RESUMO

Prenatal doppler ultrasound of the ductus venosus plays an important role in the evaluation of fetal circulation and fetal heart function. We aimed to establish the percentile of Resistivity Index (RI) and Pulsatility Index (PI) of the Ductus Venosus (DV) among normal Vietnamese fetuses with a gestation from 22 to 37 weeks in a cohort study. Normal Vietnamese fetuses with a gestation ranging from 22 to 37 weeks were examined in a cohort study. The PI and RI were recorded from the DV. We analyzed 640 participants who all fulfilled the inclusion and exclusion criteria of our study. Results: There was a significant correlation between RI, PI and gestational age as shown with the equations y = 0.077x - 0.003x2 + 3.493e - 5x3 (r=0.97) and y = 0.106x - 0.004x2 + 4.5e - 5x3 (r=0.94), respectively. A centile module was constructed for the DV RI and PI indices among normal Vietnamese fetuses with gestation from 22 to 37 weeks.

RESUMO

Castleman's disease (CD) or angiofollicular lymphoid hyperplasia is a rare disorder with unknown aetiology that can easily be misdiagnosed as lymphoma, neoplasm, or infection. Diagnosis is challenging due to its non-specific symptoms and radiologic signs as well as its rarity. We report a case of a middle-aged woman with a mass adjacent to the uterus that was accidentally detected by ultrasound; it was believed to be of ovarian origin. Subsequently, the patient underwent a successful tumorectomy. Pathological examination confirmed the hyaline-vascular type of CD in a pelvic location. This was a typical case of an asymptomatic unicentric and hyaline-vascular type of CD.

PURPOSE: This study аims to explore cement leаkаge аs а complication of percutаneous vertebroplаsty (PVP) in the treаtment of multiple osteoporotic vertebrаl compression frаctures (MOVF). PATIENTS AND METHODS: This prospective study wаs cаrried out on 32 consecutive pаtients with osteoporotic frаctures of аt leаst two vertebrаe (VB). Аll pаtients were over 50 yeаrs old аnd women аccounted for 29 out of the 32 pаtients (90.6%). PVP wаs performed under digitаl subtrаction аngiogrаphy (DSА) of аt leаst three VB, аnd 97 collаpsed VB аnd 105 VB were exаmined by PVP. Аll pаtients hаd postoperаtive computerized tomogrаphy (CT) to diаgnose аnd clаssify the complicаtions. RESULTS: One hundred аnd five vertebrаe were exаmined with PVP, аnd 36/105 (34.3%) exhibited complicаtions of cement leаkаge. Type B cement leаkаge wаs the most common complicаtion, with 19/105 (18.1%) cаses; type C аccounted for 8/105 (7.6%) cаses; аnd type S аccounted for 9/105 (8.6%) cаses. There wаs only one (0.95%) cаse of cement leаkаge moving to the pulmonаry аrtery. Аll complicаtions hаd no clinicаl symptoms аnd did not require treаtment. CONCLUSION: Cement leаkаge is quite а common complicаtion, but it usuаlly hаs no clinicаl symptoms аnd does not require treаtment. Therefore, PVP is а sаfe аnd successful technique for the treаtment of multiple osteoporotic vertebrаl compression frаctures.

BACKGROUND: The accurate diagnosis of complicated appendicitis has been improved by using various diagnostic modalities. However, no preoperative diagnostic method could completely confirm the results. Therefore, preoperative diagnosis of complicated appendicitis to have the right management is still a huge challenge. OBJECTIVES: The aim of this study was to evaluate the diagnostic value of ultrasound combined with pediatric appendicitis score for differentiation between acute uncomplicated appendicitis and acute complicated appendicitis in a pediatric population. METHODS: We prospectively evaluated 120 pediatric patients who underwent surgery for acute appendicitis from November 2017 to June 2019. Pediatric appendicitis score (PAS) was calculated and ultrasound (US) was performed before surgery. The histopathology of phlegmonous appendicitis corresponds to uncomplicated appendicitis (AUA), while gangrenous appendicitis and perforation are classified as complicated appendicitis (ACA). RESULTS: Histopathologically, the results provided a diagnosis of acute appendicitis including 86 (71.7%) patients with AUA and 34 (28.3%) children with ACA. US findings showed a sensitivity of 23.5%, the specificity of 95.4%, PPV of 66.7%, NPV of 75.9%, and an accuracy of 75%. PAS of 8 was found to be the most appropriate cutoff point compatible with ACA; it resulted in a sensitivity of 76.5% and a specificity of 84.1%. Combining ultrasound with a pediatric appendicitis score resulted in a higher specificity to distinguish complicated from uncomplicated appendicitis when compared with ultrasound or PAS solely. CONCLUSIONS: the US is highly specific but nonsensitive for detecting complicated pediatric appendicitis. Combining ultrasound with pediatric appendicitis is a very good concept to distinguish complicated from uncomplicated appendicitis in a pediatric population.

