RESUMO

The frequency of seeking emergency medical care (EC) can be mediated by the characteristics of the patient's social status, his health literacy (HL) but not only by the clinical signs of the disease, health status. The goal of the cross-sectional survey was to identify factors determining the frequency of applying for EC by the young-aged, middle-aged (18-59 years) and elderly (60-74 years) patients of the primary health organizations in the Arkhangelsk Region and the Komi Republic (North-West Russia). Logistic regression (LR) was used to identify factors mediating the fact of applying for EC; zero-inflated negative binomial regression (ZINB) - to identify factors mediating the frequency of appeals. The majority of elderly respondents in the Arkhangelsk Region (72,5%) and the Komi Republic (74,1%) applied for EC at least once during the calendar year; among the young-aged and middle-aged respondents - 45,3% and 52,1% respectively. In the group of young-aged and middle-aged respondents, a higher frequency of appeals for EC is mediated by the age, low self-esteem of the well-being and health status, a chronic disease(s) affecting daily well-being in anamnesis, a low level of HL; in the group of the elderly respondents - by the fact of absence of a spouse, low self-esteem of the well-being, a chronic disease(s) affecting daily well-being in anamnesis, low levels of HL respectively. The obtained results obtained can be used to identify the «risk group¼ of patients of the primary health organizations who have a higher probability of applying for EC, and to organize additional preventive work with the min primary health organizations.

An important role in the implementation of many physiological and pathological processes is played by the functional state of the components of the hemostasis system, the activity of which is largely determined by the influence of various humoral factors. We studied the effects of lactate, nitric oxide, and sodium glutamate on aggregation activity of platelets and coagulation properties of rat plasma in vitro. The most pronounced effect was produced by lactic acid, which manifested in a dose-dependent decrease in the parameters of ADP-induced platelet aggregation and a two-phase change in blood coagulability with progressive acidification of the medium. Nitric oxide induced less pronounced decrease in the aggregation activity of platelets. Sodium glutamate, on the contrary, stimulated platelet aggregation and similar to nitric oxide did not affect coagulation status of the plasma.

RESUMO

For the purpose of methodological justification of the use of new approaches to the organization of psychiatric care for veterans of military operations in departmental general hospital health care institutions, the analysis of existing problems in providing psychiatric care to retired combatants with mental disorders of the system of the Ministry of internal Affairs (MIA) of Russia was carried out. It is shown that the existing organizational approaches have a number of significant drawbacks and do not fully provide quality therapy and medical and psychological rehabilitation to this contingent. Proposed in the development Concept of the psychiatric service of MIA to provide targeted prevention work with ex-combatants, current employees of power structures, including early diagnosis of early forms of mental disorders, timely provision of treatment & rehabilitation help with subsequent systematic monitoring of their mental health in government health facilities. This will not only improve the quality of diagnosis of mental disorders, but also improve the treatment of psychopathological disorders, reduce the stigma of psychiatric care and systematize the directions of work with this contingent at the stages of secondary prevention of changes in mental functions.

Hydrographic data collected from research cruises, bottom-anchored moorings, drifting Ice-Tethered Profilers, and satellite altimetry in the Beaufort Gyre region of the Arctic Ocean document an increase of more than 6,400 km3 of liquid freshwater content from 2003 to 2018: a 40% growth relative to the climatology of the 1970s. This fresh water accumulation is shown to result from persistent anticyclonic atmospheric wind forcing (1997-2018) accompanied by sea ice melt, a wind-forced redirection of Mackenzie River discharge from predominantly eastward to westward flow, and a contribution of low salinity waters of Pacific Ocean origin via Bering Strait. Despite significant uncertainties in the different observations, this study has demonstrated the synergistic value of having multiple diverse datasets to obtain a more comprehensive understanding of Beaufort Gyre freshwater content variability. For example, Beaufort Gyre Observational System (BGOS) surveys clearly show the interannual increase in freshwater content, but without satellite or Ice-Tethered Profiler measurements, it is not possible to resolve the seasonal cycle of freshwater content, which in fact is larger than the year-to-year variability, or the more subtle interannual variations.

In-tissue embedded magnetic nanoparticle (MNPs) detection is one of the most interesting cases for cancer research. In order to understand the origin, the limits and the way of improvement of magnetic biosensor sensitivity for the detection of 3D mezoscopic distributions of MNPs, we have developed a magnetoimpedance biosensor prototype with a [Cu (3 nm)/FeNi(100 nm)]5/Cu(500 nm)/[FeNi(100 nm)/Cu(3 nm)]5 rectangular sensitive element. Magnetoimpedance (MI) responses were measured with and without polyacrylamide ferrogel layer mimicking natural tissue in order to evaluate stray fields of embedded MNPs of γ-Fe2O3 iron oxide. A model for MI response based on a solution of Maxwell equations with Landau-Lifshitz equation was developed in order to understand the origin of the prototype sensitivity which reached 1.3% of ΔZ/Z per 1% of MNPs concentration by weight. To make this promising technique useful for magnetically labeled tissue detection, a synthesis of composite gels with MNPs agglomerates compactly located inside pure gel and their MI testing are still necessary.

