Studies involving the Cognitive Assessment System (CAS) planning scale typically use only the subtest and scale scores without assessing the strategies employed by the participants. This study addressed this gap and examined the planning strategies used by children in the CAS2: Spanish version and their relationship with planning performance. We conducted an exploratory cross-sectional study with 26 Puerto Rican children aged 8 to 11. Results showed that no strategies were consistently used by participants according to examinees' reports (f = 0-46%), but examiners observed consistent use of some strategies such as "coded left to right, top to bottom", f = 92%; "scanned the page for the next number or letter", f = 100%. Welch's t-tests did not show relationships between participants' performance and the strategies observed by examiners, | mean differences | = 0.05-0.81, ps ≥ 0.05, nor with the strategies reported by participants, | mean differences | = 0.05-1.69, ps ≥ 0.05. These findings suggest that although the examiners may observe the use of strategies, the examinees are unaware of the strategies they use, and the strategies used are not associated with their performance. Future studies are needed to confirm these findings.

Ecuador had substantial COVID-19-mortality during 2020 despite early implementation of non-pharmaceutical interventions (NPIs). Resource-limited settings like Ecuador have high proportions of informal labour which entail high human mobility, questioning efficacy of NPIs. We performed a retrospective observational study in Ecuador's national reference laboratory for viral respiratory infections during March 2020-February 2021 using stored respiratory specimens from 1950 patients, corresponding to 2.3% of all samples analysed within the Ecuadorian national surveillance system per week. During 2020, detection of SARS-CoV-2 (Pearson correlation; r = -0.74; p = 0.01) and other respiratory viruses (Pearson correlation; r = -0.68; p = 0.02) by real-time RT-PCR correlated negatively with NPIs stringency. Among respiratory viruses, adenoviruses (Fisher's exact-test; p = 0.026), parainfluenzaviruses (p = 0.04), enteroviruses (p < 0.0001) and metapneumoviruses (p < 0.0001) occurred significantly more frequently during months of absent or non-stringent NPIs (characterized by <55% stringency according to the Oxford stringency index data for Ecuador). Phylogenomic analyses of 632 newly characterized SARS-CoV-2 genomes revealed 100 near-parallel SARS-CoV-2 introductions during early 2020 in the absence of NPIs. NPI stringency correlated negatively with the number of circulating SARS-CoV-2 lineages during 2020 (r = -0.69; p = 0.02). Phylogeographic reconstructions showed differential SARS-CoV-2 dispersion patterns during 2020, with more short-distance transitions potentially associated with recreational activity during non-stringent NPIs. There were also fewer geographic transitions during strict NPIs (n = 450) than during non-stringent or absent NPIs (n = 580). Virological evidence supports that NPIs had an effect on virus spread and distribution in Ecuador, providing a template for future epidemics in resource-limited settings and contributing to a balanced assessment of societal costs entailed by strict NPIs.

In order to characterize the quality of semen from men in an assisted reproduction center in the city of Guayaquil (Ecuador), 204 semen samples were collected from patients with fertility disorders aged 20 to 57 years, who were admitted between May 2017 and September 2018. A basic spermogram was performed on each sample, following the fabricant recommendations for the examination and processing of human semen. It was found that 27.4% of the samples presented normozoospermia. Among the disorders, it was found that 27.9% had teratozoospermia, 8.8% had oligoteratozoospermia and a higher number of patients were found to be between 30 and 39 years old. A high percentage of patients presented sperm morphology and quality values below the reference limits established by the World Health Organization.
Con el objetivo de caracterizar la calidad seminal de hombres en un centro de reproducción asistida de la ciudad Guayaquil (Ecuador), se colectaron 204 muestras de semen de pacientes con problemas de fertilidad de entre 20 y 57 años, atendidos entre mayo de 2017 y septiembre de 2018. Se realizó un espermograma básico a cada muestra, siguiendo las recomendaciones del manual para la examinación y procesamiento de semen humano. El 27,4% de las muestras presentó normozoospermia. Dentro de las alteraciones la teratozoospermia fue de 27,9%, oligoteratozoospermia del 8,8%, evidenciándose mayor número en pacientes de 30 a 39 años. ABSTRACT Several studies have been carried out to determine the presence and circulation of West Nile Virus (WNV) in several species that interact in important ecosystems of Ecuador, such as the Galapagos Islands, where presence and surveillance studies of WNV have been carried out in wild and migratory birds (2003) (2008 to 2010), penguins (2003 to 2004). Studies have also been carried out on birds from different locations in Guayaquil (2011), and on Jauneche horses (2007), but no virus has been demonstrated in any of them. Nevertheless, in the Abras de Mantequilla wetland, two studies were conducted in equines aged between 3 months to 12 years, all of them mixed race, male and female, with no previous vaccination history and with presence of symptoms only in the first study. In the two studies the serum analysis was performed by the ELISA technique (reactivity determination) and Plaque Reduction Neutralization Test (PRNT). In the first study, 8.12% (13/160) of reactivity was determined in 13 horses and 22.22% of reactivity in 2 of 9 people; and only 3.12% (5/160 horses) of the presence of IgM antibodies against WNV. In relation to the second study, 12.6% (52/412) reactivity and 10.4% (43/412 horses) confirmed the serological evidence of WNV, with a final prevalence of 6.76%. Consequently, the WNV is present and circulating in the equines of the Ecuadorian coastal zone, which is a potential risk to the public health, nevertheless there is no updated information on investigations conducted in this regard.
RESUMEN El presente es una revisión bibliográfica sobre estudios realizados para determinar la presencia y circulación del Virus del Nilo Occidental (VNO) en diversas especies que interactúan en importantes ecosistemas del Ecuador, como son las Islas Galápagos, en donde, se han realizado estudios de presencia y vigilancia del VNO en aves silvestres y migratorias (2003) (2008 al 2010) y pingüinos (2003 al 2004). También, se ha realizado estudios en aves de diversos lugares de Guayaquil (2011), y en equinos de Jauneche (2007) pero en ninguno de los lugares se evidenció la presencia del virus. Por otro lado, en el humedal Abras de Mantequilla, Coello y colaboradores realizaron dos estudios en equinos de edades entre 3 meses a 12 años, todos de raza mestiza, sexo machos y hembras, sin antecedentes de vacunación y con presencia de síntomas solo en el primer estudio. El análisis de los sueros en los dos estudios se realizó mediante la técnica de ELISA (determinación de reactividad) y la confirmación a través de Neutralización por Reducción del Número de Placas (NTRP). En el primer estudio se determinó el 8.12% (13/160) de reactividad en 13 equinos y el 22.22% de reactividad en 2 de 9 personas (no se confirmaron); del total de muestras reactivas en equinos, solo se confirmó el 3.12% (5 equinos/160) de la presencia de anticuerpos IgM contra VNO. Respecto al segundo estudio estableció el 12.6% (52/412) de reactividad y el 10.4% (43/412 equinos) se confirmó la evidencia serológica del VNO, con una prevalencia final del 6.76%. Por lo consiguiente, el VNO está presente y circulando en los equinos de la zona costera ecuatoriana, lo cual es un riesgo potencial para la salud pública, sin embargo no hay información actualizada de investigaciones realizadas al respecto.