BACKGROUND/AIM: Even though advanced magnetic resonance imaging (MRI) can effectively differentiate between medulloblastoma and ependymoma, it is not readily available throughout the world. This study aimed to investigate the role of simple quantified basic MRI sequences in the differentiation between medulloblastoma and ependymoma in children. PATIENTS AND METHODS: The institutional review board approved this prospective study. The brain MRI protocol, including sagittal T1-weighted, axial T2-weighted, coronal fluid-attenuated inversion recovery, and axial T1-weighted with contrast enhancement (T1WCE) sequences, was assessed in 26 patients divided into two groups: Medulloblastoma (n=22) and ependymoma (n=4). The quantified region of interest (ROI) values of tumors and their ratios to parenchyma were compared between the two groups. Multivariate logistic regression analysis was utilized to find significant factors influencing the differential diagnosis between the two groups. A generalized estimating equation (GEE) was used to create the predictive model for the discrimination of medulloblastoma from ependymoma. RESULTS: Multivariate logistic regression analysis showed that the T2- and T1WCE-ROI values of tumors and the ratios of T1WCE-ROI values to parenchyma were the most significant factors influencing the diagnosis between these two groups. GEE produced the model: y=exn/(1+exn) with predictor xn=-8.773+0.012x1 - 0.032x2 - 13.228x3, where x1 was the T2-weighted signal intensity (SI) of tumor, x2 the T1WCE SI of tumor, and x3 the T1WCE SI ratio of tumor to parenchyma. The sensitivity, specificity, and area under the curve of the GEE model were 77.3%, 100%, and 92%, respectively. CONCLUSION: The GEE predictive model can discriminate between medulloblastoma and ependymoma clinically. Further research should be performed to validate these findings.

INTRODUCTION: Each country has its system of the training program, but to be concordant with the world in the radiology field, the process needs to have common points or common criteria. On maintaining the integrity of intersociety collaboration in the field of radiology, it is necessary to understand each country's training program for each specialty. AIM: This retrospective study aims to compare the postgraduate thesis characteristics from various sources in the field of radiology. METHODS: This was a retrospective study evaluating data that is publicly available online and at libraries and institutional review board approval, as such, was not demanded. We selected 40 published theses from the library of Pham Ngoc Thach University of Medicine and University of Medicine and Pharmacy at Ho Chi Minh City and Hanoi Medical University in Vietnam which graduated from 2008 to 2018. Of these, there were 10 PhD, 10 specialists II, 10 master's, and 10 residency theses selected. RESULTS: A total of 40 theses were analyzed from participants with a median age of 36.5. The male/female ratio was 23/17. Most of the theses were subspecialty in diagnostic radiology (87.5%) and focused on pathological radiology (95%). Adult patients were the major objectives of the theses accounted for 87.5% with predominant materials of magnetic resonance imaging counted for 47.5%. Theses in PhD group were the largest items regarding the total number of pages as well as the number of figures, and the number of references. Nonetheless, both domestic and international publications related to all theses were truly low. CONCLUSION: The postgraduate thesis of radiology in Vietnam has many different forms but mainly focuses on diagnostic and pathological radiology with materials of magnetic resonance imaging in adults. The number of international publications regarding the thesis was very small.

INTRODUCTION: Deconstructive versus reconstructive technique remains controversial on the management of acute basilar tip artery dissection. AIM: We introduced a case report of massive dissecting aneurysm in the basilar tip artery treated with intra-aneurysm and basilar artery coiling. RESULTS: A 30-year-old male presented with sudden headache and severe vomiting. Radiographic study showed a large unruptured dissecting aneurysm in the basilar tip artery involving bilateral P1 segment. This aneurysm was treated with intra-aneurysm and basilar artery coiling. Patient was discharge after 7 days without any neurological deficits. Post-operatively, the patient received 75 mg aspirin and 75 mg clopidogrel PO per day for 3 months - then 75 mg aspirin per day for up to 1 year. Angiographic follow-up at 3 months showed a complete occlusion of aneurysmal sac and basilar tip artery without any deficits (mRS 0). Cerebral arteriography at 6 months follow-up confirmed a stable occlusion of aneurysmal sac with a minor recurrence of aneurysm in left P1 segment. CONCLUSION: Intra-aneurysm and basilar artery coiling is valuable alternative technique to treat complex basilar tip dissecting aneurysm in case of infeasible reconstructive technique. Clinical presentation, aneurysm characteristics and collateral circulation have to be investigated on each case to adopt this technique.

