RESUMO

Background: Emergency obstetric and newborn care (EmONC) in Bangladesh focusses on maternal health, whereby it addresses childbirth and postpartum complications to ensure women's health and well-being. It was transitioned to a digital platform to overcome challenges with the paper-based EmONC register and we conducted implementation research to assess the outcome. Here we outline the stakeholder engagement process integral to the implementation research process. Methods: We adopted a four-step stakeholder engagement model based on the identification, sensitisation, involvement, and engagement of stakeholders. The approach was informed by previous experience, desk reviews, and expert consultations to ensure comprehensive engagement with stakeholders at multiple levels. Led by the Maternal Health Programme of the Government of Bangladesh, we involved high-power and high-interest stakeholders in developing a joint action plan for digitisation of the paper-based EmONC register. Finally, we demonstrated this digital EmONC register in real-life settings to stakeholders at different levels. Results: The successful demonstration process fostered government ownership and collaboration with multiple stakeholders, while laying the foundation for scalability and sustainability. Nevertheless, our experience highlighted that the stakeholder engagement process is context-driven, time-consuming, resource-intensive, iterative, and dynamic, and it requires involving stakeholders with varied expertise. Effective strategic planning, facilitation, and the allocation of sufficient time and resources are essential components for successful stakeholder engagement. Conclusions: Our experience demonstrates the potential of adopting the 'identification, sensitisation, involvement, and engagement' stakeholder engagement model. Success in implementing this model in diverse settings depends on leveraging knowledge gained during implementation, maintaining robust communication with stakeholders, and harnessing the patience and determination of the facilitating organisation.

Background: Digital health records have emerged as vital tools for improving health care delivery and patient data management. Acknowledging the gaps in data recording by a paper-based register, the emergency obstetric and newborn care (EmONC) register used in the labour ward was digitised. In this study, we aimed to assess the implementation outcome of the digital register in selected public health care facilities in Bangladesh. Methods: Extensive collaboration with stakeholders facilitated the development of an android-based electronic register from the paper-based register in the labour rooms of the selected district and sub-district level public health facilities of Bangladesh. We conducted a study to assess the implementation outcome of introducing the digital EmONC register in the labour ward. Results: The digital register demonstrated high usability with a score of 83.7 according to the system usability scale, and health care providers found it highly acceptable, with an average score exceeding 95% using the technology acceptance model. The adoption rate reached an impressive 98% (95% confidence interval (CI) = 98-99), and fidelity stood at 90% (95% CI = 88-91) in the digital register, encompassing more than 80% of data elements. Notably, fidelity increased significantly over the implementation period of six months. The digital system proved a high utility rate of 89% (95% CI = 88-91), and all outcome variables exceeded the predefined benchmark. Conclusions: The implementation outcome assessment underscores the potential of the digital register to enhance maternal and newborn health care in Bangladesh. Its user-friendliness, improved data completeness, and high adoption rates indicate its capacity to streamline health care data management and improve the quality of care.

We performed whole-genome sequencing of four multidrug-resistant Enterococcus avium strains isolated from milk (4M1), feces (4F1 and 4F2), and farm soil (4S1) of mastitic dairy cows. The draft genomes of E. avium strains 4M1, 4F1, 4F2, and 4S1 were approximately 4.2 Mbp, with 39.1% GC content and 66.5× coverage.

This comprehensive review examines the immense capacity of nanowires, nanostructures characterized by unbounded dimensions, to profoundly transform the field of biomedicine. Nanowires, which are created by combining several materials using techniques such as electrospinning and vapor deposition, possess distinct mechanical, optical, and electrical properties. As a result, they are well-suited for use in nanoscale electronic devices, drug delivery systems, chemical sensors, and other applications. The utilization of techniques such as the vapor-liquid-solid (VLS) approach and template-assisted approaches enables the achievement of precision in synthesis. This precision allows for the customization of characteristics, which in turn enables the capability of intracellular sensing and accurate drug administration. Nanowires exhibit potential in biomedical imaging, neural interfacing, and tissue engineering, despite obstacles related to biocompatibility and scalable manufacturing. They possess multifunctional capabilities that have the potential to greatly influence the intersection of nanotechnology and healthcare. Surmounting present obstacles has the potential to unleash the complete capabilities of nanowires, leading to significant improvements in diagnostics, biosensing, regenerative medicine, and next-generation point-of-care medicines.

