Introduction It has been hypothesized that increasing the interstitial hydrostatic pressure within the sinonasal mucosa of patients with nasal polyposis (NP) might decrease the size of nasal polyps. Objective To evaluate the effects of positive airway pressure, delivered by a continuous positive airway pressure (CPAP) device, in patients with NP and in control subjects. Methods Twelve patients with NP and 27 healthy subjects were exposed to CPAP (20 cm H2O) for 2 hours. Visual analog scale (VAS), Nasal Obstruction Symptom Evaluation (NOSE) scale, acoustic rhinometry (AR), peak nasal inspiratory flow (PNIF) and nasal endoscopy (NE-Meltzer polyp grading system) were performed before and after the intervention, for all patients. Results The control group showed a significant worsening in nasal obstruction symptoms, as measured by VAS and NOSE ( p < 0.01), and a significant decrease in nasal patency, as measured by the PNIF and AR ( p < 0.01). For the NP group, VAS, NOSE, and AR did not differ significantly ( p = 0.72, p = 0.73, and p = 0.17, respectively), but PNIF values worsened ( p = 0.04) after exposure to CPAP. There was a statistically significant reduction in the nasal polyps' size ( p = 0.04). Conclusions Positive pressure worsened the nasal obstruction symptoms and decreased objective parameters of nasal patency in control subjects. In patients with NP, exposure to CPAP reduced the nasal polyps' size, and the nasal patency, as measured by PNIF. However, it had no significant effects in AR and in nasal obstruction symptoms.

Introduction The importance of our study lies in the fact that we have demonstrated the occurrence of mechanical dysfunction within polypoid tissues, which promotes the development of polyps in the nasal cavity. Objective To change the paradigm of nasal polyposis (NP). In this new conception, the chronic nasal inflammatory process that occurs in response to allergies, to pollution, to changes in the epithelial barrier, or to other factors is merely the trigger of the development of the disease in individuals with a genetic predisposition to an abnormal tissue remodeling process, which leads to a derangement of the mechanical properties of the nasal mucosa and, consequently, allows it to grow unchecked. Data Synthesis We propose a fundamentally new approach to intervening in the pathological process of NP, addressing biomechanical properties, fluid dynamics, and the concept of surface tension. Conclusion The incorporation of biomechanical knowledge into our understanding of NP provides a new perspective to help elucidate the physiology and the pathology of nasal polyps, and new avenues for the treatment and cure of NP.

PURPOSE: The goal of this study was to investigate the agreement between examiners who were or were not trained in the physical examination of the upper airway (UA) and the craniofacial skeleton of individuals with obstructive sleep disorders (OSD). METHOD: A systematic assessment of the UA and craniofacial skeleton was performed on 55 individuals with OSD. The participants were consecutively assessed by three otorhinolaryngologists who specialized in sleep medicine for at least 1 year (trained examiners) and two doctors who were attending a residency program in otorhinolaryngology (untrained examiners). RESULTS: When analyzing all of the parameters assessed, the concordance was better in the trained group (k = 0.694, which is considered "good") compared to the untrained group (k = 0.475, "fair") (p < 0.001). The inter-examiner agreement was also better in the trained compared to the untrained group, as follows: craniofacial (k = 0.643 vs. 0.349), nasal (k = 0.657 vs. 0.614), and pharyngeal (k = 0.729 vs. 0.276) abnormalities (p < 0.05). CONCLUSION: The overall concordance of the physical examination of the UA and craniofacial skeleton was "good" among the trained specialists and "fair" among examiners without appropriate training, despite its subjectivity.

ABSTRACT INTRODUCTION: The mandibular advancement device (MAD) is a option to treat patients with Obstructive Sleep Apnea Syndrome (OSAS). OBJECTIVE: To assess the influence of upper airway abnormalities on the success of and adherence to MAD in patients with OSAS. METHODS: Prospective study with 30 patients with mild to moderate OSAS and indications for MAD. The protocol included questionnaires addressing sleep and nasal complaints, polysomnography, and upper airway assessment. The analyzed parameters of patients who showed therapeutic success and failure and those who exhibited good and poor treatment adherence were compared. RESULTS: 28 patients completed the protocol; 64.3% responded successfully to treatment with MAD, and 60.7% exhibited good adherence to treatment. Factors associated with greater success rates were younger age (p = 0.02), smaller cervical circumference (p = 0.05), and lower AHI at baseline (p = 0.05). There was a predominance of patients without nasal abnormalities among patients treated successfully compared to those with treatment failure (p = 0.04), which was not observed in relation to adherence. Neither pharyngeal nor facial skeletal abnormalities were significantly associated with either therapeutic success or adherence. CONCLUSION: MAD treatment success was significantly lower among patients with nasal abnormalities; however, treatment adherence was not influenced by the presence of upper airway or facial skeletal abnormalities.


