RESUMO

PURPOSE: We investigated correlations between the findings of oral examinations and panoramic radiography in order to determine the efficacy of using panoramic radiographs in screening examinations. METHODS: This study included patients who visited dental clinics at National Health Insurance Service (NHIS) Ilsan Hospital for checkups during 2009–2015 and underwent panoramic radiographic examinations within 1 year prior to the oral examinations. Among the 48,006 patients who received checkups, 1,091 were included in this study. The data were evaluated using the Cohen kappa and interrater agreement coefficients. Accuracy, sensitivity, and specificity were calculated using data from the panoramic radiographs as true positive diagnoses. RESULTS: The interrater agreement coefficient for occlusal caries was 28.8%, and the Cohen kappa coefficient was 0.043 between the oral and panoramic radiographic examinations. Root caries and subgingival calculus were only found on the radiographs, while gingival inflammation was found only by the oral examinations. The oral examinations had a specificity for detecting occlusal dental caries of 100%, while their sensitivity for proximal dental caries and supragingival calculus was extremely low (14.0% and 18.3%, respectively) compared to the panoramic radiographic examinations. The oral examinations showed a relatively low sensitivity of 66.2% and a specificity of 43.7% in detecting tooth loss compared with panoramic radiography. CONCLUSIONS: Panoramic radiography can provide information that is difficult to obtain in oral examinations, such as root caries, furcation involvement, and subgingival calculus, which are factors that can directly affect the survival rate of teeth. It therefore seems reasonable and necessary to add panoramic radiography to large-scale health checkup programs such as that provided by the NHIS.

RESUMO

PURPOSE: The aim of this retrospective study was to determine the association between dental implants in the posterior region and traumatic occlusion in the adjacent premolars, using data collected during from 2002 to 2015. METHODS: Traumatic occlusion in the adjacent premolars was assessed by examining clinical parameters (bleeding on probing, probing pocket depth, fremitus, and tooth mobility) and radiographic parameters (loss of supporting bone and widening of the periodontal ligament space) over a mean follow-up of 5 years. Clinical factors (gender, age, implant type, maxillary or mandibular position, opposing teeth, and duration of functional loading) were evaluated statistically in order to characterize the relationship between implants in the posterior region and traumatic occlusion in the adjacent premolars. RESULTS: The study inclusion criteria were met by 283 patients, who had received 347 implants in the posterior region. The incidence of traumatic occlusion in the adjacent premolars was significantly higher for splinted implants (P=0.004), implants in the maxillary region (P<0.001), and when implants were present in the opposing teeth (P<0.001). The other clinical factors of gender, age, and duration of functional loading were not significantly associated with traumatic occlusion. CONCLUSIONS: This study found that the risk of traumatic occlusion in the adjacent premolars increased when splinted implants were placed in the maxillary molar region and when the teeth opposing an implant also contained implants.

RESUMO

Primary peritoneal serous papillary carcinoma (PPSPC) is a rare tumor that originates from a single or multicentric foci of peritoneum. Histologically the disease resembles primary serous papillary carcinoma of the ovary, but either involves the ovarian surface microscopically only or spares the ovaries entirely. Currently PPSPC is evaluated, staged and treated in the same way as epithelial ovarian cancer. We experienced one case of primary peritoneal serous papillary carcinoma which achieved a complete remission with carboplatin and paclitaxel, and report this with brief review of the literatures.

RESUMO

Intrauterine device is one of the most used contraception in the world. One of the major complications of intrauterine contraception is perforation through the uterine wall into the pelvic or abdominal cavity. The currently accepted treatment of choice for displaced IUD is its removal of surgical laparoscopy or laparotomy. We report on three cases with misplaced IUD in pelvic or abdominal cavity, followed by a review of the literature.

OBJECTIVE: It is controversial whether routine or selective iron supplementation during gestation is needed. The aim of this study is to evaluate whether screening with serum ferritin during the first trimester of pregnancy could be identify women who need prophylactic iron supplementation. METHODS: According to the serum ferritin level of cut-off point of 30 microgram/L during the first trimester of pregnancy and the presence of iron supplementation during gestation, the subject was divided into 4 group that were as follows: serum ferritin level of 30 microgram/L and iron- supplemented group (n=40), serum ferritin level of >30 microgram/L and non iron-supplemented group (n=15). Maternal serum ferritin, TIBC, iron, hemoglobin, hematocrit, red cell indices (MCV, MCH, MCHC, RDW) were measured before 14 weeks of gestation and after 34 weeks of gestation. RESULTS: All hematologic and biochemical markers indicated more severe anemic status in the late pregnancy than during the first trimester of pregnancy. The effect of iron supplementation was profounder on the pregnant woman whose ferritin levels were below 30 microgram/L during the first trimester of pregnancy. Regardless of iron supplementation, the group (ferritin >30 microgram/L during the first trimester of pregnancy) showed relatively higher ferritin level in late pregnancy. CONCLUSION: The screening with serum ferritin level of cut-off point of 30 microgram/L during the first trimester of pregnancy may be useful to identify women who need prophylactic iron supplementation.

