RESUMO

Objective:To investigate the application value of pelvic floor ultrasound in screening early postpartum stress urinary incontinence (SUI).Methods:A total of 220 postpartum women, who were admitted by the Department of Gynecology and Obstetrics of The People's Hospital of Hechi from January 2018 to June 2019, were selected for this study. These women were divided into SUI ( n = 52) and no SUI ( n = 168) groups according to whether SUI occurred within 42 days after delivery. Pelvic floor ultrasound parameters and clinical data were compared between the two groups. The effects of each ultrasound parameter on postpartum SUI were assessed. The receiver operating characteristic (ROC) curve was plotted for each parameter. The area under the ROC was calculated. The risk factors for postpartum SUI were investigated using the logistic regression analysis. Results:The number of deliveries (≥ 2 times), the proportion of women subjected to vaginal delivery, and neonatal weight in the SUI group were significantly higher than those in the no SUI group [ χ2 = 4.13, 3.30, t = 4.43, all P < 0.05]. There were significant differences in the bladder neck position and levator hiatus area in the resting state between the two groups ( t = 2.29, 3.09, both P < 0.05). There were significant differences in the bladder neck position, levator hiatus area, urethral rotation angle, and bladder neck mobility during the Valsalva movement between the two groups ( t = 13.14, 4.27, 15.64, 8.54, all P < 0.05). The areas under the ROC of bladder neck position and levator hiatus area in the resting state and the areas under the ROC of bladder neck position, levator hiatus area, urethral rotation angle, and bladder neck mobility during the Valsalva movement were 0.707, 0.725, 0.730, 0.771, 0.813, and 0.836, respectively. The area under the ROC of parameters used in combination was 0.849. Logistic regression analysis results revealed that the number of deliveries (≥ 2 times), bladder neck position (> 27.286 mm) and levator ani hiatus area in resting state (> 16.663 cm 2), and bladder neck position (< -2.774 mm), levator hiatus area (> 21.915 cm 2), urethral rotation angle (> 80.445°), and bladder neck mobility (> 30.501°) during the Valsalva movement were the risk factors for postpartum SUI. Conclusion:Combined use of pelvic floor ultrasound parameters is valuable for the diagnosis of postpartum SUI. Abnormal changes in the bladder neck position, levator ani hiatus area, urethral rotation angle, and bladder neck mobility are closely related to the occurrence of postpartum SUI. Therefore, combined use of pelvic floor ultrasound parameters can be preferred to screen early postpartum SUI.

Objective@#To investigate the total amount and types of fluid intake among college students in Hebei Province during spring and to provide scientific basis for fluid-intake education.@*Methods@#A simple random sampling method was used to select 156 college students from a college in Hebei Province. The information on amount and type of fluid intake was recorded using the 7-day 24-hour recording method. A separate analysis was performed on people who consumed beverages during the 7 days.@*Results@#The median intake of plain water, milk, sugar-sweetened beverages and other beverages was 866 mL, 43 mL, 43 mL and 2 mL, respectively. The median of plain water, milk, sugar-sweetened beverages and other beverages by the beverage-consumers was 845 mL, 36 mL, 63 mL and 11 mL, respectively. The intake of plain water and sugar-sweetened beverages in male students was significantly higher(Z=2.31, 3.03; 2.31, 2.11, P<0.05), and milk consumption was lower than female students of these two groups of people(Z=-2.73, -2.23, P<0.05). The intake of other beverages in male students was higher than that of female students, but only significant among beverage-consumers(Z=3.31, P<0.01). There was no significant difference in the total amount of different fluid types among different BMI(P>0.05).@*Conclusion@#College students in Hebei Province mainly drink plain water. Sex difference is observed in the types of fluid intake.

The aim of this article is to study how andrographolide-releasing collagen scaffolds influence rabbit articular chondrocytes in maintaining their specific phenotype under inflammatory environment. Physical blending combined with vacuum freeze-drying method was utilized to prepare the andrographolide-releasing collagen scaffold. The characteristics of scaffold including its surface morphology and porosity were detected with environmental scanning electron microscope (ESEM) and a density instrument. Then, the release of andrographolide from prepared scaffolds was measured by UV-visible spectroscopy. Rabbit chondrocytes were isolated and cultured and seeded on andrographolide-releasing collagen scaffolds. Following culture with normal medium for 3 d, seeded chondrocytes were cultured with medium containing interleukin-1 beta (IL-1β) to stimulate inflammation for 7 d. The proliferation, morphology and gene transcription of tested chondrocytes were detected with Alamar Blue assay, fluorescein diacetate (FDA) staining and reverse-transcriptase quantitative polymerase chain reaction (RT-qPCR) test respectively. The results showed that the collagen scaffolds prepared by vacuum freeze-dry possess a high porosity close to 96%, and well-interconnected chambers around (120.7±17.8) μm. The andrographolide-releasing collagen scaffold continuously released andrographolide to the PBS solution within 15 d, and collagen scaffolds containing 2.22% andrographolide significantly inhibit the proliferation of chondrocytes. Compared with collagen scaffolds, 0.44% andrographolide-containing collagen scaffolds facilitate chondrocytes to keep specific normal morphologies following 7 d IL-1β induction. The results obtained by RT-qPCR confirmed this effect by enhancing the transcription of tissue inhibitor of metalloproteinase-1 ( ), collagen II ( ), aggrecan ( ) and the ratio of / collagen I( ), meanwhile, reversing the promoted transcription of matrix metalloproteinase-1 ( ) and matrix metalloproteinase-13 ( ). In conclusion, our research reveals that andrographolide-releasing (0.44%) collagen scaffolds enhance the ability of chondrocytes to maintain their specific morphologies by up-regulating the transcription of genes like , and , while down-regulating the transcription of genes like and which are bad for phenotypic maintenance under IL-1β simulated inflammatory environment. These results implied the potential use of andrographolide-releasing collagen scaffold in osteoarthritic cartilage repair.

