RESUMO

The paper describes a new device for treatment of vascular erectile dysfunction--the vibrovacuum fallostimulator "Sanos". This method differs from conventional local decompression in a dynamic regimen of vacuum effect on the penis in the presence of vibration. The resultant reduced exposure of the organ to vacuum prevents such serious complications as edema, hemorrhages and necrosis of penile skin. The dynamic vacuum regimen is also effective in restoring erectile activity of the penis producing erection similar to physiological. A 10-15-day course of vibrovacuum fallostimulation was conducted in 330 patients with vascular erectile dysfunction. Recovery of arteriocavernous blood flow was registered in 92.1%. No complications occurred. The above method can be also applied for diagnosis of both insufficient arterial circulation in the penis and defects in venoocclusive mechanism of erection.

RESUMO

The authors review current classifications of chronic prostatitis and propose their own classification which reflects routes of prostatic infection, division of infectious prostatitis into obstructive and non-obstructive forms, etiology of chronic prostatitis. Original methods and facilities for prostatic drainage via transurethral and transrectal approaches are described.

RESUMO

The efficacy of prostatic drainage using transurethral vacuum aspiration (Introl-4 unit) and transrectal pneumovibromassage (PVM-R-01) was compared in 1511 patients with chronic obstructive prostatitis. The drainage was accompanied by local (urethral, rectal and urethral-rectal tripsin or chimotripsin electrophoresis) or systemic enzymic therapy (vobenzim). The highest effect of the drainage was achieved in patients pretreated for 5-7 days with systemic enzymes. Transrectal pneumovibromassage proved more effective and safe but the transurethral technique if indicated can be also used in the combined treatment.

RESUMO

Specimens obtained by aspiration at transurethral vacuum drainage of the prostate from 166 patients with obstructive prostatitis were studied before and during treatment. This study showed that the obstruction was primarily caused by epithelial and purulent "plugs" of the lobular excretory ducts. Stages of their formation and drainage technique were specified. Aspiration specimens examination was essential for diagnosis of prostatic obstruction degree in chronic prostatitis as well as for decision making on the end of the drainage therapy.

RESUMO

The clinico-immunological examination of 57 patients with chronic bacterial prostatitis was carried out. The clinical analysis made it possible to divide the patients into 3 groups characterized by the presence of chronic inflammatory diseases of other organs which had appeared before (group 1) or after (group 2) the manifestation of the symptoms of prostatitis, aw well as by the absence of concomitant inflammatory diseases (group 3). At the same time these patients were found to have changes in their immune status, most pronounced in patients of groups 1 and 2. The clinico-immunological analysis of the patients with chronic bacterial prostatitis revealed the fact that chronic bacterial prostatitis was a chronic inflammatory process linked with changes in the immune system; these changes had the signs of secondary immunodeficiency and required immunocorrective therapy.

Comparative clinical data are presented on efficacy of transurethral vacuum drainage of the prostate in patients with obstructive chronic prostatitis in the absence of systemic enzyme therapy and under treatment with enzyme medicine bobenzyme (Mucos Pharma, Germany) having proteolytic activity. The duration of the treatment in bobenzyme-treated patients reduced by 30% due to decreased viscosity of the aspirate and lysis of massive "plugs" of the ducts facilitating vacuum elimination of the inflammation substrate.

A detailed ultrasonic picture of the prostate was provided by the rectal device of Pie Medical-200 unit for 1066 patients with chronic prostatitis before and in the course of drain therapy using electrostimulator-aspirator Intraton-4. The leading ultrasonic diagnostic symptoms of chronic prostatitis are represented by various microspaces and hypoechogenic zones in different prostatic parts which reflect activity of chronic inflammatory process. The severity of the disease was determined primarily by the presence in the prostate of "true microabscesses" (microcavity of the irregular form) and pseudomicroabscesses (drop-shaped microspaces) which can be successfully drained irrespective of their number, size, location and characteristics of the content. The evidence gave grounds for design of the scheme describing pathogenesis of chronic prostatitis and its reverse dynamics in the course of drain therapy. These reverse changes closely resemble stages of ureterohydronephrosis.

486 patients with clinical symptoms of chronic prostatitis were examined. In 98 of them transurethral ultrasound has revealed undrained prostatic lobules undergoing different stages of chronic inflammation (pseudomicroabscesses). The combined treatment included transurethral vacuum drainage and direct electrostimulation of the prostate on Intraton-4 unit. Morphological examination has discovered in the aspirate plugs of two types (epithelial and pussy) occluding excretory ducts of the prostatic lobules. These plugs were responsible for poor drainage of the lobules that supported chronic inflammation. Intraton-4 proved to be an efficient modality in combined treatment of chronic prostatitis providing a complete drainage of the affected lobules with aspiration of the plugs and clinical recovery. No complications were registered.