RESUMO

Primary choriocarcinoma of fallopian tube is a extremely rare. At the time of initial presentation, symptoms are usually similar to those of traditional tubal pregnancies. The diagnosis is usually made on histological examination of a surgically resected specimen. Chemotherapy is required in most patients following initial operation including unilateral adnexectomy or salpingectomy. Primary choriocarcinoma of fallopian tube appears to have the same favorable prognosis as does uterine choriocarcinoma. We experienced one case of primary choriocarcinoma of fallopian tube, and report this with brief review of the literatures.

RESUMO

OBJECTIVE: The purpose of this study is to evaluate the clinical and pathological characteristics of granulosa cell tumor of the ovary. METHODS: Between 1990 and 2000, 27 patients with granulosa cell tumor of the ovary treated in the Department of Obstetrics and Gynecology, College of Medicine, The Catholic University of Korea were identified and reviewed retrospectively for patient profiles, International Federation of Gynecologists and Obstetricians (FIGO) disease stage, mode of therapy, and length of survival. RESULTS: The median age at diagnosis was 43.9 years (range 9-75 years). Tumors were 70.3% (19/27) stage I, 7.5% (2/27) stage II, 11.1% (3/27) stage III, and 11.1% (3/27) stage IV. There was a 15% association between granulosa cell tumor and endometrial hyperplasia. All patients underwent cytoreductive surgery, and 14 patients followed by postoperative chemotherapy. One patient received adjuvant pelvic radiotherapy and 3 patients were treated with adjuvant pelvic radiotherapy and chemotherapy. The survival rate in stage I was 100.0% after 5 years, and in stage II-IV was 62.5% after 5 years. Overall survival was 88.8% at 5 years. Mitotic index influenced the survival rate; with less 10/10 HPF the survival was 100% in 5 years, and with more or equal 10/10 HPF the longest survival was 44 months. CONCLUSION: Although most granulosa cell tumors of the ovary are slow growing, wide variation is possible, with high mitosis counts leading to a worse prognosis. However, it is difficult to predict early or late recurrences using pathologic parameters. It is therefore important that patients with theses tumors are followed up indefinitely.

RESUMO

OBJECTIVE: The purpose of this study was to evaluate female victims of sexual assault and assess the similarities and differences between them. METHODS: Case files and photographs of 59 women examined at Holy Family hospital for sexual assault during Jan 1, 1991 to Dec 31 2001 were reviewed. RESULTS: The mean age of the patients was 17.12 (+/-11.50). 45 (76.3%) of assailants were strangers to the victims. 35 (59.3%) had genital injuries and 8 (13.6%) needed surgical treatments. No victim was pregnant and 17 (28.8%) was reported to police. CONCLUSION: Sympathetic concern in the treatment of women who come forward to report the experience of sexual assaults will result in more reporting of rape. The victims need to be educated at home, at school, and by their primary care providers to avoid situations in which they could make themselves vulnerable to sexual assault, and they should be taught the importance of immediately reporting their assault and seeking medical care.

OBJECTIVE: This study was performed to evaluate the diagnostic value of polymerase chain reaction (PCR) for multiple microorganisms in female lower genital infection, because infections of the vaginal are caused by multiple microorganisms. METHODS: A total of 222 patients (161 cases of gynecologic patients and 61 cases of obstetric patients) who complained of profuse vaginal discharge or had excessive vaginal discharge were evaluated for detection of Chlamydia trachomatis, Ureaplasma urealyticum, Mycoplasma hominis, and Trichomonas vaginalis infections using PCR. RESULTS: Infecting microorganisms by PCR were found in 61 out of 161 gynecologic patients (37.6%). Among the 61 patients, single infection was present in 45 patients (78.3%), and infection by multiple microorganisms (26.6%) in the remaining 16. In these same patients, 72 showed an abundance of WBCs with the Gram stain. Among these 72 patients, 26 (74.3%) were infected with a single microorganism, and 9 (25.7%) were infected with multiple microorganisms. In 61 pregnant women, 26 patients (42.6%) were positive for infection. Single infection was found in 25 patients (96.2%) and infection by multiple microorganisms was present in one patient (3.8%). Many WBCs were observed in 19 out of the 61 pregnant women with the detection of single infection in 9 patients and none of the mixed forms. CONCLUSION: The majority of female lower genital infections are due to multiple organisms. Individual tests, cultures, and Gram staining must be done in order to detect all involved organisms which may potentially double cost and time loss. However, with the use of PCR, this can be achieved all at once. We therefore suggest that PCR may be precise and economically beneficial in the detection of female lower genital infection.

