RESUMO

Rehabilitation of an edentulous posterior mandible to restore function and arch stability can be accomplished with a removable partial denture or an implant supported fixed partial denture. If the alveolus is severely resorbed, implant placement becomes challenging due to inadequate bone and the position of the inferior alveolar nerve. This report details a situation where a mandibular fracture occurred soon after inferior alveolar nerve (IAN) transposition and simultaneous implant placement. The prosthodontic reconstruction was completed using a fixed-dental prosthesis.

RESUMO

Stable occlusion and a pleasing esthetic appearance are often difficult to achieve in patients with congenital defects and severe midfacial deficiencies. Conventional therapy, such as orthodontic treatment followed by orthognathic surgery, is often not sufficient to fully correct the dental and esthetic problems. An interdisciplinary approach for these patients should include prosthodontic treatment that will assist in establishing a harmonious occlusion and improve facial appearance. This clinical report describes the interdisciplinary approach for a young patient with a history of bilateral cleft lip and palate, spina bifida, hydrocephalus, and ventriculoperitoneal shunt. The patient was treated with conventional orthodontic treatment and orthognathic surgery that failed to fully correct the malocclusion. A removable overlay prosthesis made of crystallized acetyl resin was used to reestablish esthetics and create a stable occlusion.

AIMS: The morphometry of the distal femur was largely studied to improve bone-implant fit in total knee arthroplasty (TKA), but little is known about the asymmetry of the posterior condyles. This study aimed to investigate the dimensions of the posterior condyles and the influence of externally rotating the femoral component on potential prosthetic overhang or under-coverage. PATIENTS AND METHODS: We analysed the shape of 110 arthritic knees at the time of primary TKA using pre-operative CT scans. The height and width of each condyle were measured at the posterior femoral cut in neutral position, and in 3º and 5º of external rotation, using both central and medial referencing systems. We compared the morphological characteristics with those of 14 TKA models. RESULTS: In the neutral position, the dimensions of the condyles were nearly equal. Externally rotating the femoral cut by 3º and 5º with 'central referencing' induced width asymmetry > 3 mm in 23 (21%) and 33 (30%) knees respectively, while with 'medial referencing' it induced width asymmetry > 3 mm in 43 (39%) and 75 (68%) knees respectively. The asymmetries induced by rotations were not associated with gender, aetiology or varus-valgus alignment. CONCLUSION: External rotation may amplify the asymmetry between the medial and lateral condyles, and exacerbate prosthetic overhang, particularly in the superolateral zone. 'Central referencing' guides result in less potential prosthetic overhang than 'medial referencing' guides. Cite this article: Bone Joint J 2017;99-B:894-903.

RESUMO

The technique in this article was developed to provide a means to create prepared guide planes of proper dimension to ensure a more stable and retentive removable partial denture prosthesis (RPDP) framework when providing this service for a patient. Using commonly found clinical materials, a paralleling device can be fabricated from the modified diagnostic cast of the patient's dental arch requiring an RPDP. Polymethyl methacrylate or composite added to an altered thermoplastic form can be positioned intraorally and used as a guide to predictably adjust tooth structure for guide planes. Since it can potentially minimize the number of impressions and diagnostic casts made during the procedure, this can help achieve the desired result more efficiently and quickly for the patient.

Vinyl polysiloxane (VPS) has multiple applications in prosthodontics. This article describes how a bite registration fast-set VPS material was used to identify length overextension and/or excessive border thickness of denture flanges. In addition, the advantages of VPS over conventional materials are presented.

RESUMO

A female college student was referred by her general dentist for evaluation of the interproximal caries-like lesions discovered during her biannual visit. The lesions were determined to result from external resorption of unknown origin. After the most affected teeth had been extracted and the other teeth were restored after periodontal surgery, an interim partial removable dental prosthesis was placed. Two months later, the resorption pattern had proliferated to the rest of her dentition, thus revising the treatment plan to involve full mouth extractions and to place immediate maxillary and mandibular complete removable dental prostheses. Subsequently, 6 titanium dental implants were placed in each arch, and, after healing, opposing metal ceramic screw-retained fixed dental prostheses were inserted.

RESUMO

A patient treatment is presented to demonstrate a technique using visible-light-cured (VLC) resin to create a characterized denture base for an interim denture. This indication for characterization is routinely seen with African-Americans with dark-pigmented gingiva. Using a combination of blue VLC tray resin and pink VLC denture base resin, the mixture achieves a darker-colored denture base. This technique was used for the fabrication of an interim denture for a patient who required an immediate interim denture. The prosthesis was worn during the healing phase and allowed the patient to function while the definitive prosthesis was being fabricated.

RESUMO

The use of resilient denture liners provides an invaluable modality in removable prosthodontic treatment. Treatment of atrophic mandibles, preoperative and postoperative soft tissue management, and retention of removable prosthesis are a few applications of these materials. This article describes the clinical application of a polyphosphazene-based liner (Novus, Hygenic Corp, Dayton, OH) as a denture liner and retentive material in conventional and implant overdenture rehabilitation.

RESUMO

A technique is described for construction of an immediate transitional complete denture in one appointment using self-curing tooth color and visible light-cured resins. Conventional immediate denture therapy may be too prolonged for medically or physically compromised patients. This alternative treatment reduces laboratory, dentist, and patient time, thereby providing a fast and economical service.
