RESUMO

Aim: High expression of lytic enzymes and cytokines is related to cell proliferation in Otitis Media Chronic Suppurative (CSOM) with cholesteatoma. In addition, the process of inflammation healing and maintenance of homeostatic conditions requires Reactive Oxygen Species (ROS), which can cause significant damage to cells. To address this issue, secondary antioxidants such as Vitamins A and E are used to inhibit and neutralize the occurrence of oxidation reactions. These vitamins complement each other, with vitamin A working effectively at low concentrations of oxygen, while vitamin E functions in the opposite manner. Purpose: This study aims to investigate the roles of vitamin A and E levels in CSOM patients with Cholesteatoma. Patients and Methods: The study was conducted between July and December 2020, and the method used was an analytical observational approach with a case-control design. The sample population comprised 60 CSOM patients divided into 2 groups. These included those with and without cholesteatoma. Results: The results showed that there was no significant difference between these two groups in terms of the impact of vitamin A and E levels (respectively, p = 0.626, p = 0.864). Conclusion: Considering these results, it was discovered that vitamins A and E levels do not influence the occurrence of CSOM with or without cholesteatoma.

BACKGROUND AND OBJECTIVES: Indonesians encounter several barriers to regular functional hearing assessment. Hearing loss screening is only provided by tertiary-care hospitals that are not reachable by people in remote regions. This study aimed to develop a website-based hearing screening application that is accessible and inexpensive. Subjects and. METHODS: This comparative study was conducted between July and August 2022 in the Otolaryngology Clinic of Muhammadiyah Bandung Hospital with noise levels below 50 dB. The hearing screening was conducted using a website-based application (www.Screenout.id) and audiogram as the gold standard method. On ScreenOut, patients heard sounds with frequencies at 500, 1,000, 2,000, 4,000, and 8,000 Hz and sound intensity of each frequency at 35, 55, and 75 dB using earphones. RESULTS: A total of 133 participants were enrolled in our study. ScreenOut showed high sensitivity, specificity, accuracy, positive predictive value, and negative predictive value (90.9%, 98.9%, 93.6%, 99.4%, and 84.8%, respectively). Regarding hearing threshold, a very strong correlation was found between ScreenOut and audiogram, ranging between r=0.843 and r=0.899. Aside from that, there was no significant difference in hearing threshold values between ScreenOut and audiogram. CONCLUSIONS: Many advantages of the ScreenOut were found, including low-cost, accessibility, and easy-to-use interface, making it favorably used in low-middle-income countries such as Indonesia.

Background: Presbycusis is a gradual hearing loss caused by the ageing process. This is a chronic condition that affects the elderly population, and sensorineural progressive bilateral symmetry occurs with predominantly high-frequency hearing loss. The ability to discriminate speech decreases; hence, most of the affected patients have conversation problems, especially in noisy environments.This situation is a serious problem among elderly individuals. Social isolation, depression, and paranoia can be related to presbycusis. Objective: The aim of this study was to investigate GPx and the GSH:GSSG ratio as risk factors for presbycusis. Methods: A case-control study was conducted to determine the role of GPx activity with the GSH:GSSG ratio as a presbycusis risk factor in 60 subjects aged 55 to 75 years old during the period of August 2012 - April 2014. All of the subjects passed an ENT examination, pure tone audiometry, and tympanometry. The activity of GPx was measured with the Paglia and Valentine method, and the GSH:GSSG ratio was measured by the calorimetric method. Results: The activity of GPx and the GSH:GSSG ratio were significantly different between the groups (p<0.05), and the odds ratio for high GPx with a low GSH:GSSG ratio was 135 (CI 95%: 5.17-20,028.88). Conclusion: High GPx activity with a low GSH:GSSG ratio is a risk factor for presbycusis.
