RESUMO

A 90-years-old man comes to the emergency department with diffuse abdominal pain and fever three days. On physical examination, the abdomen is soft and depressible with air-fluid sound present and bilateral reductible inguinal hernias. A mass is palpated at the level of the roght hypochondrium with a positive Murphy´s sign whitout signs of peritoneal irritation. An abdominal CT with intravenous contrast is requested due to the suspicion of acute abdominal pathology, observing cholelithiasis and a markedly distended gallbladder with torsion. (AU)

Anemia in salmonid aquaculture is a recognized blood disorder resulting from the reduction of hemoglobin concentration and/or erythrocyte count. Because of sub-optimal oxygen supply to the tissues, as a negative impact of anemia fish will experience reduced growth and poor health. This health challenge may be linked with several factors including anthropogenic changes in the marine environment, infectious etiology (viral, bacterial, and parasitic), nutritional deficiencies, or hemorrhaging. From the mid-late summer of 2017 to 2019, Scottish salmon farming companies began to report the occurrence of anemic events in open-net marine sites. At that time, the industry had little understanding of the pathogenesis and possible mechanisms of anemia and limited the ability to formulate effective mitigation strategies. Clinical examination of fish raised suspicion of anemia and this was confirmed by generating a packed cell volume value by centrifugation of a microhematocrit tube of whole anticoagulated blood. Company health team members, including vets and biologists, reported discoloration of gills and local hemorrhages. This paper reviews various commercially significant cases and lesser-known cases of anemia in cultured salmonid species induced by various biological factors. The current methods available to assess hematology are addressed and some future methods that could be adopted in modern day fish farming are identified. An account of the most recent anemic event in Scottish farmed Atlantic salmon (Salmo salar) is presented and discussed as a case study from information provided by two major Scottish salmon producers. The percent of total marine sites (n = 80) included in this case study, that reported with suspected or clinical anemia covering the period mid-late summer 2017 to 2019, was between 1 and 13%. The findings from this case study suggest that anemia experienced in most cases was regenerative and most likely linked to blood loss from the gills.

34-year-old woman, 16 weeks pregnant, with a personal history of cervical conization and previous cesarean section.Consultation for discomfort in the right groin region and signs of inflammation in the area, without a clear tumor. She refers that inthe first pregnancy she presented the same symptoms. In the intergestational period she has not noticed these symptoms. On physicalexamination, the 16-week pregnant abdomen did not reveal clear signs of hernia, although there was slight pain on palpation. It wasdecided to request a pelvic ultrasound to rule out the presence of hernia or other soft tissue lesions in the groin region. (AU)


Mujer de 34 años, gestante de 16 semanas, con antecedentes personales de conización cervical y cesárea anterior. Consultapor molestias en región inguinal derecha y signos de inflamación en la zona, sin clara tumoración. Refiere que en el primerembarazo presentaba la misma sintomatología. En el período intergestacional no ha notado estos síntomas. A la exploraciónfísica se palpa útero, con altura correspondiente a unas 16 semanas de gestación, no se demuestran signos evidentes de hernia,aunque si leve dolor a la palpación. Se decide solicitar ecografía pélvica para descartar la presencia de hernia u otras lesiones de partes blandas en región inguinal. (AU)

INTRODUCTION: The ability of healthcare-associated infection pathogens to survive on environmental surfaces is well known. Disinfection is employed to reduce or remove these pathogens but disinfection failures still occur. One method with the potential to improve disinfection efficacy is whole-room disinfection with hydrogen peroxide (H2O2). AIM: To determine the influence of delivery system on the efficacy of low-concentration H2O2 on common healthcare-associated infection pathogens. METHODS: SanoStatic (electrostatic spray) was compared with SanoFog (fogging) in terms of performance for delivery of 5% H2O2 and trace silver ions for disinfection. The bacterial test challenges were vancomycin-resistant Enterobacterales (VRE), extended-spectrum ß-lactamase (ESBL)-producing Klebsiella pneumoniae (ESBLK), carbapenemase-producing Enterobacterales (CPE), meticillin-resistant Staphylococcus aureus (MRSA), Clostridium difficile spores, Bacillus atropheus and Geobacillus stearothermophilus commercial spore strips. FINDINGS: SanoFog and SanoStatic were effective when tested under the conditions of experimentation reported here. For VRE, ESBLK, CPE and MRSA, SanoFog and SanoStatic were comparable in performance. For C. difficile we concluded the following: SanoFog was most effective for disinfection of C. difficile spores when compared to SanoStatic. CONCLUSION: Whereas SanoFog and SanoStatic were effective against bacterial cells, the current practice of using SanoFog and SanoStatic together would be effective for disinfection of C. difficile spores based on investigations under the conditions of experimentation reported here. The spore strips results were not comparable to the results either for the vegetation cells (VRE, ESBLK, CPE, and MRSA) or for C. difficile spores.