OBJECTIVE: To evaluate the outcome of pregnancy in kidney transplanted women, and correlate this with the time of transplantation to conception. MATERIAL AND METHODS: We analyzed the outcome of 31 pregnancies from 21 allograft recipients at Kangnam St. Mary's Hospital, Catholic Medical Center from January 1990 through December 2000. For each reported pregnancy we reviewed obstetrical, medical and pediatric records. For children follow-up and for those whose obstetric procedures were taken in other hospitals, we did a questionarie and telephone interviews. RESULTS: Of the 31 pregnancies followed by us, there were 23 live-born offspring delivered by 21 mothers and 8 stillborn/abortuses, including five artificial abortuses. The mean gestational age at delivery was 36.18+/-0.6 weeks and the mean birth weight of the offspring was 2,525+/-137gm. Only 43.4% was delivered after 37weeks of gestation. Preterm delivery rate (delivered before 37weeks of gestation) was 56.5%. Common obstetric complications were intrauterine growth restriction (39.1%) preterm labor (35%), premature rupture of membrane (30%), preeclampsia (30%), and maternal complications were deterioration of renal function (5%) and rejection of allograft (18%) after delivery. Mean interval from transplantation to pregnancy was 4.7 years. Twelve pregnancies occurred within the first 2 years of transplantation, while 19 pregnancies occurred after that period. No difference was noted in neonatal body weight between 2 groups (p=0.824). There was no difference in mean interval from transplantation to pregnancy in the delivery group (5.21+/-0.9 years) vs. the abortion group (4.57+/-1.1 years) (p=0.207). All women continued with her medication during pregnancy, based on cyclosporine and/or azathioprine and steroids. No congenital anomaly was noted in the newborn. When follow up lately, one boy with neurofibromatosis and a girl with recurrent hematuria were noted. CONCLUSION: Although pregnancy in renal allograft patient is hazardous, good outcome can be expected if function of the allograft is good before conception.

RESUMO

Vulvar ateriovenous hemangioma is a rare pathologic lesion, encountered in Bartholin's glands of vulva. Little has previously described in the litherature. A painless vulvar mass occurred accidently in a healthy women after cesarean delivery. There was no obvious lesion on the skin. Excisional biopsy revealed a vascular lesion in the subcutaneous layer, consistent with arterio-venous hemangioma. The mass was sucessfully treated by excision, and the patients was asymptomatic for five months later. so, we report this case with a brief review of literatures.

OBJECTIVE: A Clinical Study of Placenta P#revia done by cesarean section and the study of preterm delivery that is major cause of premature motality even thought expected managment of placente previa. METHODS: This study was evaluated for the clinical analysis on the 641 patients with placenta previa among total deliveries of 56,120 cases at the St. Mary and Holy Family hospital of Catholic University from Jan. 1,1989 to Dec.31,1997. RESULTS: The following results were obtained: 1. The total incidence of placenta previa was 1.14%. 2. The incidence of placenta previa increases with age. 3. Placenta previa has occurred more often in multipara(60.2%) than primipara(39.8%), and placenta previa has occurred more often in women who had experienced abortion(71.8%) than women who hadnt(28.2%). 4. The types of placenta previa distributed 326 cases of totalis(50.9%), 110 cases of partialis(17.2%), 99 cases of marginal(15.4%) and 106 cases of low lying placenta(16.4%). 5. The fetal presentation distributed 557 cases of vertex(86.9%), 49 cases of breech(7.6%), 28 cases of transverse lie(4.4%). 6. The types of uterine incision included 543 cases of low sepnent transverse incision(84.7%), 46 cases of classical incision(7.2%), 9 cases of inverted T incision and 43 cases of cesarean hysterectomy(6.7%). 7. Premature delivery of placenta previa prior to completion of 37 weeks was 186 cases(29.0%) and low birth infant less than 2,500gm was 128 cases(20.0%).and perinatal mortality rate was 28.0 per 1,000. 8. During delivery, 278 cases of placenta previa(43.4%) were transfused with mean 3.6 pints. Incidence of vaginal bleeding was 49.3% of placenta previa,mean admission day until delivery was about 5 day. CONCLUSION: there is a strong association between advancing age and incidence of placenta previa, and then. the risk increases with parity & the number of abortion. Considering the mean volume of blood loss more than 3 pints of blood should be prepared befor delivery.

