PURPOSE: To investigate the accuracy of cutoff values of the morning serum cortisol (MSC) using the cortisol stimulus test (CST) insulin tolerance test (ITT) and 250 mcg short Synacthen test (SST) as the reference standard tests, to better define its clinical role as a tool in the diagnostic investigation of adrenal insufficiency (AI) AI. METHODS: An observational study was conducted with a retrospective analysis of MSC in adult patients who had been submitted to a CST to investigate AI between January 2014 and December 2020. The normal cortisol response (NR) to stimulation was defined based on the cortisol assay. RESULTS: 371 patients underwent CST for suspected AI, 121/371 patients (32.6%) were diagnosed with AI. ROC curve analysis showed an area under the curve (AUC) for MSC of 0.75 (95% CI 0.69 - 0.80). The best MSC cutoff values to confirm AI were < 3.65, < 2.35 and < 1.5 mcg/dL with specificity of 98%, 99%, and 100%, respectively. MSC > 12.35, > 14.2 and > 14.5 mcg/dL had sensitivity of 98%, 99%, and 100%, respectively, being the best cutoff values to exclude AI. Almost 25% of patients undergoing CST for possible AI had MSC values between < 3.65 mcg/dL (6.7% of patients) and > 12.35 mcg/dL (17.5% of patients), making the formal CST testing unnecessary if we consider these cutoff values. CONCLUSION: With the most modern cortisol assays, MSC could be used as a diagnostic tool, with high accuracy to confirm or exclude AI, avoiding unnecessary CST; thus, reducing expenses and safety risks during AI investigation.

The objectives of this study were to assess the biogas potential of landfilled materials and to further validate the suitability of the enzymatic hydrolysis test EHT as a valuable alternative to substitute the standardised test currently in use (BMP). Both tests were applied to a range of landfill waste samples. The waste composition and volatile solids content (VS) profile together with the BMP test results showed that the biogas potential of the waste samples was directly related to their VS content, as expected. The positive correlation between the VS and the BMP test (r=0.67) suggests that the first could be used as a primary indicator of biogas potential of waste samples. Nevertheless, it should be validated against the BMP test because, occasionally, the VS content does not equate to the biogas production. This was mainly due to the paper content of the samples which also correlates positively (r=0.77) with the BMP biogas production. The EHT results showed a higher correlation with the BMP test (r=0.91) than in previous studies which used a wider mixture of enzymes containing cellulase, hemicellulase and carbohydrase. This finding positions the EHT as a quick assessing method for the biodegradability of waste samples in future sample regimes.

In the last decade, significant advances in flow analysis have been reported, namely the extensive use of computer-controlled devices to enhance the autonomy and performance of analysers. In the present work, computer-controlled multi-syringe flow injection systems are proposed to perform the spectrophotometric determination of available iron and boron in soil extracts. The methodologies were based on the formation of ferroin complex (determination of iron) and azomethine-H reaction (determination of boron). Both determinations were performed in manifolds with similar configurations by changing the reagents present in the different syringes. In the determination of iron, elimination of Schlieren effect in the detection system was achieved through the binary sampling approach, where a three-way valve was actuated to intercalate small slugs of sample and reagent, promoting better mixing conditions for solutions with different values of refractive index. In the determination of boron, in-line sample blank measurement was attained by omitting the introduction of reagent through software control, without manifold reconfiguration. Linear calibration curves were established between 0.50 and 10.0mgFel(-1) and between 0.20 and 4.0mgBl(-1). No systematic difference was found when soil extracts were analysed by the proposed methodologies and compared to the respective reference procedures.

Leaf anatomy is described in order to contribute to the Poaceae family study. Anatomic features observed in the three studied species such as: midrib with complex vascular system, mesophyll consisting of tabular lobed chlorophyllous elements and fusoid cells, vascular bundles with double sheath, epidermis made up of long cells, short cells, micro-hairs, prickles and silica bodies correspond to the "bambusoid type" of leaf anatomy

DNA methylation patterns were evaluated during preimplantation mouse development by analyzing the binding of monoclonal antibody to 5-methylcytosine (5-MeC) on metaphase chromosomes. Specific chromosome patterns were observed in each cell stage. A banding pattern predominated in chromosomes at the one-cell stage. Banding was replaced at the two-cell stage by an asymmetrical labeling of the sister chromatids. Then, the proportion of asymmetrical chromosomes decreased by one-half at each cell division until the blastocyst stage, and chromosomes became progressively symmetrical and weakly labeled. Our results indicate that chromosome demethylation is associated with each DNA replication and suggest that a passive mechanism predominates during early development.

Hepatitis B has proved to be a major health hazard in hemodialysis patients. In order to investigate the hepatitis B virus (HBV) infection profile in the hemodialysis population of Goiânia city--Central Brazil, all dialysis patients (N = 282) were studied. The prevalence of any HBV marker (HBsAg, anti-HBs, and anti-HBc) was 56.7% (95% CI: 51.1-62.7), ranging from 33.3% to 77.7% depending on dialysis unit. HBV-DNA was detected in 67.6% and 88.2% of the HBsAg-positive serum samples, in 91.3% and 100% of the HBsAg/HBeAg-positive samples, and in 18.2% and 63.6% of the HBsAg/anti-HBe-reactive sera by hybridization and PCR, respectively. The length of time on hemodialysis was significantly associated with HBV seropositivity. Only 10% of the patients reported received hepatitis B vaccination. The findings of a high HBV infection prevalence in this population and the increased risk for HBV infection on long-term hemodialysis suggest the environmental transmission, emphasizing the urgent need to evaluate strategies of control and prevention followed in these units.

RESUMO

A 22-week-old fetus with partial trisomy 2q (q24.1-->qter) and 7q35 monosomy is described. To our knowledge, this represents the greatest unbalanced 2q chromosomal segment published in the literature. The clinical phenotype is in agreement with previous descriptions in children and allows us to propose a fetal phenotype for this chromosomal abnormality.

mos 45,X/46,X,Y with no bright fluorescence was studied in 4 patients presenting variable phenotypes, from Turner's syndrome, with or without virilization, to ambiguous external genitalia, with combined cytogenetic techniques. G-11 staining demonstrated, in all cases, that the abnormal Y was a dic(Yp). Considerations about the possibility that some of the 46,X,Yq-males attending infertility clinics may be examples of dic(Yp) are made.

RESUMO

A male with a ring 13 chromosome [r(13)(p11q34)], mild mental retardation, short stature, oligoasthenospermia, and few dysmorphisms is reported. His mother who had a poor reproductive history is carrier of a t(13q13q), featuring a dicentric NOR-negative element. The clinical significance of the r(13) and the mother's unusual karyotype are discussed.
