.013). In contrast, functionality did not exhibit a significant association with these measures (p < 0.072 and 0.217, respectively). CONCLUSIONS: Preoperative pain-related outcomes play a role in postoperative pain levels among patients undergoing rotator cuff repairs. However, they do not appear to be related to functionality. Additionally, socioeconomic status does not significantly impact either pain or functionality.

Objective: to understand the point of view of nurses in the Community of Madrid about their professional experience and its impact on their happiness. Method: a qualitative phenomenological study conducted in nurses from the Community of Madrid, who were purposively selected, looking for professionals currently working in the main areas of professional activity. Data collection was conducted through semistructured interviews. After verbatim transcription, there was subsequent analysis (coding and generation of categories and subcategories). Lincoln’s credibility, transferability, dependability, and confirmability criteria were used to guarantee the quality of the study. Results: the participating nurses claimed that they started studying by vocation, and the act of caring for others was the driver for their happiness at work. On the other hand, one of the factors with direct impact on the development of feelings of frustration, dissatisfaction and unhappiness at work was the lack of enough time to offer quality care to their patients, either due to lack of staff or excessive workload. Conclusion: although nursing professionals in their individual setting are happy in terms of personal and close care for patients and relatives, the profession seems to be going through a moment of disaffection and unhappiness in the collective setting, and this is mainly caused by aspects external to this provision of care, such as the management of their daily practice and the existing image of the profession.(AU)

RESUMO

BACKGROUND: Given the uncontrolled increase of those affected by the SARS-CoV-2 Coronavirus, COVID-19, and the possibility of the health system collapsing in Spain, the Royal Decree 463/2020, of March 14th, declared a state of alarm for the management of this health crisis. The whole population, despite their social background, were confined inside their homes during this arduous situation. There are numerous articles that endorse the idea that their home conditions, the number of people with whom they coexist with, their employment, gender, amount of physical exercise, as well as their body mass index and their diagnosis in COVID-19 are characteristics that may represent an increased risk of suffering pain and changes in their quality of life. We proposed a study to determine the impact of the confinement on pain, quality of life and public health of the Spanish population due to the COVID-19 pandemic. METHODS: An observational study was carried out during April and May 2020 through an online survey. It was statistically analysed through descriptive analysis and inferential analysis by bivariate logistic regression, ordinal logistic and Pearson's chi square. RESULTS: A total of 225 questionnaires were obtained between April and May 2020. For subjects suffering pain before confinement, significant relationships were drawn between experiencing pain and work-related activities (P=0.006), as well as, physical activity or the absence of it (P=0.033). In addition, an important association between experiencing pain and gender was discovered, both for people suffering pain before and during confinement (P=0.007 and P=0.011). Furthermore, the confinement has affected the quality of life of those affected, particularly in relation with suffering pain before and during confinement (P=0.001 and P=0.000), Job type (P=0.016) and BMI (P=0.025) and affecting more Physical Role, Emotional Role and Mental Health, followed by Physical Function. CONCLUSIONS: The presence of pain in subjects undergoing confinement is persistent, with varying intensity and frequency based on age, gender, physical activity, and work status. In any of these conditions, the quality of life of the subjects in confinement has been severely affected, which to this day creates a severe public health problem.

RESUMO

BACKGROUND: Martial arts are currently being practiced throughout the world by about 100 million people. Considering that sports injuries account for 20% of all emergencies treated in hospitals, this is an issue that should be further studied. Our objective was to determine the prevalence and type of injuries in wushu, judo, and karate practitioners in Community of Madrid, Spain. METHODS: A descriptive epidemiology study was carried out. A questionnaire was used to collect data from 457 men and women practicing karate, judo and/or wushu, including injuries from the past five years, type of injury and occurrence during training or competition. RESULTS: Injuries that disrupted training occurred in 56% of judokas, in 36.7% of karatekas and in 38.9% of wushu practitioners (WP). For judokas, the most common injury site was "shoulder/arm/elbow" (13.43%), for karatekas, it was "lower leg/foot/ankle" (9.95%), and for WP it was "groin/hip/thigh" (9.45%) (P=0.000). The most common injuries in judo and karate were "ankle sprains and joint injuries" (4.98% and 3.98%), while for WP the most common injury were "hamstring strains" (5.47%) (P=0.008). There was no relationship between occurrence of a first injury and sex, age or belt rank. CONCLUSIONS: Judo is the martial art with the highest percentage of injuries, followed by wushu and then karate.

RESUMO

BACKGROUND: Trigeminal neuralgia is a chronic disease characterized by intense facial pain that is caused by trigeminal nerve affectation. It usually affects adults from 50 years of age, and is more frequent in women. Additionally, it presents serious psychological effects that often lead to depression, which is why it is considered highly disabling. The therapeutic approach is based on the modification of nerve activity through electrical, surgical or chemical stimulation in specific regions of the nervous system. OBJECTIVE: To perform a meta-analysis of the scientific literature related to invasive and non-invasive electrical neuromodulation of trigeminal neuralgia, in order to assess their effects over pain and adverse effects. METHODS: A literature search was conducted in 4 databases, followed by a manual search of articles on invasive or non-invasive electrical neuromodulation to control the pain of trigeminal neuralgia, including the last 15 years. RESULTS: Regarding non-invasive methods, clinical trials did not present enough results in order to perform a meta-analysis. Regarding invasive methods, clinical trials meta-analysis showed no statistical differences between different treatment methods. In all cases, improvements in patients' pain were reported, although results regarding adverse effects were variable. CONCLUSION: In the treatment of trigeminal neuralgia, the continuous radiofrequency provides better short and medium-term results, but pulsed radiofrequency shows less adverse effects after treatment, and has better results in the long-term.

Objective: to explore the experience of the Nursing profession and its impact on happiness. Method: a qualitative study conducted through a discussion group. Intentional sampling was conducted, looking for profiles representative of the Nursing reality, based on their professional setting, gender, age, and years of experience. The inclusion criteria established were: participants should be currently working, and conducting their professional activity in the Community of Madrid. Results: four categories were found: definition of happiness, happiness with the profession, factors associated with happiness in the Nursing profession, and future prospects. The participants described that what makes nurses happy is caring for others, the close and direct contact with patients, without any influence by the setting of work or unit where they conducted their work, as long as it had been chosen by them. Finally, they pointed out that in the collective setting there is a situation of professional disaffection, mainly motivated by the structure where they work and the management of their work. Conclusion: in the individual setting, the act of caring for others is seen as the essential factor that generates happiness in nurses, without any influence by the setting where this is conducted, as long as nurses have control over their professional life and the environment is adequate. However, from a collective point of view, the nursing profession seems to be going through a moment of unhappiness, mainly motivated by structural and management aspects regarding their work activity
