RESUMO

PURPOSE: To analyse a population-based study of postoperative mortality and five-year survival in patients with colorectal adenocarcinoma and to compare the results to those of a previous survival study of 1970-1978 period. METHODS: Using the Register of Colorectal Cancer of Cantabria, with a population of 531.654 inhabitants according to the 1989 census, we studied 187 patients with primary colorectal adenocarcinoma. Patients with recurrence after previous excision and who were not residents of Cantabria were excluded. Postoperative mortality included 30-days postoperative period. Survival was calculated using the Kaplan-Meier method and the log-rank test was used to compare curves of probability. RESULTS: Postoperative mortality was 7.7%. Overall five-year survival was 42.4%. Specific-cancer survival was 46%. Type of surgical procedure (curative or palliative) and TNM classification showed significant differences (p < 0.05). CONCLUSION: In Cantabria the five-years survival rate in patients with colorectal adenocarcinoma is similar to published data from other population-based studies but the survival rate did not change over the last decade.

RESUMO

Twenty percent of colorectal cancers develop in elderly patients. Since the size of the geriatric population increases this percentage will get higher. The data collected from the Cantabria Colorectal Cancer Population Registry (1989-90) were divided into two groups: patients aged > or = 80 years at diagnosis and patients aged < 80. The postoperative mortality rates were 11% vs. 7%. Emergency surgery on patients over 80 years was associated with a significantly higher incidence of perioperative deaths (26%). Three-year survival rates for the two groups (20%) were not significantly different. These data suggest that elderly patients suffering from colorectal cancer can be treated by standard methods.

RESUMO

We describe six cases of congenital choledochal cyst of the Toddani type I. All were female, with a mean age of 33.5 years (16-52). The onset of symptoms ranged 8-192 months (mean 92.8), with an unexpected finding. Ultrasound scan was performed in five patients, with two accurate diagnoses, one suspicious and two wrong results. In two cases a C.T. was performed and the diagnosis was confirmed in both. A cholelithiasis was associated in four cases. Four patients (66.4%) were treated by cyst excision, Roux-en-Y hepaticojejunostomy and cholecystectomy, three of them as a primary procedure, and one more after failure of some derivative operations. All evolved well but one, requiring sphincterotomy and Wirsungtomy. Two patients (33.6%) were treated by cholecystectomy only, with a favourable evolution. Nevertheless, we emphasize the good results achieved with cyst excision and we recommend it as the procedure of choice.

RESUMO

Gastric duplications are the least frequent location for the alimentary tract duplications, these location is approximately 8%. We present a case of gastric duplication in a neonate, who had a palpable abdominal mass and feeding intolerance. Barium study of upper gastrointestinal tract and ultrasound showed an extrinsic compression of antrum and cyst abdominal mass. The surgical procedure consisted of total excision without violation of the gastric lumen. Gastric mucosal was found by hystological study. Heterotopic pancreatic tissue was also found on the major omentum closely gastric cyst duplication.

RESUMO

Prepuberal gynecomastia is extremely rare. We present a case of unilateral and idiopathic gynecomastia in a six years old male. Endocrinology test and anamnesis did not find pathology or drug ingestion. A mastectomy was performed, because the big size and progressive evolution of breast produced physicology problems. We review the literature and remark infrequent aspects of our case, also the complete clinic and pathologic study.
