RESUMO

Since the introduction of the polymerase chain reaction (PCR) technique in 1983, nucleic acid amplification has permeated all fields of biological science, particularly clinical research. Despite its importance, PCR has been restricted to specialized centers and its use in laboratories with few resources is limited. In recent decades, there has been a notable increase in the development of new isothermal technologies for molecular diagnosis with the hope of overcoming the traditional limitations of the laboratory. Among these technologies, recombinase polymerase amplification (RPA) has a wide application potential because it does not require thermocyclers and has high sensitivity, specificity, simplicity, and detection speed. This technique has been used for DNA and RNA amplification in various pathogenic organisms such as viruses, bacteria, and parasites. In addition, RPA has been successfully implemented in different detection strategies, making it a promising alternative for performing diagnoses in environments with scarce resources and a high burden of infectious diseases. In this study, we present a review of the use of RPA in clinical settings and its implementation in various research areas.

Infraspinatus muscle atrophy is common in professional volleyball players, but it is unclear whether commonly observed strength and proprioception deficits can be reversed with training. Fifty-four participants were recruited into an infraspinatus atrophy group (IAG, n = 18) and a non-atrophy group (NAG, n = 18) of elite volleyballers plus a healthy non-athletic control group (CG, n = 18). IAG were trained with a progressive, specific shoulder external rotator strengthening routine for 32 sessions over 8 weeks. Shoulder external rotation peak torque (SERPT) and threshold to detect passive movement (TTDPM) and joint position sense (JPS) were measured before and after the intervention. At baseline, no significant difference was detected in strength or proprioception between the injured and control groups, but the normal athletes were stronger and had better proprioception than either IAG or CG (P < .001). IAG (d = 2.78) and NAG (d = 0.442) improved strength significantly after training. IAG improved TTDM and JPS (P < .001, d = -0.719 and -2.942, respectively) but were still worse than NAG (P < .001). Elite volleyball players with Infraspinatus muscle atrophy have strength and proprioception deficits which can be improved by a specific exercise program to normal but not elite athlete control levels.

As lesões coronarianas já foram relatadas em espécies de peixes anádromos, com características semelhantes às observadas em aterosclerose humana, mas não em peixes cultivados sem interferência de padrões reprodutivos. Objetivou-se descrever lesões coronarianas em beijupirá (Rachycentron canadum) criado em sistema offshore, em 54 exemplares coletados ao longo do cultivo de oito meses. Amostras de tecido cardíaco de alevinos e juvenis foram submetidas à análise histopatológica e à microscopia eletrônica de transmissão (MET), para visualização de lesões coronarianas, sendo as lâminas histológicas coradas pela técnica de hematoxilina-eosina, ácido periódico de Schiff, alcian blue e reticulina de Gomori. Exames realizados pela fotomicroscopia de luz evidenciaram lesões arteriais caracterizadas por hiperplasia das túnicas íntima e média de artérias coronárias em, respectivamente, 29,63% e 79,63% dos animais, com redução do lúmen arterial. Na MET, foram observadas alterações na estrutura das coronárias com espessamento do endotélio e membrana basal, proliferação de fibroblastos e fibras colágenas, com acúmulo de material lipídico subendotelial, resíduos celulares aderidos à membrana basal e vesículas pinocitóticas com presença de lisossomos isolados. Constatou-se que os peixes da espécie Rachycentron canadum criados em cativeiro desenvolvem lesão arterial do tipo crônica degenerativa inflamatória.


The coronary lesions have been reported in species of anadromous fish with similar characteristics as those observed in human atherosclerosis, but not in farmed fish without interference of reproductive patterns. This study aimed to describe coronary lesions in beijupirá (Rachycentron canadum) farmed in an offshore system, in 54 specimens collected along an eight month cultivation period. Samples of heart tissue of fry and juveniles were subjected to histopathological analysis and transmission electron microscopy (TEM) for visualization of coronary lesions, the histology slides were stained using hematoxylin-eosin, periodic acid Schiffh, alcian blue, and reticulin of Gomori. Examinations performed by light microscopy showed arterial lesions characterized by hyperplasia of the intima and media tunics, respectively in 29.63% and 79.63 % of the animals, with reduced lumen. In TEM changes in the structure of the coronary endothelium and thickening of the basement membrane, proliferation of fibroblasts and collagen fibers, with subendothelial accumulation of lipid material, cellular debris adhering to the basement membrane and presence of pinocytotics vesicles and isolated lysosomes were observed. It has been found that the Rachycentron canadum fish species farmed in captivity develop arterial lesion of the chronic inflammatory degenerative type.

Sir, People over the age of 65 represent an increasingly large proportion of the population. In fact, ageing brings about some decrease in sexual arousal and activity [1] related to physical illness or medication needed to control a health condition [2]. Nevertheless, it is encouraging to find out that many elderly continue to be sexually active until their sixties, seventies and even eighties [3]. Information concerning the incidence of sexually transmitted infections in old people is sparse. Six retrospective studies have been published before [4-9]. They confirmed that the incidence of sexually transmitted diseases in patients over 65 years was significant for both newly acquired disease and for residual complications. Beyond that, there are sporadic case reports of HIV infection in older persons [10, 11], emphasising the persistence of risk behaviour in this population.

RESUMO

We report the case of an 84-year-old man with clinical, histological and electron-microscopic features of a Darier-like pattern of Grover's disease. The lesions had a persistent course, with a 5-month history before diagnosis and a clinical onset 2 years after the surgical removal of a cholangiocarcinoma. A significant clinical improvement was observed after a three-week course of calcipotriol ointment applied as a monotherapy regimen. Clinical uses of topical calcipotriol in dermatological conditions are reviewed with emphasis on dyskeratotic disorders.