AIM: To study the efficacy and tolerability of cortexin in the treatment of cognitive dysfunction in children. MATERIAL AND METHODS: The study included 635 patients, aged 3-7 years. Patients were divided into 4 clinical groups: group 1 (269 children with attention deficit hyperactivity disorder (ADHD)), group 2 (215 children with speech delay), group 3 (82 patients with the consequences of a perinatal lesion of the central nervous system), group 4 (69 patients with asthenic/neurotic syndrome). Attention, visual memory and thinking were assessed before and after treatment. Standard treatment of cortexin, including 10 intramuscular injections, was used. RESULTS AND CONCLUSION: The reliable effect of cortexin on cognitive impairment was shown. The best response to treatment was observed in patients with ADHD, aged 3-4 years, in particular on the tests measuring thinking. The tolerability of cortexin was good. It has been concluded that cortexin is an effective drug for treatment of children with ADHD, speech delay and consequences of a perinatal lesion of the central nervous system.

RESUMO

The purpose of the study was to analyze the foster family as tool of the social security system for elderly in remote northern territories. The significance of long-term previous relationships for building and microclimate of a foster family, relationship with the living environment as a socio-psychological support for reducing (eliminating) the risks of loneliness, social inclusion in society through a foster family has been revealed. It was noted that legally consolidated efforts of the state and family at the regional level faster respond to changing needs for various types of services, taking into account local socio-economic opportunities and cultural traditions, which helps to diversify the number of forms of services that directly affect risk reduction and meeting the needs of an elderly person. It is shown that the uncertainty associated with the organization (accessibility) of care services (palliative care) of chronically sick people in old age, worthy death, is a risk in the remote northern territories.

We studied the expression of an early activation marker CD69 in peripheral blood lymphocytes of pregnant women with a history of recurrent pregnancy loss after immunization with paternal lymphocytes. Spontaneous and phytohemagglutinin-stimulated expression of CD69 on the surface of T cells and NK cells isolated from the peripheral blood was analyzed. On gestation week 5-6, the number of T cells expressing CD69 spontaneously and after stimulation was significantly higher in women with miscarriage than in woman with prolonged pregnancy. However, the number of cells with CD56(+) phenotype expressing CD69 did not differ in these groups. No differences were found in the number of cells of all subpopulations expressing CD69 after stimulation on gestation week 12 in woman with full-term current pregnancy and in woman with physiological pregnancy.

RESUMO

Anti-HLA-antibodies were detected using cross-reaction of blood serum with allogenic T and B cells and latex microspheres coated with HLA-I and HLA-II antigens. HLA+ and HLA-sera obtained from women before and after allogeneic immunization were tested. The results obtained by these methods significantly differed. The test with latex microspheres detected antibodies to HLA-I and HLA-II antigens with high sensitivity and specificity and can be used for assessment of clinical significance of alloantibody detection when using alloimmunization in the therapy of gestation disorders.

Infestation of bee colonies and apiaries by representatives of the genus Nosema, microsporidian protozoans of European honeybees (Apis mellifera L.), causing nosemosis, in Tomsk Province was investigated. In 2012­2015, nosemosis was detected in 32 out of 124 honeybee colonies (31.3 %) and in 20 out of 64 studied apiaries (25.8 %). The maximal infestation rate of bee colonies and apiaries constituted more than 40 % in 2014­2015. N. apis pathogen was registered in 84.4 % of infected bee colonies (16 apiaries); N. ceranae was identified in 9.4 % of infected bee colonies (2 apiaries); and co-infection (N. apis and N. ceranae) was detected in 6.3 % of infected bee colonies (2 apiaries). The reasons of the spreading of the nosemosis, such as climatic conditions, control of imported bee colonies on the presence of Nosema infection, and some others are discussed.

RESUMO

The satisfaction of the quality of life (QOL) in elderly age is discussed. There have been considered the satisfaction of QOL depending on the form of social service in urban and rural areas. The key factors influencing the QOL in various forms of services is defined. They include: rural life satisfaction is largely dependent on the availability of family than on the health and financial status; in a special house for single elderly life satisfaction is determined only by the state of health due to the large number of chronic diseases, in stationary social services very important factor is loneliness, isolation that creates some feeling of being tired of life, depression, provocation the abuse of alcohol.

