RESUMO

Intracavernous injection of 20 micrograms prostaglandin E1-CD (PGE1-CD, 8 cases), 5 micrograms lipo prostaglandin E1 (Lipo PG-E1, 8 cases) or 10 micrograms Lipo PGE1 (9 cases) was performed in patients with functional impotence in order to comparatively analyze the diagnostic efficacy of these drugs. Full erection was observed in all patients who received intracavernous injection of 20 micrograms PGE1-CD or 10 micrograms Lipo PGE1. However, full erection was observed in 4 out of 8 patients administered 5 micrograms Lipo PGE1. RigiScan was used on all patients, and latency until erection (achievement of maximum rigidity of the base) after injection, maximum rigidity of the penile tip and base and circumferential expansion rate of the penile tip and base were measured. With regard to these RigiScan data and duration of erection, there were no significant differences among the 3 groups. There were no severe side effects in any of the patients. These findings indicate that 10 micrograms Lipo PGE1 and 20 micrograms PGE1-CD have similar effects and that Lipo PGE1 may be an effective drug for the diagnosis and treatment of impotence.

We studied uroflowmetry in 25 normal adult males and 100 patients with benign prostate hypertrophy (BPH). We planned a new parameter, named average flow rate increase ratio (AFRIR), based on urinary flow rate curve. One hundred patients with BPH were measured presumed circle area ratio (PCAR) based on transrectal ultrasonotomography and performed comparative studies. AFRIR was not related to voided volume of urine (VV). Although AFRIR in 100 patients with BPH was not significantly different from that in 25 normal adults males, AFRIR in 20 patients with BPH, needing surgical operation, was significantly lower than that in 25 males. We considered that AFRIR was useful for deciding on the surgical operation and these levels were below 1.5 ml/sec2. AFRIR was correlated well with maximum flow rate (MFR), average flow rate (AFR), MFR/VV x 100, AFR/VV x 100, PCAR and Area, respectively. PCAR correlated well with MFR/VV x 100 and AFR/VV x 100, respectively. Area correlated well with MFR, MFR/VV x 100, respectively. From these results, it was presumed that AFRIR was reflected clearly to the condition of urine voiding.

We reviewed the outcome of 300 cases without urinary tract infections to determine the relationship between leucocytosis and severe pain due to ureteral calculi. Leucocytosis above 9,000 cells per cmm was seen in 87.7% of the cases. Many patients (61%) had a leucocyte count between 9,000 and 12,000, but none had that exceeding 22,000. This leucocytosis can be attributed to a transient distributional alteration by means of leucopheresis to circulation sites from a storage pool such as the blood vessels in the spleen, liver, lung, etc. Since leucocytosis often accompanies ureteral calculi, caution should be exercised in distinguishing it from appendicitis.

Seventeen patients with benign prostatic hypertrophy were treated with 50 mg allylestrenol per day for a long period of time (mean: 37.7 weeks), and subjective and objective findings, transrectal ultrasonotomography, urodynamics, serum lipids and hormone levels were examined. Improvement rates of subjective and objective findings were 42.9-92.9%. A significant decrease in weight and diameter (antero-posterior, lateral) of the prostate was observed, but the difference in the height of the prostate was not significant. Increase in intravesical pressure was observed in 9 out of 14 cases (64.3%) and the decrease in area under the urethral pressure curve at functional profile length was observed in 6 out of 11 cases (54.5%). Slight increase of the serum lipid levels was observed in a few cases, but in many cases the fluctuation was within normal range. Although clear decrease in the testosterone levels was seen, decrease of libido and potency was observed in only one case (5.9%). No other side-effect was found. The overall efficacy rate was 58.8%, and clinical usefulness of Allylestrenol on benign prostatic hypertrophy was confirmed.

Prostatic tissue levels of Cefoperazone (CPZ) were studied after the administration of 3 g of CPZ, in 16 patients with prostatic hypertrophy who underwent retropubic prostatectomy. The CPZ concentration in the surgical capsule at 60 min. after the administration of CPZ was about 2 times higher than that in the gland. As compared with the histological type of removed gland, the levels in the glandular hyperplasia were slightly higher than that in the fibromuscular and mixed type hyperplasia, but the difference was not statistically significant. CPZ was found to be effective for the treatment of post-operative and bacterial prostatitis, since the CPZ levels of prostatic tissue were considerably high and above the minimal inhibitory concentration level for most bacteria.

Twenty patients with benign prostatic hypertrophy were treated with an anti-androgenic agent, chlormadinone acetate ( Prostal ), 50 mg daily for 3 months. As a control, 20 patients with prostatic hypertrophy were treated with Eviprostat which was a non-hormonal remedy consisting of plant extracts 6 tablets daily for the same duration. Subjective symptoms of 14 patients (70%) in the Eviprostat group and 18 cases (90%) in the Prostal group were improved significantly. Although there were no significant changes in size or weight of the prostate after 3 months in the patients in the Eviprostat group as measured by transrectal ultrasonotomography, remarkable reduction in size and weight of the prostate was observed after 3 months in 11 patients (55%) in the Prostal group. Four cases complained of impotency during Prostal treatment, but no other side effects to the hepatic and/or renal functions were observed.
