INTRODUCCIÓN: El tratamiento de la rosácea papulopustulosa (RPP) ha consistido durante años en el uso de tetraciclinas orales y antibióticos tópicos. Pero no es infrecuente encontrar casos de RPP resistentes al tratamiento convencional. Azitromicina oral ha demostrado ser una opción eficaz para estos pacientes no respondedores. MATERIAL Y MÉTODO: Se realizó un estudio piloto prospectivo con 16 pacientes con RPP no respondedores al tratamiento convencional (doxiciclina oral y metronidazol gel) que recibieron tratamiento con azitromicina oral. En la visita inicial (visita 1) se realizó una valoración basal del estadio clínico de la RPP, según 4 niveles de gravedad progresiva, y se inició tratamiento con azitromicina oral. A las 8 semanas de finalizar el tratamiento (visita 2) se evaluó la respuesta clínica según 3 niveles de mejoría respecto al estadio clínico basal. Posteriormente, para evaluar la eficacia de azitromicina oral a largo plazo, se realizaron visitas periódicas cada 12 semanas. RESULTADOS: Todos los pacientes que recibieron tratamiento con azitromicina oral mejoraron de su RPP. A las 8 semanas de finalizar el tratamiento se objetivó un eritema facial residual débil o nulo, con desaparición completa de las pápulas y/o pústulas en el 87,5% de los pacientes. En cuanto al mantenimiento de la eficacia a largo plazo, únicamente 2 pacientes presentaron una recidiva de lesiones inflamatorias de RPP. CONCLUSIONES: Los resultados de nuestro estudio evidencian que azitromicina oral podría ser un fármaco de gran eficacia a corto y largo plazo para el manejo de aquellos casos de RPP resistentes al tratamiento convencional

Eight weeks after completion of treatment, 14 patients (87.5%) showed complete or almost complete recovery (slight or no residual redness and complete clearance of papules and pustules). Only 2 patients experienced a new episode of inflammatory PPR lesions during follow-up. CONCLUSIONS: The findings of this pilot study suggest that oral azithromycin could be a very effective short-term and long-term treatment for RPP resistant to conventional treatment

INTRODUCTION: ß-antithrombin, the minor antithrombin glycoform in plasma, is probably the major thrombin inhibitor in vivo because of its high heparin affinity. The levels and variability of this glycoform in general population and its relevance in thromboembolic diseases is unknown since there is no specific method to measure this glycoform in clinical samples. METHODS: Plasma and recombinant α- and ß-antithrombins were purified by heparin affinity chromatography. An anti-FXa chromogenic method in presence of pentassacharide was used with two NaCl concentrations (15mM and 1.1M). This method was applied to plasma samples from 97 healthy subjects and 117 consecutive patients with ischemic cerebrovascular disease during the acute event and one year later. SERPINC1 sequencing was done in cases with antithrombin deficiency. RESULTS: High salt concentrations specifically restricted the pentassacharide-induced activation of antithrombin to the ß glycoform. ß-antithrombin displayed a normal distribution in the general population (89.5%-103.5%), with no significant variations according to age or sex. In patients, whole antithrombin values remained within the normal range. Only five cases had antithrombin deficiency during the thrombotic event, one carrying the L99F mutation in SERPINC1. Interestingly, both ß-antithrombin and the ß/whole antithrombin ratio were significantly higher in patients during the acute event but normalized after one year. CONCLUSIONS: We have developed a rapid, simple, sensitive and specific method to quantify ß-antithrombin activity using 1µL of plasma. ß-antithrombin significantly increases in patients with ischemic cerebrovascular disease during the acute event, probably by its release from the vasculature.

OBJECTIVE: To describe an accreditation system for informed consent forms (ICF) in a tertiary hospital, as an intervention to improve their quality, and to check the improvements achieved. MATERIAL AND METHODS: Following an external evaluation of the ICF quality in a public hospital in Murcia (Spain), an accreditation committee set the ICF requirements and associated procedures. Effectiveness is assessed by comparing two external evaluations carried out by the EMCA Program (2011 and 2013) and based on 19 criteria and a sample of 60 ICF for every public hospital in Murcia Region. RESULTS: To be accredited, every ICF must meet the 19 external criteria plus 5 based on legibility, readability and scientific and technical validity. A form to fill in the contents of every ICF was agreed, which would be reviewed, approved and validated for five years. Before the implementation, 8.2 defects/ICF were detected. The accreditation system obtained an 89% improvement (0.9 defects/ICF) and achieved significant improvements in 18 criteria, 16 of which are benchmarked. DISCUSSION: The accreditation system achieved a substantial improvement in the ICF (obtaining a better result in external evaluations) and guarantees their contents, legibility and readability. This system needs to be extended to other hospitals, since it is not clear whether common ICFs would be suitable. However, this improvement is structural and does not guarantee that the overall information/consent procedure is done properly, thus complementary strategies for measurement and improvement are required.