We recorded current-time (i-t) profiles for oxidizing ferrocyanide (FCN) while spherical yeast cells of radius (rc ≈ 2 µm) collided with disk ultramicroelectrodes (UMEs) of increasing radius (re ≈ 12-45 µm). Collision signals appear as minority steps and majority blips of decreased current overlayed on the i-t baseline when cells block ferrocyanide flux (JFCN). We assigned steps to adsorption events and blips to bouncing collisions or contactless passages. Yeast cells exhibit impact signals of long duration (Δt ≈ 15-40 s) likely due to sedimentation. We assume cells travel a threshold distance (T) to generate collision signals of duration Δt. Thus, T represents a distance from the UME surface, at which cell perturbations on JFCN blend in with the UME noise level. To determine T, we simulated the UME current, while placing the cell at increasing distal points from the UME surface until matching the bare UME current. T-Values at 90°, 45°, and 0° from the UME edge and normal to the center were determined to map out T-regions in different experimental conditions. We estimated average collision velocities using the formula T/Δt, and mimicked cells entering and leaving T-regions at the same angle. Despite such oversimplification, our analysis yields average velocities compatible with rigorous transport models and matches experimental current steps and blips. We propose that single-cells encode collision dynamics into i-t signals only when cells move inside the sensitive T-region, because outside, perturbations of JFCN fall within the noise level set by JFCN and rc/re (experimentally established). If true, this notion will enable selecting conditions to maximize sensitivity in stochastic blocking electrochemistry. We also exploited the long Δt recorded here for yeast cells, which was undetectable for the fast microbeads used in early pioneering work. Because Δt depends on transport, it provides another analytical parameter besides current for characterizing slow-moving cells like yeast.

Objectives: COVID-19 affects the quality of life (QoL) in a reverse way after recovery, which might be multiplied by the comorbid non-communicable diseases. This study explored the relationship between comorbidities and the QoL of people who recovered from COVID-19 in Bangladesh. Methods: The cross-sectional study was conducted among 3244 participants between June 2020 and November 2020 using a pre-tested questionnaire through over-the-phone interviews. The WHOQOL-BREF was used to explore the QoL among the study participants. A multivariable linear regression model was conducted to identify the effects of the number of comorbidities on QoL scores of all four domains. Results: Of 3244 patients who recovered from COVID-19, over one-third (39.4%) had one or more chronic diseases, such as hypertension, diabetes mellitus, ischemic heart disease, bronchial asthma/chronic obstructive pulmonary disease, chronic kidney disease, and cancer. Around 46.85% of the participants aged above 40 years presented with one to two chronic diseases, and 16.33% had three or more chronic diseases. Among all comorbidities, the participants with cancer and chronic kidney disease were found to have relatively lower scores in all four domains than other comorbidities. The lowest QoL scores were observed in the psychological domain. Those with three or more simultaneous chronic comorbidities had the lowest QoL score in all four domains: physical, psychological, social relationship, and environmental. Conclusions: Persons who recovered from COVID-19 with comorbidities undergo a lower QoL. Therefore, special attention is required to these vulnerable groups to ensure their smooth recuperation.

BACKGROUND: Disability-free life expectancy (DFLE) has been used to gain a better understanding of the population's quality of life. OBJECTIVES: The authors aimed to estimate age and sex-specific disability-free life expectancy (DFLE) for urban and rural areas of Bangladesh, as well as to investigate the differences in DFLE between males and females of urban and rural areas. METHODS: Data from the Bangladesh Sample Vital Statistics-2016 and the Bangladesh Household Income and Expenditure Survey (HIES)-2016 were used to calculate the disability-free life expectancy (DFLE) of urban and rural males and females in Bangladesh in 2016. The DFLE was calculated using the Sullivan method. RESULTS: With only a few exceptions, rural areas have higher mortality and disability rates than urban areas. For both males and females, statistically significant differences in DFLE were reported between urban and rural areas between the ages of birth and 39 years. In comparison to rural males and females, urban males and females had a longer life expectancy (LE), a longer disability-free life expectancy, and a higher share of life without disability. CONCLUSION: This study illuminates stark urban-rural disparities in LE and DFLE, especially among individuals aged < 1-39 years. Gender dynamics reveal longer life expectancy but shorter disability-free life expectancy for Bangladeshi women compared to men, emphasizing the need for targeted interventions to address these pronounced health inequalities.