Background The international recommendations of antiretroviral treatment include resistance tests to guide the treatment regimen in each patient, which is not available on a regular basis in Ecuador. Aim To describe mutations that confer resistance to antiretrovirals in a population of Ecuadorian patients. Methods Plasma samples from 101 HIV-1 patients with failure to antiretroviral therapy, divided into 15 children and 86 adults, were studied with the GS Junior (Roche) and the sequences were analyzed with the DeepChek program. Results The most frequent mutations were M184V/I, K101E/P/H, K103N/S, D30N, M46L/I, I54L/M, V82T/F/A/S/L and L90M in adults and F77L, K103N/S, M46L/I, V82T/F/A/S/L and L90M in children. High resistance to non-nucleoside reverse transcriptase (RT) inhibitors in minority viral populations of adults and children (34.9% and 70%) was detected; in children both viral populations (majority and minority viral populations) (> 45%) were protease inhibitor resistant. Patients who had a greater number of therapeutic regimens had higher levels of resistance to antiretrovirals. Most of the samples were subtype B in the TR and protease region, and CRF25_cpx in integrase. Conclusions Mutations and resistance to antiretrovirals are shown in a population of Ecuadorian patients with HIV-1. These results will make it possible to issue a warning to health authorities about the need for resistance studies.

Resumen Introducción Las recomendaciones internacionales de tratamiento anti-retroviral incluyen pruebas de resistencia para orientar el régimen de tratamiento en cada paciente, lo que no está disponible de forma estable en Ecuador. Objetivo Describir las mutaciones que confieren resistencia a anti-retrovirales en una población de pacientes ecuatorianos. Metodología A partir de muestras de plasma de 101 pacientes con VIH-1 con fallo a la terapia anti-retroviral, 15 niños y 86 adultos, se realizó pirosecuenciación con el GS Junior (Roche) y se analizaron las secuencias con el programa DeepChek. Resultados Las mutaciones más frecuentes fueron M184V/I, K101E/P/H, K103N/S, D30N, M46L/I, I54L/M, V82T/F/A/S/L y L90M en adultos, y F77L, K103N/S, M46L/I, V82T/F/A/S/L y L90M en niños. Se encontró una elevada resistencia a los inhibidores de la transcriptasa reversa (TR) no análogos de nucleósidos en poblaciones minoritarias virales de adultos y niños (34,9 y 70%, respectivamente), en los niños, tanto las poblaciones virales mayoritarias como minoritarias, fueron resistente a inhibidores de proteasa (> 45%). Los pacientes que tuvieron un mayor número de esquemas terapéuticos presentaron mayores niveles de resistencia a los anti-retrovirales. La mayoría de las muestras fueron del subtipo B en la región de la TR y proteasa, y CRF25_cpx en integrasa. Conclusiones Se muestran las mutaciones y la resistencia a antiretrovirales en una población de pacientes ecuatorianos con infección por VIH-1, que permitirán realizar un llamado de alerta a las autoridades de salud sobre la necesidad de realizar estudios de resistencia.
