We report on a case of a renal tumour that showed a solid volume in CT analysis, but could only be diagnosed and evaluated precisely using the selective i.a. DSA method thus enabling a decisive statement on operative planning.

Fifty scintigrams were performed on 45 patients using 99mTc-nanocolloid in order to test its affinity for inflammatory lesions; in these patients there was clinical and/or radiological suspicion of an inflammatory process. There was good correlation between the degree of uptake and the activity of the inflammatory process. Specificity is limited because of uptake in certain tumours (eg. sarcomas of the extremities), but difficulties in differential diagnosis are not expected to be serious. Judging by our experience, 99mTc-nanocolloid is a promising substance for the scintigraphic demonstration of inflammatory foci. The advantages of this industrially prepared tracer over In-leucocytes and Ga-citrate are discussed.

CT permits demonstration of thrombus in the iliac veins and inferior vena cava by injecting contrast material into vein in the dorsum of the foot. This technique, called CT cavography in this paper, has advantages over conventional venography or DSA and also over sonography. It is indicated following phlebographic demonstration of recent pelvic thrombosis and for demonstrating tumour thrombi in patients with carcinomas of the kidney.

The aetiology of a. hepatica aneurysm can be manifold. Among all the complications, rupture of the vessel is the most dangerous one. We report on a case which was noticed clinically due to an angina abdominalis. In this paper we outline the symptoms, all diagnostic steps and the therapeutic approach.

Case studies show that acute pancreatitis occurring independently or combined with a preceding abuse of alcohol may be the cause of fatty infiltration of the liver. These fat areas can evolve in a very short time and provoke in the case of focal incidence diagnostic problems of differentiation against abscesses or metastases. Due to this fact and because of the rapid reversibility of the fatty infiltration under therapy, the safest method to clarify the situation consists of short-term CT controls.

Tumors of the small intestine are rare as compared to other tumors of the gastrointestinal tract; clinical symptoms are unrevealing for long time, thwarting an early diagnosis. Detection of these tumors by x-ray examination is successful often only after several attempts. Case reports are given of 7 patients and difficulties of making a diagnosis are discussed. Radiological methods are described; differential diagnosis of x-ray findings is expecially difficult. A histological diagnosis by endoscopic or intraoperative biopsy should always be tried. Surgery is necessary in every case in order to restore normal passage.

A system for the documentation of radiographic diagnoses is described, based on the "documentation method for radiographic diagnosis" (Formmhold et al 1972) and on the diagnostic information system of the medical faculty of the University of Tübingen. The diagnostic indeces are dictated in relation to the findings and the diagnosis and fed on punched cards into a computer (IBM 1800). Various programmes provide patient-orientated diagnoses for clinical routine, as well as diagnoses-orientated lists of patients for demonstrations, teaching or research.

Dilatation of the renal pelvis, ureters and bladder appeared in two brothers with myotonic dystrophy (aged 26 and 29 years), probably as a result of involvement of the smooth muscles of these organs. Histochemical differentiation of skeletal muscle fibres (main fibre types I and II, after Engel) indicated a predominant size reduction in type I fibres on demonstrating myofibrillary ATPase. In addition, demonstration of this enzyme also revealed a marked shift of the fibre type proportions in favour of type I. Isolated reduction in gammaG-globulins was also demonstrated. A defect of cellular and humoral response was excluded.

Using a correct retrosternal technique, adequate insufflation of negative contrast material and good radiography, a pneumo-mediastinum is the method of choice for the investigation of retrosternal tumours with respect to their position, size and shape. By means of experiments on cadavers, a suitable needle was developed. The method of air insufflation and radiographic positioning are described. The results of our examinations are detailed and the method is discussed.

For the diagnosis of supra-renal tumours or tumours of supra-renal rests, it is necessary to employ all available radiological methods as well as the clinical data. Two cases are described in which a supra-renal tumour had been "masked" and found at operation. In the first case a cyst in the tail of the pancreas had been considered, in the second, a tumour of the renal hilum or ureter had been diagnosed. It is concluded that in the differential diagnosis of a cyst of the tail of the pancreas, supra-renal tumours must be included. Further, vascular lesions in the vicinity of the kidney associated with normal renal vasculature may be due to tumours of supra-renal origin.