Up-to-date network telemetry is the key enabler for resource optimization by capacity scaling, fault recovery, and network reconfiguration among other means. Reliable optical performance monitoring in general and, specifically, the monitoring of the spectral profile of WDM signals in fixed- and flex- grid architectures across the entire C-band, remains challenging. This article describes a two-stage spectrometer architecture amenable to integration on a single chip that can measure quantitatively the spectrum across the entire C-band with a resolution of ∼ 1.4 GHz. The first stage consists of a ring resonator with intra-ring phase shifter to provide a tuneable fine filter. The second stage makes use of an AWG subsystem and a novel processing algorithm to synthesize a tuneable coarse filter with a flat passband which isolates individual resonances of a multiplicity of ring resonances. The spectrometer is capable of scanning the entire C-band with high resolution using only one dynamic control. Due to its maturity and low loss, CMOS compatible Si3N4 is chosen for fabrication of the ring resonator and two cyclic AWGs. Complete spectrometer operation is demonstrated experimentally over a selected portion of the C-band. A novel virtual channel synthesis algorithm based on the weighted summation of the AWG output port powers relaxes the conventional AWG design requirement of a flat passband and sharp transition to stopband. The operation of the circuit is invariant to the optical path length between individual components and the algorithm corrects to some extent fabrication process variation impairments of the AWG channel spectra substantially improving robustness.

This study designed a two-stage, electrode-integrated septic tank-floating wetland system and assessed their pollutant removal performances under variable operational conditions. The two-stage system achieved mean organic, nitrogen, phosphorus, and coliform removal percentages of 99, 78, 99, and 97%, respectively, throughout the experimental run. The mean metals (chromium, cadmium, nickel, copper, zinc, lead, iron, and manganese) removal percentages ranged between 81 and 98%. Accumulated sludge, filler media, and the hanging root mass contributed to pollutant removals by supporting physicochemical and biological pathways. The mean effluent organic concentration and coliform number across the two-stage system were 20 mg/L and 1682 CFU/100 mL, respectively, during the closed-circuit protocol, which was beneath the open-circuit-based performance profiles, i.e., 32 mg/L and 2860 CFU/100 mL, respectively. Effluent organic, nitrogen, phosphorus, metals, and coliform number ranges across the two-stage system were 9-17 mg/L, 13-24 mg/L, 1-1.5 mg/L, 0.001-0.2 mg/L, and 1410-2270 CFU/100 mL, respectively during intermittent and continuous aeration periods. The air supply rate differences influenced pollutant removal depending on the associated removal mechanisms. The non-aeration phase produced higher effluent pollutant concentrations than the aeration periods-based profiles. The overall mean power density production of the septic tank ranged between 107 and 596 mW/m3; 110 and 355 mW/m3 with the floating wetland. The bioenergy production capacity of the septic tank was positively correlated to external air supply rates. This study demonstrates the potential application of the novel bioenergy-producing septic tank-floating wetland system for wastewater treatment in decentralized areas.

