RESUMO

Persistent left superior vena cava is the most common thoracic venous anomaly. It is usually asymptomatic, but it can make implanting intracardiac devices difficult.We present a novel technique to facilitate desfibrillator lead implantation in patients with persistent left superior vena cava and the absence of the right superior vena cava. We used a fixed-curve Selectra 3D 65-42 cm sheath (Biotronik), orienting it toward the tricuspid valve (TV) by rotating it counter-clockwise. During follow-up, the electrodes remained stable.Our technique was safe, simple, and feasible for patients with this complex venous anatomy.

BACKGROUND: Electrical isolation of pulmonary veins (PV) with high-power short-duration (HPSD) radiofrequency application (RFa) may reduce the duration of atrial fibrillation (AF) ablation, without compromising the procedural efficacy and safety in comparison with the conventional approach. This hypothesis has been generated in several observational studies; the POWER FAST III will test it in a randomized multicenter clinical trial. METHODS: It is a multicenter randomized, open-label and non-inferiority clinical trial with two parallel groups. AF ablation using 70 W and 9-10 s RFa is compared with the conventional technique using 25-40 W RFa guided by numerical lesion indexes. The main efficacy objective is the incidence of atrial arrhythmia recurrences electrocardiographically documented during 1-year follow-up. The main safety objective is the incidence of endoscopically detected esophageal thermal lesions (EDEL). This trial includes a substudy of incidence of asymptomatic cerebral lesions detected by magnetic resonance imaging (MRI) after ablation. RESULTS: A randomized clinical trial compares for the first time high-power short-duration and conventional ablation in order to obtain data about the efficacy and safety of the high-power technique in an adequate methodological context. CONCLUSIONS: The results of the POWER FAST III could support the use of the high-power short-duration ablation in clinical practice. REGISTRATION: ClinicalTrials.gov: NTC04153747.

BACKGROUND: The Prevention With Mediterranean Diet (PREDIMED) trial supported the effectiveness of a nutritional intervention conducted by a dietitian to prevent cardiovascular disease. However, the effect of a remote intervention to follow the Mediterranean diet has been less explored. OBJECTIVE: This study aims to assess the effectiveness of a remotely provided Mediterranean diet-based nutritional intervention in obtaining favorable dietary changes in the context of a secondary prevention trial of atrial fibrillation (AF). METHODS: The PREvention of recurrent arrhythmias with Mediterranean diet (PREDIMAR) study is a 2-year multicenter, randomized, controlled, single-blinded trial to assess the effect of the Mediterranean diet enriched with extra virgin olive oil (EVOO) on the prevention of atrial tachyarrhythmia recurrence after catheter ablation. Participants in sinus rhythm after ablation were randomly assigned to an intervention group (Mediterranean diet enriched with EVOO) or a control group (usual clinical care). The remote nutritional intervention included phone contacts (1 per 3 months) and web-based interventions with provision of dietary recommendations, and participants had access to a web page, a mobile app, and printed resources. The information is divided into 6 areas: Recommended foods, Menus, News and Online resources, Practical tips, Mediterranean diet classroom, and Your personal experience. At baseline and at 1-year and 2-year follow-up, the 14-item Mediterranean Diet Adherence Screener (MEDAS) questionnaire and a semiquantitative food frequency questionnaire were collected by a dietitian by phone. RESULTS: A total of 720 subjects were randomized (365 to the intervention group, 355 to the control group). Up to September 2020, 560 subjects completed the first year (560/574, retention rate 95.6%) and 304 completed the second year (304/322, retention rate 94.4%) of the intervention. After 24 months of follow-up, increased adherence to the Mediterranean diet was observed in both groups, but the improvement was significantly higher in the intervention group than in the control group (net between-group difference: 1.8 points in the MEDAS questionnaire (95% CI 1.4-2.2; P<.001). Compared with the control group, the Mediterranean diet intervention group showed a significant increase in the consumption of fruits (P<.001), olive oil (P<.001), whole grain cereals (P=.002), pulses (P<.001), nuts (P<.001), white fish (P<.001), fatty fish (P<.001), and white meat (P=.007), and a significant reduction in refined cereals (P<.001), red and processed meat (P<.001), and sweets (P<.001) at 2 years of intervention. In terms of nutrients, the intervention group significantly increased their intake of omega-3 (P<.001) and fiber (P<.001), and they decreased their intake of carbohydrates (P=.02) and saturated fatty acids (P<.001) compared with the control group. CONCLUSIONS: The remote nutritional intervention using a website and phone calls seems to be effective in increasing adherence to the Mediterranean diet pattern among AF patients treated with catheter ablation. Introducción y objetivos: Se describen los resultados del Registro Nacional de Ablación con Catéter correspondientes al año 2018. Métodos: La recogida de datos se ha realizado de manera retrospectiva con la cumplimentación de un formulario de recogida de datos por cada uno de los centros participantes. Resultados: Se han analizado los datos enviados por 100 centros, con un número total de procedimientos de ablación de 16.566, el más alto comunicado históricamente en este registro (con una media de 165,5+/-127,9 y una mediana de 119 procedimientos por centro). Se consolida la ablación de fibrilación auricular como el sustrato abordado con más frecuencia (n=4.234; 25,6%), seguida de la taquicardia por reentrada nodular (n=3.525; 21,3%) y el istmo cavotricuspídeo (n=3.425; 20,7%). Se observa un nuevo pico en el número de procedimientos de ablación de fibrilación auricular y aumenta la distancia respecto a los demás sustratos. La tasa total de éxito fue del 91%; la de complicaciones mayores, del 2,2% y la mortalidad, del 0,04%. El 2,1% de las ablaciones se realizaron en pacientes pediátricos. Conclusiones: El Registro Nacional de Ablación con Catéter con Catéter recoge sistemática e ininterrumpidamente los procedimientos de ablación realizados en España, lo que permite observar a lo largo de los años un aumento progresivo del número de ablaciones manteniendo una tasa de éxito elevada y unos porcentajes de complicaciones bajos

Introducción y objetivos: Se describen los resultados del Registro Español de Ablación con Catéter correspondientes al año 2017. Métodos: La recogida de datos se llevó a cabo de forma retrospectiva con la cumplimentación de un formulario de recogida de datos de cada uno de los centros participantes. Resultados: El número total de procedimientos de ablación fue de 15.284 realizados en 98 centros (mayor número de centros y de procedimientos de ablación comunicados históricamente en este registro) con una media de 156 ± 126 y una mediana de 136 procedimientos. El sustrato abordado con más frecuencia ha sido por primera vez en el registro la fibrilación auricular (n = 3.457; 22,6%), seguida del istmo cavotricuspídeo (n = 3.449; 22,5%) y la taquicardia intranodular (n = 3.429; 22,4%). La tasa total de éxito fue del 87%; la de complicaciones mayores, del 2,6% y la mortalidad, del 0,09%. Se ha producido un aumento de los procedimientos realizados sin apoyo de fluoroscopia hasta suponer un 6% del total de las ablaciones. Un 2,3% de las ablaciones se realizó en pacientes pediátricos. Conclusiones: El Registro Español de Ablación con Catéter recoge sistemática e ininterrumpidamente los procedimientos de ablación realizados en España, y esto nos ha permitido observar un aumento progresivo del número de ablaciones y de centros que las realizan manteniendo una tasa de éxito elevada y unos porcentajes de complicaciones bajos