RESUMO

Sal-like protein 4 (SALL4), a newly discovered oncogenes, plays an important role in the formation of embryo, maintenance of embryonic stem cell pluripotency and self-renewal. SALL4 not only has a close relationship with hereditary disease, benign disease, malignant tumor, blood system tumor and germ cell tumors, but also chemoresistance and poor prognosis. SALL4 obtains biological functions by adjusting different signaling pathways, transcription factors and epigenetic modulation. Further mechanism study of SALL4 and its relationship with various diseases will provide new opportunities for the treatment of diseases.

It is considered thatpoints are applied to treat various pelvis floor diseases and symptoms of different systems. Each point has similar function but with unique feature. Shangliao (BL 31) is mainly used to treat gynecologic diseases;Ciliao (BL 32) and Zhongliao (BL 33),urologic system and reproductive system diseases;Zhongliao (BL 33) and Xialiao (BL 34),reproductive system and anorectal system diseases.

Objective To analyze main influencing factors of hospitalization expenses by support vector machine modeling,and explore effective influence factors analysis methods of medical expenses. Methods Random selection of six hospitals in Zhejiang province.Using hospital electronic medical record system of the hospitals and selecting three kinds of typical diseases of internal medicine and surgery,to build the support vector machine model,BP neural network model,and multiple linear regression model for comparison of analysis results.The SVM model is used to analyze three various diseases.Results The support vector machine model based on radial basis kernel function scored the highest prediction accuracy on the hospitalization expenses,up to 96.07%.In a mixed analysis of different diseases,analysis results of all three models pointed the main influence factors of hospitalization expense as days of stay,disease types,and hospital coding for the surgery.In the analysis by diseases individually,the influencing factors, though varying with diseases, key factors remain the same. Conclusion The support vector machine in the influence factor analysis is feasible in hospitalization expenses.According to the analysis results,the single disease payment system can be made rationally, which can effectively control excessive growth of medical expenses.

@# Objective To explore the 3D gait analysis of stroke hemiplegic patients. Methods 28 stroke hemiplegic patients were examined with the 3D gait analysis system before and after 6-week rehabilitation. Results After rehabilitation, support phase time of the affected lower limb increased and swing phase time decreased (P<0.05). Step length, stride length and walking speed increased (P<0.05). There was significantly difference in range of motion (ROM) of hip deduction/abduction, flexion/extension, and knee flexion/extension, and ankle internal/external rotation, varus/eversion, dorsiflexion/plantar flexion before and after rehabilitation (P<0.05). Conclusion 3D gait analysis system can evaluate the patient's gait objectively and quantitatively, and provide the basis for rehabilitation assessment.

Objective To inVestigate the effect of emodin on hyPertroPhic scar fibroblasts ( HSFs ) and exPlore the underlying mechanism. Methods HSFs were treated by emodin at final concentrations of 0,20,40,and 80 μmol·L-1, resPectiVely,in the cultural media. Forty_eight hours later,the cells were subjected to MTS assay and flow cytometry assay with annexin V and ProPidium iodide as dyeing indicators. Whole cell lysates from the cells of eVery grouP were subjected to Western blotting to measure the Protein exPression leVels of ERK1∕2,Bcl_2,Mcl_1 and RIP1. Results The cell Viability of HSFs was inhibited by emodin in a dose dePendent manner. The mortality rate of HSFs treated with emodin for 48 h at the concentrations of 40 and 80 μmol·L-1 were 28. 6%and 68. 0%,resPectiVely,which was significantly higher than that of the control grouP ( P<0.01).Pretreatmentwith Z_VAD_FMK could Partially reduce the mortality caused by emodin (P<0.05).PhosPhorylation of ERK1∕2 and the exPression of RIP1 and Mcl_1 were inhibited by emodin. Conclusion Down regulation of ERK1∕2,RIP1 and Mcl_1 by emodin may account for the inhibited Proliferation and increased cell death of HSFs.

@#Objective To evaluate family economic burden of children with autism, or with physical disability or with intellectual disability.Methods227 parents of children with autism, children with physical disability, children with intellectual disability and normal children were interviewed for their family economic burden.ResultsThe medical cost and caring cost of children with disability were significantly more than those of normal children, and the education cost, clothes cost and amusement cost of children with disability were significantly less than those of normal children. Family income was only affected by education level of parents. Families of children with disability received more economic assistance than families of normal children except families of autistic children. More children the family had, less economic assistance the family acquired. Compared with normal children, the family economic burden of children with disability were as following, children with autism (19582.4 RMB per year), children with physical disability (16410.1 RMB per year), children with intellectual disability (6391.0 RMB per year). ConclusionFamilies of children with autism, children with physical disability and children with intellectual disability had heavier economic burden than families of normal children.