The rapid emergence of antimicrobial resistance (AMR) in Campylobacter has reinforced its status as a foodborne pathogen of significant public health concern. Resistant Campylobacter is typically transferred to humans via the consumption of contaminated animal products, particularly poultry. The genes associated with antimicrobial resistance in Campylobacter spp. are poorly understood. To address this knowledge gap, we conducted a prevalence survey of AMR Campylobacter across 84 chicken farms in two districts of Bangladesh. Pooled cloacal swabs were collected from chickens and underwent bacteriological testing for Campylobacter spp. with PCR confirmation. Antimicrobial susceptibility was tested against 14 antibiotics by disk diffusion method, and 12 resistance genes were screened in Campylobacter-positive isolates using multiplex PCR. A total of 34 (40.5%) farms were Campylobacter-positive of which 73.5% of isolates were resistant to at least 10 antibiotics. The antimicrobial susceptibility results indicate a high level of resistance against streptomycin (97.1%), clindamycin (97.1%), ampicillin (94.1%), tetracycline (94.1%), erythromycin (91.2%), ciprofloxacin (88.2%), nalidixic acid (85.3%), and imipenem (82.4%), and comparatively a low frequency of resistance to chloramphenicol (47.1%), ceftazidime (44.1%), and colistin (35.3%). Multidrug-resistant (MDR) and extensively drug-resistant Campylobacter were identified in 97.1%, and 50% of isolates, respectively. Ten resistance genes were identified including blaTEM (in 97.1% of isolates), strA-strB (85.9%), tetA (70.6%), tetB (32.4%), qnrS (23.5%), blaCTX-M-1 (20.6%), qnrB (20.6%), blaSHV (8.8%), aadB (5.9%), and qnrA (2.9%). Our findings demonstrate that resistance to ampicillin, tetracycline, and ceftazidime in Campylobacter isolates was significantly (p ≤ 0.05) associated with the presence of blaTEM, tetA, and blaSHV genes, respectively. The high rates of AMR in Campylobacter isolates from our study are not surprising given the liberal use of antimicrobials and incomplete biosecurity provisions on farms. Of particular concern are resistance rates to those classes of antibiotics that should be reserved for human use (azithromycin, ciprofloxacin, and colistin). AMR was more prevalent in chicken farms that used multiple antibiotics, engaged in prophylactic treatment of the birds, and improperly disposed of antibiotic packages. The high prevalence of MDR in chicken-derived Campylobacter isolates from the different regions of our study reinforces the need for more prudent use of antimicrobial compounds in Bangladeshi chicken farms.

Mangroves provide essential ecosystem services including coastal protection by acting as coastal greenbelts; however, human-driven anthropogenic activities altered their existence and ecosystem functions worldwide. In this study, the successive degradation of the second largest mangrove forest, Chakaria Sundarbans situated at the northern Bay of Bengal part of Bangladesh was assessed using remote sensing approaches. A total of five multi-temporal Landsat satellite imageries were collected and used to observe the land use land cover (LULC) changes over the time periods for the years 1972, 1990, 2000, 2010, and 2020. Further, the supervised classification technique with the help of support vector machine (SVM) algorithm in ArcGIS 10.8 was used to process images. Our results revealed a drastic change of Chakaria Sundarbans mangrove forest, that the images of 1972 were comprised of mudflat, waterbody, and mangroves, while the images of 1990, 2000, 2010, and 2020 were classified as waterbody, mangrove, saltpan, and shrimp farm. Most importantly, mangrove forest was the largest covering area a total of 64.2% in 1972, but gradually decreased to 12.7%, 6.4%, 1.9%, and 4.6% for the years 1990, 2000, 2010, and 2020, respectively. Interestingly, the rate of mangrove forest area degradation was similar to the net increase of saltpan and shrimp farms. The kappa coefficients of classified images were 0.83, 0.87, 0.80, 0.87, and 0.91 with the overall accuracy of 88.9%, 90%, 85%, 90%, and 93.3% for the years 1972, 1990, 2000, 2010, and 2020, respectively. By analyzing normalized difference vegetation index (NDVI), soil adjusted vegetation index (SAVI), and transformed difference vegetation index (TDVI), our results validated that green vegetated area was decreased alarmingly with time in this study area. This destruction was mainly related to active human-driven anthropogenic activities, particularly creating embankments for fish farms or salt productions, and cutting for collection of wood as well. Together all, our results provide clear evidence of active anthropogenic stress on coastal ecosystem health by altering mangrove forest to saltpan and shrimp farm saying goodbye to the second largest mangrove forest in one of the coastal areas of the Bay of Bengal, Bangladesh.