Small RNA and chaperone proteins form synergistic duos that play pivotal roles in controlling gene expression in bacteria. This is the case for Hfq, a highly pleiotropic pretranslational modulator of general protein expression, which responds to harsh environmental conditions and influences fitness and virulence in a wide range of pathogenic Enterobacteria. Given this relevancy, we evaluated the presence and potential role of Hfq in the fish pathogen Piscirickettsia salmonis, a Gram-negative bacterium that threatens the sustainability of Chilean salmon production. Using bioinformatics tools were identified and characterized two variants of Hfq, which share the consensus RNA-binding domains and the active sites described functional Hfq other bacteria. Additionally, we demonstrated that hfq-1 and hfq-2 were transcriptionally active when growing in cell-free media and in infected susceptible fish cell line. Expression of both genes differed under different growth conditions and under stress, suggesting that their roles might be independent and different, depending on the bacterial physiological status. In conclusion, we demonstrate the existence of two different and functional ORF coding for the hfq marker in marine bacteria and a preliminary analysis indicating that these two novel proteins might have relevant roles in the biology and pathogenic potential of P. salmonis.

Introducción: En 2016, España cumple una década de presencia en Líbano dentro de la operación de Naciones Unidas (UNIFIL) United Nations Interim Force in Lebanon. El objetivo del presente estudio es analizar la evolución de la asistencia sanitaria prestada en dos contingentes españoles desplegados en esa zona de operaciones con una diferencia de 10 años. Material y métodos: Estudio transversal descriptivo retrospectivo realizado durante dos periodos (del 8 septiembre al 8 de noviembre de 2006 y del 3 de septiembre al 18 de noviembre de 2016). La población a estudio fueron los pacientes atendidos en el primer escalón sanitario durante los citados periodos. Resultados: Los datos del primer y segundo intervalo respectivamente fueron: efectivos desplegados 523/562; asistencias médicas realizadas 1168/1435; enfermedades digestivas 269/423; enfermedades otorrinolaringológicas 222/147; cirugía menor 187/75; enfermedad dermatológica 175/128; traumatología 152/413; enfermedad odontológica 58/128; enfermedad oftalmológica 53/16; otras 70/105; evacuaciones a ROLE 2 4/76 y evacuaciones a ROLE 4 0/0. Conclusión: Hay similitud entre las atenciones sanitarias realizadas en ambos periodos de tiempo, excepto en las enfermedades traumatológicas donde aparece un incremento de casi un 300% en el segundo intervalo de tiempo respecto al primero


Introduction: In 2016, Spain celebrates a decade of presence in Lebanon within the United Nations Interim Force in Lebanon (UNIFIL). The aim of the present study is to analyse the evolution of the health care provided in two Spanish contingents deployed in that area of operations, comparing two periods within ten years of elapsed time. Material and method: A cross-sectional retrospective study was carried out during two periods (from the 8th of September to the 8th of November of 2006 and from the 3rd of September to the18th of November of 2016). The study population was the patients treated in the first Medical Treatment Facility during those periods. Results: The data of the first and second time interval respectively were: deployed troops 523/562; medical assistance performed 1168/1435; Digestive disease 269/423; Otorhinolaryngological diseases 222/147; Minor surgery 187/75; Dermatological disease 175/128; Traumatology 152/413; Dental disease 58/128; Opthalmologic disease 53/16; Other 70/105; Evacuations to ROLE 2 4/76 and evacuations to ROLE 4 0/0. Conclusion: There is a similarity between the healthcare provided in both periods of time, except in the traumatic diseases, because there was an increase of almost 300% during the second interval of time (AU)

