OBJECTIVE: The aim of this study is to evaluate the efficacy and safety of oxidized regenerated cellulose (ORC) in patients who underwent coronary artery bypass grafting (CABG) surgery and to compare the results of patients in whom ORC was used or not used for control of bleeding. METHOD: Pre-, intra-, and post-operative demographic and medical parameters of the patients in whom ORC was used or not used were compared. Quantitative data were analyzed with mean and standard deviation. Group differences were assessed with the Mann-Whitney U test. RESULTS: It was found that the duration of surgery, average numbers of erythrocyte and fresh frozen plasma (FFP) transfusions during surgery, average post-operative FFP transfusion count, duration of intensive care unit stay, and chest tube removal times were lower in the ORC group compared to the control group, and all these differences were statistically significant (p < 0.05 for all of these parameters). CONCLUSIONS: The study successfully demonstrated the effective and safe use of topical ORC in controlling bleeding and preventing oozing during CABG surgeries.
OBJETIVO: Evaluar la eficacia y la seguridad de la celulosa regenerada oxidada (CRO) en pacientes sometidos a cirugía de injerto de derivación de arteria coronaria y comparar los resultados de los pacientes en los que se utilizó o no la CRO para el control del sangrado. MÉTODO: Se compararon los parámetros demográficos y médicos pre-, intra- y posoperatorios de los pacientes en los que se utilizó o no CRO. Los datos cuantitativos se analizaron con media y desviación estándar. Las diferencias grupales se evaluaron con la prueba U de Mann Whitney. RESULTADOS: Se encontró que la duración de la cirugía, el número promedio de transfusiones de eritrocitos y de plasma fresco congelado durante la cirugía, el recuento promedio de transfusiones de plasma fresco congelado posoperatorias, la duración de la estadía en la unidad de cuidados intensivos y los tiempos hasta la extracción del tubo torácico fueron menores en el grupo de CRO en comparación con el grupo control, y todas estas diferencias fueron estadísticamente significativas (p < 0.05). CONCLUSIONES: El estudio demostró con éxito el uso eficaz y seguro de la CRO tópica para controlar el sangrado y prevenir la supuración durante las cirugías de derivación de arteria coronaria.

OBJECTIVES: To explore prevalence, characteristics and risk factors of COVID-19 breakthrough infections (BIs) in idiopathic inflammatory myopathies (IIM) using data from the COVID-19 Vaccination in Autoimmune Diseases (COVAD) study. METHODS: A validated patient self-reporting e-survey was circulated by the COVAD study group to collect data on COVID-19 infection and vaccination in 2022. BIs were defined as COVID-19 occurring ≥14 days after 2 vaccine doses. We compared BIs characteristics and severity among IIMs, other autoimmune rheumatic and non-rheumatic diseases (AIRD, nrAID), and healthy controls (HC). Multivariable Cox regression models assessed the risk factors for BI, severe BI and hospitalisations among IIMs. RESULTS: Among 9449 included response, BIs occurred in 1447 (15.3%) respondents, median age 44 years (IQR 21), 77.4% female, and 182 BIs (12.9%) occurred among 1406 IIMs. Multivariable Cox regression among IIMs showed age as a protective factor for BIs [Hazard Ratio (HR)=0.98, 95%CI = 0.97-0.99], hydroxychloroquine and sulfasalazine use were risk factors (HR = 1.81, 95%CI = 1.24-2.64, and HR = 3.79, 95%CI = 1.69-8.42, respectively). Glucocorticoid use was a risk factor for severe BI (HR = 3.61, 95%CI = 1.09-11.8). Non-White ethnicity (HR = 2.61, 95%CI = 1.03-6.59) was a risk factor for hospitalisation. Compared with other groups, patients with IIMs required more supplemental oxygen therapy (IIM = 6.0% vs AIRD = 1.8%, nrAID = 2.2%, and HC = 0.9%), intensive care unit admission (IIM = 2.2% vs AIRD = 0.6%, nrAID, and HC = 0%), advanced treatment with antiviral or monoclonal antibodies (IIM = 34.1% vs AIRD = 25.8%, nrAID = 14.6%, and HC = 12.8%), and had more hospitalisation (IIM = 7.7% vs AIRD = 4.6%, nrAID = 1.1%, and HC = 1.5%). CONCLUSION: Patients with IIMs are susceptible to severe COVID-19 BI. Age and immunosuppressive treatments were related to the risk of BIs.

