BACKGROUND: Phlebologic diseases have become extremely common and have major socio-economic impact. However, the percentage of dermatologists working in phlebology appears to be decreasing according to the data of the German Society of Phlebology (DGP). METHODS: To investigate the reasons for this development, we--on behalf of the DGP--sent a questionnaire to 120 German Departments of Dermatology in autumn 2012. RESULTS: In 76 returned questionnaires, the number of physicians with additional fellowship training in phlebology averaged 1.5; the average number of those who fulfill the criteria for training fellows in phlebology was 0.9. In 71.1 % of the departments there was a phlebologist. A special phlebologic outpatient clinic existed in 73.7 % of the departments. Sonography with Doppler (89.5 %) and duplex (86.8 %) was used as the most frequent diagnostic tool. For therapy, compression (94.7 %), sclerotherapy (liquid 78.9 %, foam 63.2 %, catheter 18.4 %), endoluminal thermic procedures (radio wave 28.9 %, laser 17.1 %) and surgery (especially crossectomy and stripping 67.1 %, phlebectomy of tributaries 75 %) were used. The average number of treatments was very heterogenous in the different departments. CONCLUSIONS: Phlebology definitely plays an important role in dermatology. Most departments fulfill the formal criteria for the license to conduct advanced training in phlebology. A wide spectrum of phlebological diagnostic and therapeutic procedures is available.

RESUMO

We have observed a quadratic x-ray magneto-optical effect in near-normal-incidence reflection at the M edges of iron. The effect appears as the magnetically induced rotation of approximately 0.1 degrees of the polarization plane of linearly polarized x-ray radiation upon reflection. A comparison of the measured rotation spectrum with results from x-ray magnetic linear dichroism data demonstrates that this is the first observation of the Schäfer-Hubert effect in the x-ray regime. Ab initio density-functional theory calculations reveal that hybridization effects of the 3p core states necessarily need to be considered when interpreting experimental data. The discovered magneto-x-ray effect holds promise for future ultrafast and element-selective studies of ferromagnetic as well as antiferromagnetic materials.

RESUMO

Chronic diseases usually limit patients severely in their working, social and family life. With many chronic or chronically recurring skin diseases, particularly atopic disorders, it is often impossible to define specific causes that will respond to treatment. Since a full cure cannot be achieved in constitutional dermatoses, a holistic therapeutic approach is required. This must go beyond the identification, treatment and cure of the disease, and take into account the reciprocal interactions between the patient's organic, emotional and social symptoms. Based on the bio-psycho-social model of disease and health, modern dermatological rehabilitation therefore applies a holistic concept which goes well beyond dermatological treatment, also including the dimensions of function, activity, participation and context. The holistic approach is based on findings in rehabilitation science and takes into account the principles of complexity, interdisciplinary care and individuality.

RESUMO

A quantum cavity for spin is created using a tungsten crystal as substrate of high nuclear charge and breaking the structural inversion symmetry through deposition of a gold quantum film. Spin- and angle-resolved photoelectron spectroscopy shows directly that quantum-well states and the "matrioshka" or Russian nested doll Fermi surface of the gold film are spin polarized and spin-orbit split up to a thickness of at least nine atomic layers. Ferromagnetic materials or external magnetic fields are not required, and the quantum film does not need to possess a high atomic number as analogous results with silver show.

RESUMO

The ground state electronic properties of the strongly correlated transition metal Ni are usually not accessible from the excitation spectra measured in photoelectron spectroscopy. We show that the bottom of the Ni d band along [111] can be probed through the energy dependence of the phase of quantum-well states in Ag/Ni(111). Our model description of the quantum-well energies measured by angle-resolved photoemission determines the bottom of the empty set 1 d band of Ni as 2.6 eV, in full agreement with standard local density theory and at variance with the values of 1.7-1.8 eV from direct angle-resolved photoemission experiments of Ni.

The electronic structure of the (110)-oriented terraces of stepped W(331) and W(551) is compared to the one of flat W(110) using angle-resolved photoemission. We identify a surface-localized state which develops perpendicular to the steps into a repeated band structure with the periodicity of the step superlattices. It is shown that a final-state diffraction process rather than an initial-state superlattice effect is the origin of the observed behavior and why it does not affect the entire band structure.

RESUMO

Carbon nanostripes of graphene structure prepared on the stepped Ni(771) surface have been studied by angle-resolved photoemission. The electronic structure is anisotropic: parallel to the stripe direction, a graphite-type dispersion is measured, whereas the perpendicular direction displays two entangled band structures shifted in energy with respect to each other. These are experimentally identified as the microsurface-centered band structure and its umklapp scattered image caused by the superlattice.