Introducción: El envejecimiento poblacional, el aumento en la demanda asistencial y la instauración de tratamientos avanzados para el ictus agudo han originado que las urgencias neurológicas crezcan en número y complejidad. A pesar de esto, muchos centros hospitalarios de España carecen de guardias específicas de Neurología. Métodos: Estudio prospectivo durante un año (agosto 2010-julio 2011), que describe la labor asistencial del equipo de guardia de Neurología del Hospital terciario Gregorio Marañón de Madrid. Se recogieron las características sociodemográficas, las principales afecciones, las pruebas diagnósticas y el flujo de los pacientes atendidos, utilizando un registro diario mediante una base de datos informatizada. Los resultados fueron comparados con la asistencia médica general en urgencias. Resultados: Se atendió a 3.234 pacientes (3,48% del total de urgencias médicas), con una media de 11,15 pacientes/día. La edad media fue de 59 años. Las enfermedades más frecuentes fueron la cerebrovascular (34%), epilepsia (16%) y cefalea (8%). La estancia media en urgencias fue de 7,17 h. La tasa de ingreso hospitalario fue del 40% (7,38% del total de ingresos urgentes por afección médica). Los principales destinos de los pacientes hospitalizados fueron Unidad de Ictus (39,5%) y la planta de Neurología (33%). Se realizaron 76 procedimientos trombolíticos o intravasculares en ictus agudos. El 70% de los pacientes fue valorado fuera del horario laboral. Conclusiones: La atención neurológica es frecuente, variada y compleja, representando un alto porcentaje sobre el total de paciente atendidos o ingresados en urgencias. Nuestros resultados justifican la presencia física de un neurólogo de guardia 24 h en urgencias

Conclusions: Emergency neurological care is varied, complex, and frequently necessary. Neurological cases account for a sizeable percentage of both patient visits to the emergency room and the total number of emergency admissions. The current data confirm that on-call neurologists available on a 24-hour basis are needed in emergency departments

Leishmaniasis is a major health problem and it is estimated that 12 million people are currently infected. A vaccine which could cross-protect people against different Leishmania spp. would facilitate control of this disease as more than one species of Leishmania may be present. In this study the ability of a DNA vaccine, using the full gene sequence for L. donovani gamma glutamyl cysteine synthetase (γGCS) incorporated in the pVAX vector (pVAXγGCS), and a protein vaccine, using the corresponding recombinant L. donovani γGCS protein (LdγGCS), to protect against L. major or L. mexicana infection was evaluated. DNA vaccination gave transient protection against L. major and no protection against L. mexicana despite significantly enhancing specific antibody titres in vaccinated infected mice compared to infected controls. Vaccination with the LdγGCS protected against both species but only if the protein was incorporated into non-ionic surfactant vesicles for L. mexicana. The results of this study indicate that a L. donovani γGCS vaccine could be used to vaccinate against more than one Leishmania species but only if the recombinant protein is used.

Introducción: La información obtenida mediante la historia clínica de urgencias (HCU) resulta determinante para el correcto manejo del paciente con ictus agudo. Pretendemos determinar si el empleo de un formulario estructurado (FE) para la elaboración de la HCU mejora la calidad de la información clínica.Material y métodos: Elaboramos un listado de 26 variables que consideramos importantes en el manejo del ictus agudo. Comparamos el número de variables recogidas en las HCU de pacientes ingresados en la unidad de ictus (UI) en enero-febrero 2009, antes de la implantación del FE (FE–), con los datos recogidos con el FE (FE+) (abril-mayo de 2009). Asimismo, analizamos la coincidencia con el diagnóstico definitivo al alta de la UI.Resultados: Analizamos 128 HCU, 80 FE–, 48 FE+. En las FE+, se recogió una media de 20,5 variables frente a 13,7 en las FE– (p<0,001); 16 variables se recogieron con frecuencia significativamente mayor (p<0,05) en las historias con FE+. Entre ellas destacaron: vida basal (100% vs 51%), escala Rankin previo al ictus (94% vs 1%), fecha y hora de inicio del ictus (100% vs 85%), fecha y hora de atención neurológica (100% vs 39%), NIHSS (92% vs 30%), resultado ECG (88% vs 59%), fecha y hora de realización de TC craneal (60% vs 1%). Coincidencia diagnóstica: diagnóstico nosológico/sindrómico: FE+: 94%, FE–: 60% (p<0,001), diagnóstico topográfico: FE+: 71%, FE–: 53% (p=0,03), diagnóstico etiológico: FE+: 25%, FE–: 9% (p=0,01).Conclusiones: El empleo de un FE permite mejorar cuantitativa y cualitativamente la información recogida en la HCU y aporta una mayor precisión diagnóstica