While global attention has been primarily focused on the occurrence and persistence of microplastics (MP) in urban lakes, relatively little attention has been paid to the problem of MP pollution in rural recreational lakes. This pioneering study aims to shed light on MP size, composition, abundance, spatial distribution, and contributing factors in a rural recreational lake, 'Nikli Lake' in Kishoreganj, Bangladesh. Using density separation, MPs were extracted from 30 water and 30 sediment samples taken from ten different locations in the lake. Subsequent characterization was carried out using a combination of techniques, including a stereomicroscope, Fourier transform infrared spectroscopy (FTIR) and field emission scanning electron microscopy (FE-SEM). The results showed a significant prevalence of MPs in all samples, with an average amount of 109.667 ± 10.892 pieces/kg3 (dw) in the sediment and 98.167 ± 12.849 pieces/m3 in the water. Small MPs (<0.5 mm), fragments and transparent colored particles formed the majority, accounting for 80.2%, 64.5% and 55.3% in water and 78.9%, 66.4% and 64.3% in sediment, respectively. In line with global trends, polypropylene (PP) (53%) and polyethylene (PE) (43%) emerged as the predominant polymers within the MPs. MP contents in water and sediment showed positive correlations with outflow, while they correlated negatively with inflow and lake depth (p > 0.05). Local activities such as the discharge of domestic sewage, fishing waste and agricultural runoff significantly influence the distribution of polypropylene. Assessment of pollution factor, pollution risk index and pollution load index values at the sampling sites confirmed the presence of MPs, with values above 1. This study is a baseline database that provides a comprehensive understanding of MP pollution in the freshwater ecosystem of Bangladesh, particularly in a rural recreational lake. A crucial next step is to explore ecotoxicological mechanisms, legislative measures and future research challenges triggered by MP pollution.

We present polarization-free Bragg filters having subwavelength gratings (SWGs) in the lateral cladding region. This Bragg design expands modal fields toward upper cladding, resulting in enhanced light interaction with sensing analytes. Two device configurations are proposed and examined, one with index-matched coupling between transverse electric (TE) and transverse magnetic (TM) modes and the other one with hybrid-mode (HM) coupling. Both configurations introduce a strong coupling between two orthogonal modes (either TE-TM or HM1-HM2) and rotate the polarization of the input wave through Bragg reflection. The arrangements of SWGs help to achieve two configurations with different orthogonal modes, while expanding modal profiles toward the upper cladding region. Our proposed SWG-assisted Bragg gratings with polarization independency eliminate the need for a polarization controller and effectively tailor the modal properties, enhancing the potential of integrated photonic sensing applications.

Due to its harmful effects on ecosystems and human health, microplastic (MP) pollution has become a significant environmental problem on a global scale. Although MPs' pollution path and toxic effects on marine habitats have been examined worldwide, the studies are limited to the rare biodiversity estuary region of Hatiya Island from the northern Bay of Bengal. This study aimed to investigate the MP pollution path and its influencing factors in estuarine sediments and water in rare biodiversity Hatiya Island in the northern Bay of Bengal. Sixty water and sediment samples were collected from 10 sampling sites on the Island and analyzed for MPs. The abundance of MPs in sediment ranged from 67 to 143 pieces/kg, while the abundance in water ranged from 24.34 to 59 pieces/m3. The average concentrations of MPs in sediment and water were 110.90 ± 20.62 pieces/kg and 38.77 ± 10.09 pieces/m3, respectively. Most identified MPs from sediment samples were transparent (51%), while about 54.1% of the identified MPs from water samples were colored. The fragment was the most common form of MP in both compartments, with a value of 64.6% in sediment samples and 60.6% in water samples. In sediment and water samples, almost 74% and 80% of MP were <0.5 mm, respectively. Polypropylene (PP) was the most abundant polymer type, accounting for 51% of all identified polymers. The contamination factor, pollution load index, polymer risk score, and pollution risk score values indicated that the study area was moderately polluted with MPs. The spatial distribution patterns and hotspots of MPs echoed profound human pathways. Based on the results, sustainable management strategies and intervention measures were proposed to reduce the pollution level in the ecologically diverse area. This study provides important insights into evaluating estuary ecosystem susceptibility and mitigation policies against persistent MP issues.

IMPORTANCE: When administered for seven consecutive days shortly after birth, the probiotic bacterium Lactiplantibacillus plantarum ATCC 202195 plus fructooligosaccharide (FOS) was reported to reduce sepsis and lower respiratory tract infection events during early infancy in a randomized trial in India. Since probiotic effects are often strain specific, strain-level detection and quantification by routine molecular methods enables the monitoring of safety outcomes, such as probiotic-associated bacteremia, and allows for the quality of probiotic interventions to be assessed and monitored (i.e., verify strain identity and enumerate). Despite the potential clinical applications of L. plantarum ATCC 202195, an assay to detect and quantify this strain has not previously been described. Herein, we report the design of primer and probe sequences to detect L. plantarum ATCC 202195 and the development and optimization of a real-time PCR assay to detect and quantify the strain with high specificity and high sensitivity.