The objective of the present study was to investigate the prevalence of sexual dysfunction and its associated factors among women with type 2 diabetes mellitus (T2DM). Subjects and methods: This cross-sectional comparative study comprises 150 women with diabetes and 100 healthy women without diabetes who visited the endocrinology outpatient department of Mymensingh Medical College Hospital (MMCH). The data were collected from July to December 2019. Sexual dysfunction was assessed by the 19-item Female Sexual Function Index (FSFI). Informed consent was obtained before participation. Collected data were analysed by SPSS 26. Results: More women with diabetes than control subjects reported sexual dysfunction (79% vs. 72%; p = 0.864). The global FSFI score was lower among the diabetes patients than among the healthy controls (20.8 ± 7.2 vs. 23.7 ± 4.8; p < 0.001). Patients with T2DM scored significantly lower in the domains of desire (p = 0.04), lubrication (p = 0.01), orgasm (p = 0.01), and satisfaction (p < 0.001), but not the domain of arousal (p = 0.09). A prolonged duration of diabetes was the primary contributor to orgasm problems (adjusted odds ratio, aOR 1.3, 95% CI 1.1-1.7) and painful intercourse (aOR 1.2, 95% CI 1.1- 1.5). Conclusion: Sexual problems are frequent in women with diabetes. Inclusion of sexual health in comprehensive diabetes management is crucial to address this problem as well as to improve the quality of life of female diabetes patients.

Donor-derived Strongyloides stercoralis infections in transplant recipients are a rare but recognized complication. In this case series, we report donor-derived allograft transmission of Strongyloides in three solid organ transplant recipients. Following detection of infection in heart and kidney-pancreas recipients at two different transplant centers, a third recipient from the same donor was identified and diagnosed. S. stercoralis larvae were detected in duodenal aspirates, bronchial washings, cerebrospinal fluid, urine and stool specimens. Treatment with ivermectin and albendazole was successful in two of the three patients identified. The Centers for Disease Control and Prevention was contacted and performed an epidemiologic investigation. Donor serology was strongly positive for S. stercoralis antibodies on retrospective testing while all pretransplant recipient serum was negative. There should be a high index of suspicion for parasitic infection in transplant recipients and donors from endemic regions of the world. This case series underscores the need for expanded transplant screening protocols for Strongyloides. Positive serologic or stool tests should prompt early treatment or prophylaxis in donors and recipients as well as timely notification of organ procurement organizations and transplant centers.

OBJETIVO: Em situações estressantes, as pessoas geralmente usam os dedos para retirar corpos estranhos de faringe da boca. Este artigo relata três casos de óbito de bebês após o uso dessa técnica. MÉTODOS: Um total de 26 casos de engasgamento causado por aspiração de corpos estranhos nas vias respiratórias superiores e inferiores envolvendo crianças menores de 11 anos submetidas a autopsia no Departamento de Medicina Forense da Universidade da Jordânia entre 1996 e 2006 foram revisados. RESULTADOS: O uso de busca às cegas com os dedos para retirar corpos estranhos de faringe foi relatado em três bebês durante o choro. Todas as crianças eram menores de 1 ano e haviam se engasgado com grãos-de-bico, uma bola de gude e um lápis curto. A diversidade, o tamanho, a forma, e a maciez da superfície são as principais características que fazem com que esses corpos estranhos sejam mais difíceis de ser removidos com os dedos e facilitam sua entrada via respiratória. CONCLUSÃO: Esses casos mostraram que a busca às cegas com os dedos na boca de bebês durante o choro não é somente uma manobra perigosa, mas também pode ser fatal.

This article reports on three cases of death of infants following the use of this technique. METHODS: A total of 26 cases of choking caused by foreign bodies aspiration in the upper and lower respiratory passages involving children younger than 11 years of age autopsied at the Forensic Department of University of Jordânia between 1996 and 2006 were reviewed. RESULTS: Blind finger sweep to remove pharyngeal foreign bodies were reported in three crying infants. All of these cases were younger than 1 year of age and choked on a chickpea, a marble and a short pencil. Diversity, size, shape and smoothness of the surface are the main characteristics that render the foreign bodies less easily caught by fingers and make them easily enter the respiratory passage. CONCLUSION: These cases showed that blind finger sweep in crying infants is not only dangerous but can be a fatal maneuver.

In tropical countries like Bangladesh, persons are bitten by snakes every day and a considerable number of patients die en route to the hospital. An event of consecutive neurotoxic bites on two men by a single snake was observed in the Snake Bite Study Clinic (SBSC) of the Chittagong Medical College Hospital (CMCH). Two brothers, working in their semi-pucca restaurant, were successively bitten by the same cobra on their lower limbs. Within an hour, they were taken to the CMCH. Few minutes after admission, both developed symptoms of neurotoxicity: ptosis, nasal voice, dysphagia, broken neck sign, etc. They received polyvalent antivenom (Haffkine Bio-Pharmaceuticals Company, India) and other auxiliary treatment immediately. Within few hours, neurotoxic features were completely absent. Later, the snake was captured in the restaurant kitchen and identified as monocellate cobra (Naja kauthia) by the SBSC. The elder brother developed significant antivenom reactions and both presented necrosis and ulceration at the bite sites. In these cases, immediate arrival to the hospital and early administration of antivenom resulted in successful recoveries.(AU)