RESUMO INTRODUÇÃO: O Aparelho Intraoral (AIO) é uma opção para tratamento da Síndrome da Apneia Obstrutiva do Sono (SAOS). OBJETIVOS: Avaliar a influência das alterações da VAS e esqueléticas faciais através de uma avaliação clinica sistematizada no sucesso e adesão ao (AIO) em pacientes com (SAOS). MÉTODO: Estudo prospectivo em que foram avaliados 30 pacientes com SAOS leve a moderada e indicação de AIO. Protocolo incluiu questionários de sono e queixas nasais; polissonografia e avaliação da VAS por rinoscopia anterior e oroscopia. Os parâmetros analisados foram comparados entre pacientes com sucesso e insucesso, e com boa e má adesão à terapia. RESULTADOS: Completaram o protocolo 28 pacientes. O sucesso ao tratamento foi de 64,3% e a adesão 60,7%. Os fatores associados ao sucesso foram menor idade, menor circunferência cervical e menor IAH basal. Quanto à presença de alterações nasais, houve predomínio de pacientes sem alteração nasal entre os pacientes com sucesso comparados àqueles com insucesso (p = 0,04); o que não foi observado em relação à adesão. Quanto às alterações faríngeas e alterações esqueléticas faciais, não houve significância. CONCLUSÃO: O sucesso do tratamento com AIO foi significativamente menor nos pacientes com alterações nasais, porém a adesão não foi influenciada pela presença de alterações de VAS ou esqueléticas faciais.

BACKGROUND: The few studies that examine the effect of nasal abnormalities on continuous positive airway pressure device (CPAP) adherence are controversial. The aim of this study was to evaluate the contribution of nasal abnormalities in CPAP adherence. METHODS: We included patients with moderate to severe OSA. The patients were submitted to rhinoscopy, nasofibroscopy, nasal inspiratory peak flow, and acoustic rhinometry. The patients who used a CPAP for 4 h or more per night for at least 70 % of the nights over a 6-month period were considered to have good adherence. RESULTS: Thirty-four patients finished the study. Eleven (33.4 %) were female and 23 (67.6 %) were male. Sixteen (47.1 %) patients had good adherence. The body mass index (p = 0.030), neck circumference (p = 0.006), and apnea-hypopnea index (p = 0.032) were higher, and the oxyhemoglobin saturation minimum was lower (p = 0.041) in the good adherence group. Nasal parameters showed no differences between good and poor adherence groups. In Spearman's correlation, surprisingly, there was a negative correlation between the highest number of hours of CPAP use with smaller values of nasal minimal cross-sectional areas in the supine position (r, 0.375; p = 0.029). In the linear regression model, the nasal findings that predicted increased of the CPAP use were the following: lower scores of nasal symptoms (p = 0.007) and lower nasal volume in supine position (p = 0.001). CONCLUSIONS: The majority of the nasal parameters evaluated in this study did not influence CPAP adherence.

BACKGROUND: Studies have shown a high occurrence of nasal alterations in patients with obstructive sleep apnea syndrome (OSAS), but no studies have used different methods to evaluate the nose of patients with OSAS. The objective of this study was to evaluate the nose of patients with OSAS, compare them to controls, and correlate the different methods used to evaluate the nose. METHODS: Forty-seven patients with moderate/severe OSAS and 20 controls who were matched for gender, age, and body mass index were included. Questionnaires regarding sleep and nasal symptoms, physical examination, rhinoscopy, nasofibroscopy, nasal inspiratory peak flow (NIPF), and acoustic rhinometry (AR) measurements were performed. RESULTS: In the OSAS group, 33 (70.2%) were male, with a mean age of 53.2 ± 9.1 years. In the control group, 13 (65%) were male, with a mean age of 53.7 ± 9.7 years. The OSAS group had a higher score on the nasal symptoms scale (p < 0.01) and a higher frequency of nasal alterations [presence of septal deviation, clinical complaints (p = 0.01) and hypertrophy of the inferior nasal turbinate (p < 0.01)]. The NIPF and AR parameters could not differentiate between the OSAS and control groups. There were no significant correlations among the different methods used to evaluate the nose. Lower NIPF values were capable of predicting higher apnea-hypopnea index scores (p = 0.007). CONCLUSION: Clinical complaints and nasal alterations as measured by rhinoscopy and nasofibroscopy were associated with the presence of OSAS, which was not the case for the NIPF and AR parameters. The results of different evaluation methods were not correlated with each other.