Microplastic (MP) pollution has evolved into a significant worldwide environmental concern due to its widespread sources, enduring presence, and adverse effects on lentic ecosystems and human well-being. The growing awareness of the hidden threat posed by MPs in lentic ecosystems has emphasized the need for more in-depth research. Unlike marine environments, there remain unanswered questions about MP hotspots, ecotoxic effects, transport mechanisms, and fragmentation in lentic ecosystems. The introduction of MPs represents a novel threat to long-term environmental health, posing unresolved challenges for sustainable management. While MP pollution in lentic ecosystems has garnered global attention due to its ecotoxicity, our understanding of MP hotspots in lakes from an Asian perspective remains limited. Hence, the aim of this review is to provide a comprehensive analysis of MP hotspots, morphological attributes, ecotoxic impacts, sustainable solutions, and future challenges across Asia. The review summarizes the methods employed in previous studies and the techniques for sampling and analyzing microplastics in lake water and sediment. Notably, most studies concerning lake microplastics tend to follow the order of China > India > Pakistan > Nepal > Turkey > Bangladesh. Additionally, this review critically addresses the analysis of microplastics in lake water and sediment, shedding light on the prevalent net-based sampling methods. Ultimately, this study emphasizes the existing research gaps and suggests new research directions, taking into account recent advancements in the study of microplastics in lentic environments. In conclusion, the review advocates for sustainable interventions to mitigate MP pollution in the future, highlighting the presence of MPs in Asian lakes, water, and sediment, and their potential ecotoxicological repercussions on both the environment and human health.

Waste and sanitation workers in South-Asian countries are vulnerable to injuries and diseases, including COVID-19. In Bangladesh, an intervention was implemented during COVID-19 to lower these workers' occupational health risks through training and PPE distribution. We assessed how the intervention affected their occupational health behaviors using a randomized cluster trial in 10 Bangladeshi cities, including seven intervention and three control areas. We conducted 499 surveys (Control-152, Intervention-347) and 47 structured observations (Control:15, Intervention:32) at baseline and 499 surveys (Control:150, Intervention:349) and 50 structured observations (Control:15, Intervention:35) at endline. To evaluate the impact of intervention at the endline, we used the difference in difference (DID) method. Compared to control, workers from intervention areas were more likely to have increased knowledge of using/maintaining PPEs (adjusted DID: 21%, CI: 8, 33), major COVID-19 transmission causes (adjusted DID: 27%, CI: 14, 40), and preventive measures (adjusted DID: 12%, CI: -0.6, 25), and improved attitude about using PPEs (adjusted DID: 36%, CI: 22, 49), washing PPEs (adjusted DID: 20%, CI: 8, 32). The intervention was more likely to improve workers' self-reported practice of taking adequate precautions after getting back from work (adjusted DID: 37%, CI: 27, 47) and changing/cleaning masks every day (adjusted DID: 47%, CI: 0.03, 94), and observed practices of maintaining coughing etiquette (DID: 20%, CI: 0.2, 40) at workplace and handwashing before wearing PPEs (DID: 27%, CI: 2, 52), after finishing work (DID: 31%, CI: -7, 69) & during work intervals (DID: 30%, CI: -33, 93). There was not much improvement in observed practices of mask use (DID: 1%, CI: -40, 42), handwashing before meals (DID: 2%, CI: -61, 65), and after toilet (DID: 7%, CI: -41, 55). This intervention has improved the knowledge, attitude and practice of the workers about critical occupational risk mitigation, which may be replicated in similar settings. Future interventions need to address occupational health-related injuries and health complications, introduce regular health checkups/insurance for the workers, create a balance between the quality and comfort of the PPEs and ensure a mechanism to ensure a regular supply of PPEs.

Eleven trace metals (Cd, Cr, Fe, Mn, Cu, Ni, Co, Zn, As, Pb, and Ag) in sediments of Bangladesh's ship breaking area were measured by an atomic absorption spectrometer to determine origin, contamination extent, spatial distributions, and associated ecological and human health hazards. This study found considerable quantities of Pb, Cd, Mn, Zn, and Cu when compared with standards and high levels of Pb, Cd, Zn, Cu, As, and Ag contamination according to pollution evaluation indices. Different indices indicate most of the sampling sites were highly polluted. However, spatial distribution maps indicate that trace metals were predominantly deposited in the northern and southern region. The ecological risk index revealed that Cd has the highest while Pb and As had moderate risk. Based on the health index values, Zn for both adults and children were higher than the safe limit while Mn, Pb, Cr, As, Fe, Cu, Ni, and Co for children were close to the threshold. The mean total carcinogenic risk values of Cr, As, and Ni for children and Ni for adults exceeded the permissible threshold. The cancer risk possibilities were further assessed using Monte Carlo simulation. Most trace metals have anthropogenic origins, which were attributed to ship breaking activities.