AIM: Study the circulation of respiratory viruses in children with community-acquired pneumonia (CAP) during the period from October 2012 to May 2013. MATERIALS AND METHODS: 136 children with CAP aged from 3 months to 16 years with ARI symptoms at the disease debut were studied. RNA/DNA of influenza A, B, parainfluenza (PI); adeno-, rhino-, RS-viruses, corona-, metapneumo- (MPV) and bocaviruses were detected in nasopharynx smears by PCR with hybridization-fluorescent detection in real time. Antibodies against influenza viruses A/H1N1/pdm09 California/07/09, epidemic reference strains of influenza viruses A/H1N1/Brisbane/59/07, A/ H3N2/Victoria/361/201 1, B/Wisconsin/1/10, against PI viruses type 1, 2, 3 were determined in paired sera by HAI. RESULTS: In February-March 2013 the number of children protected by antibodies against influenza decreased, and circulation of influenza viruses A/H3N2 and A/H1N1/ pdm09 was detected. Rhinoviruses and PI viruses were determined throughout the epidemic season, bocavirus and adenoviruses--during the autumn-winter period, RS-virus and MPV--during winter-spring. Coronaviruses were not detected. The peak of virus detection was established in February when the threshold of influenza and ARI morbidity was exceeded. The main pathogens of children of the first 3 years of life are rhinoviruses, RS-virus, PI viruses and bocavirus. RS-virus infection at the debut of CAP in children younger than 3 years in 55.5% of cases is associated with the development of broncho-obstructive syndrome. Bocavirus infection in 50% of cases progresses with laryngo-tracheitis and bronchiolitis. CONCLUSION: The fraction of viruses in etiologic structure ofARI in children varies depending on immune layer, season and age of children. Etiology of viral infection at the debut of CAP could only be proven using specialized laboratory studies.

RESUMO

Modern terminology on active and healthy aging used in scientific and project activities is discussed. There have been analyzed the WHO conception on active aging, which has no precise universally agreed definition, its main determinants. The directions of scientific expertise in the major European projects INNOVAGE - assessment of potentially profitable social innovations relating to the welfare and quality of life and health in old age; MOPACT - the interference between the demographic development and the main dimensions of economic and social contribution of older persons is defined. The approach to implement the policy of active and healthy longevity as a valuable asset of the modern society is underlined.

RESUMO

Interactions between quality of life for group of elderly people and social-medical care contribution to it have been discussed. There have been showed satisfaction of quality of life depending on getting care in different care system's organizations. There have been defined high level needs of elderly and aged people in medical and social care (especially of 80-89 years group in urban and rural areas). There have been exposed that elderly people living in sparsely populated areas in theirs everyday activity traditionally leaned on support on spouse's and children's help because of resources of social system protection and medical-social service less available in arctic territories which decreased the quality of life.

The level of antipaternal antileukocytic antibodies detected by flow cytometry is a parameter reflecting the efficiency of alloimmunization of women with reproductive disorders during preparation to pregnancy. The results of evaluation of antipaternal antileukocytic antibodies by two modifications of the method are presented. The optimal method for detection of antipaternal antileukocytic antibodies after immunocytotherapy is selected.

Social factors and the role of environment which influence to the aging process on the cohort of the elderly people on European North of Russia have been discussed. The indicators of aging of elderly people stated in the different living conditions with defining risk factors have been analyzed. The predominance of the individuals with high rates of aging in the men's cohort living in the rural areas has been considered. Consume alcohol is the aggravating factor accelerating the significant aging rate of women addicted to alcohol.

The contemporary approaches to the conception of healthy aging are discussed. The insufficient propaganda of healthy style of life of elderly population results in non-satisfaction of own health in old age; lack of information about opportunities to strengthen the health in the system of social services managing is also a problem. Weak skills and motivation of elder generation for taking care of the own health are registered. Gender and age features of health self-evaluation have been considered. Low meaning of spending spare time to use physical culture activity for strengthening health have been determinated.

RESUMO

Morphofunctional and biochemical properties of erythrocyte membrane were investigated in early postnatal ontogenesis in rats in norm and after prenatal immobilization stress. The transient decrease of erythrocyte membranes stability was revealed in the control rats. The ability to erythrocyte transformation and the concentration of lipid peroxidation products are increased. It has been shown by an increase of percentage discocytes and lower lipid peroxidation level that the erythrocyte membrane of the rats after prenatal stress is more stable.

RESUMO

Modern concepts and indicators of quality of life used in gerontological practices are discussed. The influence of institutional care system with the elderly person as part of the quality of life indicators is shown. The theoretical and methodological combinations of approaches as innovations on the crossing of different scientific paradigms and professional ways of solving the problem are analyzed. Structural models, tools for the measurement of the quality of life and its modification in the institutional care system are defined. This approach stimulates elderly people to be active co-producer of own quality of life, strengthen their autonomy and resource potential.