BACKGROUND: In intensive care units (ICUs), critically ill patients may be exposed to several risk factors for developing acute rhinosinusitis, including nasogastric tubes, mechanical ventilation, and prolonged periods in the supine position. The incidence of acute rhinosinusitis can be as high as 83%, the third or fourth most frequent ICU infection and it increases the risk of developing ventilator-associated pneumonia. Diagnosis and therapeutic approaches should be more aggressive than in non-ICU patients. Antral puncture plays a central role in the diagnosis and treatment of these patients. This study was designed to show the development of a method for sphenoid puncture in the diagnosis and treatment of acute infectious rhinosinusitis in critically ill patients, analyzing safety and complications. METHODS: Patients in ICUs with endoscopic and radiological diagnosis of acute rhinosinusitis were included. Maxillary puncture was performed through the inferior meatus; sphenoid puncture was performed by endoscopic identification of the sphenoethmoidal recess. An aspiration probe was introduced into the natural sphenoidal ostium, followed by aspiration of secretions and saline irrigation. RESULTS: Twenty-nine patients were included. Twenty-seven patients (93.1%) presented with sphenoidal sinusitis, 24 patients (82.7%) had maxillary sinusitis, 21 patients (72.4%) had ethmoidal sinusitis, and 13 patients (44.8%) had frontal sinusitis. Forty-seven sphenoidal and 39 maxillary punctures were performed. No major bleeding or other complications were recorded. CONCLUSION: The sphenoid puncture is a possible procedure to be performed in an ICU and might complement the paranasal puncture in cases of acute rhinosinusitis.

INTRODUCTION: Nasal polyposis is a chronic disease with unknown etiopathogenesis, although inflammatory mechanisms seem to play a role. One of several inflammatory mediators linked to nasal polyposis is Interleukin-6, which has a single nucleotide polymorphism -174 G/C that seems to promote an inflammatory reaction. OBJECTIVE: To compare the prevalence of the -174 G/C single nucleotide polymorphism between a group of patients with nasal polyposis and a control group. METHOD: Cross-sectional study with two groups (thirty two patients with nasal polyposis and fifty five controls) to investigate the -174 G/C polymorphism in blood samples. Asthma, aspirin intolerance and atopy were main exclusion criteria. IL-6 genotyping was performed using the PCR method with forward primer 5'-ATGCCAAGTGCTGAGTCACTA-3' and reverse primer 5'-GGAAAATCCCACATTTGATA-3', amplifying a 226-bp DNA fragment that contained the - 174 position. The amplified fragment can be cleaved by restriction enzyme NlaIII when the -174 position presented the C allele in two fragments of 117 and 109-bp, visualized by electrophoresis, classifying participants in GG, GC and CC. RESULTS: In the nasal polyposis group, 65.62% of the patients had the GG genotype, while in the control group, 41.82% had two G alleles, a statistically significant difference, with an odds ratio of 2.65. CONCLUSION: The -174 GG genotype was found more frequently in nasal polyposis patients than in controls, when asthma, aspirin intolerance and atopy were excluded.

UNLABELLED: The Obstructive Sleep Apnea/Hipopnea Syndrome (OSAHS) is closely related to obesity; a linear relation, however, has not been established, particularly in morbid obesity patients. AIM: To evaluate clinical and polysomnographic findings in a group of class III obese patients, and to relate these findings with the presence or absence of OSAHS. MATERIAL AND METHOD: Forty five patients with body mass index (BMI) over 40Kg/m2 were selected consecutively. A clinical history, the anthropometric examination and polysomnography were undertaken in all patients. The results of a study group and a control group (with no OSAHS) were compared. RESULTS: The sample consisted of 68.9% female and 31.1% male patients. The average age was 46.5 years (SD - 10.8 years); the average BMI was 49 (SD - 7 Kg/m2) and the average neck circumference was 43.4 cm (SD - 5.1 cm). All subjects were habitual snorers and 48.9% had daytime drowsiness. Polysomnography showed that 77.8% had an apnea/hipopnea index over 5. The findings associated with OSAHS were: younger age (p=0,02) and an increased neck circumference (p=0.004). CONCLUSION: The prevalence of OSAHS was very high, which emphasizes the importance of investigating this syndrome in patients sent for bariatric surgery. The neck circumference was the best OSAHS marked in this group of patients.