INTRODUCTION: Tobacco use, in both smoking and smokeless forms, is highly prevalent among South Asian adults. The aims of the study were twofold: (1) describe patterns of SLT and combustible tobacco product use in four South Asian countries stratified by country and sex, and (2) assess the relationships between SLT and smoking intensity, smoking quit attempts, and smoking cessation among South Asian men. METHODS: Data were obtained from South Asia Biobank Study, collected between 2018 and 2022 from 148,944 men and women aged 18 years and above, living in Bangladesh, India, Pakistan, or Sri Lanka. Mixed effects multivariable logistic and linear regression were used to quantify the associations of SLT use with quit attempt, cessation, and intensity. RESULTS: Among the four South Asian countries, Bangladesh has the highest rates of current smoking (39.9% for male, 0.4% for female) and current SLT use (24.7% for male and 23.4% for female). Among male adults, ever SLT use was associated with a higher odds of smoking cessation in Bangladesh (OR, 2.88; 95% CI, 2.65, 3.13), India (OR, 2.02; 95% CI, 1.63, 2.50), and Sri Lanka (OR, 1.36; 95% CI, 1.14, 1.62). Ever SLT use and current SLT use was associated with lower smoking intensity in all countries. CONCLUSIONS: In this large population-based study of South Asian adults, rates of smoking and SLT use vary widely by country and gender. Men who use SLT products are more likely to abstain from smoking compared with those who do not.

INTRODUCTION: The vaccination against coronavirus disease 2019 (COVID-19) has been identified as a promising strategy to reduce the severity of the pandemic. Despite the safe and effective COVID-19 vaccines, bringing socioeconomically disadvantaged people under vaccination coverage has been challenging for developing countries like Bangladesh. Therefore, this study explored the determinants of vaccine acceptance among urban slum residents of Bangladesh using the Health Belief Model (HBM) and Theory of Planned Behavior (TPB). METHODS: A face-to-face survey of 400 urban slum dwellers in two large cities in Bangladesh was conducted between July 5 to August 5, 2021. The questionnaire included vaccine acceptance, socio-demographics, health-related characteristics, trust in health authorities, reasons for vaccine hesitancy, and dimensions of HBM and TPB frameworks. Hierarchical logistic regression was performed to evaluate the association between these characteristics and vaccination acceptance. RESULTS: Around 82% (n = 327) of respondents were willing to accept the COVID-19 vaccine. In a fully adjusted model, respondents with secondary level education had higher intention (OR = 46.93, 95%CI = 1.21-1807.90, p < 0. 05) to accept COVID-19 vaccine. Respondents with bad (OR = 0.11, 95%CI = 0.01-0.35, p<0.05) or very bad (OR = 0.01, 95%CI = 0.01-0.35, p<0.05) health conditions were less interested in the COVID-19 vaccination. In regard to HBM dimensions, greater perceived susceptibility (OR = 1.75, 95% CI = 1.12-2.75, p < 0.05), and perceived benefits (OR = 3.28, 95% CI = 1.17-6.00, p < 0.001) were associated with a greater willingness to get vaccinated. In regard to TPB, higher self-efficacy in preventing illness without the vaccine increased the desire to get vaccinated (OR = 1.55, 95% CI = 1.02-2.37, p < 0.05). Fear of unknown side effects, religious beliefs, contraindications to vaccination, and insufficient information on the vaccine were the main reasons for vaccine hesitancy. CONCLUSIONS: These findings offer valuable insights for policymakers in Bangladesh to design targeted interventions that address vaccine hesitancy and increase vaccination acceptability among socially disadvantaged individuals in urban areas. Strategies should focus on providing accurate and accessible information about the vaccine, communicating its positive impact effectively, engaging with religious leaders to address misconceptions, and tailoring vaccination campaigns to meet the unique needs of different demographic groups.