PURPOSE: We sought to present our preliminary experience on the effectiveness and safety of magnetic resonance imaging (MRI)-guided, high-intensity focused ultrasound (HIFU) therapy using a volumetric ablation technique in the treatment of Association of Asian Nations (ASEAN) patients with symptomatic uterine leiomyomas. METHODS: This study included 33 women who underwent HIFU treatment. Tissue characteristics of leiomyomas were assessed based on T2- and T1-weighted MRI. The immediate nonperfused volume (NPV) ratio and the treatment effectiveness of MRI-guided HIFU on the basis of the degrees of volume reduction and improvement in transformed symptom severity score (SSS) were assessed. RESULTS: The median immediate NPV ratio was 89.8%. Additionally, the median acoustic sonication power and HIFU treatment durations were 150 W and 125 min, respectively. At six-month follow-up, the median leiomyoma volume had decreased from 139 mL at baseline to 84 mL and the median transformed SSS had decreased from 56.2 at baseline to 18.8. No major adverse events were observed. CONCLUSION: The preliminary results demonstrated that volumetric MRI-guided HIFU therapy for the treatment of symptomatic leiomyomas in ASEAN patients appears to be clinically acceptable with regard to treatment effectiveness and safety.

INTRODUCTION: The advancement of gender equality within radiology, a predominantly male profession, has currently been a significant concern. AIM: Therefore, in this original study, we aimed to investigate the gender disparity in Vietnamese radiological societies. METHODS: No ethical committee or institutional review board approval was needed since the data were publicly available. In this retrospective study, we evaluated the faculties of four main radiological societies in Vietnam: Vietnamese society of radiology and nuclear medicine (VSRNM, n = 67); Radiological society of Ho Chi Minh City (RSHCM, n = 25); Vietnamese society of ultrasound in medicine (VSUM, n = 29); and Vietnamese society of interventional radiology (VSIR, n = 18). RESULTS: There are significantly fewer women than men in faculties of four main radiological societies (15.1% vs. 84.9%). None of the women served as a professor and leader of any radiological societies. The women with a doctor of philosophy level are relatively low among the four main radiological societies. Also, female interventional and pediatric radiologists are seriously low among four main radiological societies. CONCLUSIONS: In Vietnamese radiological societies, gender disparities exist, especially about educational degrees and professorship positions. Future studies are essential to address the underlying roots of the gender gap and aid in the implementation of gender diversity programs and policies.

BACKGROUND: Magnetic Resonance Imaging (MRI) nowadays plays an important role in the evaluation of posterior fossa brain tumours in children for appropriate diagnosis, treatment planning, and follow-up. AIM: To assess the MRI features of common posterior fossa brain tumours including medulloblastomas, ependymomas, and pilocytic astrocytomas along with the postoperative parameters to contribute the local knowledge to the neuroradiology and neurosurgery fields. METHODS: The study was performed at Children's Hospital 02 from January 2016 to June 2019. In this study, all pediatric patients adopted MRI to evaluate the posterior fossa brain tumours' characteristics and then underwent surgery to eradicate the posterior fossa tumours. We retrospectively compared the baseline parameters, MRI parameters, and postoperative parameters among medulloblastomas, ependymomas, and pilocytic astrocytomas. RESULTS: There were 62 patients (27 medulloblastomas, 20 ependymomas, and 15 pilocytic astrocytomas) in this research. The main structure of medulloblastomas and ependymomas was predominantly solid, whereas the main structure of pilocytic astrocytomas was superiorly cystic (p < 0.05). Ependymoma tended to extend tumour through foramina of Luschka and Magendie (p < 0.05). Medulloblastomas chiefly showed iso intensity on T2W and FLAIR images meanwhile ependymomas and pilocytic astrocytomas predominantly appeared hyperintensity on T2W and FLAIR images. Medulloblastomas and ependymomas were mostly high intensity on DWI, and low intensity on ADC whereas pilocytic astrocytomas were usually low intensity on DWI and high intensity on ADC. After injecting CE, pilocytic astrocytomas showed a mixed intensity whereas the signal intensity of medulloblastoma and ependymoma on T1CE was generally strong. There were positive correlations between FH diameter and estimated blood loss (r = 0.289, p < 0.05); and surgical time (r = 0.312, p < 0.05). CONCLUSION: MRI plays a crucial role in demonstrating the features of posterior fossa brain tumours for appropriate diagnosis of medulloblastomas, ependymomas, and pilocytic astrocytomas. Medulloblastomas are problematic tumours and the clinicians should also take into consideration in cases of larger feet-to-head diameter of tumours to ensure the efficacy and safety surgery for patients.

INTRODUCTION: Currently, most of continents and countries have its own radiological societies which help to share and update clinical experience and scientific knowledge to make the outcome of routine work become efficacious. Although Vietnam was affected adversely due to war, it also developed fundamentally in radiological field. So as to interact better among radiological societies, we believe that the history and current condition of Vietnamese radiology should be introduced. AIM: Thus, in this paper, we aimed to describe the past, current, and future of Vietnamese Society of Radiology and Nuclear medicine. METHODS: We collected the information about Vietnamese Society of Radiology and Nuclear medicine via internet source and publications. RESULTS: Radiological society was established in 1961. Although the society developed successfully during 59 years, there are still some problematic conditions such as gender disparity, insufficiency of human resource and modern modalities. CONCLUSION: With the appearance of Vietnamese society of radiology and nuclear medicine, many doctors and technicians are close together, and Vietnam has an effective chance for radiology to intergrate into actively global radiological field.