Total 8,341 pregnant women who visited Obstetric Clinic Cheil General Hospital from Jaunary 1991 to July 1992 were screened for HBsAg. Sixty five women with positive HBsAg who delivered healthy babies during that period and their infants are included in the study. HBsAg, Anti-HBs, Anti-HBc, HBeAg and Anti-HBe were tested by radioimmunoassay and hepatitis B virus DNA were studied by polymerase chain reaction technique in both mothers' and babies serum. The results were as follow: 1) Among 8,341 pregnant women who were screened for HBsAg 374 were asymptomatic carriers(4.5%). 2) The newborn babies born to 65 mothers who were positive HBsAg showed HBsAg in 4.6%, HBeAg 75.0%, anti-HBc 98.4% and hepatitis B virus DNA in 26.2%. 3) It showed discordance of HBeAg/anti-HBe, with hepatitis B virus DNA in newborn infants. 4) Follow up study at 8~20 months showed HBeAg and hepatitis B virus DNA in 2 infants out of 20.

BACKGROUND: Perfusion scintigraphy with dipyridamole have been reported to be useful for diagnosis of coronary artery disease and the assessment of the presence and extent of myocardium at ischemic risk, especially in patients who can not undergo dynamic exercise testing. Dipyridamole, pharmacologic coronary vasodilator, also induces fall in blood pressure and rise in heart rate. The purpose of this study was to answer the question if dipyridamole induced peripheral hemodynamic responses were related to chest pain, ST changes on EKG, scintigraphic defect or extent of coronary stenosis. METHODS: Dipyridamole 99mTc-MIBI myocardial scintigraphy and coronary angiography on 43 subjects who were suspected to have coronary artery disease. The peripheral hemodynamic response was graded as absent(group 0) if there was a 10 but 10 but 20mm fall in SBP and/or >20 beats/min rise in HR. RESULTS: The overall diagnostic sensitivity and specificity for coronary artery disease of dipyridamole perfusion scintigraphy were 68%, 83% while per vessel sensitivity and specificity for coronary artery disease were 66%, 97%. The numbers of induced chest pain and ischemic ST changes among hemodynamic subgroups, were 40%, 40% in group 0, 33%, 27% in group 1 and 50%, 40% in group 2 without significant difference in each hemodynamic subgroups. Either the numbers of diseased coronary arteries or the numbers of patients demonstrationg reversible scintigraphic defects were not statically different among each subgroups. CONCLUSION: Although the peripheral hemodynamic response dose not always correlate with its central coronary effect but dipyridamlole 99mTc-MIBI myocardial perfusion scintigraphy is an useful test for diagnosis of coronary artery disease.

RESUMO

Though exercise T1-201 myocardial perfusion-scintigraphy is an important test in the diagnosis and evaluation of coronary artery disease, this test is limited in cases with poor physical conditions and unstable angina and the physical characteristics of thallium are not ideal for nuclear imaging of the myocardium. Recently, technetium-99m-labelled agent(99mTc-MIBI) has been developed to improved the quality of imaging and for easy usage in myocardial perfusion scan. Therefore, we performed dipyridamole 99mTc-MIBI myocardial scintigraphy and coronary angiography on 25 subjects who were suspeced to have coronary artery disease and the results obtanined were as follows ; 1) The overall diagnostic sensitivity of dipyridamole 99mTc-MIBI perfusion scintigraphy was 73.4%, while specificity was 66.7%. 2) Per vessel sensitivity for coronary artery disease diagnosis by the test was 71.4% and a per vessel specificity for coronary artery disease was 95.7%. 3) In all 75 coronary arteries, LAD had a sensitivity of 66.7%(10/15) and a specificity of 100%(10/10); RCA had a sensitivity of 75%(6/8) and a specificity of 82.4%(14/17); LCX had a sensitivity of 60%(3/5) and a specificity of 100%(20/20). 4) After infusion of dipyridamole, the most frequent adverse effect was chest pain which was seen in 8 cases(32%). The headache was seen in 7 cases(28%). Most of the symptoms were mild in severity and subsided spontaneously but 3 subjects were severe enough to require intravenous administration of aminophylline. In conclusion, dipyridamole 99m2011-06-17Tc-MIBI myocardial perfusion scintigraphy is a useful and safe test for diagnosis of coronary artery disease.

RESUMO

In 22 end-stage renal disease patients on regular hemodialysis, echocardiographic study including 2-D Doppler echocardiogrphy was performed to evaluate the effect of hemodialysis on cardiac performance and the degree of valvular regurgitation. After hemodialysis, there were significant improvement of ventricular contractility and reduction of circulating volume in accordance with the reduction of degree of valvular regurgitation in comparison with before hemodialysis. We postulate that the reduction of valvular regurgitation after hemodialysis is caused by relief of hypervolemic state.
