RESUMO

Results of the surgical treatment of 98 patients presenting with chronic suppurative otitis media (CSOM) and accompanying cholesteatoma. CSOM of large duration turned out to be associated with cholesteatoma in 31.3% of the patients regardless of localization of the tympanic defect. A variety of surgical interventions were undertaken depending on the extension of cholesteatoma including the intrameatal approach (n=75), endaural approach (n=16), postaural approach (n=6), and the combined approach (n=1). Forty two patients presenting with either central or peripheral peroration in the strained part of the tympanic membrane were operated with the use of the intrameatal approach. Perforation in the unstrained part of the tympanic membrane in 56 patients with chronic suppurative otitis media and cholesteatoma extending to the antrum and the cells of the mastoid process was corrected by sanation operation at different stages of the carious process using different surgical approaches. The early surgical intervention during the carious process in the patients presenting with CSOM and cholesteatoma in the phase of remission, taking into account structural peculiarities of the mastoid process and distribution of the inflammatory process, significantly reduced its extension and permitted to reconstruct the structure of the middle ear in 70% of the patients.

RESUMO

Guinea-pig experiments were made for objective assessment of the effects of high-intensity noise on the acoustic analyzer. Noise with intensity above 100 dB for 11 hours is accompanied with thresholds rise by 20 dB. In some animals hearing loss was observed for several weeks after discontinuation of a sound impact. In the presence of initially high conductive hypoacusis intensive sound load may cause short-term sound threshold lowering with further damage to neurosensory cochlear elements.

RESUMO

The prevalence of hyperacusis, and subjective ear noise in particular, tends to increase in the general population. The data available in the literature show that physiotherapeutical procedures are the most effective treatment of this abnormality. The techniques and devices using fluctuating currents were employed to treat 40 patients. A "SLUKH-OTO-1" (HEARING-OTO) device is of use now. This procedure proved to be the most effective in case of hyperacusis and subjective ear noise without hearing disorder, there was also a high percentage of positive results in chronic neurosensory hypoacusis. Treatment of subjective ear noise by this procedure is not so effective in otosclerosis and chronic otitis media.
