BACKGROUND: Inflammation is an important feature of arteriosclerotic disease, and the vulnerability of coronary plaques in acute myocardial infarction (AMI) may be related to the levels of serum C-reactive proteins (CRP). While some risk factors for early and late complications have been suggested, an accurate and definitive preprocedural risk stratification of patients undergoing percutaneous transluminal coronary angioplasty (PTCA) is still lacking. HYPOTHESIS: The study was undertaken to investigate whether early and late complications after PTCA could be predicted by evaluation of baseline serum CRP levels in patients with AMI. METHODS: Levels of serum CRP were measured in a total of 230 patients with AMI undergoing PTCA and provisional stent. They were divided into two groups: Group 1 (n = 48) with elevated CRP levels (> or = 5 mg/l) and Group 2 (n = 182) with normal CRP levels (< 5 mg/l). RESULTS: There were no significant differences in baseline clinical, angiographic, and procedural characteristics between the two groups. However, the incidence of in-hospital adverse coronary events (reinfarction, coronary reocclusion, target vessel revascularization, and death) and severe left ventricular dysfunction was significantly higher in Group 1 (18.3 vs. 6.1%, p < 0.05 and 20.9 vs. 6.1%, p < 0.05, respectively). In addition, bailout stenting was performed more frequently in Group 1 than in Group 2 (60.4 vs. 36.3%, p < 0.005). No significant late complications were noted. The serum levels of CRP were the only independent predictors of early adverse events. CONCLUSIONS: Preprocedural serum CRP level might be considered a powerful predictor of early but not late complications in patients undergoing PTCA/stent procedures.

The aim of this study was to assess the knowledge, perception, and attitude towards personal health care and menopause among women with ischemic heart disease. The study population consisted of 100 menopausal women attending the cardiac unit for coronary angiography. All women completed a 20-item questionnaire, covering information on health-care habits, life style and attitude towards menopause. About half of the study population had ever smoked, were overweight, had never tried to lose weight and did not exercise regularly. The majority regularly underwent screening mammography, Pap smear and lipid profile measurement. There was a positive statistically significant correlation between years of education and having annual mammography, Pap smear and lipid profile, performing breast self-examinations and taking an annual vacation. Sixty eight per cent of the study population had never used hormone replacement therapy (HRT), 13.2% had used HRT in the past and 18.8% were currently using HRT. There was a negative statistically significant correlation between use of HRT and age and number of children, and a positive statistically significant correlation between use of HRT and having an annual Pap smear and mammography scan. Only a relatively small percentage of the study population safeguarded their health care or used HRT. We believe that stronger efforts are needed to promote good health-related behaviors for these women.

Topically-administered ophthalmic medication may reach clinically significant serum concentrations, as evidenced by the variety of systemic side effects reported. These drugs may interact with other drugs administered orally and intravenously, increasing the frequency and severity of their side-effects. We report 4 patients examined in the emergency room because of various systemic manifestations, in whom the symptoms could be attributed either to antiglaucomatous eye drops, or to interaction between these eye drops and systemic medication. Considering the high incidence of glaucoma (0.5-2%), physicians should be aware of the systemic side-effects of antiglaucomatous eye drops and of possible interaction between such treatment and systemic medications.

RESUMO

Paroxysmal nocturnal hemoglobinuria (PNH) is a chronic hemolytic anemia of unknown etiology associated with morning hemoglobinuria, variable acholuric jaundice, and striking hemosiderinuria, all signs of hemolysis. Paroxysmal nocturnal hemoglobinuria with infarction and hemorrhage of the gut leading to emergency surgery has been reported [1]; a history of thrombosis may be found before PNH is diagnosed [2]. We describe the radiographic findings in the small bowel of a patient with paroxysmal nocturnal hemoglobinuria during an episode of moderately severe abdominal symptoms. Ours is the second reported case using barium study.