Introduction: The information obtained from the Emergency Medical Chart (EMC) is a key factor for the correct management of acute stroke. Our aim is to determine if the use of a pro-forma (PF) for filling in the EMC improves the quality of the clinical information. Material and methods:A PF was created from a list of 26 key-items considered important to be recorded in an EMC. We compared the number of items recorded in the EMC of patients admitted to our Stroke Unit (SU) in January-February 2009 (before PF was introduced) with the data obtained with the PF (April-May, 2009). We also analysed the agreement with the final diagnosis on discharge from the SU. Results:A total of 128 EMC were analysed, and the PF was used in 48 cases. The mean number of recorded items was 20.5 for the PF group and 13.7 for the non-PF charts (P<.001). Sixteen of the 26 items were recorded significant more frequently (P<.05) in the PF Group. The most notable scores being: previous baseline situation (100% vs 51%), previous Modified Rankin scale score (94% vs 1%), time of symptom onset (100% vs 85%), time of neurological evaluation (100% vs 39%), NIHSS score (92% vs 30%), ECG results (88% vs 59%), time of perform brain scan (60% vs 1%). Diagnostic agreement: nosological/syndromic diagnosis: PF group: 94%, Non-PF group: 60% (P<.001), topographic diagnosis: PF: 71%, Non-PF: 53% (P=.03), aetiological diagnosis: PF: 25%, Non-PF: 9% (P=.01).Conclusions: The use of a PF improves the quantity and quality of the information, and offers a better diagnostic accuracy

Staff occupational exposure for (18)F-FDG studies has been assessed. For this purpose, different detectors, all traceable to PTB standards, have been used. Measurements were carried out in different working areas in the facility for different procedures and 400 MBq per patient standard injected activity. A radiological map of the patient lying on the couch was obtained; the largest dose rate result was obtained in contact with the patient's abdomen. The maximum dose to the most exposed staff member for this position is 3.6 mSv y(-1), under some conservative assumptions. A typical value would be 0.9 ± 0.3 mSv y(-1) close to the abdomen, taking into account staff rotations. These results have been obtained from a sample of 50 patients and average values have been statistically tested. Particularly, a negligible probability of reaching 20 mSv in a year (assuming no incidents or contamination) was obtained (P < 0.01). Annual dose received by personnel lies well within the recommended limits (International Commission on Radiological Protection), these measurements help to optimise working procedures in the facility.

INTRODUCTION: The information obtained from the Emergency Medical Chart (EMC) is a key factor for the correct management of acute stroke. Our aim is to determine if the use of a pro-forma (PF) for filling in the EMC improves the quality of the clinical information. MATERIAL AND METHODS: A PF was created from a list of 26 key-items considered important to be recorded in an EMC. We compared the number of items recorded in the EMC of patients admitted to our Stroke Unit (SU) in January-February 2009 (before PF was introduced) with the data obtained with the PF (April-May, 2009). We also analysed the agreement with the final diagnosis on discharge from the SU. RESULTS: A total of 128 EMC were analysed, and the PF was used in 48 cases. The mean number of recorded items was 20.5 for the PF group and 13.7 for the non-PF charts (P<.001). Sixteen of the 26 items were recorded significant more frequently (P<.05) in the PF Group. The most notable scores being: previous baseline situation (100% vs. 51%), previous Modified Rankin scale score (94% vs. 1%), time of symptom onset (100% vs. 85%), time of neurological evaluation (100% vs. 39%), NIHSS score (92% vs. 30%), ECG results (88% vs. 59%), time of perform brain scan (60% vs. 1%). Diagnostic agreement: nosological/syndromic diagnosis: PF group: 94%, Non-PF group: 60% (P<.001), topographic diagnosis: PF: 71%, Non-PF: 53% (P=.03), aetiological diagnosis: PF: 25%, Non-PF: 9% (P=.01). CONCLUSIONS: The use of a PF improves the quantity and quality of the information, and offers a better diagnostic accuracy.

