RESUMO

There is a lack of institutional and regulatory structure and coordination in the public health care system in Germany. The current reform approaches to the public health service, the establishment of a Federal Institute for Public Health and the amendment of the Prevention Act offer opportunities to lay the foundations for a modern public health structure within the framework of these reforms. In this context, the present study, based on the field of health promotion and primary prevention, outlines the following five task areas 1. gathering socio-epidemiological data, 2. health communication, 3. implementation of interventions, 4. method development, evaluation and quality development and 5. discursivization; all of these are highly relevant both for practical work of all identified actors and for their coordination. Taken together, these open a window of opportunity for a coherent, nationwide infrastructure for public health that is capable of action and has the ability to adapt as needed in Germany.

Few studies from Germany have investigated the associations between parenting style and children's and adolescents' health. Little attention has been directed to whether these associations vary with familial socioeconomic or migration status. The aim of this analysis was to investigate the association between parenting style and the mental health of children and adolescents aged 11-17 years using data from the KiGGS cohort study (second follow-up). We calculated mean Strengths and Difficulties Questionnaire (SDQ) total difficulties scores stratified by parenting style (authoritative, permissive, demanding-controlling, emotional distancing). Linear regression analyses adjusted for age, gender, socioeconomic status, migration status, and family status were performed. We also analyzed moderating effects of socioeconomic and migrations status on associations between parenting style and SDQ scores. There were only small differences between the permissive and the authoritative parenting styles. Significantly higher mean scores were observed for the demanding-controlling and emotional distancing styles for both the mother and father. These associations persisted after full adjustment and did not vary by socioeconomic or migration status. Parenting behavior is an important predictor of children's and adolescents' mental health. The promotion of good relationships within families and improving parenting skills offer promising approaches for health promotion in young people.

Background: The influence of the working environment on the back health of employees is well-documented. Many companies have begun to offer employees access to services to promote back health. Factors affecting the use of these offers at the population level have received little investigation to date. The current study examined the socio-demographic factors, physical activity and health-related factors, and work-related factors associated with the use of offers of workplace health promotion for back health in Germany. Materials and Methods: In the representative population-based cross-sectional survey "German Health Update" (GEDA 2014/2015-EHIS) conducted by the Robert Koch Institute, 12,072 employees aged 18-64 years old were surveyed from November 2014 to July 2015 regarding the use of back health services in their companies. In addition to socio-demographic factors, the survey examined working hours, physical activity in leisure time, health awareness, and subjective complaints in the lower back or other chronic back problems in the last 12 months. The interaction of these factors with the utilization of back health services was tested using multiple logistic regression models. Results: Women used back health services more often than men (women: 25.5%; men: 18.1%). Female gender was associated with part-time employment (OR 0.72) and a strong to very strong level of health awareness (OR 1.40). Male gender was associated with age between 30 and 44 years (OR 1.99) and 45-64 years (OR 2.02), low socioeconomic status (OR 0.48), endurance activity of <2.5 h per week (OR 0.62), and absence of lower back pain or other chronic back conditions for the last 12 months (OR 0.48). Conclusion: The present study is the first to provide findings regarding the factors associated with the utilization of workplace health promotion to promote back health at the population level, and from the perspective of employees in Germany. The results revealed that the relevant factors for participating in offers differ for women and men. To reach more employees, workplace health promotion offers for back health should be designed specifically for each individual, considering gender and age, working hours, health awareness and behavior, and health state.

Mental health burdens are among the most common health issues in childhood and adolescence. Psychosocial resources can act as protective factors and can help in preventing the development and reduce the symptoms of mental health issues. This article discusses this relationship and the availability of these resources within the three different social status groups among 11- to 17-year-olds. The database is the second wave of the German Health Interview and Examination Survey for Children and Adolescents (KiGGS Wave 2, 2014-2017). Mental health issues were assessed via the Strengths and Difficulties Questionnaires; psychosocial resources via self-reported personal, family and social resources; social status was ascertained through a multidimensional index based on the information provided by parents on education, occupational status and income. The analyses show that 11- to 17-year-olds who have psychosocial resources are less likely to show mental health issues (independent of their social status) and that, compared to high social status, mental health issues are more frequently associated with low social status. Children from (socially) worse-off families have less access to resources. The results consequently highlight the importance of prevention and health promotion measures directed at strengthening resources. Focusing such measures on the needs of disadvantaged population groups should contribute to health equity.

RESUMO

The concept Health in All Policies (HiAP) looks at health from an inter-sectoral perspective. It has 2 main principles: firstly, investigation of the impact of policy decisions from all sectors on health and wellbeing and secondly, creation of synergies to improve health equity. HiAP serves as a foundation for dealing with the health-related challenges we face today: climate change, good nursing care, inclusion, social equity, availability of health care as well as quality of life both in urban centers and rural regions. Under the leadership of the 3 authors, a working group of Future Forum Public Health published an expert report on WHO's concept of HiAP. It provides an overview of the principles underlying this concept, foundations and state of implementation and serves as a source for decision makers and policy makers who want to include the concept into their policies and implement it in their jurisdiction.

RESUMO

Community health promotion with children requires a conceptual framework to support children and families and sustainably improve their health opportunities.Family-oriented health promotion is an approach to implement this demanding task. It focuses in particular on the user perspective of families. It can be conceptually presented along the life-stage approach. Pregnancy, early childhood and adolescence as well as transitions between age-specific socialization institutions are highly vulnerable. Additionally, burdened life situations are presented along familial adversities such as poverty, isolation, single parenting, external placement or illnesses, and life-specific approaches to health promotion and prevention are named. Family-oriented health promotion should avoid stigmatization and support families through municipalities and other settings.

RESUMO

Taking similar parliamentary reports and available expert opinions as a basis, we aim to establish specifications for the first prevention report due for release in 2019. We propose to develop a formative report based on intervention reporting under the guidance of an independent scientific board supported by an administrative office and guided by the overall aim of providing policy advice. By pooling available expertise in the field of prevention, we can promote an evidence-based focus and the development of indicators. The prevention report should contribute towards data harmonisation and the development of long-term monitoring structures. We strive to achieve arrangements with and anchor our work within federal, federal state and municipal structures.

BACKGROUND: Overweight and obesity are serious health risks for children and adolescents. Hence, various prevention projects have been initiated and implemented. Until now, a systematic overview of interventions in different settings has been lacking. AIM(S)/RESEARCH QUESTION: The aim of the "Prevention of child overweight" project (SkAP-project) is to prepare a systematic overview of the conceptual approaches used in universal prevention of overweight among children and adolescents. First of all, a comprehensive criteria catalogue will be developed based on systematic searches. In the next step the criteria catalogue will be applied to identify and characterize conceptual approaches. DATA AND METHODS: Criteria to describe conceptual approaches as well as determinants of childhood overweight were determined by systematic searches. The searches included relevant data bases and were further expanded by internet and hand search. Three settings (kindergarten, school and communities) and families are addressed by the systematic searches. Additional non-setting specific searches were conducted. RESULTS: A comprehensive criteria catalogue was developed, which allows a detailed analysis of conceptual approaches. This catalogue covers further quality criteria as well as determinants of childhood overweight. DISCUSSION: Currently, the criteria catalogue is being employed. Although the detailed analysis of conceptual approaches can be regarded as advantage of the criteria catalogue, there are also some limitations, such as the lack of necessary information provided in publications. Overall, the application will reveal an overview regarding universal prevention in childhood overweight, which is still lacking, and will support development in this field.