A Síndrome da apnéia/hipopnéia obstrutiva do sono (SAHOS) apresenta estreita correlação com a obesidade, porém não está estabelecida uma relação linear de gravidade principalmente em relação aos extremos como na obesidade mórbida. OBJETIVO: Avaliar os achados clínicos e polissonográficos em pacientes com obesidade classe III e correlacionar esses achados com a presença da SAHOS. MATERIAIS E MÉTODOS: Foram selecionados consecutivamente 45 pacientes com índice de massa corpórea superior a 40Kg/m2. Todos foram submetidos a anamnese, exame físico antropométrico e polissonografia. Os achados foram comparados entre os pacientes com e sem SAHOS. RESULTADOS: 68,9 por cento dos pacientes eram do sexo feminino e 31,1 por cento do masculino. A idade média foi de 46,5 DP 10,8 anos, a média do IMC foi 49 DP 7Kg/m2 e a média da circunferência cervical foi 43,4 DP 5,1cm. Todos os pacientes eram roncadores habituais e 48,9 por cento tinham queixa de hipersonolência diurna. Os achados polissonográficos mostraram que 77,8 por cento apresentaram índice de apnéia/hipopnéia superior a cinco. Apresentaram correlação com a presença da SAHOS: idade mais jovem (p=0,02) e maior circunferência cervical (p=0,004). CONCLUSÃO: A prevalência de SAHOS foi elevada, ressaltando a importância da sua investigação em pacientes referenciados para cirurgia bariátrica. O principal marcador da SAHOS foi a circunferência cervical.


The Obstructive Sleep Apnea/Hipopnea Syndrome (OSAHS) is closely related to obesity; a linear relation, however, has not been established, particularly in morbid obesity patients. AIM: To evaluate clinical and polysomnographic findings in a group of class III obese patients, and to relate these findings with the presence or absence of OSAHS. MATERIAL AND METHOD: Forty five patients with body mass indexex (BMI) over 40Kg/m2 were selected consecutively. A clinical history, the anthropometric examination and polysomnography were undertaken in all patients. The results of a study group and a control group (with no OSAHS) were compared. RESULTS: The sample consisted of 68.9 percent female and 31.1 percent male patients. The average age was 46.5 years (SD - 10.8 years); the average BMI was 49 (SD - 7 Kg/m2) and the average neck circumference was 43.4 cm (SD - 5.1 cm). All subjects were habitual snorers and 48.9 percent had daytime drowsiness. Polysomnography showed that 77.8 percent had an apnea/hipopnea index over 5. The findings associated with OSAHS were: younger age (p=0,02) and an increased neck circumference (p=0.004). CONCLUSION: The prevalence of OSAHS was very high, which emphasizes the importance of investigating this syndrome in patients sent for bariatric surgery. The neck circumference was the best OSAHS marked in this group of patients.

OBJECTIVE: To compare the reabsorption characteristics of fresh septal cartilage autografts, preserved homografts, and preserved autografts in the nasal dorsum of rabbits. METHODS: Rabbit nasal dorsum cartilage grafts were placed in 3 groups. The first group used fresh autologous cartilage; the second group, alcohol-preserved homologous cartilage; and the third group, alcohol-preserved autologous cartilage. Each rabbit received 2 grafts, one crushed and another noncrushed. After 16 weeks, the grafts were removed for analysis. RESULTS: No graft calcification occurred in any group. Chondrogenesis was observed in all groups. The fresh autograft group had the best results in the evaluation of the area of graft recovered and chondrocyte viability. The preserved autologous and homologous grafts did not differ in relation to any of the variables analyzed. Crushed grafts had inferior results in the area of graft recovered and chondrocyte viability compared with the noncrushed forms. No significant difference among the 3 groups was noted in the thickness of the fibrous capsule that developed around the graft. CONCLUSIONS: The fresh cartilage autograft was superior to the crushed and uncrushed preserved homografts and autografts; both types of preserved grafts had equivalent histological results. The uncrushed forms were superior to the crushed forms.