During four breeding seasons (2004-2008), 78 necropsy examinations were performed on South American fur seal pups (Arctocephalus australis gracilis) found dead on Guafo Island, southern Chile (43°36'S, 74°43'W). Tissue samples from 65 pups were examined microscopically. The primary causes of death were enteritis with microscopical lesions of bacteraemia (28.2%), starvation (23.1%), drowning (21.8%), trauma (19.2%) and stillbirth (2.6%). Those pups with enteritis and microscopical lesions of bacteraemia had haemorrhagic enteritis (100%), interstitial pneumonia (86%), periportal hepatitis (73%) and vasculitis (18%). The pups that died from starvation had atrophy of hepatocytes (61%) and cholestasis (61%). The pups that drowned had bronchoalveolar oedema (65%) and foreign bodies in the airways (65%). In animals that died from trauma, the main lesions were skull fractures (67%). This range of pathological findings is within what would be expected in a healthy otariid breeding colony.

Visceral leishmaniasis presents a serious health threat in many parts of the world. There is, therefore, an urgent need for an approved vaccine for clinical use to protect against infection. In this study, the ability of recombinant Leishmania donovani gamma-glutamyl cysteine synthetase protein (LdγGCS) alone or incorporated into a non-ionic surfactant vesicle (NIV) delivery system to protect against L. donovani infection was evaluated in a BALB/c mouse model. Immunization with LdγGCS alone or LdγGCS-NIV induced specific IgG1 and IgG2a antibodies compared to controls, with LdγGCS-NIV inducing significantly higher titers of both antibody classes (P < 0.05). Both formulations induced similar increases in splenocyte IFN-γ production following ex vivo antigen stimulation with LdγGCS compared with cells from control mice (P < 0.05). Similar levels of protection against infection were induced by LdγGCS alone and LdγGCS-NIV, based on their ability to suppress liver parasite burdens compared to control values (P < 0.01), indicating that using a carrier system did not enhance the protective responses induced by the recombinant protein. The results of this study indicate that LdγGCS may be a useful component in a vaccine against L. donovani.

The present study examines the temporal dynamics of macrophage activation marker expression in response to variations in stimulation. We demonstrate that markers can be categorized as 'early' (expressed most abundantly at 6 h post-stimulation) or 'late' (expressed at 24 h post-stimulation). Thus nos2 and p40 (IL-12/IL-23) are early markers of innate and classical activation, while dectin-1 and mrc-1 are early markers and fizz1 (found in inflammatory zone-1) and ym1 are late markers of alternative activation. Furthermore, argI is a late marker of both innate and alternative activation. The ability of interferon (IFN)-gamma to alter these activation markers was studied at both the protein level and gene level. As reported previously, IFN-gamma was able to drive macrophages towards the classical phenotype by enhancing nos2 gene expression and enzyme activity and p40 (IL-12/IL-23) gene expression in lipopolysaccharide (LPS)-stimulated macrophages. IFN-gamma antagonized alternative macrophage activation, as evident by reduced expression of dectin-1, mrc-1, fizz1 and ym1 mRNA transcripts. In addition, IFN-gamma antagonized arginase activity irrespective of whether macrophages were activated innately or alternatively. Our data explain some apparent contradictions in the literature, demonstrate temporal plasticity in macrophage activation states and define for the first time 'early' and 'late' markers associated with anti-microbial/inflammatory and wound healing responses, respectively.