RESUMO

Una de las principales características de una buena combinación farmacológica es la sinergia. Existe en el mercado, desde hace relativamente poco tiempo, una nueva combinación analgésica de ibuprofeno, un antiinflamatorio no esteroideo (AINE) muy usado en la práctica médica diaria por su buen cociente de eficacia y seguridad, y codeína, un opiáceo menor muy utilizado como segundo escalón analgésico en combinación. Hemos realizado una revisión en MEDLINE de los principales estudios publicados en la literatura médica en cuanto a eficacia y seguridad de ambos principios activos por separado, para finalizar con una aproximación tanto de la experiencia clínica mostrada en estudios con combinaciones analgésicas en general, como en las hipótesis de un posible sinergismo entre AINE y opiáceos (AU)

OBJECTIVE: To evaluate the usefulness of endoscopic assisted surgery of pituitary adenomas in transesphenoidal surgery, and in surgery of craneopharyngiomas using either minimally invasive approaches to the cranial base or transventricular approaches. MATERIAL AND METHODS: We present our preliminary experience in eleven patients operated of sellar region tumor by endoscopic assisted resection: 6 pituitary adenoma via transesphenoidal approach, 4 craneopharyngiomas 3 through supraciliar approach and 1 by transcortical transventricular approach, and 1 suprasellar cyst. RESULTS: By using the 30 degrees optic the use of endoscope allowed complete resection, confirmed by postoperative MRI, of all six pituitary macroadenomas providing control of resection of supraselar remnants. Complete resection was achieved in three out of four craneopharyngiomas, 2 of them being recurrences. Three were operated by using a supraciliar approach to the cranial base and in one case transcortical transventricular resection of a recurrent intraventricular craneopharyngioma was performed. In the case with partial resection remnant were let in place due to the close adherence to peritumoral structures. In the three craneopharyngiomas operated via supraciliar approach endoscope allowed better control of inferior aspect of ipsilateral optic nerve and internal carotid artery. In the case of intraventricular craneopharyngioma, the use of 30 degrees endoscope provide control of resection of the anterior part of third ventricle through the foramen of Monro with no additional opening. The suprasellar cyst was fenestrated. CONCLUSIONS: No matter which approach is going to be used in the resection of sellar tumors, endoscopy can play a crucial role in achieve complete resection with minimal morbidity by using minimally invasive procedures.

Objetivos: El estudio pretende evaluar la utilidad del uso del endoscopio en la cirugía de la región selar a través del abordaje transesfenoidal transnasal en los adenomas hipofisarios y a través de abordajes mínimamente invasivos a la base de cráneo o el sistema ventricular en el caso de craneofaringiomas. Material y métodos: Presentamos la experiencia preliminar en once casos intervenidos mediante cirugía asistida con endoscopia. Seis pacientes presentaban macroadenomas hipofisarios y fueron intervenidos por vía transesfenoidal transnasal. Cuatro pacientes presentaban craneofaringiomas, 2 de ellos recidivantes, que fueron abordados, 3 a través de un acceso supraciliar y uno mediante un abordaje transcortical transventricular, abordaje utilizado en un quiste supraselar intraventricular. Resultados : Se consiguió la exéresis completa confirmada por RM de los adenomas hipofisarios en los que el uso del endoscopio con óptica de 30° fue de utilidad en el control de la exéresis de los tumores con expansión supraselar. En el caso de los craneofaringiomas se alcanzó la exéresis completa en 3 de ellos uno de los cuales era recidivante, 2 por vía supraciliar y otro transcortical transventricular. En el caso restante, un craneofaringioma recidivante, la exéresis fue parcial por la íntima adherencia de la cápsula tumoral a las estructuras circundantes. En los 3 casos de acceso supraciliar, el endoscopio fue útil para el control de la exéresis del tumor localizado inferior al nervio óptico y la carótida interna ipsilaterales. En el acceso intraventricular el craneofaringioma que ocupaba el tercio anterior y medio del tercer ventrículo pudo resecarse a través del foramen de Monro, mediante una óptica de 30° que permitió controlar y resecar el resto tumoral del tercio anterior. El quiste fue fenestrado. Conclusiones: En cualquiera de las posibles vías de abordaje a la región selar, el uso de la cirugía asistida por endoscopia favorece una mayor radicalidad en la resección mediante el uso de abordajes mínimamente invasivos (AU)

The schwannomas are tumors arising from nervous tissue. It appears very rarely in the nose and paranasal sinuses. We report a case of a female whit nasal obstruction that presents a nasal neurinoma schwannoma

The intensity distribution of the ultrasonic energy is, after the frequency, the most significant parameter to characterize ultrasonic fields in any sonochemical experiment. Whereas in the case of low intensity ultrasound the measurement of intensity and its distribution is well solved, in the case of high intensity (when cavitation takes place) the measurement is much more complicated. That is why the predicting the acoustic pressure distribution within the cell is desirable. A numerical solution of the wave equation gave the distribution of intensity within the cell. The calculations together with experimental verification have shown that the whole reactor behaves like a resonator and the energy distribution depends strongly on its shape. The agreement between computational simulations and experiments allowed optimisation of the shape of the sonochemical reactor. The optimal geometry resulted in a strong increase in intensity along a large part of the cell. The advantages of such optimised geometry are (i) the ultrasonic power necessary for obtaining cavitation is low; (ii) low power delivered to the system results in only weak heating, consequently, no cooling is necessary and (iii) the "active volume" is large, i.e. the fraction of the reactor volume with high intensity is large and is not limited to a vicinity close to the horn tip.

RESUMO

Numerical simulations have been carried out in order to characterize the ultrasonic field propagation and to obtain the spatial distribution of the mechanical effect derived from it. The results have been compared with those obtained with different classical physical methods (calorimetry, aluminium foil erosion, thermal probes) and have given useful information about the influence of the presence of probes and auxiliary tools in the ultrasonic field. All these information have been used for the development of the Part II of this work: analysis of chemical effects, providing an accurate picture of the reaction environment in the sonoreactor used (20 kHz, 100 W supplied by Undatim) for further uses in sonoelectrochemical studies.

RESUMO

A new electrochemical redox probe has been investigated in order to characterize the local production of radicals during the cavitation events. The results have been compared with those obtained with Fe(CN)6(3-)/Fe(CN)6(4-) (electrochemical probe for local mechanical effects) and classical chemical methods such as iodide and Fricke dosimeters (chemical probes for global effects).