UNLABELLED: Obstructive sleep apnea syndrome is a complex disease, which the etiology is multifactorial and incompletely understood. Surgery with genioglossus advancement is indicated in hypopharynx obstruction. AIM: evaluate the efficacy and complications of genioplasty technique for genioglossus muscle advancement in patients with obstructive sleep apnea-hypopnea syndrome (OSAHS). METHODS: polysomnography, physical examination and cephalometric analysis were performed in 10 non-obese patients, who presented mild or moderate OSAHS, an apnea-hypopnea index (AHI) between 5 and 30, with hypopharynx obstruction and mandibular retrognathia. RESULTS: the AHI preoperative mean of 12.4+/-4.6 decreased to a postoperative mean of 4.4+/-5.7 (p < 0,001). When 50% reduction of preoperative AHI was chosen as a parameter, its rate was 70% (7/10). Cephalometric analysis revealed an increase in the posterior airway space (PAS) in all patients, from a preoperative mean value of 7.9+/-2.3mm to a postoperative mean value of 10.8+/-2.5mm (p < 0,001). CONCLUSION: genioplasty for genioglossus advancement seemed to reduce OSAHS signs, thus, it can be considered as an option for the surgical treatment of patients with hypopharynx obstruction. Data collected supports this surgical procedure as an option in patients with OSAHS and mandibular retrognathia.

A Síndrome da Apnéia-Hipopnéia do Sono Obstrutiva (SAHSO) é um doença de fisiopatologia complexa, e sua etiologia é multifatorial e não entendida completamente. Cirurgia com avanço do músculo genioglosso é indicado em obstrução na região de hipofaringe. OBJETIVO: Avaliar a eficácia e complicações da técnica de mentoplastia para avanço do músculo genioglosso em pacientes com síndrome da apnéia-hipopnéia do sono obstrutiva (SAHSO). MÉTODOS: Polissonografia, exame físico e análise cefalométrica foram realizados em 10 pacientes não-obesos, que apresentavam SAHSO leve ou moderada, índice de apnéia-hipopnéia (IAH) entre 5 e 30, obstrução de hipofaringe e retrognatismo mandibular. RESULTADOS: IAH pré-operatório médio foi de 12,4±4,6 diminuindo a média pós-operatória para 4,4±5,7 (p < 0,001). Quando 50 por cento de redução do IAH pré-operatório foram escolhidos como parâmetro, o índice de sucesso foi de 70 por cento. A análise cefalométrica revelou um aumento do espaço aéreo posterior (EAP) em todos os pacientes, a média pré-operatória foi de 7,9±2,3mm para uma média pós-operatória de 10,8±2,5mm (p < 0,001). CONCLUSÃO: Mentoplastia para avanço do músculo genioglosso parece reduzir os sinais da SAHSO, podendo ser considerada uma opção com tratamento cirúrgico em pacientes com obstrução na hipofaringe. Os dados coletados indicam esse procedimento como opção em pacientes com SAHSO e retrognatismo mandibular.

O nariz desviado é resultado de um conjunto de alterações anatômicas e por isso existe um grande número de técnicas para sua correção, sendo que um único método não deve ser utilizado em todos os casos. OBJETIVO: Avaliar um novo método para a correção destas deformidades, o uso do spreader graft no lado convexo do nariz desviado. FORMA DE ESTUDO: Prospectivo. MATERIAL E MÉTODO: Seis pacientes com nariz desviado foram submetidos à rinoplastia com colocação de spreader graft no lado convexo do desvio nasal e acompanhados por um período de 2 anos. RESULTADOS: Todos os pacientes apresentaram melhora da estética facial. Conclusão: Este estudo mostrou ser possível utilizar o spreader graft no lado convexo do desvio nasal em casos específicos.


A deviated nose is the result of a set of anatomical abnormalities, and for this reason there are many methods for correcting such defects. Therefore we should not use only one single method in all cases of nasal deformities. AIM: In this prospective study we propose a new method using a spreader graft on the convex side of the deviated nose. METHODS: We performed rhinoplasty in six patients by inserting a spreader graft in the convex side of the deviated nose and followed them for two years. RESULTS: All six patients presented an improvement in the external appearance of their noses. Conclusions: This study showed that in some particular cases, the spreader graft technique can be used successfully to correct deviated noses.