Au chain structures have been prepared on Ni(110). Au6 s,p-derived features in photoemission spectra are identified as quantum-wire states due to their strong dispersion along the chains and absence of dispersion perpendicular to the chains in agreement with our ab initio calculation of the electronic structure. Spin analysis reveals that the states have minority-spin character showing that the confinement of electrons in the chain structure depends on the electron spin.

The design of a versatile high-precision eight-axis ultrahigh-vacuum-compatible polarimeter is presented. This multipurpose instrument can be used as a self-calibrating polarization detector for linearly and circularly polarized UV and soft-x-ray light. It can also be used for the characterization of reflection or transmission properties (reflectometer) or polarizing and phase-retarding properties (ellipsometer) of any optical element. The polarization properties of Mo/Si, Cr/C, Cr/Sc, and Ni/Ti multilayers used in this polarimeter as polarizers in transmission and as analyzers in reflection have been investigated theoretically and experimentally. In the soft-x-ray range, close to the p edges of Sc, Ti, and Cr, resonantly enhanced phase retardation of the transmission polarizers of as much as 18 degrees has been measured. With these newly developed optical elements the complete polarization analysis of soft-x-ray synchrotron radiation can be extended to the water-window range from 300 to 600 eV.

Using X-ray diffractometry and spectral measurements, the structure and properties of graded X-ray optical elements have been examined. Experimental and theoretical data on X-ray supermirrors, which were prepared by the magnetron sputtering technique using precise thickness control, are reported. Measurements on graded aperiodic Si(1-x)Ge(x) single crystals, which were grown by the Czochralski technique, are also presented. The lattice parameter of such a crystal changes almost linearly with increasing Ge concentration. The measurements indicate that Si(1-x)Ge(x) crystals with concentrations up to 7 at.% Ge can be grown with a quality comparable to that of pure Si crystals.

RESUMO

The high-brilliance synchrotron radiation source BESSY II will provide space for the installation of 14 insertion devices. At the time of writing (July 1997), eight insertion devices are funded. The first light for use by scientists will be available in January 1999. The undulators are designed for a scanning operation in parallel with the monochromator. This modus requires tight tolerances concerning the drive system. The first insertion device to be installed, a U-49 undulator, has been assembled and magnetically measured. The r.m.s. phase error is 2 degrees. An endpole configuration using coils with less than 64 amp-turns makes the angular kick smaller than +/-3 micro rad over a gap range 15-50 mm.

The high precision of 0.1 arcsec required for the positioning of optical elements in new two-axes monochromators at the undulator beamlines at BESSY II has led to the development of UHV-compatible high-precision angle encoders. Mounted directly on the rotation axes, they provide substantial advantages over measuring systems connected outside the vacuum vessel. Making use of a fast closed-loop control system, an accuracy of 0.1 arcsec at a resolution of less than 0.01 arcsec has been experimentally verified.

RESUMO

At BESSY II, a third-generation 1.7 GeV storage ring is under construction. A planar elliptical undulator will be installed as a source of X-rays that have a high degree of circular polarization. Radiation in the first, third and fifth harmonics will cover the energy range 87-1330 eV. The beamline will essentially consist of a plane-grating monochromator working with collimated light in the dispersion plane. A single set of optical elements can be used to handle the two angularly separated beams of opposite helicity from the double undulator. The degree of circular polarization ranges from 73 to 100%, and a flux of up to 5 x 10(13) photons s(-1) (100 mA)(-1) can be achieved. A maximum spectral resolution of about 13 000 will be possible at 100 eV using a 20 micro m slit.

At the German synchrotron radiation facility BESSY II, the VUV photon energy range between 15 and 600 eV is covered by the first, third and fifth harmonics of the U125 undulator. The beamline connected to this source is a plane-grating monochromator (PGM) with a variable deviation angle. The plane grating is illuminated by collimated radiation, allowing very flexible use of the monochromator. Depending on users' demands, it can be operated at high spectral resolution, high-order suppression or in high-flux mode, simply by software control.

RESUMO

At the BESSY II storage ring, the Physikalisch-Technische Bundesanstalt (PTB) will operate insertion devices dedicated to radiometric use. One branch of the appendant beamline system will be equipped with a grazing-incidence monochromator, described here. The monochromator concept is based on a plane grating operated in parallel light; therefore exact focusing is maintained for all photon energies irrespective of the angular setting at the grating. The monochromator has been optimized for small higher-order transmittance and high power throughput, as required by radiometric applications in the wide photon energy range from 20 eV to 1900 eV.
